심실중격결손증의 외과적 고찰 (Clinical Analysis Of Ventricular Septal Defect)

  • Two hundred one patients of ventricular septal defect, which were operated at Seoul National University Hospital, were analysed on clinical background during the period from January, 1975 to December, 1980. The results were as follows: 1. Of the 201 patients, 118 patients were male [58.7%] and 83 patients were female [41.3%]. Their age ranged from 15 months to 40 years, and the mean age was 8.7 years. 40% of the patients were between 4 and 8 years. 2. The most common symptoms showed frequent U RI and exertional dyspnea. 3. On Kirklin`s anatomical classification, type I constituted 26.9%, type II 58.2%, type III 12.4%, and type IV 1.0%. We showed marked increased incidence of type I VSD as compared to Caucasians* 4. 46 cases were associated with other congenital cardiac diseases. They were PDA [13 cases], AI[11 ], ASD[6], PS[10], MI[4], and Double aortic arch [1]. 5. In 128 patients, who had complete hemodynamic data and were not associated with other congenital cardiac diseases, an attempt was made to correlate the EKG findings with the hemodynamic data, and defect size with the hemodynamic data. The children had variable distribution of PA syst. pr. and Rp/Rs. But most of adults had $R_P$/$R_S$of 0.15 or less. As $P_P$/$P_S$increased, the rate of operative complication increased also. 6. When a normal EKG pattern was present, $Q_P$/$Q_S$and $R_P$/$R_S$and $P_P$/$P_S$were relatively low. When EKG findings were LVH pattern, there was diastolic volume overload to left ventricle. As RVH, there was systolic pressure overload to right ventricle. And as BVH, there was mixed pattern of diastolic volume overload to left ventricle and systolic pressure overload to right ventricle. 7. Among patients in defect was less than 1 $cm^2$ per $M^2$ of BSA, $Q_P$/$Q_S$was less than 2:1, and $R_P$/$R_S$less than 0.25, and PAsyst. pr. less than 50 mmHg, and $P_P$/$P_S$was less than 0.5. But patients with the defect greater than 1 $cm^2$ per $M^2$ of BSA had no correlationship between $Q_P$/$Q_S$, $R_P$/$R_S$, PAsyst. pr. and defect size in each other. Most of patients with the defect greater than 2 $cm^2$/$M^2$ BSA, $R_P$/$R_S$was greater than 0.5. 8. Operative mortality rate was 9.5% [19 cases] among 201 patients. And complication rate including mortality rate was 22.9% [46 cases].

고페리틴혈증의 원인과 적혈구 수혈 (Causes of Hyperferritinemia and Red Blood Cell Transfusion)

  • 배경: 만성 적혈구 수혈 환자에서 철과잉 여부를 알아보기 위해 페리틴 검사를 시행하는데, 페리틴은 체내 저장철의 양을 반영하지만, 염증, 감염 등에서도 비특이적으로 증가할 수 있다. 본 연구에서는 고페리틴혈증을 보이는 환자에서 페리틴이 증가한 원인을 분석하고 적혈구제제 수혈과의 연관성을 알아보고자 하였다. 방법: 2017년 1월부터 12월까지 본 의료기관에 내원하여 페리틴 검사를 시행한 환자들의 의무기록을 후향적으로 검토하였다. 시행한 페리틴 검사에서 1,000 ng/mL이상의 결과를 보인 경우를 고페리틴혈증으로 정의하고 연구대상으로 정하였다. 페리틴 검사와 함께 시행된 혈청 철, TIBC (Total iron bind capacity), CRP (C-reactive protein) 검사결과와 진단명 등 의무기록을 조사하여 고페리틴혈증의 원인을 분석하였다. 결과: 해당 기간 동안 고페리틴혈증 결과를 보인 건은 238명 환자에서 417건이었다. 질환별로는 혈액종양질환이 31명에서 125건(30.0%)으로 가장 많았으며, 감염질환이 두번째로 많았다. 철과잉이 의심되는 경우는 33명 환자에서 119건이었으며, 이 중 검사 시행 전 1년 동안 적혈구제제수혈이 8단위를 초과한 경우는 12명(76건)이었다. 결론: 고페리틴혈증을 보이는 환자에서 실제 철과잉으로 판단되는 경우, 기저질환의 영향이 있었고 만성 적혈구제제 수혈과도 연관이 있었다. 체내 철 저장 상태를 정확히 파악하기 위해서 페리틴 외에 CRP, 철 포화도를 함께 측정하는 것이 도움이 된다. 습관적인 철분제제 처방, 잦은 수혈 처방시에 철과잉 발생 여부에 주의를 기울여야 한다.

직무순환의 개인발전이 직무만족도에 미치는 영향에 관한 연구 -업무효율성의 매개효과와 업무과부하의 조절효과를 중심으로- (A Study on the Effect of Individual Development of Job Rotation on Job Satisfaction - Focused on the Mediating Effect of Work Efficiency and the Moderating Effect of Work Overload -)

  • 직무순환은 사회복지시설의 종사자들이 여러 직무와 다양한 경험을 할수 있도록 하기 위해 일정기간마다 타 직무 또는 업무로 변경하여 근무할수 있도록 하는 제도로서 인사이동, 업무분장, 직무이동 등의 명칭으로 사용하고 있다. 본 연구는 직무순환의 개인발전이 업무효율성과 직무만족도에 미치는 영향을 실증적으로 분석하기 위해 전남 목포시에 소재중인 사회복지시설 종사자를 대상으로 2019년 5월 21일부터 5월 24일까지 설문조사를 실시하였다. 본 연구의 독립변수로 직무순환의 개인발전, 종속변수로 직무만족도, 매개변수로는 업무효율성, 조절변수로는 업무과부하로 설정하였다. 모든 변수들의 문항은 Likert 5점 척도를 활용하였고, 가설을 검증하기 위해 실증 분석 도구로서SPSS 24.0 버전을 사용하였으며, 기술 통계, 신뢰도분석을 실행하였으며, SPSS PROCESS macro 4.0을 활용하여 매개효과, 조절효과 분석을 실시하였다. 본 연구 결과는 다음과 같다. 첫째, 직무순환의 개인발전은 직무만족도에 유의한 영향을 미치지 않는 것으로 분석되었다. 둘째, 업무효율성은 직무순환의 개인발전과 직무만족도 간의 관계를 완전 매개하는 것으로 나타났다. 마지막으로, 업무효율성과 업무과부하의 상호작용항은 직무만족도에 부적 영향을 미쳐 조절된 매개효과가 있는 것으로 나타났다.

하중진폭제어에 따르는 피로균열전파거동 (Fatigue Crack Propagation Behaviors under the Controlloed Stress Amplitude)

  • The effect of mechanical, properties in the plastic zone near the crack-tip was investigated, under various controlled loading conditions, i.e., ${\Delta}K$ increasing, ${\Delta}K$ decreasing, and single overload test. For both ${\Delta}K$ decreasing test and ${\Delta}K$ increasing test with constant stress ratio, it is found that the ratio of material constant m'( ${\Delta}K$ decreasing test) to material constant m( ${\Delta}K$ increasing test) is larger than 1 for n<0.1, and it is equal to 1 for 0.10.2. A modified crack growth rate equation based on Forman's equation which applied stable region of fatigue crack propagation in ${\Delta}K$ decreasing test is proposed. Within the limit of this single overload test, an empirical relation between among the retardation ratio (Nd/ $N^{*}$), the strain hardening exponent (n) and the percent peak load (%PL) has been established.established.

임상간호사의 직무 과부하, 자기효능감, 정서적 소진과 이직의도와의 관계 (The Relationship among Job Overload, Self-efficacy, Emotional Exhaust and Turnover Intention in Clinical Nurses)

  • Purpose: This study was to identify the relationship among clinical nurse's job overload, self-efficacy, emotional exhaust and turnover intention. Methods: The study was conducted through a survey on 537 clinical nurses in five general hospitals. The data were collected by self-reporting questionnaires and analysed by the SPSS/WIN 17.0 program using descriptive statistics, t-test, ANOVA, Pearson correlation coefficient and stepwise multiple regression. Results: The mean score of turnover intention was 3.04. Turnover intention according to age, total career, job objective, and work off showed significant differences. The significant positive correlations between emotional exhaust and turnover intention (r=.531, p<.001), job overload and turnover intention (r=.514, p<.001) were found. The significant factors influencing turnover intention were emotional exhaust (${\beta}$=.488, p<.001), career (${\beta}$=.175, p<.001), job overload (${\beta}$=.119, p= .003), and number of night (${\beta}$=.117, p=.002), which accounted for 40.3% of the variance. Conclusion: Thought this result, nursing managers should analyze the reasons for high turnover intention, focusing on the main factors affecting turnover intention and consider the solutions to decrease the turnover intention.

Association between Transfusion-Related Iron Overload and Liver Fibrosis in Survivors of Pediatric Leukemia: A Cross-Sectional Study

  • Purpose: Patients who receive frequent blood transfusions are at an elevated risk of developing hepatic fibrosis due to iron overload in the liver. In this study, we evaluated the effectiveness of transient elastography (TE) (FibroScan®) for assessing liver fibrosis in patients with pediatric cancer. Methods: We enrolled 106 consecutive cases of acute leukemia in individuals under 21 years of age. The participants were followed for 2 years. Based on their serum ferritin (SF) levels, the patients were divided into two groups: group 1 (SF≥300 ng/mL) and group 2 (SF<300 ng/mL). A liver FibroScan® was performed, and a p-value of less than 0.05 was considered statistically significant. Results: Among the various parameters in the liver function test (LFT), alkaline phosphatase was significantly higher in a subgroup of patients aged 5-8 years in group 2 compared to those in group 1. The indices of liver fibrosis determined by TE, including the FibroScan score, controlled attenuation parameter score, steatosis percentage, and meta-analysis of histological data in viral hepatitis score, as well as indirect serum markers of liver fibrosis such as the aminotransferase (AST)/alanine aminotransferase (ALT) ratio, Fibrosis 4 score, and AST to platelet ratio index, did not differ significantly between the two groups. The association between the TE results and LFT parameters was only significant for ALT. Conclusion: Transfusion-associated iron overload does not have a significant correlation with severe liver fibrosis. FibroScan® is not a sensitive tool for detecting early stages of fibrosis in survivors of pediatric leukemia.

Effect of Hfe Deficiency on Memory Capacity and Motor Coordination after Manganese Exposure by Drinking Water in Mice

  • Excess manganese (Mn) is neurotoxic. Increased manganese stores in the brain are associated with a number of behavioral problems, including motor dysfunction, memory loss and psychiatric disorders. We previously showed that the transport and neurotoxicity of manganese after intranasal instillation of the metal are altered in Hfe-deficient mice, a mouse model of the iron overload disorder hereditary hemochromatosis (HH). However, it is not fully understood whether loss of Hfe function modifies Mn neurotoxicity after ingestion. To investigate the role of Hfe in oral Mn toxicity, we exposed Hfe-knockout ($Hfe^{-/-}$) and their control wild-type ($Hfe^{+/+}$) mice to $MnCl_2$ in drinking water (5 mg/mL) for 5 weeks. Motor coordination and spatial memory capacity were determined by the rotarod test and the Barnes maze test, respectively. Brain and liver metal levels were analyzed by inductively coupled plasma mass spectrometry. Compared with the water-drinking group, mice drinking Mn significantly increased Mn concentrations in the liver and brain of both genotypes. Mn exposure decreased iron levels in the liver, but not in the brain. Neither Mn nor Hfe deficiency altered tissue concentrations of copper or zinc. The rotarod test showed that Mn exposure decreased motor skills in $Hfe^{+/+}$ mice, but not in $Hfe^{-/-}$ mice (p = 0.023). In the Barns maze test, latency to find the target hole was not altered in Mn-exposed $Hfe^{+/+}$ compared with water-drinking $Hfe^{+/+}$ mice. However, Mn-exposed $Hfe^{-/-}$ mice spent more time to find the target hole than Mn-drinking $Hfe^{+/+}$ mice (p = 0.028). These data indicate that loss of Hfe function impairs spatial memory upon Mn exposure in drinking water. Our results suggest that individuals with hemochromatosis could be more vulnerable to memory deficits induced by Mn ingestion from our environment. The pathophysiological role of HFE in manganese neurotoxicity should be carefully examined in patients with HFE-associated hemochromatosis and other iron overload disorders.

진성 리더십이 기술스트레스를 통해 지식관리시스템 이용의도에 미치는 영향: 기술스트레스의 매개효과 및 피드백의 조절효과 분석 (The Effect of Authentic Leadership on Intention to Use Knowledge Management System through Techno-stress: Analysis of the Mediating Effect of Techno-stress and the Moderating Effect of Feedback)

  • 본 연구는 조직 지식관리시스템에 의해 발생 가능한 기술스트레스를 완화하는 조건을 제시하는 것에 목적이 있다. 세부적으로, 연구는 지식관리시스템의 기술 과부하와 기술 복잡성이 지식관리시스템 이용의도에 미치는 부정적 요인임을 밝히고, 진성 리더십과 피드백이 기술스트레스를 완화하는 것을 제시한다. 연구는 지식관리시스템을 도입한 조직의 구성원에게 설문을 실시하였다. 2021년 5월 온라인 설문을 실시하였으며, 417개의 표본을 확보하였다. 가설검증은 AMOS 22.0을 활용하여 구조방정식모델링을 반영하여, 주 효과 분석과 조절 효과 분석을 실시하였다. 분석 결과, 진성리더십이 지식관리시스템 이용의도를 감소시키는 기술 과부하와 기술 복잡성의 영향을 완화 시켰다. 그리고, 피드백이 기술스트레스와 이용의도 간의 부정적 관계를 조절하였다. 연구는 조직원 관점에서 지식관리시스템 도입 및 적용 시 문제될 수 있는 기술스트레스 완화에 대한 조직 차원의 전략적 방향(진성리더십 및 피드백 활동 강화)을 제시한 관점에서 시사점을 가진다.

보육교사의 직무스트레스가 이직의도 및 전직의도에 미치는 영향과 개인적 성취감의 조절효과 (The Effects of Child Care Teachers' Job Stress on Job Turnover, Occupational Turnover and the Moderating Effect of Personal Accomplishment)

  • The purpose of this study was to examine the effects of stress on child care teachers' with relation to job turnover, occupational turnover and the moderating role of personal accomplishment. The subjects were 153 child care teachers in Gyeonggi-Do. Data was analyzed through Pearson's correlation, multiple regressions using the SPSS12.0. This study showed that work overload and administrative problems in child care centers were significant predictors of a teachers' intention to change their job. Also, the stresses of human relations such as with children or co-workers were significant predictors of a child care teachers' intention to change occupation. Personal accomplishment was suggested to reduce the negative results of work overload and stress.

사무직 근로자의 역할 관련 스트레스 요인과 대처전략, 스트레스 반응간의 관계 (The Relationships of Role-Conflict and Role-Overload, Coping Strategies, and Stress Response of Clerical Workers)

  • This study was designed to identify the relationships of role-conflict and role-overload, coping strategies, and stress response of clerical workers. The subjects were 115 clerical workers from two company. The instruments were the role-conflict and role-overload scale developed by Lee(1989), coping scale developed by Latack(1986), and the stress response scale developed by Chang(1993). Analysis of data was done by descriptive statistics, Cronbach's $\alpha$, Pearson correlation coefficietnt, ANOVA. The results of this study were summarized as follows ; 1. The average score for the level of stress response was $83.6({\pm}9.1)$. The high risk stress group was 86.1% of the clerical workers, the latent stress group members was 13.9%, and healthy group was 0%. The overall stress level was very high. 2. Considering the coping strategies by sociodemographic factors, the use of control coping strategies showed significant differences by work-time. The use of symptom management coping strategies showed significant differences by sex, age, married status, year of career, pay, and type of work. The organizational coping strategies showed significant differences by year of education and type of work. 3. The clerical workers who had worked below 44 hours per week complained significantly lower stress response(F=4.942, p<.05). 4. Above 65% of all respondents answered that they needed all of organizational level coping strategies. 5. The role-confilct positively related to escape and organizational level coping strategies(r=.562, r=.495, p<.05). The role-overload didn't related to individual level coping strategies, but positively related to organizational level coping strategies(r=.320, p<.05). 6. The control coping strategies and the escape coping strategies positively related to symptom management, organizational level coping strategies(r=.409, r=.324, r=.316, r=.339, p<.05). 7. The relationship between role-related stress and stress response revealed positive correlation(r=.482, r=.431, p<.05), and the relationship between stress response and control, symptom management coping strategies revealed negative cor-relation(r=-.3204, r=-.3146, p<.05). In conclusion, this study revealed that relationship of role related stress, coping strategies, stress response was statistically significant. The result of this study support that coping strategies vary across situation of stress. Based on survey results, future studies should consider coping strategies in specific stress situation and concept of symptom management coping strategy. Also organizational or training interventions could be developed and initiate to help to increase the use of adaptive coping strategies in light of individual and situational differences.
