이중 에너지 엑스레이 흡수기의 가동 시간에 따른 골밀도 값의 평가 (The Bone Mineral Density Value According to the Operating Time of the Dual Energy X-ray)

  • Purpose: Recently, the performance of the X-ray tube was very much improved by the power generation of the technology. However, the overload of equipment is occurred by the increment of the equipment operating time according to the increment of the examination number of cases. The X-ray dose can change by heat occurrence of the X-ray tube due to this. Moreover, the change of the bone mineral density value is possible to occur. Therefore, We tries to whether the change of the bone mineral density value of each equipment according to the difference of the examination number of cases and operating time occur or not. Materials and Methods: The BMD value was measured by the Aluminum Spine Phantom and the European Spine Phantom in each equipment, in order to find out about the difference of the time general classification bone mineral density value by using the Dual energy X-ray absorptiometry. And after scanning each phantom by using X-ray dose meter (Unfors Mult-O-Meter), a dose was measured by the same condition. As to, an average and standard deviation were found and the change of each equipment much BMD value was compared and it evaluated. Results: $Mean{\pm}SD$ of each equipment by using the Aluminum Spine Phantom, A equipment was $1.174{\pm}0.002$, $1.171{\pm}0.005$, $1.173{\pm}0.005$, B equipment was $1.186{\pm}0.003$, $1.187{\pm}0.003$, $1.185{\pm}0.003$, C equipment was $1.180{\pm}0.003$, $1.182{\pm}0.004$, $1.183{\pm}0.002$, D equipment was $1.188{\pm}0.004$, $1.185{\pm}0.003$, $1.185{\pm}0.004$. By using the European Spine Phantom, A equipment was $1.143{\pm}0.006$, $1.153{\pm}0.009$, $1.161{\pm}0.003$, B equipment was $1.134{\pm}0.004$, $1.13{\pm}0.008$, $1.127{\pm}0.015$, C equipment was $1.143{\pm}0.006$, $1.134{\pm}0.01$, $1.133{\pm}0.006$, D equipment was $1.14{\pm}0.001$, $1.122{\pm}0.002$, $1.131{\pm}0.008$, altogether included in the normal range. Conclusion: There was no significant change of the BMD value of using a phantom by time zones. Therefore, if the quality control is made to use the extent management method of the equipment for beginning in the present application, the reliability of the BMD equipment will be able to be enhanced.

간헐적 전방온혈심정지액에서 희석되지 않은 고농도 포타슘의 사용 (Use of Undiluted Potassium Solution in Intermittent Antegrade Warm Blood Cardioplegia (IAWBC))

  • 배경: 냉혈심정지액은 저온에서 적혈구응집현상을 방지하고 미세순환을 좋게 하기 위해 심정지액을 4 : 1로 희석하여 사용하도록 권장되었는데 간헐적전방온혈심정액(IAWBC)에서는 심정지액을 냉각하지 않기 때문에 희석할 필요가 없이 고농도 포타슘을 바로 사용할 수 있게 되었다. 본 연구는 IAWBC에서 희석되지 않은 고농도 포타슘 사용의 안전성과 유용성을 알아보고자 한다. 대상 및 방법: 관상동맥우회로 조성수술을 받은 환자 중 IAWBC를 이용한 환자 30명을 대상으로 하였다. 1 : 4로 희석된 온혈심정지액을 사용한 군을 dilutedplegia군으로 하고 희석되지 않은 포타슘을 사용한 군을 microplegia군으로 하였다. dilutedplegia군의 심정지액은 1 : 4 delivery kit를 이용하여 상행대동맥뿌리에 주입되었고 microplegia군에서는 희석되지 않은 포타슘을 infusion pump를 이용하여 산소포화 된 혈액에 직접 연결하여 대동맥뿌리에 주입하였다. 걸과: microplegia군이 16명, 대조군인 dilutedplegia군이 14명으로 나이, 성별, 좌심실 구획률, 이식혈관 수, 대동맥차단시간, 수술 후 심근효소치에서는 두 군에서 차이가 없었다(p>0.05). 모든 환자에서 수술 후 심근경색과 수술사망은 없었다. dilutedplegia군에서 사용된 crystalloid심정지액의 양은 1346$\pm$597 mL (평균$\pm$표준편차)이었고 microplegia군에서는 28$\pm$9mL이었다. 체외순환 중 적혈구 구획률은 microplegia군에서 24$\pm$3%로 dilutedplegia군의 21$\pm$4%에 비해 약간 높았으나 통계학적 의의는 없었다. 체외순환 중에 수혈을 받은 환자는 microplegia군에서 4명인데 비해 dilutedplegia군에서는 11명으로 높았다(p<0.05). 수술 중 환자의 소변량과 혈액여과한 양에서 microplegia군이 959$\pm$410 mL와 1481$\pm$784 mL로 dilutedplegia군의 1250$\pm$810mL와 1689$\pm$548 mL에 비해 통계학적으로 의의 있게 적었다(p<0.05). 결론: 관상동맥우회로 조성수술에서 전방온혈심정지액을 사용할 때 희석되지 많은 고농도 포타슘은 fliud overload와 수혈을 피하고 delivery kit를 사용하지 않음으로써 효과적이고 만족할 만한 심근보호 효과를 보였다.

헬릭스각의 변화에 따른 밀링공구의 변위 특성 연구 (The Characteristics of the Milling Tool Deflection According to the Variation of Helix Angle)

  • In the end milling operation the deflection of the cutter is an important factor affecting the accuracy of machining, with implications on the selection of cutting parameters and economics of the operation. Several studies were devoted to the end mill deflection and its effects, notably, providing a useful insight into the problem. Although the deflection affects adversely the accuracy, the flexibility of the cutter is beneficial in attenuating the overload in a sudden transient situation, as well as in attenuating chatter. The deflection of the end mill was studied both experimentally with strain gauge, tool dynamometer, laser measuring apparatus and on a finite element model of the cutting using ANSYS software. The deflection of machining tool with various helix angles was studied with FEM simulation and experiment. ANSYS analysis performed on the finite element model of the end mill provides deflection results which agree within 15.0% with the experimental ones.

간호원이 근무중에 경험하는 스트레스 측정을 위한 도구개발 연구 (The Development of the Stress Measurement Tool for Staff Nurses working in the Hospital)

  • The purpose of this study is to propose the baseline data for developing the stress measurement tool for staff nurses working in the hospital. Two hundred and fifteen stan nurses in Seoul National University Hospital were participated during the period from Feb. 28 to Mar. 10, 1984. Though the pretest and literature review a questionnaire was constructed with 63 stressors which were experienced by the nurses in the hospital. Subjects were given instruction to rate 1∼6 likert type scale according to the level of stress experienced by each stressor described. Reliability of the tool was tested by Cronbach's Alpha, and the result was α=0.94871. Factor analysis was applied to organize 63 items together. As the result, 15 factors were obtained and these factors explained 66.3% of variance. The 15 factors were: 1) Work overload 2) Role conflict as a profession 3) Lack of professional knowledge and skill 4) Interpersonal problem 5) Conflict in nurse doctor relationship 6) Work conflict with doctors 7) Emotional burden due to the limitation of medicine 8) Poor treatment 9) Unsatisfactory relationship with supervisor 10) Low reward 11) Unsatisfactory relationship with subordinate 12) Poor physical environment of unit 13)Responsibility for extra affairs 14)Unfamiliar situation 15)Night duty.

신생아집중치료실 간호사의 캥거루 돌보기에 대한 인식과 장애 (Perception and Barriers to Kangaroo-Mother Care Among Neonatal Intensive Care Unit Nurses)

  • Purpose: To investigate the perception and barriers of Kangaroo-Mother Care (KMC) among nurses in Neonatal Intensive Care Units (NICU). Methods: Participants were 131 nurses working in NICU who completed self-report questionnaires which included information regarding perception, barriers, and practice of KMC. Collected data were analyzed using SPSS 18.0 program for descriptive statistics, t-test, ANOVA, and Pearson correlation coefficients. Results: Of the participants, 33.6% reported the practice of KMC in their NICU, with 75.6% wanting to receive training in KMC and 31.3% having received KMC education. Most of the participants agreed that KMC enhances attachment, parental confidence, and effective breast feeding but they reported a negative perception in providing KMC for premature infants weighing less than 1000 grams or intubated premature infants. Major barriers to practicing KMC were safety of infants, possible work overload for nurses, as well as absence of consistent guidelines. Barriers to KMC among nurses who received the KMC training were lower than nurses who did not receive the KMC training (t=-2.11, p=.037). Conclusion: Education program and standardized clinical practice protocol should be developed to foster the positive perception and to reduce nurse barriers to KMC.

Salubrinal Alleviates Pressure Overload-Induced Cardiac Hypertrophy by Inhibiting Endoplasmic Reticulum Stress Pathway

  • Pathological hypertrophy of the heart is closely associated with endoplasmic reticulum stress (ERS), leading to maladaptations such as myocardial fibrosis, induction of apoptosis, and cardiac dysfunctions. Salubrinal is a known selective inhibitor of protein phosphatase 1 (PP1) complex involving dephosphorylation of phospho-eukaryotic translation initiation factor 2 subunit $(p-eIF2)-{\alpha}$, the key signaling process in the ERS pathway. In this study, the effects of salubrinal were examined on cardiac hypertrophy using the mouse model of transverse aortic constriction (TAC) and cell model of neonatal rat ventricular myocytes (NRVMs). Treatment of TAC-induced mice with salubrinal ($0.5mg{\cdot}kg^{-1}{\cdot}day^{-1}$) alleviated cardiac hypertrophy and tissue fibrosis. Salubrinal also alleviated hypertrophic growth in endothelin 1 (ET1)-treated NRVMs. Therefore, the present results suggest that salubrinal may be a potentially efficacious drug for treating pathological cardiac remodeling.

Premature Failure Analysis of Servovalve Components for a Thermal Power Plant

  • The premature failure of a servovalve used for six months in a thermal power plant has been analyzed. The servovalve was made of stainless steel, containing 16Cr-0.44Mo, along with other elements. An overload of oil-supply pumping and an abnormal increase in the oil flux were observed during operation. A study revealed that erosion and corrosion could be the main causes of the failure. The visual examination of the servovalve did not show any appreciable damage. However, corrosion and erosion of the servovalve were observed using scanning electron microscopy (SEM). Upon examination of the servovalve, the corrosion was found to have occurred throughout the bushing and spool; however, erosion occurred at only the edge-side. In addition, the condition of the electrohydraulic control system (EHC) oil was investigated with respect to its satisfaction of the management standard.

잡종 개에서 이첨판 폐쇄 부전증이 합병된 난원공 개존증 (Patent Foramen Ovale Complicated with Chronic Mitral Valvular Insufficiency in A Mixed Dog)

  • 12연령의 암컷 잡종개 (체중 6.2 kg)가 심한 복부 팽만, 지속적 기침 그리고 운동 불내성을 주증으로 진료 의뢰 되었다. 진단상에 복수, 경정맥 노장, 심한 좌우 심첨부 수축기 잡음 (4/6 단계), 후대정맥 확장을 동반한 전반적 심비대와 좌심방 확장이 관찰되었다. 심장 초음파에서는 퇴행성 이첨판막 그리고 삼첨판막 심내막증과 양심실성 울혈성 심부전이 관찰되었다. 좌우단락의 난원공 개존증 또한 있었다. 좌심실 수축능은 용적 과부하의 정도와 관계되어 떨어져 있었다. 진단적 결과에 기초하여, 본 증례는 ISACHC IIIa 심부전의 이첨판막과 삼첨판막 심내막증이 합병된 난원공 개존증으로 진단되었다. 이 개는 푸로세마이드 (2 mg/kg, q12hr, PO), 피모벤단 (0.3 mg/kg, q12hr, PO)와 스피로노락톤 (1 mg/kg, q12hr)으로 치료되었다. 임상 증상들은 약물 치료 후에 점차적으로 개선되었다.

복잡선천성 심장기형 완전교정수술후 시행한 복막투석의 임상적 고찰 (Peritoneal Dialysis after Correction of Complicated Congenital Heart Disease in Children)

  • 본 연세대학교 심장혈관센타에서는 1992년 I월부터 1993년 12월가지 체외순환을 시행하여 완전교정 을 시행한 복잡심장기형 환아 198명중 22예에서 복막투석을 시행하였다. 이들의 나이는 10일에서 6세 (평균 14.8$\pm$ 17.8개월)이었으며 복막투석의 적응증으로는 수분과부하상태가 10예, 혈장칼릅치가 5. Smmo111 이상인 경우가 3예, 그리고 충분한 양의 이뇨제를 투여함에도 소변양이 Imllkglhr 이하가 시간 이상 지속된 경우 9예에서 시행하였다. 체외순환이 90분 이상인 경우와 그 이하인 경우를 비교할 때 90분 이상 시행한 경우에 의미있게 많이 복막투석을 필요로 하였다(P<0.05). 1개월 미만 환아 13예중 5예(38%)에서 시행하였으나 그 이상의 나이와 비교할 때 의미있게 높지는 않았다(p : 0.08). 16예에서 신기능이 회복되었으나 4명은 호흡부전 또는 폐혈증으로 사망하였고 신기능이 회독되지 않 은 6예는 저심박출증이나 폐부종 등으로 모두 사망하였다. 아직도 사망률은 높으나 체 외순환 후 빈뇨, 수분과부하, 또는 고칼륨증이 있을 때에는 조기에 복막투석을 시행하는 것이 환자 치료에 도움이 되리라 사료된다.

Inhibition of Histone Deacetylase Activity Diminishes Pressure Overloaded Cardiac Hypertrophy in Mice

  • To explore the role of histone deactylase (HDAC) activation in an in vivo model of hypertrophy, we studied the effects of Trichostatin A (TSA). TSA subjected to thoracic aortic banding (TAB)-induced pressure stress in mice. In histological observations, TAB in treated mice showed a significant hypertrophic response, whereas the sham operation remained nearly normal structure with partially blunted hypertrophy. TSA treatment had no effect (measured as HW/BW) on sham-operated animals. TAB animals treated with vehicle manifested a robust ~50% hypertrophic response (p<0.05 vs sham). TAB mice treated with 2 mg/kg/day TSA manifested a blunted growth responses, which was significantly diminished (p<0.05) compared with vehicle-treated TAB mice. TAB mice treated with a lower dose of TSA (0.5 mg/kg/day) manifested a similar blunting of hypertrophic growth (~25% increase in heart mass). Furthermore, to determine activity duration of TSA in vitro, 1 nM TSA was added to H9c2 cells. Histone acetylation was initiated at 4 hr after treatment, and it was peak up to 18 hr, then followed by significantly reduced to 30 hr. We also analyzed the expression of p53 following TSA treatment, wherein p53 expression was elevated at 4 hr, and it was maintained to 24 hr after treatment. ERK was activated at 8 hr, and maintained till 30 hr after treatment suggesting an intracellular signaling interaction between TSA and p53 expression Taken together, it is suggested that HDAC activation is required for pressure-overload growth of the heart. Eventually, these data suggest that histone acetylation may be a novel target for therapeutic intervention in pressure-overloaded cardiac hypertrophy.
