In the present report, we describe the successful use of miniscrews to achieve vertical control in combination with the conventional sliding MBT$^{TM}$ straight-wire technique for the treatment of a 26-year-old Chinese woman with a very high mandibular plane angle, deep overbite, retrognathic mandible with backward rotation, prognathic maxilla, and gummy smile. The patient exhibited skeletal Class II malocclusion. Orthodontic miniscrews were placed in the maxillary anterior and posterior segments to provide rigid anchorage and vertical control through intrusion of the incisors and molars. Intrusion and torque control of the maxillary incisors relieved the deep overbite and corrected the gummy smile, while intrusion of the maxillary molars aided in counterclockwise rotation of the mandibular plane, which consequently resulted in an improved facial profile. After 3.5 years of retention, we observed a stable, well-aligned dentition with ideal intercuspation and more harmonious facial contours. Thus, we were able to achieve a satisfactory occlusion, a significantly improved facial profile, and an attractive smile for this patient. The findings from this case suggest that nonsurgical correction using miniscrew anchorage is an effective approach for camouflage treatment of high-angle cases with skeletal Class II malocclusion.
Purpose: The aim of this study was to compare the root surface areas of the maxillary permanent teeth in Thai patients exhibiting anterior normal overbite and in those exhibiting anterior open bite, using cone-beam computed tomography (CBCT). Materials and Methods: CBCT images of maxillary permanent teeth from 15 patients with anterior normal overbite and 18 patients with anterior open bite were selected. Three-dimensional tooth models were constructed using Mimics Research version 17.0. The cementoenamel junction was marked manually. The root surface area was calculated automatically by 3-Matic Research version 9.0. The root surface areas of each tooth type from both types of bite were compared using the independent t-test (P<.05). The intraclass correlation coefficient was used to assess intraobserver reliability. Results: The mean root surface areas of the maxillary central and lateral incisors in individuals with anterior open bite were significantly less than those in those with normal bite. The mean root surface area of the maxillary second premolar in individuals with anterior open bite was significantly greater than in those with normal bite. Conclusion: Anterior open-bite malocclusion might affect the root surface area, so orthodontic force magnitudes should be carefully determined.
This study was to investigate the effect of functional pressures of the tongue and lips on the incisor relationship. The incisor relationship was devided into two categories; one is vertical relationship which is subdevided into open bite, normal overbite and deep bite on the basis of overbite, and the other is anteroposterior relationship which is subdevided into cross bite, normal overjet and large overjet on the basis of overjet. The functional tongue and lip pressures exerted to incisors were measured with subminiature pressure sense from the 99 subjects, 19 of normal overbite and overjet, 26 of open bite, 18 of deep bite, 17 of cross bite and 19 of large overjet with age of 17-20, and cephalograms were taken from the same subjects. Functional pressures were analyzed and correlated to craniofacial veriables. The results of present investigation led to the following conclusions. 1. There were no differences in functional and maximum pressures by the tongue and lips exerted to maxillary incisors between normal occlusion, open bite, deep bite, cross bite and large overjet. 2. Significant differences in functional and maximum pressures by the tongue and lips exerted to mandibular incisors have been shown to exist between open bite and deep bite, but no differences between cross bite and large overjet. 3. Equilibrium between tongue pressures and lip pressures did not exist. 4. Significant differences in the ratio of upper functional and maximum pressures to lower pressures of the tongue and lips exerted to upper and lower incisors have been shown to exist between open bite and deep bite, and no differences between cross bite and large overjet. 5, There was significant correlation between functional and maximum pressures exerted to mandibular incisors and craniofacial variables, but not significant correlation between functional and maximum pressures exerted to maxillary incisors and craniofacial variables.
For the purpose of investigating mesiodistal axial inclination of posterior teeth in normal occlusion group, open bite and deep bite group and investigating the correlationship between the axial inclination of posterior teeth and overbite of anterior teeth, a cephalometric study was performed on the subjects consisted of normal occlusion group(40), open bite group(71 : Angle's Class I, Class II, division 1 25, Class III 25) and deep bite group(64 : Angle's Class I 23, Class II, division 1 21, Class III 20). Mesiodistal axial inclination of posterior teeth to occlusal, mandibular and palatal plane were measured. The findings of this study were as follows : 1. Upper and lower posterior teeth were more mesially inclined to occlusal plane in open bite group than in deep bite group. 2. Lower posterior teeth were more mesially inclined in deep bite group than in open bite group in Angle's Class II, division 1 malocclusion but there were no significant differences in Angle's Class I and Class III malocclusion. 3. There was no significant correlationship between the axial inclination of posterior teeth to each plane and overbite of anterior teeth in open bite group. 4. There was a significant correlationship between the axial inclination of upeer and lower second premolar to occlusal plane and overbite of anterior teeth in Angle's Class I, Class II, division 1 and Class III malocclusion.
The prupose of this study was to obtain standard measurements of the tooth size, Bolton ratio, width and length of dental arch and basal arch, overbite and overjet of the children who have normal dentition. The plaster cast of 97 children(47 boys and 50 girls) among the contestants in 1992, 1994, 1995 Healthy Dentition contest in Seoul were measured and following results were obtained. 1. Means and standard deviations of the mesio-distal maximum width of the permanent teeth, Bolton ratio, width and length of the dental arch and basal arch of the upper and lower dentition and overbite and overjet of the children were obtained. 2. Mesio-distal width of the teeth, width and length of the dental arch and basal arch of the upper and lower dentition of the boys were larger than those of the girls. 3. Bolton Overall ratio, Anterior ratio and overjet of the boys were larger than those of the girls and overbite of the boys were smaller than those of the girls, but no significant differences were noted between the boys and the girls(p>0.05). 4. In the comparision of the Healthy Dentition Contestants with Korean adults of Shur, all teeth of the Contestants were larger than those of Korean adults, especially upper and lower bicuspids(p<0.01). In the comparision of the Healthy Dentition Contestants with Caucasians, all teeth except upper and lower central incisor and upper first molar of the contestants were larger than those of Caucasians(p<0.05).

Objective: The purpose of this study was to examine the effectiveness and mechanism of clear aligner therapy for the correction of anterior open bite in adult nonextraction cases. Methods: Sixty-nine adult patients with anterior open bite were enrolled and classified into Angle's Class I, II, and III groups. Fifty patients presented with skeletal open bite (mandibular plane angle [MPA] ≥ 38°), whereas 19 presented with dental open bite. Fifteen cephalometric landmarks were identified before (T1) and after (T2) treatment. The magnitudes of planned and actual movements of the incisors and molars were calculated. Results: Positive overbite was achieved in 94% patients, with a mean final overbite of 1.1 ± 0.8 mm. The mean change in overbite was 3.3 ± 1.4 mm. With clear aligners alone, 0.36 ± 0.58 mm of maxillary molar intrusion was achieved. Compared with the Class I group, the Class II group showed greater maxillary molar intrusion and MPA reduction. The Class III group showed greater mandibular incisor extrusion with no significant vertical skeletal changes. Conclusions: Clear aligners can be effective in controlling the vertical dimension and correcting mild to moderate anterior open bite in adult nonextraction cases. The treatment mechanism for Class III patients significantly differed from that for Class I and Class II patients. Maxillary incisor extrusion in patients with dental open bite and MPA reduction with mandibular incisor extrusion in patients with skeletal open bite are the most significant contributing factors for open bite closure.
This study was designed to evaluate the relationship of mandibular condylar position to overbite depth and compare the findings from transcranial radiographs and those, from individualized corrected tomographs in determination of condylar position. The subjects consisted of 20 control subjects (male 8, female 12), and 10 open-bite patients (male 3, female 7) and 23 deep-bite patients (male 17, female 6). The mean age was 23.3 years for the control group, 21.5 years for open-bite group, and 23.2 years for deep-bite group. Transcranial radiographys and individualized corrected tomographys in centric occlusion were taken from right and left temporomandibular joints of each sueject. The results were as follows. 1. In the 20 normal subjects showing no symptoms of TM disorder, the incidence of condylar retrusion was $27.5\%$, middle condylar position $60.0\%$, and anterior displacement $12.5\%$. 2. There was significant correlation between the bite depth and observed condylar position. 3. Only $45.2\%$ of the findings from transcranial radiographs coincided with those from individualized corrected tomographs in determining condylar position.
