Inflammatory pseudotumor is characterized by the presence of a mass that may mimic malignancy and is composed of spindle cells mixed with variable amounts of extracellular collagen, lymphocytes, and plasma cells It is benign tumor commonly involves lung and orbit and rarely that occurs in the head and neck The cause and pathogenesis of inflammatory pseudotumor still remain controversial. Some bacterial infection or chronic irritation may have prompted some speculation that an initially reactive process can change into an overt neoplastic disease. It tends to affect children and young adults than old ages. We present a case of inflammatory pseudotumor involving the tongue base, in a 73-year-old patient with repeated oral bleeding that aggravated over a few days. Clinical examination showed angiomatous polyp like findings in oropharynx that easy bleeding tendency. CT images showed well defined soft tissue mass with focal enhancement. After surgery, tumor was finally diagnosed inflammatory pseudotumor by histopathologic examinations showed plasma cells and inflammatory cells in variable fibrotic tissues.
Carcinoma ex pleomorphic adenoma is an uncommon malignant salivary gland tumor that arises from a long-standing pleomorphic adenoma. The carcinomatous component of carcinoma ex pleomorphic adenoma can possess virtually any histologic subtype of salivary gland cancer. We experienced a case of a 61-year-old patient who presented with a right parotid mass that was initially palpated 20 years ago, with a sudden increase in size in the last few months. Radiological and cytological findings from fine needle aspiration biopsy could not exclude malignancy. Total parotidectomy and selective neck dissection were performed for treatment, and carcinoma ex pleomorphic adenoma with mixed carcinoma components of salivary duct carcinoma and myoepithelial carcinoma was diagnosed. After receiving postoperative radiation of 6000 cGy over 6 weeks, there has been no recurrence up to the 18-month follow-up. We report this rare case of carcinoma ex pleomorphic adenoma with mixed malignancy subtypes, accompanied by a review of literature.
The objectives of this study are to identify the actual educational contents of management for head nurse and to propose the educational subjects according to identity the needs of head, charge, and staff nurses. The subjects were investigated the actual Management Development Programs and educational needs of head nurses and prospective nurse manager(charge nurse, staff nurse with a lot of clinical experiences) in general hospitals. The tools were composed of two questionnaires: One was developed from the literature review for making items to measure actual situation. The other was revised Katz's model for measurement of educational needs. The first respondents of actual situation were 27 general hospitals with over 400 beds in Seoul and the second respondents were 89 head nurses, 67 charge nurses and 136 nurses at 3 hospitals by convenient sampling out of 27 general hospitals. Data were collected by telephone interview, mail questionnaire and visiting from 7th of October through 30th of November in 1997. In data analysis, general characteristics of the respondents and actual status of Management Development Programs were analyzed by frequency and percentage. Educational needs according to general characteristics were analyzed by ANOVA The results were as follows: 1. Actual situation of Management Development Program 1) Seven hospitals(26%) had Management Development Program for prospective managers and 14 hospitals (52%) for head nurses. 2) Education Department existed in 14 hospitals (52%). 3) One hospital(4%) had top level managers took part in the Management Development. 4) Two hospitals selected head nurse, who had finished courses of Management Development. Eight hospitals(30%) assessed educational needs. The assessment tools consisted of making a question via questionnaire(75%), determining at department meeting(12%) and interview(13%). 5) Educational programs had 3 types: 10 lecture type, 7 discussion type and 4 role play type programs. 6) One hospital evaluated the change of learner's attitude. 7) Four hospitals scored educational point, but that was measured only by attending. 8) Actual Management Development Programs were as follows. parenthesis indicates the number of hospitals. The results of needs on Management Development subject 1) The educational needs of all respondents on 3 skill domains showed positive agreement to strongly positive agreement. 2) High priority(more than 4.5) items were 12 of 24 Human skill items(50%), 1 of 6 Technical skill items(16%), and 2 of 13 Conceptual skill items (15%). 3) Out of high priority items, 8 items were instituted. 4) All respondents showed high needs on 3 skill domains regardless of 3 positions (head nurse, charge nurse, and nurse). Educational needs of Human skill domain, according to position were 108. S, 108.7, 106.8 (mean score = 72) , needs of Technical skill domain were 26.5, 26.6, 26.I(mean score=18), and needs of Conceptual skill domains were 56.9,56.7, 55.1(mean score=39). 5) Needs of 3 skill domains according to clinical career showed significant difference. Out of respondents, nurses with career of over 16years showed lowest degree of needs in Human skill domains(F=4.47, P=.004) and Conceptual skill domain(F=2.93, P=.034). 6) Educational needs according to educational background were not significant difference. But out of respondents, nurses educated at 3-year junior college relatively showed lowest needs in all of the 3 skill domains. With the above-mentioned findings, further study is necessary for generalization of this study at hospitals with different bed size and location. Also it is needed to study about management skill of nurse and charge nurse, and effective educational method.
PURPOSE: The purpose of this study was to apply dynamic neuromuscular stabilization (DNS) to subjects with forward head posture (FHP) and to compare its effects on respiratory function as against the conventional neck stabilization exercise and neck stretching and extensor strengthening exercises. METHODS: The whole-body posture measurement system was used to measure the degree of FHP, and a spirometer and a respiratory gas analyzer were used to measure the respiratory function. After the intervention was completed, the changes over time were analyzed in the DNS group, the neck stabilization exercise group, and the neck stretching and extensor strengthening exercise group. The inter-group difference in the changes was also analyzed. A repeated ANOVA was performed to compare the respiratory function according to the period between the three groups, and the least significant difference (LSD) method was used for the post hoc test. RESULTS: After the 6-week exercise period, respiratory functions, such as forced vital capacity (FVC), forced expiratory volume for 1 second (FEV1), forced expiratory volume for 1 sec/forced vital capacity (FEV1/FVC), maximum oxygen intake (VO2max), and the volume of expired gas (VE), significantly improved according to the period (p < .05), but no inter-group differences were found. CONCLUSION: DNS is an effective training method, and can be applied along with neck stabilization exercise and neck stretching and extensor strengthening exercises, which are widely used in clinical practice, to people with FHP who cannot directly perform neck exercises to improve their respiratory function.

Over the course of the last 4-5 years, India's northeastern region have widely used Autoclaved Aerated Concrete (AAC) blocks to construct load-bearing masonry structures. The aim of this investigation is to examine the shear characteristics of AAC masonry triplet assemblage strengthened by using two techniques, i.e., the bead joint (BJ) and the bed-head joint (BHJ) technique. Three unique variations of wire mesh were involved in the strengthening method. Furthermore, three strengthening configurations were used to strengthen each of the three wire mesh variations and the two-strengthening method, i.e. (-), L and (Z) configuration. The unreinforced and reinforced triplet masonry wallets were tested under direct shear test. From the results obtained, the 'BJ'triplet masonry wallets observed an enhanced in shear strength of about 2.23% to 23.33 % whereas the 'BHJ' triplet masonry wallets observed an enhanced in shear strength of about 22.92% to 50.69%. The "BHJ" strengthening method effectively enhance the shear strength of the triplet masonry wallets compared to the "BJ" and the "UR" wallets with an increase in capacity as the wire mesh strength increases. Furthermore, in terms of the strengthening configuration, the (Z) configuration performs better, followed by the (L) and (-) configuration demonstrating the strengthening configuration effectiveness.
A seawater pumps in the nuclear power plant is responsible for providing cooling water to other components all the time. Because of the depreciation, the seawater pump with current impeller consumes too much power for maintaining the total head. Therefore the objective of this study is to reduce power with maintaining certain the total head by cutting the current impeller. By using a commercial CFD code, FLUENT, the overall performance of seawater pump with current and modified impeller was simulated. Also Affinity law was applied at pumps with various impeller diameter and evaluated the validity of the affinity law. The numerical results show that the pump efficiency is quite irrelevant to the diameters of the impellers and the pump efficiency becomes worse over the designed flow rate. And affinity law result and numerical one show good agreements at small change of impeller diameter. One of the impeller diameters was decided to modify and was applied to the nuclear power plant with the numerical study above.
Flash floods are one of the types of natural hazards which has severe consequences. Flash floods cause high mortality, about 5,000 deaths a year worldwide. Flash floods usually occur in mountainous areas in conditions where the soil is highly saturated and also when heavy rainfall happens in a short period of time. The magnitude of a flash flood depends on several natural and human factors, including: rainfall duration and intensity, antecedent soil moisture conditions, land cover, soil type, watershed characteristics, land use. Among these rainfall intensity and antecedent soil moisture, play the most important roles, respectively. Flash Flood Guidance is the amount of rainfall of a given duration over a small stream basin needed to create minor flooding (bank-full) conditions at the outlet of the stream basin. In this study, the Sejong University Rainfall-Runoff model (SURR model) was used to calculate soil moisture along with FFG in order to identify flash flood events for the Geum basin. The division of Geum river basin led to 177 head-water catchments, with an average of 38 km2. the soil moisture of head-water catchments is considered the same as sub-basin. The study has measured the threshold of flash flood generation by GIUH method. Finally, the flash flood events were used for verification of FFG. The results of the validation of seven past independent events of flash flood events are very satisfying.
We report a case of pseudoaneurysm of the parietal division of the superficial temporal artery (STA) secondary to iatrogenic head injury due to Gardner traction. A 54-year-old man presented with a pulsatile, cystic, and painless mass in the right anterior temporal region which developed three weeks after head fixation via Gardner traction. At the time of discovery, the mass was 10 mm in diameter, compressible and disappeared after manual compression of the proximal STA. A bruit was audible over the mass, which was thought to be a pseudoaneurysm. A computed tomography angiogram (CTA) showed a pseudoaneurysm of the parietal division of the right ST A. The tip of the pseudoaneurysm was thrombosed and was both red and tender. The pseudoaneurysm was thought to be filled with infected thrombus, and the mass was resected with ligation of the proximal and distal ends of the STA. A pseudoaneurysm of the STA should be suspected when there is a history of possible vessel injury, such as a history of head-pin fixation, and when a patient presents with a pulsatile, cystic mass near the temple. Pseudoaneurysms can be successfully treated by excision.
실험실 수조에서 조석 및 조류를 발생시키기 위해서는 수조내에 흐름의 분출부와 흡입부를 적절히 설치해야 한다. 분기관은 관수로 측면에 설치된 다수의 구멍을 통하여 관내ㆍ외 압력차에 의해 물을 분출 또는 흡입하는 장치이다. 수조 폭에 따라 균등한 단위폭당 유량을 발생시키기 위해서는 각 구멍의 크기를 적절히 결정해야 하며 이를 위해서는 공급류량, 공급수두, 수두손실, 구멍에서의 분출 및 흡입류량 등의 수리특성을 파악하여야 한다. 본 연구에서는 에너지 방정식과 연속방정식으로부터 비롯되는 비선형 연립방정식을 구성하고 Newton 해법을 적용하여 분출 및 흡입분기관의 수리특성을 파악하였다. 아울러, 자동제어 밸브가 부착된 분기관이 수조폭에 걸쳐 일정한 유량을 유지해야하는 조석수조에 효과적으로 사용될 수 있음을 확인하였다.
Purpose : To observe changes in side effects and fatigue of the cancer patients receiving radiation therapy on the head/neck and the chest over a period from the start to the end of the therapy, and to analyze correlation between side effects and fatigue. Method : Twenty seven patients receiving radiation therapy 5 days per week for longer than 6 weeks participated in the present study. Fatigue was measured for when healthy, before-, and after-therapy. Side effects was surveyed by structured questionnaire on the last day of each therapy. Result : The results of this study were as follows. 1. Major side effects of the head/neck patients were skin irritation, change of taste, sore throat and xerostomia, while the chest patients experienced fatigue, skin irritation, anorexia, difficulty swallowing and cough increased with therapy. 2. Although fatigue was significantly changed for when healthy and before-therapy (F=60.25, p <.05) and also for before- and after-therapy, no statistical significance was demonstrated in fatigue of both the chest head/neck patients (p> .05). 3. Fatigue and side effects showed high correlation form the 4-th week with after the therapy Conclusion : The present results could be of great use to develop systematic intervention technique, leading to practical help for patients, since side effects and fatigue change to a large degree depending on the disease and the timing and technique of the therapy.
