2014년부터 2017년까지 전북대학교 치과병원에서 외래 전신마취 하에 치과치료를 받은 증례에 관한 연구 (A SURVEY OF DENTAL TREATMENT UNDER OUTPATIENT GENERAL ANESTHESIA AT CHONBUK NATIONAL UNIVERSITY DENTAL HOSPITAL BETWEEN 2014~2017)

  • There are increasing number of patients having dental treatment under outpatient general anesthesia. There were researches that studied patients treated under outpatient general anesthesia in Chonbuk National University Dental hospital between 2005~2013. However, there has been an absence of research since 2013. Therefore, the aim of this research is to study cases regarding outpatient general anesthesia visiting Chonbuk National University hospital between 2014~2017 and analyze the trends of these patients. The medical records of 409 cases from January, 2014 to December, 2017 were reviewed. From the patients' charts, data on sex, residence, visiting dates, disability status, and types and number of treatments was collected and investigated. The number of cases has increased from 86 in 2014 to 140 in 2017. Among 409 cases, 275 cases(67.2%) had disabilities, with mental disabilities consisting most of the disabilities. 192 cases were aged under 20 years old mostly with no disabilities, 142 cases were 20~39 years old mostly with mental disabilities, and 75 cases were 40 years old or more mostly with physical disabilities. The conservative treatment was performed most frequently in both disabled and non-disabled patient groups. There was an increasing trend in the number of patients treated under outpatient general anesthesia, and more than half were disabled patients. In order to meet this demand, Chonbuk National University Dental Hospital increased the number of general anesthesia pracrice. To follow this trends, the implementation of safe outpatient general anesthesia system and cooperation among teams is needed to improve the quality of treatments.

  • Background: Sedation or outpatient general anesthesia is a necessary method for a successful dental treatment for children and the disabled who have a difficulty of cooperation. The aim of this study was to assess the patients who had dental treatment under sedation or outpatient general anesthesia at department of pediatric dentistry, Chonnam national university dental hospital and Gwangju dental clinic for the disabled. Methods: 875 patients who had sedation from January 2009 to September 2013 and 88 patients who had outpatient general anesthesia after May 2011 were assessed for this study. Patient's distribution and treatment patterns were examined. Results: The number of patients has been increasing in both sedation and outpatient general anesthesia. In case of sedation, the proportion of male patients was higher and 4-6 years age group was the highest. Most patients have resided in Gwangju buk-gu and the percentage of restorative treatment was the highest. In case of outpatient general anesthesia, the proportion of male patients was higher than that of sedation. Two groups, 10-19 and 20-29 years age, consisted most of patient. Most patients have resided in Gwangju buk-gu and Jeollanam-do, and the percentage of periodontic and preventive treatment was the highest. Conclusions: Sedation or outpatient general anesthesia for dental treatment has been increasing for children and the disabled who have a difficulty of cooperation. Therefore, it is important to improve treatment environment under sedation and general anesthesia through continuous research and studies.

A retrospective analysis of outpatient anesthesia management for dental treatment of patients with severe Alzheimer's disease

  • Background: The number of patients with Alzheimer's disease is growing worldwide, and the proportion of patients requiring dental treatment under general anesthesia increases with increasing severity of the disease. However, outpatient anesthesia management for these patients involves great risks, as most patients with Alzheimer's disease are old and may show reduced cardiopulmonary functions and have cognitive disorders. Methods: This study retrospectively investigated 43 patients with Alzheimer's disease who received outpatient anesthesia for dental treatment between 2012-2017. Pre-anesthesia patient evaluation, dental treatment details, anesthetics dose, blood pressure, duration and procedure of anesthesia, and post-recovery management were analyzed and compared between patients who underwent general anesthesia or intravenous sedation. Results: Mean age of patients was about 70 years; mean duration of Alzheimer's disease since diagnosis was 6.3 years. Severity was assessed using the global deterioration scale; 62.8% of patients were in level ${\geq}6$. Mean duration of anesthesia was 178 minutes for general anesthesia and 85 minutes for intravenous sedation. Mean recovery time was 65 minutes. Eleven patients underwent intravenous sedation using propofol, and 22/32 cases involved total intravenous anesthesia using propofol and remifentanil. Anesthesia was maintained with desflurane for other patients. While maintaining anesthesia, inotropic and atropine were used for eight and four patients, respectively. No patient developed postoperative delirium. All patients were discharged without complications. Conclusion: With appropriate anesthetic management, outpatient anesthesia was successfully performed without complications for dental treatment for patients with severe Alzheimer's disease.

외래소수술시 적용하는 치과국소마취 술식들 (Dental Local Anesthetic Techniques for Minor Operations at Outpatient Clinic)

  • The most widely used method of pain control in dental practice is to block the pathway of painful impulses by local anesthesia. For a maximum effect of pain control at outpatient minor operation, regional anesthesia including nerve block must be performed. This issue includes various dental local anesthetic techniques on mandible, maxilla and considerations for selection of dental local anesthetics according to patient's age, duration of operation. Additionally, current conscious sedation technique combined with local anesthesia will be introduced for control of patient's anxiousness. Based on patient's general condition, severity of operation, local anesthesia sometimes must be changed into general anesthesia to avoid failure of dental practice. The exact choice of anesthetic techniques for minor operations at dental outpatient clinic must not be underestimated for complete success of pain control.

  • 90 handicapped patients with dental problem treated under outpatient general anesthesia at one day operation room of kyungpook National University Hospital during 2000-2005 were analyzed. Following are the conclusions. 1. For those 90patients treated under general anesthesia, 62(68.9%)patients had autism or mental retardation. 2. The most difficult work for the preparation of general anesthesia was preoperative examination(53.3%). 3. The parent's satisfaction level after treatment under general anesthesia was high(75.6%). 4. 36(40%) of the patients responded to periodic recall check till now.

  • The purpose of this study is to survey the status of oral care of adult patients with disabilities and satisfaction degree of outpatient general anesthesia for dental treatment. 30 adult patients who underwent general anesthesia for dental treatment at clinic for the disabled in Seoul National University Dental Hospital enrolled for telephone survey. Many patients (83.3%) had mental problems to do social communication. And someone of family members (mainly mother) had cared for the patient but there was only one patient who got regular oral care. The satisfaction level of outpatient general anesthesia and nursing care for dental treatment was very high(100%).

  • Background: Dental treatment under outpatient general anesthesia is an option for disabled person, children and dentally disabled person who have the difficulty of cooperating voluntary with dental care. The aim of this study was to access the patients who had dental management under general anesthesia at Chungnam dental clinic for the disabled. Methods: This survey is consisted of 114 severely disabled and dentally disabled person who undergone dental treatment under general anesthesia from January to June 2011. Data were collected by individual dental and anesthesia records. Results: Of the 114 patients studies, the 45 patients was the dentally disabled and the 69 patients was the severely disabled. Rate of dental treatment among male patients was higher than among female patients. The dentally disabled was an similar age distribution. In the severly disabled, above 13-year-old was higher than below 12-year-old. The most popular induction method of general anesthesia was sevoflurane inhalation after induced sedative effect of nitrous oxide inhalation. The severly disabled who had definitely negative behavior was applied inhalation of high concentration sevoflurane. Conclusions: Chungnam Dental clinic for the disabled provide dental management for the disabled and the dentally disabled.

Telephone follow-up care for disabled patients discharged after receiving dental treatment under outpatient general anesthesia

  • Background: Patients were subjected to post-discharge follow-up (by telephone) in order to investigate the potential complications of outpatient general anesthesia or deep sedation that could develop in disabled dental patients discharged from the hospital. The ultimate aim of this study was to establish an appropriate response measure for such complications. Methods: The caregivers of 79 disabled patients who underwent dental procedures under general anesthesia at our outpatient clinic were interviewed over telephone. Necessary care instructions were provided during the phone calls when required. The patient satisfaction level regarding the telephonic follow-up care was surveyed by additional telephone calls. Results: Most of the patients did not suffer any serious complications; however, some reported fever and bleeding. The data obtained in this study can be utilized towards the development of caregiver education pertaining to the ambulatory general anesthesia of dental patients with disabilities. Conclusions: Additionally, we hope that the findings of this study will help minimize the effects of complications experienced by disabled dental patients undergoing ambulatory general anesthesia, as well as increase the overall patient satisfaction level.

Nonsustained ventricular tachycardia during outpatient anesthesia: a case report

  • During the perioperative period, anesthesiologists frequently observe cardiac tachyarrhythmia. Ventricular tachycardia is very rare in non-cardiac surgeries. However, it can be fatal when it occurs. Therefore, anesthesiologists should be watchful so as to not to miss ventricular tachycardia and take the appropriate steps to manage it promptly. We present a case, with a review of related literature, in which a non-sustained ventricular tachycardia was observed in a patient who visited the hospital for dental treatment.

  • 수술전, 후의 준비 및 처치를 적절히 할 수만 있다면 수술을 받은 후 귀가하는 외래수술은 특히 소아환자와 보호자로부터 환영받을 수 있는 안전하고 경제적인 방법이다. 대상환아의 선택시에는 수술의 종류와 소요시간 그리고 환아의 상태 등을 종합적으로 판단하여 수술후 합병증을 야기하지 않고, 빠른 회복이 가능한 대상을 선별토록 해야 하며 특히 소아의 연령에 따른 인지의 정도 및 심리적 다양성을 충분히 감안하여 마취유도시 까지는 정신외상을 주지 않도록 환아와의 rapport형성에 최대한의 배려를 해야 하며 수술후의 통증 및 회복시 야기될수 있는 부작용에 대하여는 가능한 한 예방이 바람직하나 초래된 경우에는 신속한 처치를 해주므로써 환아의 고통을 경감시켜 주도록 해야한다. 최근에 등장한 여러가지 이상적인 마취약제들과 아울러 합리적인 투여방법의 발달로 신속하고 부드러운 마취유도 및 수술이 끝난 후의 빠른 회복 그리고 부작용의 최소화가 가능하게 되었다. 그러나 국내의 의료현실상 아직은 이상적인 약제들을 의사의 판단에 따라 자유롭게 사용할 수 있는 여건이 되지 못하며 소아의 통증치료에 대하며 의료전이 소극적이며 수술후 관리에 대한 보호자들의 이해수준이 낮으므로 수술 당일 바로 환아를 병원에서 퇴원시키는 것은 상당한 위험성을 내포하고 있기 때문에 소아에서의 외래마취는 성인의 경우와는 달리 아직은 보편화되지 못하고 있는 실정이다.
