Effects of Progressive Resistance Training on Body Composition, Physical Fitness and Quality of Life of Patients on Hemodialysis

  • Purpose: To investigate the effect of progressive resistance training (PRT) on body composition, physical fitness, quality of life, lipid and nutritional profile of patients on hemodialysis (HD). Methods: A non equivalent comparison group pretest and posttest design study was used with 40 participants who were randomly assigned to the exercise group (20 participants) and the comparison group (20 participants). The exercise group received PRT for 30 minutes per session, 3 sessions a week, for 12 weeks, while the comparison group received usual care. The PRT consisted of upper and lower body exercises using elastic bands and sandbags. Outcome measures evaluated were: body composition, physical fitness, quality of life, and lipid profile. Results: Skeletal muscle mass, grip, leg muscle strength, and quality of life all improved significantly in the exercise group. Body fat rate, total cholesterol and triglyceride rate decreased significantly in the exercise group. Conclusion: These results suggest that PRT improves body composition, physical fitness, quality of life, and lipid profile of patients on HD. PRT using elastic bands and sandbags can be utilized as part of a regular care plan for these patients.

Study of Analyzing Outcome of Building and Introducing System for Preserving Full-Text of e-Journal

  • Today, most researchers conduct their studies through the full-text of e-journals. Therefore, an important base for domestic development of science and technology is to obtain the full-text of quality e-journals by overseas researchers and to provide it to Korea's researchers. This study aims to build a system based on the National Archiving Center for the full-text of e-journals and to make a service system for providing them to the public by acquiring the full-text of quality overseas e-journals. To do this, an analysis was made of the outcome of introducing such a system for full-text of e-journals in comparison with the investment. As a result, 112 more institutions, that is, from 47 institutions to 159 institutions, have introduced the system as of 2012, and the number of downloaded full-texts increased at least 2.17 times.

  • Objective: The self-decisions of the client regarding the meaningful work as a therapeutic approach of client-orientation. The Cognitive Orientation to daily Occupational Performance (CO-OP) is an occupation-oriented problem-solving approach. The purpose of this study was to describe the goals and intervention protocols of CO-OP in those affected by stroke. Design: A systematic review. Methods: Using EBSCOhost, PubMed, and ProQuest databases, we searched studies published in the past decade that utilized the CO-OP intervention. An initial search revealed 71,171 potential articles. After applying our search criteria to screen the titles, abstracts, and full-text, we included 7 articles that met our inclusion and exclusion criteria. In this study, we used the patient, intervention(s), comparison, outcome method to analyze the 7 selected studies. We analyzed the frequency of goals and intervention protocols. Results: Seven articles met our selection criteria; these studies included participants with an almost normal cognitive function from inpatient and outpatient rehabilitation facilities. CO-OP was used for 237 goals; the most used goal was the instrumental activities of daily living. The training procedure used 3 types of self-selecting goals in the activities. One of the goals was not trained, but was only evaluated to determine the generation effect. The most common outcome measurements included the Canadian Occupational Performance Measure and the Performance Quality Rating Scale. Conclusions: This research provided information about the effectiveness of CO-OP and selecting the correct evaluation tool to assess the efficiency of the intervention. This study suggests that treatment with CO-OP in occupational therapy is effective and that it outlines common protocols.

The Comparison of Outcome between Thromboaspiration and Aggressive Mechanical Clot Disruption in Treating Hyperacute Stroke Patients

  • Objective : Stroke is the third leading cause of death in the Republic of Korea. Time is the most important factor in hyperacute stroke. Yet, there had been no protocol for mechanical thrombolysis. We have treated patients with hyperacute stroke by mechanical thrombolysis for 3 years. In current study, we analyzed the outcome of mechanical thrombolysis. Methods : From March 2008 to February 2011, 36 patients were treated with mechanical thrombolysis. Initially we treated the patients by aggressive mechanical clot disruption (AMCD) who were admitted within 6 hours after the symptom onset. If revascularization was not achieved, balloon angioplasty was performed, followed by stenting or temporary endovascular bypass was performed. The result in 15 cases was not so successful. Since then, we started using the thromboaspiration method as the first line treatment of the mechanical thrombolysis. Results : After using the thromboaspiration, we had better results in recanalization rate, modified Rankin Score (mRS) and reperfusion injury compared to AMCD. The recanalization rate was 80.85%, mRS is 2.85, and there was only 0.09% hemorrhagic formation. Conclusion : Even though thromboaspiration is not statistically significant due to the limited numbers of patients enrolled in this study, we think it is a good way in mechanical thronbolysis for hyperacute stroke.

야뇨(夜尿)의 침구(鍼灸) 치료(治療) 연구(硏究) - 최근 중의 잡지를 증심으로 - (Acupuncture for childhood nocturnal enuresis in Traditional Chinese Medical Journals)

  • Objectives : This study aimed to investigate acupuncture trend of childhood nocturnal enuresis in current Traditional Chinese Medical Journals. Methods : During the period from January 2000 to August 2004, we searched all Traditional Chinese Medical Journals in oriental medical library in dongguk university. We selected 28 literatures on acupuncture treatment for nocturnal enuresis in children Results : 28 studies are included and classified by diagnosis criteria and outcome criteria. And reclassified into two large groups, case report and comparison study. Totally 2324 cases, complete cures are 1701, significant improvements 2239, complete cure rate 73.2%, total improvement rate 97.3% 33 acupuncture points are used and main meridians are bladder, conception vessel and governor vessel. Conclusion : There remains a debate about diagnosis and outcome criteria. But Acupuncture is very effective and safe in the treatment of enuretic children.

특발성 말초성 안면신경마비(Bell's Palsy)에 대한 한방치료와 양방병용치료의 비교 고찰 (Comparative Clinical Study between Oriental Medicine and Oriental-Western Medicine Treatment on Bell's palsy)

  • Objective : Lately variable oriental-western medicine treatment have been used for Bell's palsy. The purpose of this study is comparison of clinical treatment outcome between oriental medicine group and oriental-western medicine group. Materials and Methods : From 08-01-2000 to 07-30-2001, 30 Bell's palsy out-patients who visited within 5days after onset and treated more than 3weeks at the Department of Acupuncture & Moxibustion, Bundang CHA Oriental Medicine Hospital were selected for two groups. One group(A group) was treated by oriental medicine therapy(Acupunture, Herb, Physical therapy), the other group(B group) was treated by oriental-western medicine therapy(Acupunture, Herb, Physical therapy, Administration of prednison). Two group was composed of 15 patients respectively and measured by HBGS(House-Brackmann Grading System) and DEFS(Detailed Evaluation of Facial Symmetry of Pillsbury and Fisch) at first visit and 3weeks after. Result and Conclusion : A group was marked more higher than B group in treatment outcome. But we discovered that it is not significant differences between two groups.

Effects of periodic air-exposure and nutrients on the competition of Ascophyllum nodosum and Fucus vesiculosus germlings

  • To examine the relationship between competition and environmental factors, the germlings of Ascophyllum nodosum (L) Le Jolis and Fucus vesiculosus L. were cultured in monocultures and mixtures of the two species under two different exposure and nutrient levels. Both intra- and inter-specific competition were examined in comparison of the mortality and growth of germlings in monocultures and mixtures of the two species. The mortality of germlings increased with increasing density and emergence periods both in the monoculture and mixtures of the two species, and the mortality of Ascophyllum was significantly higher than that of Fucus both in submerged and emerged treatments. The growth of germlings of both species reduced with increasing density but F. vesiculosus always grew faster than Ascophyllum. The values of log output ratio were more than 0.1, indicating that Fucus 'won' in the competitive battles with Ascophyllum under two nutrient- and air-exposure levels. Log output ratio was greater in high than in low nutrients, indicating that the growth of Fucus is more enhanced than that of Ascophyllum in high nutrients. In the present study, the outcome of inter specific competition between germlings of Fucus vesiculosus and Ascophyllum nodosum was slightly altered by duration of emergence and nutrient concentration, but not to such an extent as to change the outcome.

부인과 간호단위 입원 환자에 적용되는 간호진단-간호결과-간호중재의 연계 확인 (Identification of Nursing Diagnosis-Outcome-Intervention Linkages for Inpatients in Gynecology Department Nursing Units)

  • Purpose: The aim of this study was to identify the nursing diagnosis-outcome-intervention (NANDA-NOC-NIC) linkages for gynecology inpatients shown in their electronic nursing records. Methods: This retrospective and descriptive research was conducted in two steps and based on the 287 electronic nursing records for 253 patients. First, nursing diagnoses, outcomes and interventions were collected. To identify major nursing diagnoses, a comparison was done with the top 10 nursing diagnoses from this research and with previous research selected using a content validity index developed by a team of professionals. Second, nursing outcomes and interventions that were associated with major nursing diagnoses were identified. Results: Nineteen nursing diagnoses, 12 nursing outcomes, and 40 nursing interventions were collected. The top 5 major nursing diagnoses were identified and 7 nursing outcomes and 18 nursing interventions associated with these diagnoses were checked. Conclusion: The identified NANDA-NOC-NIC linkages can contribute to improving nursing practice and will help in the establishment of standardized nursing care.

APACHE III 시스템을 이용한 병원간 중환자실 치료결과 비교분석 (Interhospital Comparison of Outcome from Intensive Care Unit with APACH III Scoring System)

  • The objective of this study was to evaluate outcome for the patients of the intensive care unit, using APACHE III prognostic system. We prospectively collected the information of 429 patients in intensive care units at 2 tertiary care hospitals and 4 secondary care hospitals in PUSAN who had been admitted from December 1, 1993 to February 28, 1994. The results were as follows. 1. APACHE III scores were various from 0 to 173. But the distribution of the scores were similar between tertiary care hospitals and secondary care hospitals. 2. The mortality rate significantly increased as APACHE III score rised (p<0.001). Within the interval of same score, generally, the mortality of operative patients was higher in secondary care hospitals but in the case of nonoperative patients higher in tertiary care hospitals. 3. When the tertiary care hospitals compared with secondary for ratio of the predicted mortality rate to the actual mortality rate, there was little difference. 4. When we compared the 6 hospitals, one hospital had significantly better results and another hospital was significantly inferior (p<0.05).

가슴압박만 하는 심폐소생술과 30 : 2 표준 심폐소생술 방법에 의한 가슴압박 질의 비교 (Comparison of chest compression quality between compression-only CPR and 30 : 2 conventional CPR)

  • Purpose: This study aimed to compare the chest compression quality between compression-only cardiopulmonary resuscitation (CPR) and conventional CPR. Methods: This study involved 123 participants randomly assigned to either the chest compression-only CPR training and conventional CPR training. After training for 120 min, the participants performed CPR for 4 min and the CPR quality was evaluated. The primary outcome was evaluated as the mean compression depth, and the secondary outcome was calculated as the proportion of chest compressions with an appropriate depth among the total chest compressions. Results: The mean compression depth was more deeper in conventional CPR than in compression-only CPR (57mm vs. 53mm, p <.001). The proportion of appropriate chest compression depth was also more higher in conventional CPR (98.8% vs. 68.6%, p <.001). As compared with every minute over time for a total of 4 min, the mean compression depth and the proportion of appropriate chest compression depth were deeper and higher after 1 min in conventional CPR than in compression-only CPR. Conclusion: The results suggested that conventional CPR is a better method than compression-only CPR in terms of chest compression quality.
