본인부담상한제 적용 요양병원 환자의 의료이용가수요 예측요인 분석 (An Aanalysis of Predictive Factors of Medical Service Overuse for Inpatients Applied Out-of-Pocket Maximum in Long-Term Care Hospitals in South Korea)

  • Background: The out-of-pocket maximum is one of the distinctive healthcare systems which sets a ceiling on co-payment in order to reduce the burden of households from the unpredictable medical expenditure. However, this leads to an increase in the demand for healthcare services especially in long-term care hospitals (LTCHs) in Korea. Methods: This study analyzed the influence factor of medical service overuse of 165,592 inpatients in LTCHs which out-of-pocket maximum is applied, by utilizing data from the National Health Insurance Service (2016). Based on Anderson Model, the medical service overuse, as a dependent variable, was defined as long-stay admission more than 180 days at the LTCHs. Independent variable was comprised of predisposing factors (gender, age), enabling factors (income level, types of out-of-pocket maximum) and need factors (illness level, patient use of tertiary hospital). Results: The most powerful factor of medical service overuse in LTCHs was availability of pre-payment for the out-of-maximum (odds ratio [OR], 191.66; p<0.001). This tendency was found in high income level status (p<0.001). Furthermore, mild inpatients (OR, 1.50; p<0.001) which had no experience with the tertiary hospitals (OR, 2.06; p<0.001) were more relevant to the medical service overuse in LTCHs, compared to the severe inpatients. Conclusion: It is suggested that a separate standard of out-of pocket maximum with regards to LTCHs is required to secure the beneficial functions of long-term hospitals and prevent unnecessary financial leakage to achieve sustainable and financially sound National Health Insurance.

요양병원 장기입원에 대한 본인부담상한제 개편 영향 분석 (Impact of Adjusted Out-of-Pocket Maximum Rules of Long-stay Admissions in Long-Term Care Hospitals)

  • Purpose: This study aimed to determine the effect of adjusted out-of-pocket maximum rules in the 'differential co-payment ceiling', which means having a higher burden of co-payment, that expanded to the entire ceiling level in long-stay admission patients in long-term care hospitals(LTCH). Methodology: We used health insurance claim data between January 1, 2022, and December 31, 2022 received from the National Health Insurance Service. The study populations were inpatients in long-term care hospitals more than 1 days during the study period. We performed the difference in characteristics of the LTCH patient of the differential and general ceiling by the chi-square test. We estimated the change of the population, cost, and co-payments per person under the assumption of restructuring. Finding: Based on adjusted out-of-pocket maximum rules in 2023, it was expected that the number of benefits decreases at the high-income level while increasing at the low-income level. The burden of health expenditure after reimbursement of co-payment ceiling, is expected to increase by 65.1% in the highest medical necessity, whereas the low medical necessity would decreases compared to 2022. Practical Implications: The results demonstrate that the current out-of-pocket maximum rules do not reflect the needs of medical necessity. This study suggested the need to reflect the medical necessity in LTCH on the out-of-pocket maximum rules in the future.

본인 부담상한제와 민영 실손의료보험의 상호작용 (Interaction between Out-of-Pocket Maximum and Indemnity Health Insurance)

  • 본 연구는 건강보험의 본인부담상한제와 민영실손의료보험의 관계에서 야기되는 쟁점을 진단하고, 제도 개선을 위한 정책 과제를 제안하는 것을 목적으로 한다. 문헌연구를 통해 본인부담상한제 초과 환급금 미지급으로 인한 소비자 피해 실태와 건강보험 보장성 강화에 따른 실손보험의 역할 변화를 분석하였다. 연구 결과, 일방적 약관 해석과 불완전판매 관행이 소비자 권익을 침해하고 있으며, 실손보험의 보장 축소에도 불구하고 보험료 인하로 이어지지 않는 문제점이 확인되었다. 이에 상품 구조 합리화, 위험률 산출 투명화, 소비자 정보 제공 강화 등의 제도 개선과 함께, 건강보험과 민영보험의 합리적 역할분담을 위한 사회적 합의 도출이 시급함을 강조하였다. 본 연구는 의료보장체계의 발전적 재편을 위한 정책적 시사점을 제공한다는 점에서 의의가 있다.

한국의 소득수준 간 의료이용 차이의 계량적 분석: 2015 (Econometric Analysis of the Difference in Medical Use among Income Groups in Korea: 2015)

  • Background: The purpose of this study is to estimate empirically whether there is a difference in medical use among income groups, and if so, how much. This study applies econometric model to the most recent year of Korean Medical Panel, 2015. The model consists of outpatient service and inpatient service models. Methods: The probit model is applied to the model which indicate whether or not the medical care has been used. Two step estimation method using maximum likelihood estimation is applied to the models of outpatient visits, hospital days, and outpatient and inpatient out-of-pocket cost models, with disconnected selection problems. Results: The results show that there was the inequality favorable to the low income group in medical care use. However, after controlling basic medical needs, there were no inequities among income groups in the outpatient visit model and the model of probability of inpatient service use. However, there were inequities favorable to the upper income groups in the models of probability of outpatient service use and outpatient out-of-pocket cost and the models of the number of length of stay and inpatient out-of-pocket cost. In particular, it shows clearly how the difference in outpatient service and inpatient service utilizations by income groups when basic medical needs are controlled. Conclusion: This means that the income contributes significantly to the degree of inequality in outpatient and inpatient care services. Therefore, the existence of medical care use difference under the same medical needs among income groups is a problem in terms of equity of medical care use, so great efforts should be made to establish policies to improve equity among income groups.

  • An experiment has been performed using a facility, which simulates the safety depressurization system (SDS) and in-containment refueling water storage tank (IRWST) of APR1400, an advanced PWR being developed in Korea, to investigate the dynamic load resulting from the blowdown of steam from a steam generator through a sparser. The influence of the key parameters, such as air mass, steam pressure, submergence, valve opening time, and pool temperature, on frequency and peak toads was investigated. The blowdown phenomenon was analyzed to find out the real cause of the initiation of bubble oscillation and discrepancy in frequencies between the experiment and calculation by conventional equation for bubble oscillation. The cause of significant damping was discussed and is presumed to be the highly tortuous flow path around bubble. The Rayleigh-Plesset equation, which is modified by introducing method of image, reasonably reproduces the bubble oscillation in a confined tank. Right after the completion of air discharge the steam discharge immediately follows and it condenses abruptly to provide low-pressure pocket. It may contribute to the negative maximum being greater than positive maximum. The subsequently discharging steam does not play as at the driving force anymore.

크레메진의 만성신부전증 환자 투석도입 지연효과에 대한 최대지불의사액(willingness-to-pay) 추청 (Estimating willingness-to-pay for Kremezin in delaying the initiation of dialysis treatments among patients with chronic renal failure)

  • To assess the economic value of pharmaceutical therapy with Kremezin, we investigated the maximum amount of willingness-to-pay (WTP) of patients with chronic renal failure (CRF) for a hypothetical effect of Kremezin in delaying the initiation of dialysis treatments. A face-to-face survey was carried out in a sample of 141 CRF patients from 2 dialysis centers, composed of 82 hemodialysis patients, 38 peritoneal dialysis patients, and 21 non-dialysis CRF patients. Using a bidding game method with a starting point of 320,000 Won, which is the average monthly out-of-pocket payment for dialysis treatment, we asked the study subjects how much they would pay per month to receive Kremezin therapy. The mean out-of-pocket monthly WTP for Kremezin was 310,000, 430,000, and 520,000 Won (p<0.05, repeated one-way ANOVA)) when Kremezin delays the initiation of dialysis treatments by 1, 2, and 4 years. Significant correlation between the respondent's WTP and income $(r=0.266{\sim}0.368,\;p<0.05)$ confirmed the construct validity of the WTP instrument. Regression results showed that patients with a higher education, with diabetes as a major causes of CRF, and undergoing hemodialysis treatments tended to express higher WTP for Kremezin. The economic value of WTP from the perspective of patients varied from 310,000 to 520,000 Won depending on the effect size of Kremezin. The mean WTP was higher than 32,000 Won, only when the hypothetical effect of Kremezin in delaying the initiation of dialysis is for 2 years. This implies that Kremezin might be the preferred choice of therapy by CRF patients if it delays the initiation of dialysis treatment for at least 2 years.

우측 개흉술을 이용한 한국형 인공심장(AnyHeart)의 이식기법 (Surgical Technique for Korean Artificial Heart(AnyHeart) Implantation Using a Right Thoracotomy Approach)

  • 배경: 한국형 인공심장(AnyHeartTM)은 단심보조장치, 체네 이식형 양심보조장치, 인공심장, 그리고 생명 유지를 위한 체외순환 장치 등 다양한 형태로 사용이 가능한 장치이다. 그 중에서도 체내 이식형 양심보조장치는 가장 독특한 형태이다. 양심보초장치 이식의 외과적 술기는 주로 삽관으로 구성되어 있고, 흉골 정중절개는 외과의사가 심장의 노출을 충분히 할 수 있어서 가장 선호되어 온 방법이다. 그러나 환자가 흉골 정중절개를 이미 시행 받은 경우나, 또는 차후에 환자 대부분이 흉골 정중절개가 필요한 것을 감안하면 재개흉에 따른 합병증은 양심보조장치 이식에 있어서 중요하게 고려해야 할 부분이다. 일반적인 심장 수술의 임상경험에 근거하여, 저자들은 양심보조장치 이식에 있어서, 도관의 상관을 위한 접근이 심장의 우측에서도 가능할 것이라고 가정하였다. 이번 연구의 목적은 동물실험에서 양심보조장치의 이식을 위한 우측 개흉술의 수술 기법을 개발하고자 하였다. 대상 및 방법: 지난 2년 동안, 30차례의 AnyHeart$^{TM}$ 이식실험 중 16례가(11 calves, 3 canines, and 2 sheep) 우측 개흉술로 이루어졌다. 기계장치는 14례의 동물에서 체내 이식형 양심보조장치로 시술 되었으며, 체외형 양심보조장치와 체내 이식형 좌심실보조장치로 시술된 경우가 각각 1례씩이었다. 수술은 4번째 늑간를 통하여 우측 흉강으로 진입하였고, 양심보조장치의 경우에 우측 유입 도관은 우심방의 free wall에 삽관 하였고, 유춘도관은 인조혈관을 이용하여 주폐동맥애 측단 문합하였다. 좌측 유입도관은 심방격을 통하여 좌심방에 삽관하였고, 유출도관을 무명동맥에 인조현관을 이장하여 측단 문한하였다. 각각의 도관은 피하터널을 통해 우측 옆구리에 위치한 펌프와 연결하였다. 결과: Fitting test와 초기 실험의 5례를 제외하고 모든 실험 동물은 수술 후 회복되었다. 펌프의 유출량은 최고 6.5L/min였고, 평균 3~3.5L/min로 측정되었다. 수술과 관련된 사망이나 이환율은 없었다. 모든 실험 도물은 부검을 하였으며, 부검 결과 도관이 위치는 모두 각각의 심방내에 적절하게 위치하고 있었다.

국민건강보험 발전방향 (Future Direction of National Health Insurance)

  • It has been forty years since the implementation of National Health Insurance (NHI) in South Korea. Following the 1977 legislature mandating medical insurance for employees and dependents in firms with more than 500 employees, South Korea expanded its health insurance to urban residents in 1989. Resultantly, total expenses of the National Health Insurance Service (NHIS) have greatly increased from 4.5 billion won in 1977 to 50.89 trillion won in 2016. With multiple insurers merging into the NHI system in 2000, a single-payer healthcare system emerged, along with separation policy of prescribing and dispensing. Following such reform, an emerging financial crisis required injections from the National Health Promotion Fund. Forty years following the introduction of the NHI system, both praise and criticism have been drawn. In just 12 years, the NHI achieved the fastest health population coverage in the world. Current medical expenditure is not high relative to the rest of the Organization for Economic Cooperation and Development. The quality of acute care in Korea is one of the best in the world. There is no sign of delayed diagnosis and/or treatment for most diseases. However, the NHI has been under-insured, requiring high-levels of out-of-pocket money from patients and often causing catastrophic medical expenses. Furthermore, the current environmental circumstances of the NHI are threatening its sustainability. Low birth rate decline, as well as slow economic growth, will make sustainment of the current healthcare system difficult in the near future. An aging population will increase the amount of medical expenditure required, especially with the baby-boomer generation of those born between 1955 and 1965. Meanwhile, there is always the problem of unification for the Korean Peninsula, and what role the health insurance system will have to play when it occurs. In the presidential election, health insurance is a main issue; however, there is greater focus on expansion and expenditure than revenue. Many aspects of Korea's NHI system (1977) were modeled after the German (1883) and Japanese (1922) systems. Such systems were created during an era where infections disease control was most urgent and thus, in the current non-communicable disease (NCD) era, must be redesigned. The Korean system, which is already forty years old, must be redesigned completely. Although health insurance benefit expansion is necessary, financial measures, as well as moral hazard control measures, must also be considered. Ultimately, there are three aspects that we must consider when attempting redesign of the system. First, the health security system must be reformed. NHI and Medical Aid must be amalgamated into one system for increased effectiveness and efficiency of the system. Within the single insurer system of the NHI must be an internal market for maximum efficiency. The NHIS must be separated into regions so that regional organizers have greater responsibility over their actions. Although insurance must continue to be imposed nationally, risk-adjustment must be distributed regionally and assessed by different regional systems. Second, as a solution for the decreasing flow of insurance revenue, low premium level must be increased to an appropriate level. Likewise, the national reserve fund (No. 36, National Health Insurance Act) must be enlarged for re-unification preparation. Third, there must be revolutionary reform of benefit package. The current system built a focus on communicable diseases which is inappropriate in this NCD era. Medical benefits must not be one-time events but provide chronic disease management. Chronic care models, accountable care organization, patient-centered medical homes, and other systems that introduce various benefit packages for beneficiaries must be implemented. The reimbursement system of medical costs should be introduced to various systems for different types of care, as is the case with part C (Medicare Advantage Program) of America's Medicare system that substitutes part A and part B. Pay for performance must be expanded so that there is not only improvement in quality of care but also medical costs. Moreover, beneficiaries of the NHI system must be aware of the amount of their expenditure through a deductible payment system so that spending can be profiled and monitored. The Moon Jae-in Government has announced its plans to expand the NHI system; however, it is important that a discussion forum is created so that more accurate analysis of the NHI, its environments, and current status of health care system, can take place for reforming NHI.
