A Clinical study on the Therapeutic effect of Kamihyunggyeyungyotang for Pediatric Recurrent Otitis Media with Effusion (소아(小兒) 재발성(再發生) 삼출성(渗出性) 중이염(中耳炎)에서 가미형개연교탕(加味荊芥連翹湯)의 치료효과(治療效果)에 대한 임상적(臨床的) 연구(硏究))

  • Otitis media with effusion(OME) is the second most common disease in childhood after upper respiratory tract infection. Antibiotic treatment and ventilation tube insertion are the common treatment. The emergence of drug-resistant streptococcus pneumoniae (DRSP) has implications for the primary care provider who treats acute otitis media(AOM) in children. OME need not be treated with antibiotics unless the effusion has been present for 3 to 4 months. Tympanostomy tubes are an effective treatment for both chronic OME and recurrent AOM. But the complications of tympanostomy tubes are serious Kamihyunggyeyungyotang is known to have antiinflmmatory and antiallergic effect. In this study, we investigated the clinical efficacy of the Kamihyunggyeyungyotang on recurrent otitis media with effusion prospectively by using pneumatoscopy, tympanogram, pure tone audiometry, and radiologic study. The patients who had treated by antibiotics was used as control. The statistical analysis was done by Mann-Whitney test and the significance was considered when the p value was less than 0.05. The general outcome was significantly higher in Kamihyunggyeyungyotang group than in antibiotic group. The recovery rate from grade 3 to 0 in pneumatoscopy was 42.5% but the control was 6%. Hearing gain was improved 71% and pnuematization was returned 70%.

  • Obiectives : Otitis media is the disease which indicates every inflammatory situation in the middle ear. This includes the inflammation in the ear canal and mastoid sinus. In this study, the effect of Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for three children patients with otitis media were evaluated. Methods : After treatment with the Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for 3~6 weeks, we evaluated the effect by the figure of otoscope and complaints of patients. Results : In three cases, the pus in the figure declined dramatically and symptoms of patients got better after 3~6 weeks treatment. Conclusions : The Sa-Am acupuncture Bi-Jung-Geouk(脾正格) and Bojoong-ikgi-tang(補中益氣湯) for otitis media was effective. It will be attempted to the patients with it.

Comparison of Autophagy mRNA Expression between Chronic Otitis Media With and Without Cholesteatoma

  • Background and Objectives: Autophagy is known to be associated with pathogen infection. However, the expression of autophagy-related proteins has not been studied in chronic otitis media without cholesteatoma (COM) or with cholesteatoma (CholeOM). This study aimed to determine whether there is a difference between COM and CholeOM in autophagy-related gene mRNA expression. Subjects and Methods: For 47 patients with chronic otitis media, the inflammatory tissues were classified into granulation tissue (COM) or cholesteatoma (CholeOM) according to biopsy results. Results: PI3K mRNA expression (COM vs. CholeOM, mean±SD, 0.009±0.010 vs. 0.003±0.004; p=0.004) was lower, whereas Beclin-1 mRNA expression (0.089±0.107 vs. 0.176±0.163; p=0.034) was higher in the CholeOM group. Expression of PI3K mRNA in the CholeOM group was lower than that in the COM subgroups with presence of bacteria (0.022±0.019 vs. 0.001±0.001; p=0.001), otorrhea (0.049±0.068 vs. 0.003±0.004; p=0.004), and hearing loss over 40 dB (0.083±0.130 vs. 0.003±0.004; p=0.005). Conclusions: The data suggested that different autophagy proteins play important roles in chronic otitis media according to the presence or absence of cholesteatoma.

A Case Report of Atopic Dermatitis with Otitis Media with Effusion and Chronic Rhinitis (삼출성 중이염과 만성 비염을 동반한 아토피 피부염 1례)

  • Objectives: This study aims to report a case of atopic dermatitis with otitis media with effusion and chronic rhinitis improved by oral administration of Galgeungabanha-tang. Methods: The patient of this case had symptoms such as erythema, abrasions, scales, and itching. She also complained of rhinorrhea, nasal obstruction, cough, sputum, and ear fullness. Considering the fact that she was sensitive to heat, but did not sweat well, and respiratory symptoms often occurred, Galgeungabanha-tang was prescribed. Results: After medication, the symptoms of atopic dermatitis significantly improved from 10 to 2 point. The subjective symptoms of chronic rhinitis and otitis media with effusion also improved considerably. In addition, nasal swelling and purulent exudate almost disappeared. The tympanic membrane, which was swollen due to amber exudate, also changed to a normal gray-white color. Conclusions: Atopic dermatitis accompanied by chronic rhinitis and otitis media was improved after 45 days of medication.

Malondialdehyde Levels in Middle Ear Fluid from Patients of Otitis Media with Effusion

  • Otitis media with effusion (OME) is an inflammatory disease of the middle ear cleft. Oxygen free radicals have been implicated in a variety of inflammatory disorders. Oxygen free radicals may also be involved in the pathogenesis of OME. To evaluate the involvement of oxygen free radicals in the pathogenesis of OME, the level of malondialdehyde, which gives an index of lipid peroxidation by oxygen free radicals, was measured by the reaction with thiobarbituric acid. Malondialdehyde level in the middle ear fluid from the OME group was higher than that in the normal control group. Malondialdehyde level in the middle ear fluid from a mucoid subgroup was higher than that in the serous subgroup. Malondialdehyde levels in the middle ear fluid from the serous subgroup was significantly correlated with symptom duration. The Pearson correlation coefficient between malondialdehyde levels in the middle ear fluid from the serous subgroup and symptom duration was 0.842 (P<0.05). These results indicate that lipid peroxidation by oxygen free radicals may be involved in the pathogenesis of human OME.

Nomogram plot for predicting chronic otitis media in Korean adults

  • Nomogram is useful for predicting the prevalence of each patient through the scoring system without a complex formula. Because there are few studies on chronic otitis media (COM) in adults, this study aims to identify the relevant risk factors for COM in Korean adults and to build a nomogram for the risk factors. The Health Interview Survey data subset, derived from the Sixth Korean National Health and Nutrition Examination Survey (KNHANES VI), was used to evaluate the participants. Of the participants, the weighted prevalence of COM was 5.3%. Residence, earphone use, atopic dermatitis, allergic rhinitis, chronic rhinosinusitis, and subjective hearing status were identified as risk factors for COM. Using 6 risk factors, we propose a nomogram for COM, and use AUC to verify the discrimination of the nomogram.

The Causative Organisms of Otitis Media Accompanying Otorrhea in Children and Their Antimicrobial Susceptibility (소아에서 이루를 동반한 중이염의 원인 및 항균제 감수성)

  • Purpose : A great deal of youngsters suffer from otitis media, for which antimicrobials are frequently prescribed. Increased antimicrobial resistance forces physicians to judiciously use antimicrobial agents in treating patients with acute otitis media. There have however been few references with regard to otitis media in Korean children, and authors proceeded investigation to look for the causative agents of otitis media in Korean children and their antimicrobial susceptibility. Methods : The study included 65 patients younger than 15 years old who had been cared at the department of pediatrics and otolaryngology in Hanyang University Hospital from July 1994 to June 1999, and diagnosed of otitis media with otorrhea which contained microorganisms isolated in otorrhea culture. The medical records were reviewed for demographic data, isolated organisms and their antimicrobial susceptibility. Results : Among 65 patients, 37(57%) were boys and 28(43%) girls. Distribution of the patients was reciprocal to the age of the patients; 27 patients(41.5%) were younger than 1 year old, 24(36.9%) were 1 to 3 years old with the average of 2.9 years of age. Staphylococus aureus was isolated in 32 patients(49.2%), Streptococcus pneumoniae in 19 patients(29.2%) Haemophilus influenzae in 9 patients(13.8%), Streptococcus oralis in 3 patients(4.6%), Moraxella catarrhalis in 1 patient(1.5%). The isolated microorganisms were not different whether patients had cleft lip/palate or not. The antibiotic resistance rates of S. aureus were ${\geq}90%$ to erythromycin, imipenem, cephalothin, and clindamycin, 86.2% to oxacillin, 25% to chloramphenicol, 12.5% to trimethoprim/sulfamethoxazole(TMP/SMX), and 0% to vancomycin and teicoplanin. The antibiotic resistance rates of S. pneumoniae were 71.4% to penicillin and greater than 60% to erythromycin, tetracycline, TMP/SMX, 7.1% to chloramphenicol, and 0% to vancomycin and teicoplanin. The antibiotic resistance rates of H. influenzae were 55% to ampicillin and TMP/SMX, and 0% to chloramphenicol, ceftriaxone, aztreonam, imipenem and ciprofloxacin. Conclusion : With otorrhea culture, the causative organisms of otitis media appear to be S. aureus, S. pneumoniae and H. influenzae. The high antibiotic resistance rates of the isolated organisms should affect the choice of antibiotics in treating patients with otitis media. Prospective investigations utilizing tympanocentesis in microbiologic studies are needed.

One Case of Otomycosis with Chronic Suppurative Otitis Media Treated with Korean Medical Treatment (만성 화농성 중이염에 동반된 이진균증 환자 한방 치료 1례)

  • Objectives : Otomycosis is superficial fungal infection of external auditory canal accompanied with itching, pain, discharge, hearing loss, tinnitus and ear fullness. And chronic inflammation like chronic suppurative otitis media exacerbates otomycosis. The purpose of this study is to report one case of Korean medical treatment for otomycosis with chronic suppurative otitis media. Methods : This study was conducted with one case of otomycosis with chronic suppurative otitis media patient hospitalized in Ophthalmology, Otolaryngology & Dermatology Clinic of Korean medical Hospital. This patient was treated with Korean medical treatment including herbal medicine, acupuncture, pharmacopuncture and venesection. After the treatment, we took picture of external auditory canal by otoscope and evaluated subjective symptoms. Results : After the treatment, this patient had improved condition of external auditory canal and subjective symptoms. Conclusions : This study suggests that Korean medical treatment may be effective for otomycosis with chronic suppurative otitis media.

  • Congenital stapes fixation may develop alone or with other systemic anomalis. Acquired stapes fixation is found in otosclerosis or in chronic otitis media. It is our intent to review 4 cases (5ears) of congenital stapedial fixation and 2 cases (2 ears) of acquired stapedial fixation following chronic otitis media, treated by stapedectomy.
