Effects of DSG on Osteoblastic Cell from Rat Calvariae in the Presence of Dexamethasone (단치소요산가미방이 Dexamethasone 처리한 랫드의 두개골 세포에 미치는 영향)

  • It is well known that glucocorticoid may induce osteoporosis as its side effect in long-term therapy. The inhibition of osteoblast by glucocorticoid is also recognized as its action mechanism of decreased bone formation. In this study, the effect of DSG, Danchisoyosangamibang, on the differentiation and function of osteoblastic cells was investigated. The osteoblastic cells were isolated from rat calvariae using collagenase treatment. The cell counting, enzyme activity assay, MTT assay, collagen content assay were done to determine the cell proliferation, intracellular alkaline phosphatase (ALP) activity, bone martrix production, and cell apoptosis. DSG enhanced the cell proliferation after the culture for 10 days. ALP activity and total protein synthesis, and intracelluar collagen synthesis were increased time dependently when the cells were treated with DSG in the presence of dexamethasone. And, DSG restored calvarial cell function decreased by dexamethasone.

  • Compared with the earlier technique of dual photon absorptiometry (DPA) using $^{153}Gd$ radionuclide source, dual energy X-ray absorptiometry (DXA) has advantages of higher precision, accuracy and shorter scanning time. Despite the change from DPA to DPX, the nuclear medicine physicians has remained one of major suplier of this service due to long-standing use of DPA. Among many kinds of bone densitometries, DXA is the "gold standard" for the noninvasive diagnosis of osteoporosis. Especially there is no role for peripheral devices in the monitoring of patients on therapy. But, there are some areas of controversy related to the application of DXA, such as proper site of measurement, accurate interpritation, appropriate use of T-score, and the reference population young database. And the accuracy, precision, and quality control issues relating to bone density measurement are important subjects. To address these issues, the International Society for Clinical Densitometry (ISCD) has convened two Position Development Conferences and addressed official positions. This review deals the key elements of ISCD position paper and other important issues on the management of bone densitometry.

  • Sarcopenia is the degenerative loss of muscle mass and function with aging. Recently sarcopenia was recognized as a clinical disease by the International Classification of Disease, 10th revision, Clinical Modification. An imbalance between protein synthesis and degradation causes a gradual loss of muscle mass, resulting in a decline of muscle function as a progress of sarcopenia. Many mechanisms involved in the onset of sarcopenia include age-related factors as well as activity-, disease-, and nutrition-related factors. The stage of sarcopenia reflecting the severity of conditions assists clinical management of sarcopenia. It is important that systemic descriptions of the disease conditions include age, sex, and other environmental risk factors as well as levels of physical function. To develop a new therapeutic intervention needed is the detailed understanding of molecular and cellular mechanisms by which apoptosis, autophagy, atrophy, and hypertrophy occur in the muscle stem cells, myotubes, and/or neuromuscular junction. The new strategy to managing sarcopenia will be signal-modulating small molecules, natural compounds, repurposing of old drugs, and muscle-specific microRNAs.

  • Isoflavone found in Leguminosae is one of natural phytoestrogens and its effect on bone remodeling is one of key investigational interests in terms of estrogen replacement therapy (ERT). As commonly known, osteoporosis is one of hormonal deficiency diseases, especially in menopausal women. When estrogen is blocked, local factors such as 1L-1$\beta$ and IL-6 that are related in bone resorption are increased and enhance osteoclastogenesis, which is responsible for bone resorption. In the present study, we investigated the effect of isoflavones (Isocal) extracted from Sophorae Fructus on bone resorption in vitro. (omitted)

  • Isoflavones have been a central subject in natural phytoestrogens found in Leguminosae. Their effects on bone formation and remodeling are an important turning point in that they can act like estrogen by binding on estrogen receptors on target cell surface. We, therefore, believed that isoflavones may be applied in estrogen deficiency disease such as osteoporosis in terms of estrogen replacement therapy (ERT). As commonly known, osteoporosis is one of hormonal deficiency diseases, especially in menopausal women. (omitted)

  • Bisphosphonate-related osteonecrosis of the jaw (BRONJ) occurs mainly in female patients. In males the occurrence rate is low, which seems to be related to the low incidence of osteoporosis in men. Unfortunately, BRONJ tends to be ignored in general dental clinics in male patients with a history of osteoporosis treatment. BRONJ occurred in a male patient due to the clinician's lack of interest in the patient's history. In this case, the male patient was on bisphosphonate therapy because of a orchiectomy, and a dental treatment was performed without consideration of his medical history, resulting in BRONJ. We performed careful examinations and treatment with antibiotics and surgical operations. The postoperative healing was successful. In light of this particular case, we concluded that careful listening to the patient's history is very important.

Effects of the Herbal-Acupuncture with Corni Fructus Extract at Eumgok(KI10) on Osteoporosis in Ovariectomized Mice (음곡(陰谷)(KI10) 산수유 약침이 난소적출 생쥐의 골다공증에 미치는 영향)

  • Objective & Method: The purpose of this study is to investigate the effects of herbal-acupuncture with Corni Fructus extract (CF-HA) at Eumgok(KI10) on osteoporosis in ovariectomized (OVX) ddy mice. I carried out several experimental items to analyze the changes in body weight, uterine weight, uterus index, tibial length, the ash bone weight, tibial BMD, the levels of serum ALP, osteocalcin, Ca, P and the levels of tibial Ca, P, Ca/P ratio, and we performed histological and histomorphological analysis as well. Result: 1. CF-HA at Eumgok(KI10) significantly did not increase the level of bone mineral density in overiectomized mice. 2. CF-HA at Eumgok(KI10) significantly decreased the level of serum phosphorus in ovariectomized mice and sinificantly increased the level of serum calcium in ovariectomized mice. 3. CF-HA at Eumgok(KI10) significantly increased the levels of tibial calcium and phosphorus in ovariectomized mice. 4. CF-HA at Eumgok(KI10) significantly decreased the number of tibial osteoclast like cell in ovariectomized mice. 5. In the histomorphometric analysis of tibia, GPL(growth plate length) was significantly decreased by CF-HA at Eumgok(KI10) in ovariectomized mice. Conclusions: These results suggest that CF-HA at Eumgok(KI10) may have useful therapy effects on osteoporosis in ovariectomized mice.

The Associated Factors of Bone Mineral Density in Postmenopausal (폐경후에 골밀도의 관련인자 분석)

  • Objectives: The objective of this study was to investigate the Associated factors of bone mineral density in postmenopausal was measured for 36 normal in the women from July 14.2000 to august 24,2000. Methods: The data were collected from women who visited Physical Examination Center of a Catholic university hospital located in Taegu. Bone mineral density(BMD) of lumbar spine was measured using energy absorptiometry. Results: The bone mineral density of the lumbar spine decreased with aging. The bone mineral density of the lumbar spine increased with number of delivery and number of abortion decreased. The mean bone mineral density of the lumbar spine of postmenopausal women in age less than 50 and 50 ${\sim}$ 59 were 0.79 g/c$m^2$, the lumbar spine of postmenopausal women in age greater than 60 was 0.69 g/c$m^2$. The mean bone mineral density of the lumbar spine of postmenopausal women in mean age 56.1. Conclusions: In the multiple regression of risk factors to bone mineral density(BMD) of lumbar spine were correlated with bone mineral density T-scores(p<0.05). Osteoporosis is a major public health problem among the elderly, demanding effective strategic approach for prevention and treatment.

Adjunctive recombinant human parathyroid hormone agents for the treatment of medication-related osteonecrosis of the jaw: a report of three cases

  • Teriparatide has been effective in treating people diagnosed with medication-related osteonecrosis of the jaw (MRONJ). However, its efficacy is not well established to be accepted as a standard of care. The objective of this paper was to investigate the efficacy of recombinant human parathyroid hormone for the treatment of MRONJ. We report three cases of MRONJ patients with osteoporosis as the primary disease who were treated with a teriparatide agent along with other adjunctive measures. Each patient was administered a teriparatide injection subcutaneously for 16 weeks, 36 weeks, or 60 weeks. Surgical intervention including partial resection, sequestrectomy, decortication, and saucerization took place during the teriparatide administration. Complete lesion resolution was identified clinically and radiographically in all three patients. In patients diagnosed with MRONJ, teriparatide therapy is an efficacious and safe therapeutic option to improve healing of bone lesions. These findings demonstrate that teriparatide in combination with another therapy, especially bone morphogenetic protein, platelet-rich fibrin, or antibiotic therapy, can be an effective protocol for MRONJ.

  • This study aims to investigate the effect of Early Weight Bearing and Vibration Exercise and is focusing on the difference between changes if experimental before and after on 30 stroke patients. The obtained results are as follows; First, the chang due to Early Weight Bearing and Vibration in the comparison of experimental duration, Early Weight Bearing and Vibration Exercise showed improvement of Bone Density(P<.05). Second, in the comparison of change according to Early Weight Bearing and Vibration Exercise, Bone Density was significantly changed between two experimental group(P<.05). Third, the chang due to Early Welght Bearing and Vibration Exercise, Bone Density of sound femoral head and affected femoral head was significantly changed(P<.05). Fourth, the chang due to Early Weight Bearing and Vibration Exercise, Bone Density of femoral head was significantly changed between two experimental group(P<.05).
