Ossifying fibroma in the maxilla and orbital floor: report of an uncommon case

  • Benign fibro-osseous lesions occur when normal bone is replaced by cellular fibrous connective tissue and mineralized structures. One rare type of these lesions is the ossifying fibroma (OF). The aim of this study is to report an unusual case of OF in a 57-year-old female. Physical examination showed facial asymmetry without any tenderness, fluctuation, ocular pain, or ophthalmoplegia. Imaging exams revealed a solid mass involving the left maxilla and orbital floor. Surgical resection was performed without any complications or sequelae, and the histopathological results confirmed OF. Although recurrence is rare in this condition, the patient remains under follow-up.

  • Fibro-osseous lesions are a poorly defined category of conditions affecting the jaws and craniofacial bones, and include developmental lesions, reactive or dysplastic lesions, and neoplasms. Fibrous dysplasia and ossifying fibroma are the 2 main types of fibro-osseous lesions affecting the jaw, and ossifying fibroma is a true benign neoplasm of the bone-forming tissues with several well-recognized variants ranging from innocuous to extensively aggressive lesions. However, multiple simultaneous fibro-osseous lesions of the jaw bones involving all quadrants are exceedingly rare. One such case diagnosed by conventional radiography and computed tomography is discussed here.

  • The peripheral odontogenic fibroma (WHO type) is a relatively rare and characteristically benign and unencapsulated, exophytic gingival mass of fibrous connective. Odontogenic epithelium is found within the gingival mass, but usually appears to playa minor role when compared to the fibrous component. The connective tissue is ranged from markedly cellular to relatively acellular and well collagenized. but the connective tissue in this case report appears less cellular. Peripheral odontogenic fibroma must be differentiated histologically from peripheral ossifying fibroma, Peripheral cemental epithelial odontogenic tumor and Peripheral ameloblastoma. The author reports the following conclusions after clinical and radiological examination, excisional biopsy and reviewing literatures. 1. Peripheral odontogenic fibroma is rare lesion and frequently occurs in interdental papila as a form of fibroblastic connective tissue including odontogenic epithelium within the lesion. 2. Peripheral odontogenic fibroma must be differentially diagnosed with Peripheral ossifying fibroma by including less cellular connective tissue, odontogenic epithelium and dysplastic dentin 3. Treatment consists of surgical excision including removal of alveolar bone which is eroded under the lesion

백악질화성섬유종에 관한 연구 (A STUDY OF THE CEMENTIFYING FIBROMA)

  • Cementifying fibroma is an odontogenic tumor of periodontal ligament origin, consisting of a proliferation of fibrous tissue that forms cementum. Cemento-ossifying fibroma, cementifying fibroma and ossifying fibroma are difficult to distinguish on clinical and radiographic exmination, and on histopathology. However, when the calcified product is represented by ovoid or curvilinear deposits, the lesion is often referred to as cementifying fibroma. Cementifying fibromas of the jaws are well-circumscribed, generally slow-growing lesions. Although most lesions appear to be limited to the tooth-bearing areas, a few have extended into the angle-ramus area or encroached on the maxillary sinus. Radiographically, the cementifying fibroma could present as a radiolucent, radiopaque or mixed density lesion, depending on the degree of maturity. We have observed two cases of cementifying fibroma occurred in the left maxillary premolar-molar area of 38-year-old woman and 35-year old man. We obtained that two cases were shown the followed results; 1. Clinically, main clinical symptom was facial swelling and pain at palpation 2. Radiographically, well-circumscribed radiolucent lesion and radiolucent lesion with radiopaqu e foci were seen on the left maxillary premolar-molar areas. Loss of lamina dura and root resorption or divergency were also seen. 3. Histopathologically, many cementum-like calcifications were observed in the hypercellular connective tissue.

  • 본과에서는 하악에서 발생한 거대한 백악질-골성 섬유종에서 원발병소부위에 대한 광범위한 외과적 적출술 및 후방 장골이식을 통해 환자의 심미적, 기능적 안정성을 이루었으며 이환치아의 발거와 함께 술후 악간고정을 통하여 얇아진 하악하연부의 병적골절을 방지하였다. 현재까지 병소의 재발은 없고 양호한 결과를 보이고 있으며 향후 보철수복과 함께 지속적인 평가를 요할 것으로 사료된다.

  • The author has observed a case of ossifying fibroma occured in the left mandibular body area of 19 years old woman. In the serial roentgenograms, the author has drawn following conclusions: 1. The lesion is circumscribed and demarcated from the surrounding bone tissues. 2. The cortical bone of the involved area was expanded and thinned markedly on the site. 3. The radiopacity was increased with more or less mottled appearance on the site.

Psammomatoid Juvenile Ossifying Fibroma of the Maxilla Misdiagnosed as Fibrous Dysplasia: A Clinicopathologic Case Report

  • Juvenile ossifying fibroma (JOF) is a variant of the ossifying fibroma and includes two histopathological subtypes: trabecular and psammomatoid. Psammomatoid JOF (PJOF) in craniofacial structures should be distinguished from other fibro-osseous lesions, such as fibrous dysplasia (FD), considering the difference in the treatment protocols. Here, we present a rare case of PJOF that was initially misdiagnosed as a case of FD and emphasize the importance of considering JOF in the differential diagnosis of patients with craniofacial fibro-osseous lesions. A 4-year-old boy demonstrated progressive enlargement of the zygomaticomaxillary area on his left side for the last 6 months. The patient was diagnosed as a case of FD based on the clinical features and radiographic findings, and was operated considering the rapid progression. To achieve facial symmetry, contouring of the zygomatic bone and arch was performed. However, the patient demonstrated rapid enlargement at the 3-month postoperative follow-up. The decision was made to surgically remove the tumor due to visual field impairment. Intraoperatively, a rubbery mass, which was separated from the surrounding cortical bone, was identified and excised. The lesion was confirmed as PJOF by histopathological examination. The possibility of PJOF should not be ruled out in the differential diagnosis of patients with fibrous-osseous lesions. In the event of suspected PJOF, accurate diagnosis should be made through definitive biopsy.

  • 저자는 20세 남자환자에게 발생한 섬유골성병소에 대한 문헌고찰과 더불어 섬유골성병소중 치주인대로부터 유래된 백아질골섬유종에 대해 En-bloc절제술과 결손부에 대해 이물성형재료인 $Pyrost^{\circledR}$이식을 시행한 뒤 현재까지 재발소견없이 양호한 치유상태를 보여 이에 보고하는 바이다.

유리혈관부착 비골 이식술을 이용한 골종양의 치료 (Treatment of Bone Tumor with Free Vascularized Fibular Graft)

  • In certain low-grade malignant bone tumors such as chondrosarcoma or frequent recurrent benign bone tumors as ossifying fibroma, radical treatment may provide a good chance for cure. And large bony defect after the radical treatment can be filled with the massive bone graft. Recent advances in clinical microsurgery have made free vascularized bone graft a clinical reality, and Taylor in 1975, first reported the technique of free vascularized fibula graft for the reconstruction of large tibial defect with excellent clinical results. We tried wide excision and free vascularized fibula graft in 5 patients with ossifying fibroma and one patient with chondrosarcoma from January 1984 to December 1994 and followed for more one year. The shortest bony defect was 7cm and the longest bony defect was 20cm and mean bony defect was 13cm. All patients were evaluated clinically and roentgenographycally on basis of functional recovery and bony union. All patients showed satisfactory functional recovery with sound bony union and showed bony hypertrophy. And, local recurrence was not seen.

화골성 섬유종의 치료 (Treatment of Ossifying Fibroma)

  • 목적: 장골에 발생한 8예의 화골성 섬유종의 치료 결과를 조사하였다. 대상 및 방법: 1994년 5월부터 1999년 3월까지 본원에서 진단받은 8명의 환자를 대상으로 하였으며, 평균 연령은 7.9세이며, 경골이 7예, 비골이 1예였다. 모두 조직 검사를 시행했다. 보존적 치료를 시행하였으며, 반복되는 골절, 갑자기 커지는 종괴, 심한 굴곡 변형, 가관절을 보이는 경우 수술을 시행하였다. 방사선 추적 검사를 통해 재발 여부를 관찰하였다. 평균 추시 기간은 78.4개월이었다. 결과: 1차 치료로서 경과 관찰 2예, 소파술 2예, 골막하 절제술 1예, 골막을 포함한 광범위 절제술 3예를 시행하였다. 경과 관찰하였던 2예와 골막을 포함한 광범위 절제술을 시행하였던 3예는 재발하지 않았으며, 소파술 시행하였던 2예와 골막하 절제술을 시행하였던 1예에서 재발하였다. 소파술을 시행 후 재발하였던 2예 가운데 1예는 경과 관찰 중이며, 1예는 심한 굴곡 변형을 보여 골막을 포함한 광범위 절제술 시행하였다. 골막하 절제술 시행한 예는 증상이 없고 악화 소견없어 경과 관찰하였다. 2차 치료를 시행한 3예에서 재발이나 악화 소견은 보이지 않았다. 결론: 화골성 섬유종은 가능한 보존적인 치료를 시행하며, 수술 적응증이 되는 경우 골막을 포함한 광범위 절제술이 재발이 적을 것으로 생각된다.
