Florid Osseous Dysplasia 의 치험 1예 (FLORID OSSEOUS DYSPLASIA;REPORT OF A CASE)

  • 본 증례는 약 3~4년 전부터 우측 하악 골체부에 골성종창 및 간헐적 둔통이 있어 타병원에서 부분절제술을 시행받았으나 재발의 증세를 보여 내원한 60세 여자환자에 대해 병소부위를 포함하여 partial mandibulectomy와 동시에 metal plate와 iliac bone을 이용한 즉시 재건술을 시행하였으며 조직 병리학적 검사결과 florid osseous dysplasia로 진단 하였고, 술후 6개월이 경과한 현재까지 재발이나 별다른 합병증없이 양호한 치유경과를 보여주고 있다.

Mandibular Reconstruction with Vascularized Osseous Free Flaps: a Review of the Literature

  • Purpose: This article reviews a few of the commonly used types of vascularized osseous free flaps in maxillofacial reconstruction, which still represents the gold standard of restoration. We also discuss the developing concepts in maxillofacial reconstruction. Recent findings: Most of the literature reconfirms the established patterns of reconstruction with the aid of vascularized osseous free flaps. This method of free-tissue transfer is also feasible in cases of osteoradionecrosis or bisphosphonate-related osteonecrosis of the jaw. These flaps are also suitable for prosthetic restoration using osseointegrated dental implants. Summary: Vascularized osseous free flaps still remain the standard of care. Improvements upon the free-tissue transfer method employing vascularized osseous free flaps, such as distraction osteogenesis, tissue engineering, and imaging techniques, currently require further development, but these technologies could lead to improved outcomes of maxillofacial reconstruction in the near future.

Osseous outgrowth on the buccal maxilla associated with piezosurgery-assisted en-masse retraction: A case series

  • Piezoelectric surgery is a novel surgical approach used in orthodontic treatment for rapid tooth movement. This paper presents a case series wherein osseous outgrowths were observed in response to piezosurgery-assisted en-masse retraction. Sixteen patients requiring upper premolar extractions were treated with miniscrew-supported en-masse retraction and received minimally invasive decortication via piezosurgery. Computed tomography (CT) of the maxillary anterior region was performed to investigate the nature of the outgrowths. In 8 of the 16 patients, hemispheric or disc-shaped osseous outgrowths were observed on the sites where piezosurgery was performed during retraction. CT images revealed that these outgrowths were alveolar bone. This case series presents a previously unreported osseous response to piezosurgery-assisted tooth movement during orthodontic treatment. The response is mostly transient and is observed in 50% of the treated patients, suggesting a bone turnover that can be assessed clinically and radiographically.

경두개방사선사진과 자기공명영상을 이용한 측두하악관절 골변화에 관한 연구 (Transcranial radiograph and magnetic resonance imaging in the evaluation of osseous changes of the temporomandibular joint)

  • Purpose: To evaluate the diagnostic accuracy of transcranial radiographs and magnetic resonance imaging (MRI) of the temporomandibular joint (TMJ) in the assessment of osseous changes of the condylar head and articular eminence. Materials and Methods: Osseous changes of the TMJ were evaluated in forty-three patients. Osseous changes of the condylar head and articular eminence were observed in 41 joints and 64 joints, respectively on transcranial radiographs, and 48 joints and 59 joints, respectively on MRI. Results: The flattening, sclerosis, erosion, and osteophyte formation of the condylar heads were observed in 36.6%, 43.9%, 12.2%, and 7.3%, respectively on transcranial radiographs compared with 35.4%, 20.8%, 37.5%, and 6.3%, respectively on MRI. While, the flattening, sclerosis, and erosion of the articular eminences were observed in 26.6%, 67.2%, and 6.2%, respectively on transcranial radiographs compared with 32.2%, 59.3%, and 8.5%, respectively on MRI. Conclusion: There were no statistical differences between transcranial radiographs and MRI scans in the detection of osseous changes of the TMJ. However, MRI scans were superior to the transcranial radiographs in the detection of erosion of the condylar head (p<0.01).

  • Since the fibro-osseous lesion is not a specific diagnostic term, the author studied clinically and radiologically 44 cases which had been diagnosed as fibro-osseous lesion in SNUDH (1972- 1981. 12).The obtained results were as follows. 1. Clinico-radiologically, the cases of fibro-osseous lesions were divided into two groups. 2. The first group was fibrous dysplasia (21 cases). 3. The second group was tumors of periodontal ligament origin, including ossifying fibroma, cementifying fibroma and cemento-ossifying fibroma (23 cases). 4. In most cases the chief complaint was painless swelling of the jaw and the mean age of the fibrous dysplasia (24.6 yrs) was a little younger than that of the periodontal ligament origin lesions (29.2 yrs). 5. In fibrous dysplasia, maxilla was more often involved and showed ground-glass or smoke pattern radiologically. 6. The tumors of periodontal ligament origin occurred more in female, mandible and radiologically showed varying amounts of radiopaque foci in well-circumscribed osteolytic lesion.

  • Purpose: A 6-year-old boy with osteogenic sarcoma of the left humerus underwent bone scintigraphy. Tc-99m MDP was accumulated not only in the primary tumor but also in the osseous and extraosseous (pulmonary and pericardial) metastases. Osteogenic sarcoma directly produces osteoid, both in the primary and metastatic lesions. Tc-99m MDP is avidly taken up by tumor osteoid. At initial presentation, only 2% of cases have both pulmonary and osseous metastases. The patient had osseous, pulmonary, and pericardial metastases at presentation. This case presents that increased uptakes of Tc-99m MDP by the primary and metastatic tumor were demonstrated on bone scintigraphy at presentation.

  • Cemento-osseous dysplasias are a group of disorders known to originate from periodontal ligament tissue and involve, essentially, the same pathological process. They are usually classified into three main groups: periapical, florid, and focal cemental dysplasias depending on their extent and radiographic appearances. Radiographically, florid cemento-osseous dysplasia (FCOD) appears as dense, lobulated masses, often symmetrically located in various regions of the jaws. The best management for the asymptomatic FCOD patient consists of regular recall examinations with prophylaxis. The management of the symptomatic patient is more difficult. A case of FCOD occurring in a 52-year-old edentulous Korean female is reported which is rare with regard to race and sex.

Computer-assisted horizontal translational osseous genioplasty: a simple method to correct chin deviation

  • Background: Different genioplasty techniques are applied for the adjustment of chin area deformities such as chin deviation. Results: Thirty patients with simple facial asymmetry due to chin deviation underwent computer-assisted horizontal translational osseous genioplasty. In this technique, a surgical guide was used to cut a bone strip from the side where the chin should be transferred to; then, the same bone strip was used for the filling of the gap that was formed on the opposite side. Conclusion: According to the experience gained from this study, the authors believe that computer-assisted horizontal translational osseous genioplasty is a simple and reliable technique for patients with facial asymmetry due to chin deviation.

혀 기저부의 골성 분리종 2예 (Two cases of osseous choristoma on tongue base)

  • Osseous choristoma is a rare, benign proliferative osseous lesion, which is defined as the growth of normal tissue in an abnormal location. The etiopathogenesis for its formation is unknown, but various hypotheses have been proposed. Treatment of choice is en-bloc resection, and no recurrence has been reported. Here, we report the two cases of osseous choristoma, presented with a mass on the base of the tongue with/without globus symptom and were treated with surgical excision.

  • Fibro-osseous lesions are a poorly defined category of conditions affecting the jaws and craniofacial bones, and include developmental lesions, reactive or dysplastic lesions, and neoplasms. Fibrous dysplasia and ossifying fibroma are the 2 main types of fibro-osseous lesions affecting the jaw, and ossifying fibroma is a true benign neoplasm of the bone-forming tissues with several well-recognized variants ranging from innocuous to extensively aggressive lesions. However, multiple simultaneous fibro-osseous lesions of the jaw bones involving all quadrants are exceedingly rare. One such case diagnosed by conventional radiography and computed tomography is discussed here.
