Since many years ago and in spite of the existence of national regulations and international conventions ratified by the countries that are conformed the Central American Region (CAR), the seagoing maritime sector of this Region, has developed its activity without any type of safety measures. Therefore, a large number of people are known to die, disappear and suffer from serious injuries each year. These maritime casualties devastate the families affected by these events, and severely impact the local social and economic well-being. While all these accidents occurred in this Region, non government or any other official records are available recording these casualties Due to the lack of any historical written reports about maritime casualties occurred in this Region, the compilation of this type of accidents information and data was carried out directly by the researchers in the field. Also, all the information about the CAR Maritime Organization Components was too compiled by the authors from the respective National Maritimes Authorities. From the compiled information was elaborated the CAR maritime casualties data base and from the analysis of the above data base were determined the main causes of the maritime accidents occurred in the CAR. The lack of safety measures onboard and the ignorance of the presence and influence of atmospheric phenomena were among main factors that had been caused the maritime casualties in this Region. By other hand, from the analysis of the CAR maritime organization components, the existence of one level of organization very similar among them was determinate. The objective of this research was to analyze the role of the CAR maritime organization in the prevention of the occurrence of maritime casualties. The results of this study provides general understanding of the causes of this type of accidents in the CAR and basis for support in improving safety navigation system and measures so that the number of fatalities and maritime accidents may be reduced in the future.

This research is based on the information compiled on the occurrence of maritime casualties onboard ships registered under Central American Region (CAR) flag States. Due to nonexistence of writing reports in anyone of the countries that are integrating this Region, the information was compiled by the author through personal communication with the people involved in the casualties. The information was compiled by typing up the text and digitizing on the computer for the respective data base. For each casualty occurred, the following information was compiled, date and type of the casualty, position of the ship at the time of the casualty, dimension of the ship, number of people affected by the casualty, and cause and consequences of the casualty. Based on the number of people affected (death and disappear) by the casualties occurred was calculated the mortality rate by 100,000 persons and then compared with the Japanese mortality rate. Furthermore were analyzed the CAR maritime authorities competences and then were compared to that the Japanese counter part. In addition, the implementation rate of the International Maritime Organization (IMO) and International Labor Organization (ILO) conventions ratified by the CAR countries were analyzed. The objective of this research was to compiled and analyze the occurrences of maritime casualties happened on board the ships registered under the CAR Flag States, in order to determine the causes of these accidents. The results of the analysis enable us to better understand of the maritime safety situation of the ships that are registered under the CAR flag States. In order to reduce the occurrence of maritime casualties are proposed a series of measures based on the differences found then the comparison between CAR and Japan. Based on the results of this research, is possible to conclude that the inaccessibility of atmospheric information and the lack of safety measures onboard has been the main cause of the maritime casualties happened in the Pacific side of the CAR.

Adoption of wireless communication facilities in mobile devices leads to increased vulnerability in individual privacy. One of such cases was discovered when a smart mobile phone of Paris Hilton at Oscar Award Ceremony was hacked a Swedish group of hackers. In this study, I wondered what sort of personal information could be exposed to hackers in such cases. In the course of survey, it was recognized that technical analysis of flash memory in mobile devices to check what kinds of data are stored there is technically almost impossible, since they are usually built in a proprietary manner. No generic tools could apply to discover their contents. Having recognized technical difficulties, it was inevitable to resort to a questionnaire survey to see awareness level with regard to personal privacy. We collected response from three hundred respondents by posting the questionnaire at World Survey on-line research site. What we have discovered was quite astonishing that even personal residence registration numbers have been found from nine of every ten respondents. Other data revealed include phone numbers, names, and personal bank accounts.

The circadian clock regulates various physiological processes, including bone metabolism. The nuclear receptors Reverbs, comprising Rev-erbα and Rev-erbβ, play a key role as transcriptional regulators of the circadian clock. In this study, we demonstrate that Rev-erbs negatively regulate differentiation of osteoclasts and osteoblasts. The knockdown of Rev-erbα in osteoclast precursor cells enhanced receptor activator of nuclear factor-κB ligand (RANKL)-induced osteoclast formation, as well as expression of nuclear factor of activated T cells 1 (NFATc1), osteoclast-associated receptor (OSCAR), and tartrate-resistant acid phosphatase (TRAP). The overexpression of Rev-erbα leads to attenuation of the NFATc1 expression via inhibition of recruitment of c-Fos to the NFATc1 promoter. The overexpression of Rev-erbα in osteoblast precursors attenuated the expression of osteoblast marker genes including Runx2, alkaline phosphatase (ALP), bone sialoprotein (BSP), and osteocalcin (OC). Rev-erbα interfered with the recruitment of Runx2 to the promoter region of the target genes. Conversely, knockdown of Rev-erbα in the osteoblast precursors enhanced the osteoblast differentiation and function. In addition, Rev-erbα negatively regulated osteoclast and osteoblast differentiation by suppressing the p38 MAPK pathway. Furthermore, intraperitoneal administration of GSK4112, a Rev-erb agonist, protects RANKL-induced bone loss via inhibition of osteoclast differentiation in vivo. Taken together, our results demonstrate a molecular mechanism of Rev-erbs in the bone remodeling, and provide a molecular basis for a potential therapeutic target for treatment of bone disease characterized by excessive bone resorption.
Objectives: This study was conducted to evaluate the inhibitory effect of polygoni cuspidati radix (PCR) extract on osteoclast differentiation. Methods: MTT-assay was performed to estimate cytotoxicity of PCR extract in BMMs stimulated with RANKL. Tartrate resistant acid phosphatase (TRAP) staining, TRAP activity and RT-PCR were performed to know the inhibitory effect on osteoclast differentiation. actin ring formation were analysed to observe the effect of PCR extract. Results: PCR decreased the number of TRAP positive cells and TRAP activities in BMMs stimulated with RANKL and M-CSF. PCR restrained the formation of actin ring. PCR down regulated the induction of NFATc1, c-Fos, TRAP and OSCAR by RANKL. PCR inhibited NF-${\kappa}B$ activity by inducing degradation of $I{\kappa}B{\alpha}$. Conclusions: We suggest that PCR Extracts can be an effective therapeutic agent on osteoclast differentiation caused by diseases such as osteoporosis.

Vaccinium oldhamii (V. oldhamii) has been reported to exert a variety of the pharmacological properties such as anti-oxidant activity, anti-cancer activity, and inhibitory activity of ${\alpha}$-amylase and acetylcholinesterase. However, the anti-inflammatory activity of V. oldhamii has not been studied. In this study, we aimed to investigate anti-inflammatory activity of the stem extracts from V. oldhamii, and to elucidate the potential mechanisms in LPS-stimulated RAW264.7 cells. Among VOS, VOL and VOF, the inhibitory effect of NO and PGE2 production induced by LPS was highest in VOS treatment. Thus, VOS was selected for the further study. VOS dose-dependently blocked LPS-induced NO and PGE2 production by inhibiting iNOS and COX-2 expression, respectively. VOS inhibited the expression of pro-inflammatory cytokines such as $IL-1{\beta}$, IL-6 and $TNF-{\alpha}$. In addition, VOS suppressed TRAP activity and attenuated the expression of the osteoclast-specific genes such as NFATc1, c-FOS, TRAP, MMP-9, cathepsin K, CA2, OSCAR and ATPv06d2. VOS inhibited LPS-induced $NF-{\kappa}B$ signaling activation through blocking $I{\kappa}B-{\alpha}$ degradation and p65 nuclear accumulation. VOS inhibited MAPK signaling activation by attenuating the phosphorylation of ERK1/2, p38 and JNK. Furthermore, VOS inhibited ATF2 phosphorylation and blocked ATF2 nuclear accumulation. From these findings, VOS has potential to be a candidate for the development of chemopreventive or therapeutic agents for the inflammatory diseases.

Objective: The objective of this study was to define a new composite trait for Holstein dairy cows and evaluate the possibility of joint improvement in milk and fertility traits. Methods: A data set consisting 35,882 fertility related records (days open [DO], calving interval [CI], and number of services per conception [NSC], and total milk yield in each lactation [TMY]) was collected from 1998 to 2016 in Polish Holstein-Friesian breed herds. In this study TMY, DO, CI, and lactation length of each cow was used to obtain composite milk and fertility traits (CMF). Results: Moderate heritability (0.15) was estimated for composite trait that was higher than heritability of female fertility related traits: DO 0.047, CI 0.042, and NSC 0.014, and slightly lower than heritability of TMY 0.19. Favourable genetic correlations (-0.87) were estimated between CMF with TMY. Spearman rank correlation coefficients between breeding value of CMF with DO, CI, and TMY were high (>0.94) but with NSC were moderate (0.64). Selection on CMF caused favourable correlated genetic gains for DO, CI, and TMY. Different selection indices with different emphasis on fertility and milk production were constructed. The amount of correlated genetic gains obtained for DO and total milk production according to selection in CMF were higher than of genetic gains obtained for DO and TMY in selection indices with different emphasis on milk and fertility. Conclusion: The animal selection only based on a composite trait - CMF proposed in current study would simultaneously lead to favourable genetic gains for both milk and fertility related traits. In this situation CMF introduced in current study can be used to overcome to limitations of selection index and CMF could be useful for countries that have problems in recording traits, especially functional traits.

Complications experienced during lymphatic surgery have not been ubiquitously reported, and little has been described regarding how to prevent them. We present a review of complications reported during the surgical management of lymphedema and our experience with technical considerations to reduce morbidity from lymphatic surgery. A comprehensive search across different databases was conducted through November 2020. Based on the complications identified, we discussed the best approach for reducing the incidence of complications during lymphatic surgery based on our experience. The most common complications reported following lymphovenous anastomosis were re-exploration of the anastomosis, venous reflux, and surgical site infection. The most common complications using groin vascularized lymph node transfer (VLNT), submental VLNT, lateral thoracic VLNT, and supraclavicular VLNT included delayed wound healing, seroma and hematoma formation, lymphatic fluid leakage, iatrogenic lymphedema, soft-tissue infection, venous congestion, marginal nerve pseudoparalysis, and partial flap loss. Regarding intra-abdominal lymph node flaps, incisional hernia, hematoma, lymphatic fluid leakage, and postoperative ileus were commonly reported. Following suction-assisted lipectomy, significant blood loss and transient paresthesia were frequently reported. The reported complications of excisional procedures included soft-tissue infections, seroma and hematoma formation, skin-graft loss, significant blood loss, and minor skin flap necrosis. Evidently, lymphedema continues to represent a challenging condition; however, thorough patient selection, compliance with physiotherapy, and an experienced surgeon with adequate understanding of the lymphatic system can help maximize the safety of lymphatic surgery.

Background Educational resources on the internet are extensively used to obtainmedical information. YouTube is the most accessed video platform containing information to enhance the learning experience of medical professionals. This study systematically analyzed the educational value of microsurgery-related videos on this platform. Methods A systematic review was conducted on YouTube from April 18 to May 18, 2020, using the following terms: "microsurgery," "microsurgical," "microsurgical anastomosis," "free flap," and "free tissue transfer." The search was limited to the first 100 videos, and two independent reviewers screened for eligible entries and analyzed their educational value using validated scales, including a modified version of the DISCERN score (M-DISCERN), Journal of the American Medical Association (JAMAS) benchmark criteria, and the Global Quality Score (GQS). Evaluation of video popularity was also assessed with the video power index (VPI). Results Of 356 retrieved videos, 75 (21%) were considered eligible. The educational quality of videos was highly variable, and the mean global scores for the M-DISCERN, JAMAS, and GQS for our sample were consistent with medium to low quality. Conclusions A limited number of videos on YouTube for microsurgical education have high-educational quality. The majority scored low on the utilized criteria. Peer-reviewed resources seem to be a more reliable resource. Although the potential of YouTube should not be disregarded, videos should be carefully appraised before being usedas an educational resource.

The effect of PHAR-DBH-Me, a cannabinoid receptor agonist, on different cardiovascular responses in adult male rats was analyzed. The blood pressure was measured directly and indirectly. The coronary flow was measured by Langendorff preparation, and vasomotor responses induced by PHAR-DBH-Me in aortic rings precontracted with phenylephrine (PHEN) were analyzed. The intravenous injection of the compound PHAR-DBH-Me (0.018-185 ㎍/kg) resulted in decreased blood pressure; maximum effect was observed at the dose of 1,850 ㎍/kg. A concentrationdependent increase in the coronary flow was observed in a Langendorff preparation. In the aortic rings, with and without endothelium, pre-contracted with PHEN (10-6 M), the addition of PHAR-DBH-Me to the superfusion solution (10-12-10-5 M), produced a vasodilator response, which depends on the concentration and presence of the endothelium. L-NAME inhibited these effects. Addition of CB1 receptor antagonist (AM 251) did not modify the response, while CB2 receptor antagonist (AM630) decreased the potency of relaxation elicited by PHAR-DBH-Me. Indomethacin shifted the curve concentration-response to the left and produced an increase in the magnitude of the maximum endothelium dependent response to this compound. The maximum effect of PHAR-DBH-Me was observed with the concentration of 10-5 M. These results show that PHAR-DBH-Me has a concentration-dependent and endothelium-dependent vasodilator effect through CB2 receptor. This vasodilation is probably mediated by the synthesis/release of NO. On the other hand, it is suggested that PHAR-DBH-Me also induces the release of a vasoconstrictor prostanoid.
