Treatment of Syringomyelia due to Chiari Type I Malformation with Syringo-Subarachnoid-Peritoneal Shunt

  • Chiari type I malformation is a tonsillar herniation more than 3 mm from the level of foramen magnum, with or without concurrent syringomyelia. Different surgical treatments have been developed for syringomyelia secondary to Chiari's malformations: craniovertebral decompression with or without plugging of the obex, syringo-subarachnoid, syringo-peritoneal, and theco-peritoneal shunt placement. Shunt placement procedures are useful for neurologically symptomatic large-sized syrinx. In this paper, authors define the first successful treatment of a patient with syringomyelia due to Chiari type I malformation using a pre-defined new technique of syringo-subarachnoid-peritoneal shunt with T-tube system.

  • In orthopedics, hip arthroplasty is the operation that replaces damaged hip joint to artificial joint. In hip arthroplasty, quite better result can be achieved if robot is applied to machine cavity in bone, especially when cementless stem is used. So several kinds of robots were introduced for hip arthroplasty, but they used MRI, CT Scan, vision analysis and real time tracking of bone position for registration of robot. To overcome shortage of conventional robot surgery, gauge based registration method was proposed and small robot was designed. In this method, small robot is mounted on femur, and its position is determined by gauge registration method. Operation procedure was performed on model femur and result was analyzed. This robotic hip surgery system is expected to more adaptable in operation room.

  • The usages of botulinum toxin were most commonly for the treatment of spasmodic dysphonia in the otolaryngology field. It has been not only widely used in otolaryngology-Head Neck surgery but also plastic surgery, ophthalmology, rehabilitation medicine, and orthopedics. Now botulinum toxin is used such as blepharospasm (excessive blinking), strabismus, cosmetic, muscle spasms, upper motor neuron syndrome, severe primary axillary hyperhidrosis (excessive sweating), cervical dystonia (spasmodic torticollis), chronic migraine, bruxism, and achalasia. The indication of this drug still gradually expanding with the times. In this articles, the author will demontrate how to use the botulinum toxin for treating cricopharyngeal spasm, arytenoid dislocation, sialocele, Frey syndrome, contact granuloma, bilateral vocal fold paralysis, and mutaional falsetto instead of conventional surgical treatment.

  • Spinal fixation systems provide surgical versatility, but the complexity of their design reduces their strength and fatigue resistance. There is no published data on the mechanical properties of such screws. Screws were assembled according to a vertebrectomy model for destructive mechanical testing. A group of two assemblies was tested in static compression. One group was applied to surface a grit blasting method and another group was applied to surface a bead blasting method. Modes of failure, yield, and ultimate strength, yield stiffness, and cycles to failure were determined for six assembles. Static compression 2% offset yield load ranges was from 327 to 419N. Fatigue loads were determined two levels, 37.5% and 50% of the average load from static compression ultimate load. An assembly of bead blasting treatment only achieved 5 million cycles at 37.5% level in compression bending.

  • The purpose of this paper is to describe the transition of a 1,100 beds teritary hospital from 50% softcopy operation to full PACS operation. For the past 2 years, radiologists and clinicians have been using PACS to provide softcopy services to the outpatient clinics and inpatient wards of orthopedics surgery, neurosurgery and neurology as well as emergency room, surgical intensive care unit, medical intensive unit, pediatrics intensive care unit and neonatal intensive care unit. The examinations requested by these departments account for about 50% of hospital's radiological exams. In September 1996, we began the second phase of PACS implementation and installed additional workstations (102) in the remaining wards and clinics, interfaced to PACS additional imaging modalites, and increased the capacity of both the image server (256 Gbytes) and optical juke boxes (3 Tbytes). As of January 1997, we are in the final phase of moving away from conventional film system to full PACS operation.

  • The first step implementation of a hospital-wide Picture Archiving Communications System (PACS) at a newly built hospital Samsung Medical Center (SMC), is described. Current clinical operation encompasses the fiber optics delivery of direct-interfaced magnetic resonance imager (LRI), X-ray computed tomography (CT). digital subtraction angiography (DSA) and computed radiography (CR) digital images via high performance file server to the departments of radiology, neurosurgery, orthopedics surgery, neurology, emergency room and the surgical intensive care unit.

  • After ACL reconstruction. abrasion or wear of graft appeared frequently because of contact stresses between femoral tunnel and ACL. To minimize these problems. optimal shape of femoral tunnel is necessary. In this study. we evaluate friction force by degree of wear due to abrasion of soft tissue and develop 3-dimensional FEM model using ANSYS 5.5.1 version to analyze stress growths between femoral tunnel and ACL, We conclude that femoral tunnel angle must be slacked parallel to tunnel direction to minimize contact stress.

  • Lesion of long head of biceps tendon is one of the causes making an anterior shoulder pain. According to the anthropometric study of the bicipital groove the meanings of the width, depth and the medial wall angle were suggested. Confirming the images and location of the long head of biceps tendon itself would be helpful in diagnosis.

  • MicroRNAs have been demonstrated to regulate proliferation and apoptosis in many types of cancers, but biological functions in osteosarcomas remain relatively unknown. Here, we found expression of miR-802 to be up-regulated in osteosarcoma tissues in comparison with adjacent normal tissues. Enforced expression of miR-802 was able to promote cell proliferation in U2OS and MG63 cells, while miR-802 antisense oligonucleotides (antisense miR-802) inhibited cell proliferation. At the molecular level, our results further revealed that expression of p27, a negative cell-cycle regulator, was negatively regulated by miR-802. Therefore, the data reported here indicate that miR-802 is an important regulator in osteosarcoma, our findings contributing to a better understanding of important mis-regulated miRNAs in this tumour type.

  • There are various diseases in soft tissue tumors that occur in subcutaneous tissue, and hematoma generally would be occurred in traumatic or coagulopathic condition. We report a case of hematoma on left gluteal area that was diagnosed by ultrasonography and was resected in 70 year old woman who visited the hospital because of left gluteal mass detected by chance two month ago and slowly growing.
