Down-regulation of Long Non-coding RNA TUG1 Inhibits Osteosarcoma Cell Proliferation and Promotes Apoptosis

  • Objective: To investigate the expression level of TUG1 and one of its transcript variants (n377360) in osteosarcoma cells and assess the role of TUG1 in proliferation and apoptosis in the U2OS cell line. Methods: TUG1 and n377360 expression levels in patients with osteosarcomas and the U2OS human osteosarcoma cell line were evaluated using real-time quantitative PCR. U2OS cells were transected with TUG1 and n377360 siRNA or non-targeting siRNA. MTS was performed to assess the cell proliferation and flow cytometry was applied to analyze apoptosis. Results: We found significantly higher TUG1 and n377360 expression levels in osteosarcoma tissues compared with matched non-tumorous tissues. In line with this, suppression of TUG1 and n377360 expression by siRNA significantly impaired the cell proliferation potential of osteosarcoma cells. Furthermore, inhibition of TUG1 expression significantly promoted osteosarcoma cell apoptosis. Conclusions: The overexpression of TUG1 and n377360 in osteosarcoma specimens and the functional role of TUG1 and n377360 regarding cell proliferation and apoptosis in an osteosarcoma cell line provided evidence that the use of TUG1 or n377360 may be a viable but an as yet unexplored therapeutic strategy in tumors that over express these factors.

An Analysis of the Foot and Ankle-related Questions on the Korean Orthopedic In-Training Examination (대한 정형외과 전공의 평가시험에서 족부-족관절 분야 문항 분석)

  • Purpose: The purpose of this study was to evaluate the questions pertaining to the foot and ankle on the Korean Orthopedic In-Training Examination (KOITE) between 2012 and 2016. Materials and Methods: Questions regarding the foot and ankle on KOITE during the five-year period (2012~2016) were analyzed. Number of foot and ankle questions, topics, taxonomic classification, imaging modalities, and cited references related to each question were analyzed. Results: The average number of foot and ankle questions was 35, accounting for 7.0% (35/500) of all KOITE questions. All questions were categorized into three topic areas: disease (51.4%, 18/35), trauma (31.4%, 11/35), and basics (17.1%, 6/35). Taxonomy 3 (decision-making type questions) was the most common (42.9%, 15/35). References that are commonly used for each question were Campbell's Operative Orthopedics (100%, 35/35) and the textbook of The Korean Orthopaedic Association (74.3%, 26/35). Conclusion: This analysis can be valuable to not only orthopaedic surgery residents to improve their knowledge of orthopedics, but also to instructors for optimizing their education programs.

Role of dexmedetomidine as adjuvant in postoperative sciatic popliteal and adductor canal analgesia in trauma patients: a randomized controlled trial

  • Background: The effect of dexmedetomidine as an adjuvant in the adductor canal block (ACB) and sciatic popliteal block (SPB) on the postoperative tramadol-sparing effect following spinal anesthesia has not been evaluated. Methods: In this randomized, placebo-controlled study, ninety patients undergoing below knee trauma surgery were randomized to either the control group, using ropivacaine in the ACB + SPB; the block Dex group, using dexmedetomidine + ropivacaine in the ACB + SPB; or the systemic Dex group, using ropivacaine in the ACB + SPB + intravenous dexmedetomidine. The primary outcome was a comparison of postoperative cumulative tramadol patient-controlled analgesia (PCA) consumption at 48 hours. Secondary outcomes included time to first PCA bolus, pain score, neurological assessment, sedation score, and adverse effects at 0, 5, 10, 15, and 60 minutes, as well as 4, 6, 12, 18, 24, 30, 36, 42, and 48 hours after the block. Results: The mean ± standard deviation of cumulative tramadol consumption at 48 hours was 64.83 ± 51.17 mg in the control group and 41.33 ± 38.57 mg in the block Dex group (P = 0.008), using Mann-Whitney U-test. Time to first tramadol PCA bolus was earlier in the control group versus the block Dex group (P = 0.04). Other secondary outcomes were comparable. Conclusions: Postoperative tramadol consumption was reduced at 48 hours in patients receiving perineural or systemic dexmedetomidine with ACB and SPB in below knee trauma surgery.

Survey on Medical doctors' awareness and perceptions of Bisphosphonate-related osteonecrosis of the jaw (비스포스포네이트 관련 악골괴사 (Bisphosphonate-Related Osteonecrosis of the Jaw)에 관한 의사의 인식도 조사)

  • The objective of this study was to identify bisphosphonate-related osteonecrosis of the jaw (BRONJ) awareness and experience level of patients by medical doctors who prescribes bisphosphonate being used, analyze dental examination referral reality and to utilize its result as basic education data for early diagnosis of BRONJ and its prevention. The study was carried out through a self-administered questionnaire distributed among a sample 192 residents and specialists. They belonged to family medicine, internal medicine and orthopedics of 6 tertiary medical centers located in Seoul. The survey consisted of 22 questions; general characteristics, bisphosphonate therapy, awareness of BRONJ, implementation level of dental examination referral. Among 192 medical doctorss, 78.1% (n=150) showed awareness of BRONJ. Only 8.9% (n=17) had correct response in all 5 BRONJ knowledge questions. Dental examination referral by medical doctors was implemented in below 30% of the total patients. At the time of bisphosphonate administration, specialist of oncology most highly recognized necessity of dental examination referral and it was represented in the order of endocrinology, rheumatology, family medicine, orthopedics specialists. As recognition of medical doctors for BRONJ and implementation level of dental referral were represented to be low, it is considered that enhancement of BRONJ recognition for medical doctors and development of high accessible education program for increasing implementation rate of dental examination referral would be required.

  • This Paper proposes designing and implementing the database system storing the medical images. This system collects the medical image when doctors operate and diagnose the patients using the endoscope in the orthopedics, then stores the medical image data to database. Therefore. system avoids duplicated medical data, retrieves and updates the medical data effectively. The medical image data can be shared to the multiple users and application programs. This system consists of the five components. that is, the input module acquiring the medical image from the endoscope. the modulo storing the medical image. the database design and implementation storms the patient's disease history and the medical image data, user friendly interface design and implementation, and the simple data retrieval engine. The features of the system are followed. The image catcher program using DirectShow is portable any image catcher board And because the image catcher algorithm is implemented as a public module, The throughput can be increased during the development of video and audio contents on internet.

Comparison between fiber-reinforced polymers and stainless steel orthodontic retainers

  • Objective: The aim of this study was to examine the properties of fiber-reinforced composite and stainless steel twisted retainers for orthodontic retention. Methods: Three different span lengths (5.0, 8.0, and 14.0 mm) of fiber-reinforced composite were investigated. The three fiber-reinforced composite retainer groups were subdivided according to the storage condition (dry and wet), resulting in a total of six groups. Each stainless steel and fiber-reinforced composite group was comprised of six specimens. The three-point bending flexural test was conducted using a universal testing machine. ANOVA was used to assess differences in the maximum load and maximum stress according to the span length, material, and storage condition. Post-hoc comparisons were performed if necessary. Results: The maximum stress and maximum load were significantly (p < 0.001) associated with the span length, material, and storage condition. The significant interaction between the material and span length (p < 0.001) indicated the differential effects of the material for each span length on the maximum stress and maximum load, with the difference between materials being the highest for the maximum span length. Conclusions: Our findings suggest that fiber-reinforced composite retainers may be an effective alternative for orthodontic retention in patients with esthetic concerns or allergy to conventional stainless steel wires.

Overexpression of CXCR4 is significantly associated with cisplatin-based chemotherapy resistance and can be a prognostic factor in epithelial ovarian cancer

  • The chemokine receptor 4 (CXCR4) plays an important role in the growth, angiogenesis and metastasis of various cancers, including epithelial ovarian cancer (EOC). However, the correlation between CXCR4 and the clinical response of EOC patients to chemotherapy remains unknown. 124 EOC patients were recruited to assess the relationship between CXCR4 and the response to cisplatin-based chemotherapy. The results showed that patients with a higher CXCR4 expression had a significantly lower chemosensitivity, a poorer progression-free survival and a lower overall survival than those with lower CXCR4 expression. In addition, knockdown of CXCR4 by small interfering RNA suppressed cell proliferation and resulted in G1/S arrest, increased apoptosis and chemosensitivity in both cisplatin-sensitive A2780 cells and cisplatin-resistant cell A2780/cis in vitro. Our data suggest that CXCR4 is one of the key molecules in cisplatin-based chemotherapy for EOC patients and that CXCR4 inhibition is a potential strategy to address the chemoresistance of EOC.

Strain Rate Dependent Poroelastic Behavior of Bovine Vertebral Trabecular Bone

  • It is widely accepted that the pressure variation of interstitial fluid is one of the most important factors in bone physiology. In order to understand the role of interstitial fluid on porous bony structure, a consideration for the biomechanical interactions between fluid and solid constituents within bone is required. In this study, a poroelastic theory was applied to investigate the elastic behavior of calf vertebral trabecular bone composed of the porous solid trabeculae and the viscous bone marrow. The poroelastic behavior of trabecular bone in a uniaxial stress condition was simulated using a commercial finite difference analysis software (FLAC, Itasca Consulting Group, USA), and tested for 5 different strain rates, i.e., 0.001, 0.01, 0.1, and 10 per second. The material properties of the calf vertebral trabecular bone were utilized from the previous experimental study. Two asymptotic poroelastic responses, the drained and undrained deformations, were predicted. From the predicted results for the simulated five strain rates, it was found that the pore pressure generation has a linearly increasing behavior when the strain rate is the highest at 10 per second, otherwise it showed a nonlinear behavior. The pore pressure generation with respect to the strain was found to be increased as the strain rate increased. The elastic moduli predicted at each strain were 208.3, 212.2, 337.6, 593.1, and 602.2 MPa, respectively. Based on the results of the present study, it was suggested that the calf vertebral trabecular bone could be modeled as a poroelastic material and its strain rate dependent material behavior could be predicted.

  • Synovial sarcoma is rare soft tissue tumor mesenchymal origin. Osseous involvement of synovial sarcoma is rare. A 24-year-old man presented with pain and swelling of the lower extremity. MRI of the lower extremity demonstrated a large mass encircling tibia with osseous involvement. Surgical excision of the mass was done and the mass was diagnosed as biphasic synovial sarcoma with bone marrow involvement. We also discuss the other imaging findings of synovial sarcoma on MRI.
