Mastoid osteoma: A rare incidental finding in an orthodontic patient

  • Mastoid osteomas of the temporal bone are rare, benign, and usually asymptomatic tumors. However, depending on their size and extension, mastoid osteomas may cause facial palsy, a sensation of ear fullness, pressure-related pain, hearing loss, recurrent external ear infections, and chronic discharge. The etiology of mastoid osteomas is still unknown, but congenital, infectious, and traumatic factors have been proposed. Surgical treatment may be performed with minimal postoperative morbidity. In this article, the authors report a case of a 48-year-old woman seeking orthodontic treatment with an unusual retroauricular protruding mass, including the diagnostic process and differential diagnosis. This case supports the essential role of cone-beam computed tomography to analyze and identify the lesion as a mastoid osteoma.

Parry-Romberg Syndrome 환자에서 악정형 및 교정 치료 (Orthopedic and Orthodontic Treatments of a Patient with Parry-Romberg Syndrome)

  • Parry-Romberg syndrome(PRS) is a degenerative disease characterized by progressive hemifacial atrophy. A 10-year-old girl who had been treated for linear scleroderma at the dermatologic department visited the orthodontic department. The frontal facial photograph showed mild facial asymmetry. On the left side, mild atrophy of soft tissue, enophthalmos, cheek depression, and dry skin with dark pigmentation were observed. The radiograph showed the hypoplasia of both the maxilla and mandible on the left side. This case report describes the treatment of a patient with PRS for 7 years. To minimize the effect of progressive atrophy on the facial growth, a hybrid appliance was used. The facial photos and radiographic records were periodically taken to analyze the progression of PRS. Although it is impossible to prevent the progression of facial asymmetry, it appears to be possible to limit the atrophic effect. After the stabilization of PRS, the orthodontic treatment by the fixed appliance was performed. Additionally, autologous fat graft was performed three times at 6 month intervals. After the treatment, the patient had a confident smile and facial asymmetry was improved.

Orthodontic treatment in a patient with Moebius syndrome: A case report

  • Moebius syndrome (MBS) is a congenital neurologic disorder that causes cranio-facial abnormalities. It involves paralysis of the VI and VII cranial nerves and causes bilateral or unilateral facial paralysis, eye movement disorder, and deformation of the upper and lower limbs. The orofacial dysfunctions include microstomia, micrognathia, hypotonic mimetic and lip muscles, dental enamel hypoplasia, tongue deformity, open bite or deep overbite, maxillary hypoplasia, high arched palate, mandibular hyperplasia or features indicating mandibular hypoplasia. This case report presents a 7-year-old male patient who was diagnosed with MBS at the age 2 years. The patient displayed typical clinical symptoms and was diagnosed with Class II malocclusion with a large overjet/overbite, tongue deformity and motion limitation, and lip closure incompetency. Treatment was initiated using a removable appliance for left scissor bite correction. After permanent tooth eruption, fixed appliance treatment was performed for correction of the arch width discrepancy and deep overbite. A self-ligation system and wide-width arch form wire were used during the treatment to expand the arch width. After 30 months of phase II treatment, the alignment of the dental arch and stable molar occlusion was achieved. Function and occlusion remained stable with a Class I canine and molar relationship, and a normal overjet/overbite was maintained after 9.4 years of retainer use. In MBS patients, it is important to achieve an accurate early diagnosis, and implement a multidisciplinary treatment approach and long-term retention and follow-up.

치과 교정 환자 부모의 인성(SCL-90-R)과 환자 협조도의 상관관계 (Correlation between The Parents' Character of Orthodontic Patients and Patients Cooperation)

  • 교정 치료에서 치과의사가 아무리 좋은 진료 술식을 가지고 있더라도, 환자의 협조도가 따르지 않는다면 좋은 치료 결과가 나오기를 기대할 수 없다. 본 연구에서는 환자부모의 인성을 연구하여 자녀의 협조도와의 상관관계를 규명고자 하였다. 환자부모의 인성검사는 scl-90-r로 간이 인성검사를 하였고, 환자의 협조도는 약속시간, 장치의장착률, 장치의 파손, 진료비 납부율를 통해 협조도를 구했다. 환자 부모 인성검사 내용은 신체화척도, 강박증, 대인예민성, 우울척도, 불안척도, 적대감, 공포불안척도, 편집증 척도, 정신증 척도, 부가적 문항이었다. 여기서 상호상관관계를 알기위해 원도우 통계처리용 SPSS12.0 프로그램을 사용한 결과는 협조도와 부모인성관계는 상관관계가 없었고, 아버지와 협조도의 관계에서 아버지의 정신 증에서 약간의 상관관계는 있었으나, 무시할 만한 수준이었다. 어머니와 협조도의 상관관계는 거의 나타나지 않았다. 그러나 협조도간의 상관관계에서 약속과 장착, 파손, 납부는 통계적으로 유의할만한 결과가 나왔다. 장착과 납부는 양의 상관관계를 보였다.

  • Upper canine is described as 'cornerstone' of maxilla, and its importance is implicated by long root and good alveolar support. The incidence of impaction of upper canine is the second most frequent next to the third molar because it takes a long period of time to develop, and has a complicated path of eruption, and erupts lately. Generally, the patient who has a palatally impacted canine visit the clinic primarily due to a missing of canine after 12-13 years old. Palatally impacted canine is different from labially impacted canine in its cause and treatment process. It is due to malposition or anomalous lateral incisor rather than arch length deficiency. Once the impaction is identified, the first stage of the treatment is to localiz the lesion by radiographic examinations or others and according to severity, orthodontic traction, or transplantation should be considered, and comprehensive diagnosis and treatment plan of malocclusion should be estsblished. Properly managed impacted canine can provide funtion and esthetic through proper diagnosis and treatment of extraction of canine is not indicated.

Correction of Angle Class II division 1 malocclusion with a mandibular protraction appliances and multiloop edgewise archwire technique

  • A Brazilian girl aged 14 years and 9 months presented with a chief complaint of protrusive teeth. She had a convex facial profile, extreme overjet, deep bite, lack of passive lip seal, acute nasolabial angle, and retrognathic mandible. Intraorally, she showed maxillary diastemas, slight mandibular incisor crowding, a small maxillary arch, 13-mm overjet, and 4-mm overbite. After the diagnosis of severe Angle Class II division 1 malocclusion, a mandibular protraction appliance was placed to correct the Class II relationships and multiloop edgewise archwires were used for finishing. Follow-up examinations revealed an improved facial profile, normal overjet and overbite, and good intercuspation. The patient was satisfied with her occlusion, smile, and facial appearance. The excellent results suggest that orthodontic camouflage by using a mandibular protraction appliance in combination with the multiloop edgewise archwire technique is an effective option for correcting Class II malocclusions in patients who refuse orthognathic surgery.

구치부 반대교합이 없는 총생 증례의 급속구개확장에 의한 치료 (Crowding with no posterior crossbite treatment by rapid Palatal expansion)

  • 중등도 내지는 심한 총생과 회전이 있는 12.5세 여자 환자의 증례에 대한 보고이다. 발치를 하지 않고, 상하악 치열궁의 확장에 의해 치료하였다. 구치부의 반대교합이 없었지만, 상악의 경우 급속구개확장을 시행하였다. 양악의 총생과 치아 회전이 해소되었으며, 교합기능과 안모 심미의 개선을 이루었으며, 적절한 수평 및 수직 피개교합을 이룩하였다. 최근에 많이 사용되는 구치부 반대교합이 없는 경우에서의 급속구개확장은, 진단학적 기반 및 적응증에 관한 재평가가 필요하다고 생각된다.

교정 진단을 위한 피개교합심도지수 (Overbite Depth Indicator, ODI) 의 새로운 평가 (A NEW APPROACH TO USE OVERBITE DEPTH INDICATOR IN ORTHODONTIC DIAGNOSIS)

  • Since 1984, many patients have been treated with Multiloop Edgewise Archwire (MEAW) Technique and diagnosed with ODI (Overbite Depth Indicator) and APDI (Anteroposterior Dysplasia Indicator) by the authors. 234 samples of them were selected randomly for the statistical analysis (age, sex, Angle's classification, treatment period, extraction, ODI etc.). Especially, ODI was analysed statistically and its application methods were reviewed. The results and conclusions were as follows: 1. On the 150 patients with normal overbite, the mean values of Class I, II, III malocclusion were $67.5^{\circ}$, $72.2^{\circ}$ and $59.0^{\circ}$. They were significantly different on the level of p < 0.01. 2. In normal overbite samples, ODI decreased with the increase of APDI and the correlation coefficient was -0.54. It seems that this result reflects the characteristics of AB to mandibular plane angle. 3. The regression equation was Y = - 0.57X + 114.64, where X is APDI and Y is ODI. In cases of small or large APDI, it seems to be absurd that the patient's ODI is compared with the mean ODI to differentiate diagnostically the open bite or deep bite tendency from the normal.

간접 골성 고정원을 이용한 골격성 III급 부정교합의 절충 치험례 (Class III nonsurgical treatment using indirect skeletal anchorage: A case report)

  • 성인에서의 골격성 III급 부정교합의 치료의 원칙은 하악골의 후방이동을 동반한 수술적 교정치료이나, 다양한 문제로 인해 수술적 방법을 선택하기 어렵고 부조화의 정도가 심하지 않은 환자의 경우 비수술적 절충 치료를 선택할 수 있다. 비수술적 절충치료를 시행하는 경우 교정치료의 한계와 치주적인 부분에 대한 주의가 필요하다. 본 교실에서는 간접 골성 고정원을 이용하여 성인에서의 골격성 III급 부정교합을 비수술적으로 절충 치료 하였기에 보고하고자 한다.

Clinical Validity of Tooth Size Measurements Obtained via Digital Methods with Intraoral Scanning

  • Purpose: Dental diagnostic records derived from study models are a popular method of obtaining reliable and vital information. Conventional plaster models are the most common method, however, they are being gradually replaced by digital impressions as technology advances. Moreover, three-dimensional dental models are becoming increasingly common in dental offices, and various methods are available for obtaining them. This study aimed to evaluate the accuracy of the measurement of dental digital models by comparing them with conventional plaster and to determine their clinical validity. Materials and Methods: The study was conducted on 16 patients' maxillary and mandibular dental models. Tooth size (TS), intercanine width (ICW), intermolar width (IMW), and Bolton analysis were taken by using a digital caliper on a plaster model obtained from each patient, while intraoral scans were manually measured using two digital analysis software. A one-way analysis of variance test was used to compare the dental measurements of the three methods. Result: No significant differences were reported between the TS, the ICW and IMW, and the Bolton analysis through the conventional and two digital groups. Conclusion: Measurements of TS, arch width, and Bolton analysis produced from digital models have shown acceptable clinical validity. No significant differences were observed between the three dental measurement techniques.
