Comparing locus of control in orthodontic patients of a university dental hospital with those of a private dental clinic. (치과의원과 대학치과병원 교정환자의 통제소재 특성)

  • Objective: Many factors are considered when a patient chooses the type of dental institution they wish to receive treatment from. Numerous studies have been conducted regarding this issue. However, more research is needed in analyzing the patient's psychological aspects when he/she decides to choose the dental institution they wish to receive treatment from. Methods: A locus of control test was utilized to analyze the psychological aspects in orthodontic patients of a university dental hospital and a private dental clinic. The locus of control test is known to be useful in predicting a human being's behavior. Both intrinsic and extrinsic locus of control tests were carried out in 934 orthodontic patients attending Seoul National University Dental Hospital and 1466 orthodontic patients from a private dental clinic. Results: The orthodontic patients at the university dental hospital showed a stronger extrinsic locus of control compared with those from the private dental clinic. Conclusion: The orthodontic patients at the university dental hospital value the doctor as the authority figure and the treatment quality more than those at the private dental clinic.

Strategic surgical-combined orthodontic treatment planning of patient with missing incisors on maxilla: a case report (상악 전치부 결손 환자의 수술을 동반한 전략적 교정치료 증례)

  • Proper positioning of maxillary incisors is key to success of surgery combined treatment. Establishing surgery plan would be a difficult job if maxillary incisors are lost. Patient who lost all of her maxillary incisors due to accident came for orthodontic treatment. Through careful modification of maxillary archform, pre-surgical orthodontic treatment was conducted with four prosthetic space consolidation. Position of incisors was decided by help of 3D prosthetic set-up, and 1-jaw surgery was planned. After relative short treatment period of 28 months, final prosthesis was done. When alveolar bone loss happens, harmonious prosthesis of upper incisors is difficult. Utilizing mandibular set-back surgery and incisor positioning using 3D set-up could make a better environment for treatment outcome. Strategic pre-surgical orthodontic treatment can allow shorter time and less number of prosthetics.

  • The Objective of this research was to construct the multimedia database system that was necessary for the education of the practitioners and the students. To establish this system, there were technical problems as follows 1) The processing of the images, 2) The rapid processing of the information with the computer network, 3) The development of diagnosis tools, 4) The technique to establish the database system, 5) The link between the database system and the internet, and 6) The method to educate through many cases. The analysis for diagnosis and treatment planning were provided as two parts : model analysis and cephalometric analysis. As a model analysis, arch length discrepancy and Bolton tooth ratio were provided for the part of patient information. Cephalometric analyses were provided in the part of initial diagnosis. The Cephalometric analysis ver 2.0 and the PowerCeph pro 3.3.5 were used to show Tweed, Steiner and Jarabak analysis. In the main part, Kim's analysis and some measurements were added. THE STUDY OF ARCH DIMENSIONAL CHANGES BEFORE AND AFTER ORTHODONTIC TREATMENT IN ANGLE CLASS I MALOCCLUSION CASES (Angle씨 I급 부정교합 환자에서 교정치료 전, 후의 악궁크기 변화에 관한 연구)

  • The purposes of present study were to identify possible relationships between post-treatment changes and post-retention changes. The patient's models were composed of 58 samples, and were classified non-extraction group (30 samples) and extraction group (28 samples). For each sample the first models were taken prior to the start of treatment, the second models just after the end of treatment, and the third models two years after. The results were as follows: 1. In the cases of non-extraction group, increases were in intermolar width of maxilla, interbicuspid width of maxilla and arch perimeter of mandible during treatment period, but decreases were in the same measurements during post-retention period. 2. In the cases of extraction group, decreases were in intermolar width of mandible, interbicuspid widths of maxilla and mandible, arch length of mandible, arch perimeters of maxilla and mandible during treatment period. 3. Significant decreases were in the irregularity index of both extraction and non-extraction group during treatment period. 4. There were significant differences of arch dimensional changes in intermolar widths of maxilla and mandible, interbicuspid widths of maxilla and mandible, arch lengths of maxilla and mandible, arch perimeters of maxilla and mandible between non-extraction and extraction group.

  • The symphyseal mandibular fractures due to accidents happened in form of collapsed transeverse arch and multiple teeth loss. And the collapsed transverse arch in mandible occurs with unilaterally or bilaterally. So that patient needs to recover arch width. Conventional approaches, however, we difficult to get appropriate transverse arch correction. Distraction osteogenesis is a unique form of clinical tissue engineering and biologic process of new bone formation between bone segments that are gradually separately by incremental traction. Distraction osteogenesis is considered that great potential for correcting transverse mandibular deficiencies. Tn this paper, distraction osteogenesis applied to patients who had a unilateral or bilateral collapsed arch width in mandible. But it was necessary secrutinize consideration about periodontal conditions, biomechanical vectors, TMJ adaptations, and neuromuscular change during distraction osteogenesis period.

  • This case report describes the treatment of a 66 year old adult patient with a diagnosis of severe obstructive sleep apnea who was intolerant of nasal continuous positive airway pressure (nCPAP) treatment and oral appliance therapy. An alternative treatment of snoring and obstructive sleep apnea (OSA) with 2 orthodontic microimplants anchored to the mandible providing skeletal anchorage for mandibular advancement was implemented. After a 2 week healing period, a custom designed facemask provided extraoral anchorage to which the microimplants were connected to for titratable mandibular advancement. Microimplant Mandibular Advancement (MiMA) therapy resulted in resolution of the symptoms of severe OSA with a reduction of the apnea-hypopnea index (AHI), snoring and OSA symptoms.

THE CASE REPORT OF THE SKELETAL ANGLE'S CLASS II MALOCCLUSION WITH THE UPPER CENTRAL INCISOR MISSING (상악 중절치 결손을 동반한 골격성 II급 부정교합 환자의 치험예)

  • The causes of the missing teeth are classified as congenital missing, trauma and extraction due to dental caries, variable problems are occured clinically by the missing teeth. The missing of the upper incisors especially would assume a serious aspect, and could be treated by three methods of orthodontic treatment, prosthodontic treatment and autotransplantation of the premolar teeth. The patient of this report had the skeletal class II malocclusion with the left upper central incisor missing, and have been treated with the fixed appliance after extraction of the right upper central incisor and both lower second premolars. The results were obtained as follows: 1. Treatment was done for 1 year 6 months. 2. Normal overbite and overjet were achieved. 3. Cuspal interdigitation was obtained normally. 4. Space problem was resolved with resin restoration of the upper lateral incisors. 5. The upper canines were used as the upper laterals after cuspal contouring. 6. Retention would be required with adequate retainers for a long time to prevent relapsing after treatment.

A CEPHALOMETRIC STUDY ON THE SOFT TISSUE PROFILE CHANGES BY ORTHODONTIC TREATMENT IN FEMALE PATIENTS (여자 부정교합자의 치료전후 연조직 측모 변화에 관한 두부 방사선학적 연구)

  • This study was undertaken to investigate soft tissue profile changes by orthodontic treatment in female patients. Traditional cephalometric appraisal yields data of dubious scientific value, the soft tissue profile forms were evaluated by finite element method. The subject was divided into three groups according to Angle's classification and each group was composed of 25 female patients averaged aged 12-14 years at the start of treatment. The changes in soft tissue form were evaluated by computing the degree of distortion in each triangle after treatment compared with the triangle before treatment. The conclusions were as follows; 1. The soft tissue profile forms were evaluated by finite element method and independent evaluation of each element by local changes was possible. 2. Maximum and minimum principal strains showed marked variability depending on the particular finite element and each group and Class II, III sample was greater than Class I sample. 3. Soft tissue size changes as a result of orthodontic treatment was not related to those of shape. 4. Soft tissue changes by orthodontic treatment were variable in individual patient, and were not related to Angle's classification.

  • The purpose of this report is to present the cause and treatment of posterior open bite. Posterior open bite is the open bite limited to posterior teeth. These problem usually are attributed to a mechanical interference with the eruption process, either ankylosis or some soft tissue interference. But, in some patient, lateral open bite is due to a disturbance of the eruptive mechanism itself and other disease or side effect following other treatment. Distinguising cause of posterior open bite, lack of eruption due to some external interference with eruption or primary failure of eruption mechanism, is important clinically because this determines the prognosis for orthodontic treatments. The characteristics, differential diagnosis with posterior open bite due to other etiologies and the treatment of these problems are presented and discussed,.

Magnetic resonance imaging appearance of foreign-body granulomatous reactions to dermal cosmetic fillers

  • Foreign body granulomas can develop after the injection of various cosmetic filling materials into the facial area to flatten wrinkles. Clinically, reactive lesions are easily mistaken for soft-tissue neoplasms or cysts. This report presents a case of foreign body granuloma in a 52-year-old female patient complaining of a painless swelling in the nasolabial region. Both clinical and histological features are described, underscoring the diagnostic role of magnetic resonance imaging findings.
