부정교합 치료기간의 예측에 관한 연구 (A STUDY HO THE PREDICTION OF DURATION OF ORTHODONTIC TREATMENT IN MALOCCLUSION)

  • To predict duration of orthodontic treatment of malocclusion, 304 male and female patients whose orthodontic treatment was finished in department of orthodontics, Infirmary of dental college, Chosun University were studied. The duration of treatment was studied according to types of malocclusion, beginning ages of treatment, Hellman's dental ages, the determination of tooth extraction, types of tooth extraction and types of orthodontic appliance. The following results were obtained. 1. The duration of treatment was 18 months in Class I malocclusion, 20 months in Class II malocclusion and 24 months in Class III malocclusion. 2. The more early treatment was done, the more duration of treatment was needed according to beginning age of treatment and Hellman's dental ages. 3. Treatment of tooth extraction case was needed for 78 days more than that of non-extraction case. 4. The duration of treatment with unilateral extraction of premolars was the shortest (598 days) and that with single arch extraction of premolars was the longest (685 days) according to types of tooth extraction. 5. The duration of treatment by removable appliance was the shortest (237 days) and the combination of removable appliance, headgear and full banded appliance was the longest (1425 days) according to types of orthodontic appliance. 6. The duration of treatment was 18 months in typical orthodontic treatment with 4 extraction of premolar and full banded appliance.

Outcomes of comprehensive fixed appliance orthodontic treatment: A systematic review with meta-analysis and methodological overview

  • Objective: The aim of this systematic review was to assess the occlusal outcome and duration of fixed orthodontic therapy from clinical trials in humans with the Objective Grading System (OGS) proposed by the American Board of Orthodontics. Methods: Nine databases were searched up to October 2016 for prospective/retrospective clinical trials assessing the outcomes of orthodontic therapy with fixed appliances. After duplicate study selection, data extraction, and risk of bias assessment according to the Cochrane guidelines, random-effects meta-analyses of the mean OGS score and treatment duration were performed and 95% confidence intervals (CIs) were calculated. Results: A total of 34 relevant clinical trials including 6,207 patients (40% male, 60% female; average age, 18.4 years) were identified. The average OGS score after treatment was 27.9 points (95% CI, 25.3-30.6 points), while the average treatment duration was 24.9 months (95% CI, 24.6-25.1 months). There was no significant association between occlusal outcome and treatment duration, while considerable heterogeneity was identified. In addition, orthodontic treatment involving extraction of four premolars appeared to have an important effect on both outcomes and duration of treatment. Finally, only 10 (39%) of the identified studies matched compared groups by initial malocclusion severity, although meta-epidemiological evidence suggested that matching may have significantly influenced their results. Conclusions: The findings from this systematic review suggest that the occlusal outcomes of fixed appliance treatment vary considerably, with no significant association between treatment outcomes and duration. Prospective matched clinical studies that use the OGS tool are needed to compare the effectiveness of orthodontic appliances.

치열교정환자의 투명 교정장치에 대한 지식 및 자가관리행태 (Knowledge and self care behaviors of orthodontic patients using clear aligner)

  • Objectives : The purpose of the study is to investigate the knowledge and self care behaviors of orthodontic patients using clear aligner. Methods : A total of 181 orthodontic patients using clear aligner in Daegu filled out the self-reported questionnaire from September 6 to October 18, 2010. and 162 subjects completed orthodontic treatment. Results : There existed a significant difference in knowledge and self care behaviors between gender and wearing duration of clear aligner(p<0.01). Treatment duration influenced significantly on the score of self care behaviors(p<0.01) and the high score of self care behavior showed in short duration of treatment. Conclusions : Self care behaviors were closely correlated with treatment duration. An information for the clear aligner will be helpful to improve the knowledge and self care behaviors.

A comparative study of combined periodontal and orthodontic treatment with fixed appliances and clear aligners in patients with periodontitis

  • Purpose: With the increasing prevalence of orthodontic treatment in adults, clear aligner treatments are becoming more popular. The aim of this study was to evaluate the effect of orthodontic treatment on periodontal tissue and to compare orthodontic treatment with fixed appliances (FA) to clear aligner treatment (CAT) in periodontitis patients. Methods: A total of 35 patients who underwent orthodontic treatment in the Department of Periodontology were included in this study. After periodontal treatment with meticulous oral hygiene education, patients underwent treatment with FA or CAT, and this study analyzed patient outcomes depending on the treatment strategy. Clinical parameters were assessed at baseline and after orthodontic treatment, and the duration of treatment was compared between these two groups. Results: The overall plaque index, the gingival index, and probing depth improved after orthodontic treatment (P<0.01). The overall bone level also improved (P=0.045). However, the bone level changes in the FA and CAT groups were not significantly different. Significant differences were found between the FA and CAT groups in probing depth, change in probing depth, and duration of treatment (P<0.05). However, no significant differences were found between the FA and CAT groups regarding the plaque index, changes in the plaque index, the gingival index, changes in the gingival index, or changes in the alveolar bone level. The percentage of females in the CAT group (88%) was significantly greater than in the FA group (37%) (P<0.01). Conclusions: After orthodontic treatment, clinical parameters were improved in the FA and CAT groups with meticulous oral hygiene education and plaque control. Regarding plaque index and gingival index, no significant differences were found between these two groups. We suggest that combined periodontal and orthodontic treatment can improve patients' periodontal health irrespective of orthodontic techniques.

External root resorption after orthodontic treatment: a study of contributing factors

  • Purpose : The purpose of this study was to examine the patient- and treatment-related etiologic factors of external root resorption. Materials and Methods : This study consisted of 163 patients who had completed orthodontic treatments and taken the pre- and post-treatment panoramic and lateral cephalometric radiographs. The length of tooth was measured from the tooth apex to the incisal edge or cusp tip on the panoramic radiograph. Overbite and overjet were measured from the pre- and post-treatment lateral cephalometric radiographs. The root resorption of each tooth and the factors of malocclusion were analyzed with an analysis of variance. A paired t test was performed to compare the mean amount of root resorption between male and female, between extraction and non-extraction cases, and between surgery and non-surgery groups. Correlation coefficients were measured to assess the relationship between the amount of root resorption and the age in which the orthodontic treatment started, the degree of changes in overbite and overjet, and the duration of treatment. Results : Maxillary central incisor was the most resorbed tooth, followed by the maxillary lateral incisor, the mandibular central incisor, and the mandibular lateral incisor. The history of tooth extraction was significantly associated with the root resorption. The duration of orthodontic treatment was positively correlated with the amount of root resorption. Conclusion : These findings show that orthodontic treatment should be carefully performed in patients who need the treatment for a long period and with a pre-treatment extraction of teeth.

Patients' and parents' concerns and decisions about orthodontic treatment

  • Objective: Patients' and parents' expectations are important in orthodontic treatment decision making. The literature generally demonstrates the perceived benefits of orthodontic treatment, but patients' and their parents' concerns about orthodontic treatment have not been investigated comprehensively. The aim of this study was to identify patients' and parents' concerns about orthodontic treatment and compare them according to sex, age, and treatment demand level. Methods: One hundred and eighty-nine children and their parents were interviewed about concerns related to orthodontic treatment. Patients and parents were asked about orthodontic treatment decisions. Answers were recorded as "yes," "no," or "don't know." Chi-squared and Fisher's exact tests were used to compare concerns between age groups, sexes, and treatment demand levels. Kappa statistics were used to assess agreement between patients and their parents. Results: Concerns about orthodontic treatment were gathered under 10 items as follows: "feeling pain," "the appearance of braces," "being teased," "avoiding smiling," "speech problems," "dietary changes," "problems with transportation," "economic problems," "long treatment duration," and "missing school." There was no statistically significant difference in concerns between the sexes or age groups. Some concern items and treatment demand were inversely related in patients. Conclusions: The results of this study demonstrate patients' and parents' concerns about orthodontic treatment. Differences between the concerns of patients with different treatment demands imply that children might reject orthodontic treatment because of their concerns. Appropriate consultation of patients addressing their concerns may help reduce anxiety and improve the acceptance of treatment.

Effects of self-ligating brackets and other factors influencing orthodontic treatment outcomes: A prospective cohort study

  • Objective: The purpose of this study was to evaluate the effects of self-ligating brackets (SBs) and other factors that influence orthodontic treatment outcomes. Methods: This two-armed cohort study included consecutively treated patients in a private practice. The patients were asked to choose between SBs and conventional brackets (CBs); if any patient did not have a preference, he or she was randomly allocated to the CB or SB group. All patients were treated using an identical archwire sequence. Evaluated parameters were as follows: treatment duration, number of bracket failures, poor oral hygiene, poor elastic wear, extraction, use of orthodontic mini-implants (OMI), OMI failure, American Board of Orthodontics (ABO) Discrepancy Index (DI), arch length discrepancy, and ABO Cast-Radiograph Evaluation (CRE) score. Stepwise regression analysis was performed to generate the equation for prediction of the CRE. Results: The final sample comprised 134 patients with an average age of 22.73 years. The average DI, CRE, and treatment duration were 21.81, 14.25, and 28.63 months, respectively. Analysis of covariance showed a significant difference in CRE between the CB and SB groups after adjusting for the effects of confounding variables. Stepwise regression analysis using four variables, namely extraction, SB use, poor elastic wear, and additional appliance use, could explain only 25.2% of the variance in the CRE. Conclusions: Although the CRE was significantly better for CBs than for SBs, the clinical significance of this result seems to be limited. Extraction, SB use, poor elastic wear, and additional appliance use may have significant effects on treatment outcomes.

구개측으로 매복된 상악견치의 교정치료기간에 영향을 미치는 요소 (Factors that influence treatment duration for patients with palatally impacted maxillary canines)

  • 구개측으로 매복된 견치와 인접구조물 간의 위치관계가 전체적인 교정치료기간에 어떤 영향을 미치는지 알아보고자 편측성 구개측 견치 매복으로 진단받고, 교정적인 견인을 하여 치료를 완료한 환자 36명의 초진 시 파노라마 방사선사진을 대상으로 연구하였다. 전체 치료기간의 평균인 21개월을 기준으로 나누었을 때, 장기치료군에 비해 단기치료군에서 인접 측절치의 치근에 견치의 치관이 겹치는 정도가 더 크게 나타났다. 치료기간을 예측하기 위한 중회귀분석에서 상악견치의 치관에서 치조정까지의 교합면에 대한 수직거리를 나타내는 HCV (Height of the canine crown vertically)와 하악견치간폭경이 각각 0.142와 0.115의 예측상수$(r^2)$를 보였으며, 두 인자를 같이 고려하였을 경우에는 0.208의 예측상수를 보였다. 나이에 따른 분류에서 치료기간, 상악 견치 치관에서 치조정까지의 수직거리(HCV), 중절치 정중선에 대한 견치의 각도가 초기 영구치열기까지는 점진적으로 감소를 하다가 완성 영구치열기에서 증가하였다. 본 연구의 결과, 구개측으로 매복된 견치의 견인을 시행할 때에는 초기 영구치열기에 견치의 치관첨이 교합면에 가깝고 직립이 되어 있을 경우에 전체 교정치료 기간이 짧아진다는 것을 알 수 있었다.

A posteriori registration and subtraction of periapical radiographs for the evaluation of external apical root resorption after orthodontic treatment

  • Purposes: This study employed a posteriori registration and subtraction of radiographic images to quantify the apical root resorption in maxillary permanent central incisors after orthodontic treatment, and assessed whether the external apical root resorption (EARR) was related to a range of parameters involved in the treatment. Materials and Methods: A sample of 79 patients (mean age, $13.5{\pm}2.2years$) with no history of trauma or endodontic treatment of the maxillary permanent central incisors was selected. Periapical radiographs taken before and after orthodontic treatment were digitized and imported to the Regeemy software. Based on an analysis of the post-treatment radiographs, the length of the incisors was measured using Image J software. The mean EARR was described in pixels and relative root resorption (%). The patient's age and gender, tooth extraction, use of elastics, and treatment duration were evaluated to identify possible correlations with EARR. Results: The mean EARR observed was $15.44{\pm}12.1pixels$ (5.1% resorption). No differences in the mean EARR were observed according to patient characteristics (gender, age) or treatment parameters (use of elastics, treatment duration). The only parameter that influenced the mean EARR of a patient was the need for tooth extraction. Conclusion: A posteriori registration and subtraction of periapical radiographs was a suitable method to quantify EARR after orthodontic treatment, and the need for tooth extraction increased the extent of root resorption after orthodontic treatment.

Distalization with a modified C-palatal plate for severe upper crowding and a missing lower incisor

  • This case report presents the orthodontic treatment of a 25-year-old patient with skeletal Class II and severe maxillary arch crowding, moderate mandibular arch crowding, anterior crossbite, and a missing lower incisor. He was treated with molar distalization using a modified C-palatal plate and temporary anchorage devices to create sufficient space for retraction. The total treatment duration was 21 months. After treatment, his occlusion and smile esthetics showed significant improvement. The modified C-palatal plate represents a treatment modality that enhances the prospects of non-extraction treatment and reduces the need for extraction.
