Study on Analysis of Nose Shape from the viewpoint of Inspection Diagnosis (망진(望診)에서 비(鼻) 형태(形態)의 중요성 연구)

  • There are two methods in inspection diagnosis(望診), the viewing shape(觀形) and the inspecting color(察色). Acquiring proper analysis from viewing shape(觀形) requires several standards of shape that can be compared with. We can identify major defects and flaws of the shape from comparing it with the standards that is defined as very normal shape. Also, It can eventually provide us interpretation of health related meanings of the objective shapes. Those standards need symmetrical and proportional details. In this research, I put some physiognomy and inspection diagnosis related documents, and standards for restorative plastic surgery together to suggest standards on viewing shape diagnosis of nose which is one of the most important spot in facial inspection. It is expected to improve delicacy of Oriental medine's inspection diagnosis, as the result of the comparison between plastic surgery and inspection diagnosis.

Breast Reconstruction make use of Contralateral Breast Tissue after Mastectomy (유방절제술 후 반대편 유방조직을 이용하여 유방 재건한 증례)

  • Purpose: Unilateral breast reconstruction after mastectomy confront the challenges of recreating a natural appearing breast mound and achieving symmetricalness of the breasts. If the patient's remaining breast is large compared to reconstructed breast, the most common procedure is reduction mammoplasty of the large breast side. The authors experienced a new method of breast reconstruction using the excess breast tissue from the contralateral breast after breast reduction. Methods: The excess tissue from the contralateral breast after vertical reduction mammoplasty with superior pedicle and remaining lower breast tissue were transferred to the mastectomy site breast through the subcutaneous tunnel on the chest wall. The main blood supplies of the flap are perforator branches of the 4th, 5th and 6th anterior intercostal artery. After elevating and detaching the flap on the lower lateral area of the breast, the turn overed flap is fixed on the upper portion of the chest wall of the mastectomy site. Results: On two cases of the breast reconstructions, remaining excess breast tissue from reduction mammoplasty was transferred to the contralateral breast side as pedicles. Both patient and operator were satisfied with the outcome of the reconstruction as the breasts were symmetrical and natural shape. Conclusion: We have performed unilateral breast reconstruction using the excess breast tissue after reduction mammoplasty of the contralateral breast. As Oriental women's breasts are relatively smaller than that of Caucasian women, delayed breast reconstruction cases of Oriental women with large breasts(macromastia) seem to be ideal for this procedure.

Correction of the Epicanthal Fold with Invisible Scar (눈에 띄는 흉의 생성 없는 내안각췌피 교정술)

  • The epicanthal fold is common natural finding in Asian eyes. It is very common that patients, who look for oriental blephaloplasty for double eye lid, request correction epicanthal fold. It is very often difficult to achieve satisfactory results if the correction of the epicanthal fold is not corrected concomitantly. Many authors described surgical procedures for correction of epicanthal folds. But most of them leave additional scars which tend to be hypertrophic and noriceable. We achieved satisfactory result in epicanthoplasty with invisible scar, using combination of procedures, such as upward incision, double eyelid operation, anchoring suture of the medial upper lid skin to the medial canthal tendon, trans-nasal root subcutaneous mattress suture of the epicanthal fold itself and combined rhinoplasty. For past six years (1998 to 2003) 17 patients have been operated with one of these mentioned procedures. The mean follow up was 4months (2weeks to 6months). All patients were satisfied and no major complication was noted. This method can be one of the effective procedures for correcting the Asian epicanthal fold for avoiding potential visible scar and ancillary procedure in double eye lid blephaloplasty.

The Changes of Bone and Soft Tissue after Maxillary Anterior Segmental Osteotomy and Advancement Genioplasty (상악전방분절절골술과 턱끝전진술 후 안면골격과 연부조직의 변화)

  • Purpose: Mid and lower facial convexity is more common in Oriental people than in Caucasian. Bimaxillary dentoalveolar protrusion is characterized by procumbent teeth, protruding lips, acute nasolabial angle, gummy smile, receding chin, facial convexity. Especially, pure maxillary dentoalveolar protrusion is less frequent than bimaxillary dentoalveolar protrusion. Therefore, it is important to make an accurate decision for the operation throughout the history taking, cephalogram, dental cast to arrive at accurate diagnosis and surgical plan. Methods: From December 2002 to June 2004, ten patients with maxillary dentoalveolar protrusion and microgenia were corrected by maxillary anterior segmental osteotomy and advancement genioplasty. 10 patients were analyzed by preoperative and postoperative clinical photography, posteroanterior and lateral cephalograms. Results: No major complications were occurred throughout the follow-up period except one of the over-recessed, otherwise most of the patients were satisfied with the result. Conclusion: We could correct the occulusal relationship with teeth and improve lower facial profile, asthetically and functionally, by maxillary anterior segmental osteotomy and advancement genioplasty.

  • One of the most common cosmetic surgery, the blepharoplasty can be divided two method. there were incisional method and sutured method. Authors try to understand the anatomic difference between the oriental upper eyelids and apply the surgical techniques of constructing upper eyelid crease of oriental case effectively. And we discussed the postoperative complications of the blepharoplasty and proposed the solve of these problems. The sutured method of blepharoplasty was more simple technique than conventional incision method and had low occurrence of complications. Even though slight recurrence, suture method was more esthetic due to no scar formation.

Transareolar-Perinipple Dual Pockets Breast Augmentation (횡유륜 유두주위절개를 통한 이중포켓 유방확대술)

  • Purpose: Many options are available for the incision and pocket selection in breast augmentation. Each method has its advantages and disadvantages. To leave an invisible operation scar and to achieve easier pocket dissection by the central location of the incision on the breast, we made a transareolar-perinipple incision. To overcome the disadvantages of the transareolar incision, originally advocated by Pitanguy in 1973, we modified the direction of incision line and dissection plane. Methods: To avoid the injury of 4th intercostal nerve responsible for nipple sensation, we made perinipple incision on the medial side of the nipple instead of trans-nipple incision and made the transareolar incision as 11-5 o'clock on the left side and 1-7 o'clock on the right side instead of 3-9 o'clock on both sides. To avoid the possible infection and breast feeding problem caused by the injury to the lactiferous duct, and the possible implant hernia caused by the incisions lying on a same plane of pocket dissection, we made a subcutaneous dissection just above the breast tissue medially down to the bottom of breast tissue and made a subglandular or subfascial pocket, which may avoid the injury of lactiferous duct and create different planes for skin incision and pocket dissection. Other advantages of the transareolar-perinipple incision include easier pocket dissection, less chance of hematoma, and as a result less postoperative pain because of the central location of the approach which allow finger dissection and meticulous bleeding control with direct vision, without any specialized instrument such as an endoscope or long mammary dissectors. As for pocket selection, we made dual pockets. We prefer subglandular or subfascial pocket. Also, we made a subpectoral pocket in the upper 1/4 of the pocket to add more volume on the upper part of the augmented breast, which can make aesthetically more desirable breasts in thin Asian women with small breasts. Possible disadvantages of our method are subclinical infection and scar widening, which could be overcome by meticulous operation techniques, antibiotic therapy, and intradermal tattooing. Results: From September, 2003 to August, 2005, 12 patients underwent breast augmentation using round smooth surface saline implants by our method. During the mean follow-up period of 13 months, there were no complications such as infection, hematoma, capsular contracture, and sensory change of nipple, and results were satisfactory. Conclusion: We suggest breast augmentation via transareolar-perinipple incision and dual pockets(subpectoral-subglandular or subfascial) as a valuable method in thin oriental women with small breasts.

Oriental-western Cooperative Treatment Clinical Sudy of Side Effect after Filler Injection Glabella Region (필러시술이후 발생한 부작용의 한.양방 병행치료 1례)

  • Objective : The purpose of this study is to report the oriental-western cooperative treatment on opthalmic and dermatologic disease caused by adverse effect of filler injection galbella region. Method : After filler injection, a patient suffers from spontenous blineness and palsy of oculomotor and trochlear nerve on right eye and erythema on glabella and nasal region. She is admitted to kyung-hee oriental hospital for 2 weeks with oriental(herbal-medicine, acupunture and nega treatment) and western(department of opthalmology, dermatology and plastic surgery measurement) treatment. Result & Conclusion : Nerve palsy was recovered near to normal. And skin legion was proceeded to recovery period without any complication. This recovery speed with oriental-western cooperative treatment is much faster than usual treatment.

REDUCTION MALARPLASTY WITH BONE OSTECTOMY & REPOSITIONING ONLY USING INTRAORAL APPROACH (구강내 접근법만을 통한 골절단술과 재위치술을 이용한 협골 축소술)

  • The zygoma is protruded on both sides of the midface and plays an important part in determining the impression of face. There are much different esthetic consideration of zygomatic prominence between the oriental and western. Because slight prominent zygoma is thought to be esthetic in the western, there has been done many augmentation therapy. But, in the oriental, prominent zygoma, especially in the female, is thought to be aggresive and manlike image, there has been done many zygoma reduction therapy. There are two methods to reduce the prominent zygoma, one is shaving method and the other is zygoma repositioning with ostectomy. The approach to zygoma and zygomatic arch without surgical morbidity is very difficult, so this problem has been difficult subject to oral & maxillofacial surgeon in a long period. We report the treatment case of facial asymmetry with the unilateral zygoma reduction & genioplasty using only intraoral approach with the retrospective study.

Calcific Tendinitis of the Shoulder by Oriental Medical Treatments : 4 Cases Report (한방치료로 호전된 견관절의 석회화 건염 4례 보고)

  • This study was performed to report the effect of oriental medical treatments on the 4 patients with calcific tendinitis of the shoulder. The 4 patients were treated by cupping, acupuncture, pharmacopuncture on tender points of the greater tuberosity of humerus. The state of patients was measured by visual analog scale(VAS) and shoulder range of motion(ROM). After mean 3.25 times of treatments, the significant recovery of shoulder ROM was measured and the significant pain relief was seen as the average of VAS changed from 8.5 to 1. These results suggest that oriental medical treatments including cupping, acupuncture, pharmacopuncture are effective to improve the calcific tendinitis of the shoulder. However more rigorous study should be followed.

A PHOTOMETRIC STUDY OF THE FACIAL PROFILES OF KOREAN ADULT WOMAN (한국 성인 여성의 안모에 대한 실물 사진 계측학적 연구)

  • The balance of facial sop tissue is important factor for the treatment of maxillofacial deformity patients. There are many cephalometric analysis of facial bone but a few soft tissue analysis of face for the orthognathic surgery, especially in the oriental people. This study aimed at examining the mean value of the soft tissue measurement of 120 Korean adult women with the linear and angular measurement and comparing with Caucasian. The results ere as follows. 1. In the frontal linear measurement, Korean women have larger interocular distant and width of nose, but the width of mouth appeared less than Caucasian. 2. In the frontal angular measurement, Korean women have larger inclination of the eye than Caucasian. 3. In the lateral linear measurement, Korean women have more prominent upper and lower lip position than Caucasian and have longer upper facial height comparing with lower facial height than Caucasian. 4. In the lateral angular measurement, Korean women have more convex facial profile and less nasolabial and geniolabial angle. 5. In the lateral facial ratio, Korean women have larger proportion in the lower department below the stomion.
