CDSS를 이용한 한방 U-Healthcare 서비스 모델 구현 (A Implementation of Oriental Medicine U-Healthcare Service Model Using CDSS)

  • 고도화된 의료 서비스 발전으로 인해 유비쿼터스 헬스 케어(Ubiquitous Healthcare)의 실현이 가속화되고 있다. 이러한 배경에 따라 헬스 케어 서비스 모델이 제시되고 있고, 여러 가지 환경과 조건에 따라 그에 부합한 다양한 헬스 케어 서비스 모델이 연구되고 있다. 이에 발맞춰 세계 의료 시장은 서양 의학의 절대 우위시대를 벗어나 자연 친화적인 전통 의학이 각광 받는 시대로 재편되고 있다. 하지만 이러한 세계 의료 시장의 추세에도 불구하고 국내의 U-Healthcare 시장은 전통 의학에 적합한 서비스 모델이 부족한 실정이다. 이로 인해 국내의 전통 의학인 한방 분야에서 효율적으로 운영이 가능한 한방 U-Healthcare 서비스 모델의 제시가 요구되는 실정이다. 따라서 본 논문은 한방 분야에 효율적으로 적용 가능한 헬스케어 서비스 모델로, 퍼지 룰 기반 방법을 적용한 CDSS 처리에 의해 환자의 한방 측정 데이터를 분석하고 이를 효과적으로 진단할 수 있는 방법을 제안하였다. 분석결과 한방 분야에서는 제안 모델이 기존 양방 모델에 비해 보다 적합하고 장기간 운영 시 양방에 비해 높은 효과가 있다는 것을 알 수 있었다. 향후 연구방향으로 제안 모델의 부족한 부분인 데이터 표준 및 보안에 대해서 추가적인 연구가 필요할 것으로 사료된다.

한방의료 만족도에 대한 문헌적 고찰 (A literature study about the satisfaction with oriental medical services)

  • Objectives This study aimed to analyze and summarize the satisfaction with oriental medical services in korea. Methods From 3 Korean databases (National Assembly Library, Korea Education Research Information Sharing Service, and National Discovery for Science Leaders), published between 2001 and 2011, we were obtained 13 studies that involved the satisfaction of oriental medical services. Results We found that the kindness of doctors and other staffs was significant determinant of satisfaction with oriental medical services. The positive recognition about treatment effect and the preference to combined medical system were also influenced to satisfaction with oriental medical services. Generally it was necessary for evaluation of satisfaction to improve facilities and environments of hospitals. Conclusions The satisfaction with oriental medical services would be related with the kindness of doctors and other staffs, the recognition about treatment effect, the preference to combined medical system and environments of hospitals. Also to confirm the reasons for the satisfaction with oriental medical services, further studies should be conducted using the highest methodological standards.

한방산업의 정의와 분류에 대한 연구 (A research on the Korean medicine industry of define and classification)

  • 1. To define 'Korean medicine industry' through study on existing medicine related industries, Korean medicine industry means all industrial activities related to Korean medicine. It covers material resources such as herbs and products made with herbs, medical instruments, Korean medical service and related information service based on Korean medicine theories. 2. According to Korea National Statistical Office standard industrial branch, Korean medicine industry was classified as a large branch. There were industries such as agriculture, food and beverage manufacture, publishing, copy of prints and record media, manufacture of compound and chemical products, medical service, manufacture of precise optical instruments, wholesale trade and product mediation, retail trade, restaurant, research and development, education service, health preservation service, entertainment, culture and sports industry related to the Korean medicine industry. 3. If we classify this according to the industry branch of English economists Clark, Colin Grant, herb cultivating industry will be classified as primary industry, manufacture of foot and beverage related to Korean medicine, secondary industry and wholesale and retail sales of herb, research and development, education, health preservation, social welfare, tertiary industries.

공중보건한의사의 효율적인 활용방안에 대한 보건소장의 인식 및 태도 (Health Center Director's Cognition and Attitude on the Strategies for Utilizing Oriental Public Health Doctors)

  • The proportion and role of public sector in health care industry is very small in Korea. Asymmetric distribution of health care resources is one of the major health care concerns. This issue is so important that it raises a question of accessibility, availability, continuity of care and equity of rural area people's health care utilization. To solve these problems and to satisfy the basic demand of oriental medical service in rural areas, the oriental public health doctors were placed in rural health centers since 1998. The main objectives of this study are twofold: to measure the cognition and attitude of health center directors on the strategies for utilizing oriental public health doctors and to provide basic data for improving the health manpower management program. Data have been collected by way of the self-administrative questionnaires. Developing the questionnaire, the literature review on the previous studies and delphi method were carried out. The response rate was 38.7%. The results of this study are summarized as follows; 1. community people respond positively on the oriental medical service activity in health center. 2. In regard to workloads of oriental public health doctor, 'appropriate' was 81.1% and 'burdensome' was 18.2%, respectively. 3. The 94.0% of respondents thought that the oriental medical service will be continued. 4. To activate oriental medical service in health center, the sufficient budget and provision of aid workers is a necessity. 5. The 75.5% of health center directors respond positively on the allocation of oriental public health doctor to health sub-centers. 6. Health center directors agreed that oriental public health doctor should perform the clinical service as well as prevention and health promotion activity. These results recommend that oriental medical service in health center should be continued gradually, and oriental public health doctors working at health center perform their work efficiently. Undoubtedly, their activity should be more focused on health promotion and disease prevention than daily patient care. For achieving this objective, more support of governmental policy is essential for utilizing oriental public health doctor and better health of the rural area community people.

최근 3년간 대구 달성군 보건소 한방진료실의 진료현황에 대한 통계적 연구 (The Statistical Analyses of Oriental Medical Office in a Public Health Center of Dalseong-gun, Daegu Metropolitan City During Recent 3 years)

  • Objective : We investigated the actual condition of oriental medical office in a public health center. Methods : We classified patients who visited an oriental medical office iin a public pealth center of Dalseong-gun, Daegu Metropolitan City during recent 3 years by KCPJMAIN(data processing system), according to location, age, time(month, year), sex, disease, insurance, etc. Results : As a local category, the residents in Hyeunpung-Myeon higher than 80% of the patients who visited the public health center. The patients aged over 60 occupied 80% of the patients. The frequency of the treatment was more than 10 in those patients aged over 60. The number of the patients was the highest in May when there was a change for the past 36months, whereafter the number has sustained decline for 3-5 months and repeated decreasing. As a gender category, the female inpatients were 10732(82%). This statistics shows that these musculoskeletal system disease occupied large part in them. As a heath insurance category, the patients who were insured by health care were 12454(96.30%). Conclusion : It should need to enable the rural residents who have difficulty benefitting from medical service to reach the service by making their access to the oriental medical office in public health center easier. Most of all, the support from both government and municipality should be urged to accomplish it. plus, it should be included not only boosting doctors' reliance but also improving the capability and services of doctors in public heath center. In conclusion, the treatment service in public health center should be diversified beyond musculoskeletal system disease and the identity transform of public health center should be needed to appeal to young generation.

한국인 수면장애 환자의 최근 3년간 한방 진료 양태 (Oriental Medical Treatment Pattern of Korean Patients with Sleep Disorders)

  • Objectives: Though there are many studies about sleep disorder, no research has been performed on the utilization of oriental medicine as a treatment. Therefore, the oriental medical treatment pattern of Korean patients with sleep disorders was examined herein using the Health Insurance Review and Assessment Service (HIRAS). Methods: The medical records of patients with sleep disorders (G47) or nonorganic sleep disorders (F51) as a main diagnosis were adopted from the HIRAS database from 2011 to 2013. Analysis was performed on the number of patients and cost per patient, with comparison between oriental and western medicine in terms of gender, age, patient care service type, and hospital type. Results: 1) Regarding sleep disorders, the medical visits and insurance charges have been increasing. Western medicine was utilized 8 times more often than oriental medicine during 3 years. 2) There were 2.5 times more women than men. 3) Among all ages, the 50~59 year group had the highest representation. 4) In comparison of average portions of patient care type over 3 years, outpatients were the majority, while the number of visits of outpatients and hospitalization has been increasing. 5) Comparison of average portion of oriental hospital type over 3 years revealed oriental clinics to be used most. The use of general hospitals was higher in western medicine treatment, while public health centers used oriental medicine more. 6) Regarding average oriental medical cost per patient over 3 years, the total was 88,000 won, with 353,000 won for hospitalization and 85,000 won for outpatients. The outpatient cost has been increasing. 7) In line with 6, oriental medical hospitals cost 126,000 won, local clinics were 85,000 won, and etc. was 95,000 won. Average costs of all types have increased during 3 years, except oriental medical hospitals in 2013. Conclusions: This study provided objective information about the epidemiologic characteristic of oriental medicine used for treatment of sleep disorder. For expansion of oriental medical demand for sleep disorder, this study would be helpful in understanding the recent status.

현대사회의 발병인자에 대한 고찰 (A study on Etiologic factors in modern society)

  • In spite of the development of scientific and medical technology, the rate of prevalence and death of the chronic disease gets higher. Therefore people come to know prevention is more important than treatment in disease. In Western, it is nearly after 1970's when it is revealed that the major factor of chronic disease comes from habitual behaviors in everyday life. But that is common idea in Oriental Medicine since thousand years. In Oriental Medicine, the main cause of disease is composed of natural factor including climate condition, living-life factor, psycho-social factor, constitutional factor, inbody-pathologic-substance factor, and other factors including habits of using drug and medical service, injury, environmental pollutions, and occupational etiologic factors. Among these, the major factors of disease in modern society are lifestyle factors like the tendency of choosing and intaking food, the level of physical activity and labor, smoking and drinking alcohol, psychological tendency, the habits of using drug and medical service and so on. This is a practical aspect of oriental medicine where the most important etiologic factor comes from human behaviors, not from external factors like germ or virus. So that etiology in Oriental Medicine where improvement in the way of life is needed to prevent disease would play an important role in modern society.

한방의료 이용에 대한 문헌적 고찰 (A literature study about utilization of oriental medical services)

  • Objectives : This study aimed to analyze and summarize the utilization of oriental medical services in korea. Methods : From 3 Korean databases (National Assembly Library, Korea Education Research Information Sharing Service, and National Discovery for Science Leaders), published between 2001 and 2011, we were obtained 17 studies that involved the utilization of oriental medical services. Results : We found that women, elderly patients, and patients with low education levels and those with musculoskeletal and cerebrovascular diseases preferred to use oriental medical services. Other variables such as trust and kindness of staff and positive perceptions of the treatment effect were found to be significant determinants for utilizing oriental medical services. Conclusions : The utilization of oriental medical services would be related with gender, age, education levels and diseases. Also to confirm the reasons for patients to choose oriental medical services, further studies should be conducted using the highest methodological standards.

뇌졸중 환자의 양${\cdot}$한방 협진에 대한 연구 (A Study on Cooperative Treatment with Both the Western and Oriental Medical Department in C.V.A patients)

  • The primary purpose of this study was to provide the basic materials for C.V.A inpatients' actual use of medical service at the hospital with both the western and oriental medical department and the attitude on cooperative treatment. The results of this study were as follow: First, the subjects' general characteristics including job, monthly mean income and age made statistical differences to their pathological characteristics such as part of primary paralysis, detailed name of disease and cause of elicitation. Second, their general characteristics including religion and job produce statistical difference to their actual use of medical service, like medical institution form, term of treatment and type of medical institution at first-aid. Third, through the awareness of cooperative treatment system, the effect of C.V.A treatment and the shorten of the C.V.A treatment term were higher at oriental medical department inpatients and cooperative treatment serviced inpatients than western medical department inpatients and cooperative treatment non-serviced inpatients. Fourth, the biggest problem on current dual medical system is increase medical expenses and the biggest reason on not vitalized cooperative service is prejudice of both parts.

Determinants of Inpatients Satisfaction and Intent to Revisit Oriental Medical Hospitals

  • Objectives: This study aimed to investigate the determinants of inpatients satisfaction and intent to revisit oriental medical hospitals. Methods: The independent variables of the study contain the level of satisfaction with doctors, nurses, staffs, medical fee, environment, facilities and hospital life. Each variable was measured by three to eight items about the level of satisfaction with various aspects of hospital ward life. The level of overall patient satisfaction was used as an intervening variable and the level of intent to revisit was used as a dependent variable. The sample used in this study consisted of 268 inpatients from 3 oriental medical hospitals located in Chungnam Province. Data were collected with a structured and self-administrated questionnaire and analysed using path analysis. Results: The major findings of the study were as follows : First, it was found that the 3 variables of hospital life satisfaction, ancillary staffs satisfaction, and metropolitan residents has significant positive effect on the level of overall patient satisfaction. Second, the overall satisfaction and the satisfactory level of facilities, medical fee, and quality of ward life were found to have significant effects on the level of intent to revisit of respondents. Conclusions: The results of the study indicate that oriental medical hospitals should make an effort to improve the overall satisfaction of inpatients, especially focusing on the facilities convenience, medical price, and hospital ward life which will lead to high level of intent to revisit of inpatients.
