Symptomatic Indications of Chongpesagan-tang according to Objective Analysis of Clinical Usage in Oriental Medicine Hospital - In Case of Stroke Patients - (태음인(太陰人) 청폐사간탕(淸肺瀉肝湯)의 응용예를 통한 증후분석(證候分析) - 중풍환자를 중심으로 -)

  • In Oriental medicine, the subjective process of diagnosis often makes it difficult to explain the indications objectively, in spite of its excellent therapeutic value. The authors' objective Was to present more objective indications of Chongpesagan-tang(淸肺瀉肝湯), a prescription predominantly used to treat stroke in Oriental Medicine, We analyzed clinical symptoms of 105 patients who were hospitalized in the Department of Circulatory Internal Medicine, College of Oriental Medicine. Kyung Hee University, Seoul, who were given Chongpesagan-tang and improved without side effects, The symptoms analyzed included general symptoms which are usually checked in the process of Oriental Medicine Diagnosis: the so called Four Diagnosis(四診). We rearranged and selected symptoms which were predominantly found among other symptoms. We found some symptoms of which the patients frequently complained: sweating 84%, red tongue 74%, white tongue coat 65%, constipation 35%, tight pulse 21%, headache 20%, thirst 20%, chest discomport 17%, etc, We believe that these suggest an objective indication of Chongpesagantang.

  • Objectives : This cliniclal study was to evaluate the effect of Cheongsangsahwatang (淸上瀉火湯) treatment on the headache after subdural hemorrhage. Methods : We treated two patients who have headache after subdural hemorrhage by oriental medical therapy, specially Cheongsangsahwatang (淸上瀉火湯). We used VAS(Visual Analog Scale) to investigate the effect of oriental medical therapy, specially Cheongsangsahwatang (淸上瀉火湯). Reasults : After prescription of Cheongsangsahwatang (淸上瀉火湯), VAS scores on headache and accompanying symptoms decreased. Conclusions : This study suggests that Cheongsangsahwatang (淸上瀉火湯) is effective in the treatment of headache after subdural hemorrhage.

Study on Practical Prescription and It's Drug Composition for the Treatment of Diabetic Nephropathy (당뇨병성신병증(糖尿病性腎病證)의 활용 처방 및 약물에 관한 연구)

  • Based on the theories and clinical data of oriental medicine, traditional medicinal prescriptions known to be effective to diabetic nephropathy were collected. The prescriptions were carefully examined and analyzed in order to be used as fundamental material for experiments and in clinicals. More than 40 publications related to diabetes were arranged and analyzed. Of the complications, diabetic nephropathy part of the publications were specifically focused during analysis. Data were analyzed and classified according to the quantity, prescription, differentiation of symptoms, signs and addition and subtraction of each medicine. Frequently used medicines were statistically analyzed. The most frequently used prescription was Yukmigihwang-tang(六味地黃湯) based medicine and prescriptions of its addition and subtraction, which was 15% of the total prescriptions cited. The most frequently used medicine was Astragali Radix, mentioned 192 times in the prescriptions. Hoelen was next with 180 citations, followed by Dioscoreae Rhizoma with 147 times, Rehmanniae Radix with 140, Corni Fructus with 131, Salviae Miltiorrhizae Radix with 106, Angelicae Gigantis Radix with 101, Alismatis Rhizoma with 95, Rhei Rhizoma with 90, Atractylodis Rhizoma Alba with 84, Rehmanniae Radix Preparata with 78, Leonuri Herba with 74, Moutan Cortex Radicis with 66, Aconiti Lateralis Preparata Radix with 66 Cnidii Rhizoma with 65, Pseudostellaria heterophylla with 62, Liriopis Tuber with 55, Lycii Fructus with 52, Rhei Rhizoma with 49, Codonopsis Pilosulae Radix with 44, Paeoniae Radix Rubra with 44, Schisandrae Fructus with 42, Polyporus with 42, Achyranthis Radix with 41 and Euryales Semen with 40. Medicines prescribed more than 30 grams a day included Astragali Radix, Imperatae Rhizoma, Benincasae Pericarpium, Leonuri Herba, Dioscoreae Rhizoma, Plantaginis Semen, Polyporus, Hoelen, Halloysitum Rubrum, Achyranthis Radix, Arecae Pericarpium, Phaseoli Angularis Semen, Coicis Semen, Rhei Rhizoma, Salviae Miltiorrhizae Radix, Sargassum, Ostreae Concha, Pseudostellaria heterophylla, Epimedii Herba, Rehmanniae Radix, Scrophulariae Radix and Polygonati Rhizoma. No scientific reports on the traditional medicinal aspects of diabetic nephropathy was searched. This analysis report would be able to provide the basis of developing new drug candidates for the treatment of diabetic nephropathy, as well as securing the EBM for the traditional medicines already being prescribed to the patients.

A Literature Review of the Acupuncture and Moxibustion Methods in UiRimCh'walYo ("의림촬요(醫林撮要)" 침구법(鍼灸法)의 의사학적(醫史學的) 고찰(考察))

  • Objectives : Investigate the situation of Joseon Dynasty's Acupuncture and Moxibustion. Methods : the Acupuncture and Moxibustion methods in UiRimCh'walYo (${\ulcorner}$醫林撮要$\lrcorner$ 'Essentials of Oriental Medical Doctors') were studied. Results and Conclusions : First of all, the Acupuncture and Moxibustion methods in UiRimCh'walYo are narrated according to specific symptoms like diarrhea and stomachache, and not organized by the origin of illness or some abstract nature of a disease. In addition, it excluded complicated Acupuncture and Moxibustion methods, only adopting a couple of Acupuncture Bleeding methods and Moxibustion methods to simplify the technique as much as possible. Secondly, the Acupuncture and Moxibustion methods in UiRimCh'walYo, along with those in DongUiBoCam and Ch'imGuYoGyol introduce ways to perform moxibustion on the Umbilical Middle and Elixir Field. By focusing on the similarities between the three comtemporary medical works, it is possible to assume the existence of a unique acupuncture method using moxibustion on the Umbilical Middle and Elixir Field. Thirdly, the Acupuncture and Moxibustion methods in UiRimCh'walYo didn't go into detailed differentiation of symptoms and just concisely described one or two treatment methods for each symptom and simplified the prescription down to the core acupuncture points. This shows that the Acupuncture and Moxibustion methods in UiRimCh'walYo didn't seek its own cure but was edited in order to act as an appendix to Herbal Medicine. When integrating the conclusions above, it can be said that UiRimCh'walYo strived to use acupuncture and moxibustion simply and effectively inside the Oriental medicine's large boundaries of Herbal Medicine and Acupuncture and Moxibustion. Harmony with Herbal Medicine, an easy-to-find organization, and simple, easy-to-do prescription are what UiRimCh'walYo was about.

Varied Flavonoid Contents of Citri Unshii Pericarpium Extracted from Several Traditional Herbal Prescriptions (진피의 복합처방에 따른 유효성분 추출율의 변화)

  • Objecitves: Peel of Citrus unshiu (Rutaceae) has been used in traditional herbal prescriptions. Hesperidin and naringin contents of Citri Unshii Pericarpium were determined and compared to herbal prescriptions. Then, the different extractions from prescriptions are explained with traditional herbal theories. Methods: The herb and herbal prescriptions were extracted in boiled water. A quantitative analysis was made of hesperidin and naringin using HPLC with-reversed-phase CIS column and a UV detector at 204nm. Elution was carried out at 1.0ml/min with $38\%$ methanol. Results and Conclusions: The extract of Citri Unshii Pericarpium for 180 min was the better method for extracting naringin and hesperidin. The flavonoid contents in remedies are different among several traditional herbal prescriptions. The reason for the difference can be explained with traditional drug interaction theories.

The effects of Hagochosan(HGCS) on hyperlipidemia in rats induced by high cholesterol diet (하고초산(夏枯草散)이 고지혈(高脂血) 유발(誘發) SHR 백서(白鼠)에 미치는 영향(影響))

  • In order to investigate the effect of HGCS on Hypercholesterolemia in rats diagnosed with hypertension, changes of related factors in blood and tissues of animal models were studied. The results are as following: At 250 ${\mu}g/m{\ell}$ or lower doses of HGCS, human fibroblast cells showed a survival rate of more than 90%. HGCS significantly reduced the total cholesterol and LDL cholesterol levels in blood observed in hypercholesterolemia rats. On the other hand, HGCS signicantly increased the blood HDL cholesterol level, compared to that of the control group. HGCS significantly reduced triglycerides caused by hypercholesterolemia. However, glucose level was decreased insignificantly. Significant decrease of hyperoxidative lipid level in tissues due to hypercholesterolemia was observed in HGCS treated group. Significant increase of SOD and catalase activities in tissues due to hypercholesterolemia were observed in HGCS treated group. From the results above, the anti-hypercholesterolemic effect of clinically used HGCS has been experimentally proved. More active prescription of HGCS to hypercholesterolemia and related patients should be available in the future.

  • Objective Medication is one the most important things in treatment for children, but prescribing traditional herbal medicine for them is very difficult. Therefore, we made 4 new forms of oriental herbal medicine that has a better taste, smell, and color. These new forms also have a texture that make medicine easier to chew. Methods We made up a question of 20 children visiting $\bigcirc\;\bigcirc$ university hospital. Results and Conclusions 1. Children preferred liquid or pill type of herbal medicine. 2. The preference sensory test of the traditional medicine and new form of medicine had not much difference. 3. On the test for preferences, it resulted as they liked the 4th form of new medicine which was with little bit more of sweet and sour taste, and the 3rdformwithnewcolors. 4. They hated traditional herbal medicine of the taste, smell, color, sensation of texture of material.

Anti-apoptotic Effect of Bojungbangam-tang Ethanol Extract on Cisplatin-Induced Apoptosis in Rat Mesangial Cells

  • Cisplatin is a anti-neoplastic agent which is commonly used for the treatment of solid tumor. Cisplatin activates multiple signal transduction pathways involved in the stress-induced apoptosis in a variety of cell types. Cytotoxicity of cisplatin was detected in rat mesangial cells and the value of $IC_{50}$ is about 20 ${\mu}M$. The treatment of cisplatin to rat mesangial cells showed the apoptotic bodies and DNA fragmentation. The activation of caspase-3, -8, and -9 and proteolytic cleavage of PARP were observed in the rat mesangial cells treated time-dependently with cisplatin. The activation of ERK, p38 and JNK was also observed in the apoptosis induced by cisplatin in rat mesangial cells. The ethanol extract of Bojungbangam-tang (EBJT), a new hergal prescription composed of nine crude drugs, inhibited cisplatin-induced apoptosis in rat mesangial cells. EBJT reduced sub-G1 peak (apoptotic peak) in cisplatin-treated rat mesangial cells. The cisplatin-induced ERK and JNK activation in rat mesangial cells were blocked by EBJT, but EBJT had no effect on p38 activation. Taken together, these results con suggest that EBJT prevents cisplatin-induced apoptotic cell death in rat mesangial cells through inhibition of ERK and JNK activation.

Herbal medicine prescription analyses of bronchiectasis patients with claim data during 5 years (2013~2017) (최근 5개년 (2013~2017)간 기관지확장증(J47) 환자에게 처방한 급여한약제제 현황 분석 - 건강보험청구자료 중심으로)

  • Objectives : Korean national health insurance data is a useful real-world data representing whole medical bills submitted to Health Insurance Review Agency. This study aims to understand recent benefit trend of insurance herbal preparations for treating bronchiectasis(disease code J47) utilizing insurance data. Methods : We reviewed national health insurance claims data from 2013 to 2017 which have main diagnosis or sub diagnosis code of J47 and with the record of prescribing insurance herbal medication. Frequency analysis was performed to analyze the most frequently prescribed prescription. Results & Conclusions : Both the number of claims statement(770 to 1,746cases) and patients(266 to 484) of insurance herbal preparations increased considerably from 2013 to 2017. Top 10 preparations based on the number of claims statement were 'Samso-eum', 'Yeonkyopaedok-san', 'Socheongryong-tang', 'Bojungikgi-tang', 'Hyangsapyungwi-san', 'Yijin-tang', 'Saengmaek-san', 'Jaeumganghwa-tang', 'Ojeok-san' and 'Gungha-tang'. Top 10 preparations based on the number of patients were 'Samso-eum', 'Socheongryong-Tang', 'Saengmaek-san', 'Yeonkyopaedok-san', 'Haengso-tang', 'Hyangsapyungwi-san', Yijin-tang', 'Jaeumganghwa-tang', 'Bojungikgi-tang' and 'Hyeonggaeyeongyo-tang' in respectiv order. Claims of top 10 frequent preparations occupied more than 60% of total claims. We hope this finding to be utilized as basic data for future research of evidence-based bronchiectasis treatment utilizing Korean traditional medicine.

Study On Ungi Prescription Of 25 Organic Constitution (운기(運氣) 25체질(體質)의 처방(處方)에 대한 연구(硏究))

  • There were so many discussions about the importance of Ungi theory, but little about its application. But, in Korea, there is particular Ungi medical system. I studied the 25 organic constitutions and their prescriptions and concluded like below. In "Ounyukgihanihakbogam(五運六氣漢醫學寶鑑)", "Ounyukgihak(五運六氣學)", there are 25 organic constitutions, and each of them has its basic prescription. Like above, In "Ounyukgigeummoksuhwatosangsaengsanggeukbeop(五運六氣金木水火土相生相克法)", "Ounyukgigyeongheomcheobang(五運六氣經驗處方)", and "Jesaenglon(濟生論)", there are also 25 organic constitutions, and each has basic prescription. Moreover, there are so many prescriptions for diseases can often occur. I classified these as three groups, "Ounyukgihak(五運六氣學)" is groupⅠ, "Ounyukgihanihakbogam(五運六氣漢醫學寶鑑)" is Ⅱ, and the other books is Ⅲ, and compared each basic prescriptions. GroupⅠand Ⅱ use similar prescriptions, but groupⅢ doesn't. This Ungi medical system has little relation with "Somun(素問)". And I couldn't find the organic constitutions based on Ungi from other country's oriental medical book. 25 organic constitutions have much sameness in prescriptions. These prescriptions are used in real medical system, but not wide. Of course, it needs more research, but it has another position in oriental medicine.
