System Analysis of Disease Classification of Oriental Medicine Diagnosis and Study for Improvement Method (한방진단명의 질병분류체계 분석과 개선방안 연구)

  • Background : To examine the difference between ICD-10 and The Korean standard classification of disease(oriental medicine), and to aim at improve the practical use as statistical data. It is one of the reason of disease classification. On that account we convert the many to many correspondence presenting classification of oriental medicine into many to one correspondence. Method : The study tracked out 155 patients discharged from the university hospital which is located in Gyeonggi Province and managing hospital and oriental medicine hospital from July to October this year. The period of this study was from August 1 to November 18. We compared correspondence between the two services' diagnosis(hospital services and oriental medicine hospital services) at the same time and attempted many to one correspondence classification. That is for production of statistical data. Result : We investigated the group which have had medical treatment experience of two kinds of services at the same time. The result of this investigation was that the same oriental medicine diagnosis used differently in western medicine diagnosis. 44.5% was accorded with western medicine diagnosis. Correspondence of the western medicine diagnose with the top of the Korean standard classification of disease(oriental medicine) list's western medicine diagnosis was 13.5%. For many to one correspondence classification for statistics, one western medicine diagnosis was selected for one oriental medicine diagnosis. In case of the main diagnosis(I sign) was not enough to explain oriental medicine diagnosis' characteristic, we chose multiple other diagnosis, so other diagnosis(II sign) about patient's cause of disease could be selected for supplement after we examined the patient's records. The statistics was possible with this many to one correspondence. Conclusion : The result of this study about correspondence between western medicine diagnoses and those of oriental medicine confirms that The Korean standard classification of disease(oriental medicine) is hard to be standardized with western medicine diagnosis. Therefore, according to this study, we use new many to one correspondence classification, multiple oriental medicine diagnoses with one ICD-10, which can be used by statistical data.

Development and Evaluation of Ontology for Diagnosis in Oriental Medicine (한의진단 Ontology 구축과 평가)

  • The goal of this study is to develop knowledge representation method for the construction and evaluation of ontology for diagnosis in oriental medicine. To develop the expert system for decision making on diagnosis and treatment, the systematic and structural knowledge which can be processible in EMR(Electronic Medical Record) must be precedent, and the Computational Process which control the system as well. This study set up an ontology as a trial model to represent the oriental medical knowledge into the machine processible one. Protege 2.1 has been used to build the ontology, and the serialization format of our ontology is the XML document based on OWL. The components of oriental medical diagnosis was arranged with the combination of symptoms which belong to the certain symptom patterns. Then natural language which expresses the oriental medical diagnosis components were converted into the logical sentence, and individual characteristic symptoms into each values of specific properties. In addition to the study, the diagnosis software for oriental medicine was developed and it used the ontology which we developed. Sequently, we tested the software to confirm the appropriateness of ontology. The result of the test shows that diagnostic questions are automatically formulated according to the diagnosis components of this ontology and that as such diagnostic results are induced. Therefore, the ontology system in this study will be efficient to develop the diagnosis program and useful as a tool for doctors to make decision. But, it is not recommendable to apply the system to the clinical environment until the clear diagnosis standards are introduced, and the more reliable diagnosis program can be developed based on the more appropriate ontology mentioned above.

The Study on Relationship of Dampness-Phlegm Tongue Diagnosis to hyperlipidemia in Stroke Patients (중풍 환자의 습담 설진과 고지혈증의 연관성에 관한 연구)

  • Objectives : This study was aimed to clarify the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients. Methods : We analyzed the data of 1405 patients with acute stroke in 10 oriental medical hospitals from November 2006 to December 2008. We classified patients into two groups, dampness-phlegm and non dampness-phlegm by tongue diagnosis such as the white coating of the tongue, thick coating of the tongue, swollen tongue and teeth printed tongue which is oriental medical diagnosis. And We analyzed their characteristics with type of stroke and lipid blood level. Results : 1. There was no significant difference of stroke type between the dampness-phlegm tongue diagnosis group and the non dampness-phlegm tongue diagnosis group. 2. The ratio of SVO was higher in dampness-phlegm tongue diagnosis group (76.06%) than the non dampness-phlegm tongue diagnosis group (61.26%). 3. According to the blood test, the dampness-phlegm tongue diagnosis group showed higher in total cholesterol, HDL cholesterol, LDL cholesterol and triglyceride than the control group. 4. Total cholesterol and HDL cholesterol were significant higher in dampness-phlegm tongue diagnosis than the control group. Conclusion : According to the analysis, the relationship between the tongue diagnosis of dampness-phlegm and hyperlipidemia in acute stroke patients were more clarified. Based on these results, more prospective studies are to be done with more clinical data.

Researches Trend of Korea Institute of Oriental Medicine in Sasang Constitution Field - from 2007 to 2010 (한국한의학연구원의 사상체질 연구동향 - 2007년부터 2010년까지의 논문을 중심으로 -)

  • Objective : The aim of this study is to investigate current status of Sasang constitutional medicine researches and to provide source in setting a development strategy and making a policy on the Sasang constitutional medicine. Method : This study analyzed trends in research as represented in the 142 articles of Korea Institute of Oriental Medicine in the field of Sasang constitution medicine from 2007 to 2010. Result : The main themes of studies were Sasang constitution clinical study (68, 47%) and developing instruments for measuring diagnosis (48, 33%), especially pulse diagnosis (25, 17%). Genetic researches and trends studies on Sasang constitutional medicine were respectively 17 (12%) and 8 (5%). As a result, Korea Institute of Oriental Medicine established Sasang clinical information bank, and discovered clinical character of Sasang constitution to develope various diagnosis tools include constitution survey form and pulse diagnosis instrument. Genetic researches also discover constitution related genes such as FTO and MC4R, and explore association between heredity and Sasang constitution. Conclusion : The above mentioned achievements of Korea Institute of Oriental Medicine were limited to diagnosis process, unquestionably studies for reproducibility and authenticity of constitution diagnosis were important mission of Sasang constitutional medicine. However, to develop the advance of Sasang constitutional medicine, not only constitution diagnosis but also constitutional treatment studies should be conducted.

Concepts on Appearance Diagnosis and Four HyungSang Types - Fish, Bird, Horse, and Turtle - (장상논(臟象論)과 어조주갑류(魚鳥走甲類)에 대한 고찰)

  • After a research on appearance diagnosis and fish, bird, horse, and turtle types, this study concluded as following. Appearance diagnosis is a technique that exams five jang organs by color and shape of eye, nose, mouth, and ear. Four HyungSang types-including fish, bird, horse and turtle types-are evaluated upon the external shape that are created while type of seed changes. Appearance diagnosis focuses on five jang organs. Four HyungSang types focus on external shape, but are eventually related to five jang organs. Thus, two different techniques possess a point in common; five jang organs. The assignment of nose, eye, ear and mouth to the Five elements varies between two techniques. On the viewpoint of formation, appearance diagnosis assigns them to metal-water-wood-fire-earth and movement, and four HyungSang types do to wood-fire-earth-metal-water and constitution. On the viewpoint of body/use and static/dynamic, appearance diagnosis assigns them to metal-water-wood-fire-earth and constitution, and four HyungSang types do to wood-fire-earth-metal-water and movement. If nose, eye, ear, and mouth are assigned by on four HyungSang types, which are based on external shape, the assignment can be utilized in diagnosis. If, however, they are assigned by appearance diagnosis, it can be focus on treatment. Five jang organs and six fu organs are affected by internal conditions because they are located inside of human body, while four HyungSang types are affected by external conditions because it deals with external shape. If a disease occurs in the developed part of the body, it would be difficult to be cured because four HyungSang types depend on external shapes.

Research on Oriental Medicine Diagnosis and Classification System by Using Neck Pain Questionnaire (경항통 설문지를 이용한 한의학적 진단 및 분류체계에 관한 연구)

  • Objectives : The purpose of this thesis is to help the preparation of oriental medicine clinical guidelines for drawing up the standards of oriental medicine demonstration and diagnosis classification about the neck pain. Methods : Statistical analysis about Gyeonghangtong(頸項痛), Nakchim(落枕), Sagyeong(斜頸), Hanggang (項强) classified experts' opinions about neck pain patients by Delphi method is conducted by using oriental medicine diagnosis questionnaire. The result was classified by using linear discriminant analysis (LDA), diagonal linear discriminant analysis (DLDA), diagonal quadratic discriminant analysis (DQDA), K-nearest neighbor classification (KNN), classification and regression trees (CART), support vector machines (SVM). Results : The results are summarized as follows. 1. The result analyzed by using LDA has a hit rate of 84.47% in comparison with the original diagnosis. 2. High hit rate was shown when the test for three categories such as Gyeonghangtong and Hanggang category, Sagyeong caterogy and Nakchim caterogy was conducted. 3. The result analyzed by using DLDA has a hit rate of 58.25% in comparison with the original diagnosis. The result analyzed by using DQDA has a accuracy of 57.28% in comparison with the original diagnosis. 4. The result analyzed by using KNN has a hit rate of 69.90% in comparison with the original diagnosis. 5. The result analyzed by using CART has a hit rate of 69.60% in comparison with the original diagnosis. There was a hit rate of 70.87% When the test of selected 8 significant questions based on analysis of variance was performed. 6. The result analyzed by using SVM has a hit rate of 80.58% in comparison with the original diagnosis. Conclusions : Statistical analysis using oriental medicine diagnosis questionnaire on neck pain generally turned out to have a significant result.

Computational Methods for Traditional Korean Medicine : A survey (한의 정보의 계산적 방법 조사)

  • Traditional Korean Medicine (TKM) has been actively researched through various approaches, including computational methods. This paper aims at providing an overview of domestic studies using the computational techniques in TKM field. A literature search was conducted in Korean publications using OASIS system, and major studies of data mining in TKM were identified. A review was presented in six diagnosis fields, including sasang constitution diagnosis, eight constitution diagnosis, tongue diagnosis, pattern diagnosis for stroke, diagnosis based on ontology, diagnosis for cause of disease. They collect clinical data themselves for experiments and primarily applied a algorithm of decision tree, SVM, neural network, case-based reasoning, ontology reasoning, discriminant analysis. In the future, there needs to identify which algorithm is suitable to diagnosis or other fields of TKM.

Traditional Korean Medicine Diagnosis System Based on Basic Ontology (기초 온톨로지 기반 한의 진단 시스템)

  • We in this paper design and implement a traditional korean medicine diagnosis system based on basic ontology. If doctors put the symptoms or tongues or pulses of a patient in the diagnosis system, they can be recommended for the diagnosis results. To support the doctors decision, the diagnosis system make the inference based on the basic ontology and compute the similarity between symptoms of patient and those of ontology. The diagnosis systems also provide the learning mechanism about diagnosis results which save the results in the ontology and reuse them in the next diagnosis. Thus, doctors can share their knowledge for the diagnosis by exchanging their ontology each other. In future, we will expand the knowledge of the basic ontology continuously so that doctors can get the more accurate diagnosis results. We also implement the prescription function and integrate it to the diagnosis system.

  • Modern Objectification of Pulse Diagnosis, One of the Four Diagnosis Method of Oriental Medicine, is necessarily project to improving Oriental medical clinic status and quality by standardization of diagnosis database. At that, accurate measurement equipment or devices(sensor, system and instruments, etc,) to exactly detect MacSang(脈診 : the parameter and subject of pulse diagnosis) have not developed yet. Existing Pulse diagnosis devices are not satisfy clinical needs and medical equipments quality. We study for pulse diagnosis system, that CD is satisfying oriental medical clinic needs, is ensuring accuracy and reappearance to support in clinical diagnosis and treatment, is guaranteeing the quality of medical equipments. theoretical base and convenience.

A Comparison of Controlled and Uncontrolled Hypertension Groups Regarding Comprehensive Diagnosis of Qi Blood Water and Quality of Life (고혈압 환자에서 혈압 조절 여부에 따른 기혈수(氣血水) 변증(辨證)과 삶의 질 비교)

  • This study is about a comparison of controlled and uncontrolled hypertension groups regarding comprehensive diagnosis of Qi blood water and quality of life. We surveyed "controlled and uncontrolled hypertension patients" using questionnaires for comprehensive diagnosis of Qi blood water, SF-36 and HTN QoL (Measurement Scale for the quality of life in hypertensive patients). There was no difference in comprehensive diagnosis of Qi blood water between the controlled and uncontrolled hypertension groups. Within the controlled hypertension group, the patients diagnosed with a Qi deficiency, Qi stagnation, Qi counterflow, blood deficiency, and water retention received lower total scores in SF-36 and HTN QoL than in undiagnosed patients. Within the uncontrolled hypertension group, the patients diagnosed with Qi deficiency, blood deficiency, and water retention got lower total scores in SF-36 and HTN QoL than in undiagnosed patients. These results were statistically significant. These results are insufficient that we and use comprehensive diagnosis of Qi blood water for a diagnosis tool of hypertension. But if we have better studies that make up for weak points, these results will help to make a diagnosis tool for hypertension.
