한방신경정신과 영역의 Alzheimer형 치매 관련 연구현황 (The Current Status about Alzheimer's Dementia in the Journal of Oriental Neuropsychiatry for Evidence Based Medicine)

  • Objectives: Alzheimer's Dementia is somewhat common in clinical stages, and there are many reports and papers regarding this subject. We explain the present state of Alzheimer's Dementia in JON (The Journal of Oriental Neuropsychiatry) for the benefits of clinical practice. Methods: We searched for articles in JON from 1990 to 2014 and selected ones that were related to Alzheimer's Dementia; then, we analyzed the data and placed it in four categories, like experimental study, case study, Clinical Data-Analysis study, and clinical trial. Results: 1) We found 47 articles relating to Alzheimer's Dementia in JON from 1997 to 2014: 41 experimental studies, 1 case study, 1 Clinical Data-Analysis study, and 4 Clinical trials. 2) There were Chung-kyung Acupuncture Method, Auricular acupuncture, and Cha-rak Acupuncture Method to treat Alzheimer's Dementia. 3) There were many additional treatments, like aroma therapy. 4) In the experimental study, they reported on several herbs, including herbal prescriptions that affected the suppression of related mRNAs and genes. 5) There were 4 clinical trials regarding herbal prescription that was effective for treating Alzheimer's Dementia. Conclusions: In JON, regarding Alzheimer's Dementia, the majority dealt with experimental studies. Therefore, we also have to expand our sight into other fields of study. We need more clinical trials and case studies for the treatment of Alzheimer's Dementia in Oriental Neuropsychiatry.

한방신경정신과 영역의 수면장애 관련 연구현황 (The Current Status about Sleep Disorder in the Journal of Oriental Neuropsychiatry for Evidence Based Medicine)

  • Objectives : Sleep Disorders are very common in clinical stages and there are many reports and papers regarding this subject. I try to understand the present state of sleep research in JON (The Journal of Oriental Neuropsychiatry) for the benefits of clinical practice. Methods : We searched and read every article on JON from 1990 to 2012 and selected ones that are related to Sleep Disorder; then analyzed the data to 5 categories, like literature study, case study, Clinical Data-Analysis study, experimental study, and clinical trial. Results : 1. We Found 41 articles relating to sleep disorder in JON form 1999 to 2012, and there were 9 literature studies, 13 case studies, 13 Clinical Data-Analysis studies, 4 experimental studies, and 2 clinical trials. 2. There were Sa-Am Acupuncture Method, Pharmacopuncture, electroacupuncture, and Auricular acupuncture to treat Sleep Disorder. 3. There were many additional treatments like Purgative Therapy, etc. 4. They reported the ordinary sleep pattern of Soeumin and Soyangin, too. 5. In the experimental study, they reported several herbs, herbal prescription on the expression of melatonin receptors, and Punsimgeimgamibang on insomnia, anxiety, emotionality in rats. 6. There were 2 clinical trials, one for Hwabyung-patient with insomnia by acupuncture treatment and another for insomnia in the elderly by ETF-I program. 7. There were 2 case of other sleep disorder (enuresis, sleepwalking) and 2 literature study of dream. Conclusions : In JON regarding sleep disorder, the majority was due to insomnia. Therefore, we also have to expand our sight into other sleep diseases. We need more clinical trials and experimental researches for the construction of EBM Sleep Disorder in Oriental Neuropsychiatry.

한.중 간호교육제도 및 교육과정 비교연구 (Comparative Study on Nursing Education System of Korea and China)

  • This study is a descriptive comparative investigated one to analyze nursing education system and curriculum of Korea and China trans-culturally. 1) Education System The basic level of nursing education in Korea consisted of 65 3-year- junior college of nursing (7379 students) and 48 4-year-bachelor of nursing college (2345 students) in 2000 showing more 3-year-junior college of nursing and its students. In China, western nursing as well as Chinese nursing education system were operating. In 2000, 501 western school of middle technical nursing, 29 school of middle technical nursing of middle level education, and 89 3-year western and 24 Chinese junior college of nursing, and 42 4-year western bachelor of nursing college and 10 Chinese of high level education have been established. The presence of Chinese school of middle technical nursing system seemed to be in slower development in nursing than Korea, but that of Chinese nursing education seemed to be advanced with its national identification prior to Korea. Post graduate continuous education for RN-Diploma and RN-BSN program has been opened as in Korea. The Hosa(護士) system in school of middle technical nursing in China reflects lower level of education than Korea. But it can be a merit, other than in Korea, without nurses aids, when they are acting under supervision of nurses and led by them, and there presents a special course for promotion up to high level education. Graduate school in Korea is divided into general type opening a curriculum for MS in 1960 and as of 2000, 21 general types for majoring in nursing. The PhD course in Korea was established in 1978, and after that the PhD courses have been opening in 14 universities at present. China established master degree course in 1991 and as of 7 colleges are ongoing, and the doctoral course is now under planning, resulting slower development than Korea. 2) Education of theory and clinical practice in Korea and Chine (1) Korea's 3-year junior nursing college have 51 subjects, 49 subjects in China, which was not different. China was following education of ideology and medical. 4-year Bachelor of Science College has 92 subjects in Korea with cultural subjects and essential major studies/elective in theory education in Korea, while 63 subjects in China, showed wider selection in Korea's education. (2) Korea's 3-year and 4-year nursing colleges performed clinical practice education parallel with theoretical education for a certain period, block or theory/practice system. While China's 3-year and 4 or 5-year-colleges educated the theory first and then practice for one year in the last grade, integrating each situation of the departments and the theory. (3) Korea's oriental nursing theory in nursing education was performed in 28 colleges of 65 nationwide ones of 3-year junior nursing colleges, but only one school was educating clinical practice. In 4-year bachelor of nursing colleges, the oriental nursing theory was done in 14 among 48 investigated. And 1-4 subjects of them were doing, and 4 schools performed of clinical practice, showing more reinforced than the junior colleges. China's 3-year and 4-5-year western nursing colleges taught two subjects of Chinese medicine nursing. China's 3-year & 4-5-year College of Chinese medicine nursing, theory of Chines medicine nursing education taught eight subjects. (4) 5-year colleges of Chinese medicine western integrated nursing, theory of Chinese medicine nursing education consisted of twelve Chinese medicine nursing subjects and two of Chinese medicine western integrated nursing subjects. China was tempting a new development of a pattern of Chinese medicine nursing subjects reinforced. 3) The verification of Korean and Chinese nurse's license. The verification test of Chinese nurses license is differentiated at the level of education other than in Korea. Expire date is 2 years and a qualified test must be done to a renewal. And the continuing education hours are 72 per year, which is more enforced than Korean nurses (10 hours a year). In accordance with WTO regulations, we should prepare for opening foreign hospital, educating oriental nursing subjects. And on this, it is recommendable to settle a basic frame research to run the oriental nursing practice ongoing. 1. It is desirable to develop the oriental nursing subjects to apply its idea to the western nursing and differentiate Korean nursing. 2. It is desirable to certify oriental nurse's characters, to expand and develop the nursing areas to practice it, and to establish the oriental nursing system. 3. It is expectable to promote Korean nursing specialization to develop the oriental nursing as a professional and to create its demand.

피로를 호소하는 외래환자에 대한 임상적 관찰 (Clinical Investigation of Fatigue Among Outpatients)

  • Objectives: Fatigue is a common symptom experienced by many people who visit Oriental medical clinics or hospital. However, there has been little study about the fatigue in the Oriental medical academic world. For this reason, we attempted to investigate the present status of fatigue of outpatients, and its relation with Health Practice Index(HPI). Methods: The subjects were 63 outpatients who visited the Tonification Clinic in Kyunghee Oriental Medical Center between January 1, 2001 and July 31, 2001. Their chief complaint was fatigue and they did not have any physical or mental problem. They were given a questionnaire which included questions reflecting general characteristics, fatigue degree and health habits. We measured degree of fatigue by Chalder scale et al. Health habits were investigated about 5 articles out of 'Breslow 7 Health habits'. Results: Among the subjects, 48 people(76.2%) were considered as 'fatigue patients' by the Chalder scale. Of this 48 fatigue patients, 27 people(56.3%) had manifested fatigue for more than 6 months. The average of scale II for all the patients was 14.05, which indicates moderate degree of fatigue. They complained fatigue, drowsiness and general weakness, dryness and discomfort of the eyes, headache, shoulder pain and neck stiffness, dizziness, heat in the upper part of the body, and poor concentration. There were no differences in degree of fatigue according to Health habits including exercise or not, smoking or not, the frequency of drinking, hours of sleeping, and body mass index. Conclusions: Many people complain fatigue symptom. Therefore doctors should have more interest in fatigue and care. This study can provide standards of prognosis of fatigue patients. Also prospective studies are needed to find relationship between health habits and fatigue degree.

사상의학(四象醫學)이 가진 한국(韓國) 한의학적(韓醫學的) 특성(特性)에 대한 고찰(考察) (A Study on Korean Medical Properties of Sasang Constitutional Medicine)

  • Sasang constitutional medicine was born on basis of constitutional cognition in Korean History. Only if we look at Sasang constitutional medicine on the environment of korean unique culture, geography and historical tradition, the true feature come into view, and on the contrary we could grasp to which degree it has general properties. Sasang constitutional medicine pursue inner principles of human being, that has been the most important traditions of Korean medicine. Also that properties are results of continuous confirming academic universality and constant efforts to look for the medicine that is most fit to clinical practice. For the purpose of developing world medicine, we must grow that specific characteristics of Sasang constitutional medicine.

국립한의대 설치 필요성에 대한 일반인의 인식도 조사 (General Survey on the Necessity of Establishment School of Oriental Medicine at the National University Level)

  • Subjects in this study comprised of general public (907), high school students (772), individuals associated with Oriental medicine (660), and 60 majoring in western medicine, totalling 2,413 individuals. Survey was conducted on the necessity of establishing Oriental medicine school at the national university level and the following result were obtained: - 78.3% (1847 individuals) were in favor of establishing Oriental medicine school at the national university level. - For the validity of establishment, responders expressed opinions of standard and virtuous education, higher quality education, standardized practice, research on difficult to cure diseases, and obtaining competitiveness in the world market. - One to three schools were considered as an appropriate number of schools with less than 80 students per class, Class size may be adjusted from existing schools (52.5% favored decrease in size) and (46.3% favored increase in size). - Educational and research facilities must be accopanied with schools of Oriental medicine as well as clinical training facilities, herbal pharmaceutical research centers, and fundamental medical centers. - Many favored 6 year curriculum as the most appropriate system and the school of Oriental medicine should be established within the university. Based on the information gathered in this survey, we may recognize the limitations of Oriental medicine schools at the private institutional level and support the establishment of Oriental medicine schools at th national university level. This establishment may play as a steeping stone for advancement in education, standardization of research and treatment, and commercialization of Oriental medicine of benefit the general public.

한방처방비법등의 수집분석방법 및 자료제공자등의 권리에 대한 법적보장방안연구 (A Study of Method about Gathering-analysis, of Legal Guarantee Device about Offerer Rights in Oriental Herbal Prescription-mystique)

  • Objectives : First, to restore reliability of oriental prescriptions that are effective against incurable diseases, Second, in the short term, to secure the validity of traditional remedies with the help of accumulated data and in the long term, develop new therapeutic methods and prescriptions in order to broaden its aspect in the field of medicine. Third, to maintain predominance on oriental therapeutic methods and put it into practical use based upon the results obtained by this paper. Methods : In the primary sub-thesis(herbalogical analytic methods applied on unknown oriental prescriptions), models of analytic method and in gathering information that could be put into practice are being revised In the secondary sub-thesis(method in gathering and analysing in dealing with oriental prescription), several statistical approaches and analysis on data that has been gathered are being revised. In the tertiary sub-thesis(research on legal guarantee of the offerer rights), an alternative scheme that covers the limitations of the legislative content in dealing with offerer rights is being revised Results : This research has revealed several problems, including those which were foreseen, in proceeding with the project. The prospect of the involvement of the medical personnel engaging in the field of oriental medicine was remarkably bright. Given the condition that the project will proceed as it follows, unknown oriental prescriptions and remedies which have been particularly ignored will eventually play an important role in clinical practice. Conclusions : It is clear to everyone that these oriental remedies will remain ignored by the public unless they gain popularity. Strict verifications on these oriental remedies are definitely needed in order to overcome this limitation. Finally, it should provide a momentum in the field of medicine in gaining popularity to the public.

한양방 병용치료에 대한 문헌고찰 (A Literature Review of the East-West Medical Combined Treatment)

  • Objectives : This study aims to analyze the papers of the east-west medical combined treatment and suggest the research direction. Methods : 183 research papers were collected by using 25 keywords including combination(in korean words, 병용), collaborative practice(in korean words, 협진) in Korean Medical Database(KMbase) and Oriental Medicine Advanced Searching Integrated System(OASIS). We analysed the type of studies, the kind of diseases, the treatment method, the kind of herbs and drugs in frequent use, and the result of research. Results : Among the analysed 183 papers, clinical studies are 89 cases and experimental studies are 94. 5 cases of clinical studies are Randomized Controlled Trial(RCT). The cancer(50 papers) and diseases of the circulatory system(25 papers) occupied mostly in treatment studies. Because Combined treatment was actively progressed in oriental medicine, treatment was mainly applied the combined oriental medical treatment with herbs and western drugs. In herbal medicine, Mahwangyounpae-tang(麻黃潤肺湯) and Eunkyo-San(銀翹散) were frequently used in the papers. In western drugs, Cyclophosphamide, Cisplatin, and Mitomycin C were frequently used in the papers. 154 papers introduced the treatment effect, 14 papers announced the safety, and 4 papers mentioned the side effect. Conclusions : We suggested several future research direction as follows. Clinical studies based on experiment studies must be more activated and many RCT shoud be shown. Experimental group in clinical studies should be clearly separated to confirm the treatment method is effective or isn't effective. Studies about the side effect must be expanded, and every study should be confirmed both the treatment effect and safety.

표준화 난임환자를 활용한 한의대생의 진료 및 의사소통 수준연구 (A study about the medical communication proficiency of Korean traditional medical students using standardized patients of Infertility)

  • Objective : The purpose of this study is to assess the communication and medical interview level of the students in the college of korean traditional medicine using Standardized Patients(SP). Method : 35 students of 5th grade in the college who are doing their clinical practice class participated and conducted medical interview on standardized infertile female patient. SP and professor who observed the interview evaluated the students' communication skill and medical interview level using the assessment sheet composed of 23 questions. Results : 1. Satisfaction of SP was 1.66 of 3. In the communication section, students conducted well in appropriate voice and no inappropriate interruption but they didn't show good practice in considerate physical examination, understandable explanation relatively. 2. The scores that were acquired by students in informing the time required, obtaining agreement at initiating and closing, family history taking, checking mental status and physical examination 2 were low in the medical interview section. 3. There were some differences in the communication and medical interview scores by the students' sex (p<.01), age(p<.05) and medical camp experience(p<.05). But we couldn't find any evidence about the relation between character type and the communication and medical interview scores(p>.62). Conclusion : On the whole, students who participated this study had difficulties in building relationship with patient and conducting medical interview. They need to improve their ability in those fields by appropriate education before their graduation.

한방병원 병상 실태와 한방 간호의 교육 현황에 따른 한방간호사 수요 예측에 대한 고찰 (The Present Educational Conditions and the Demanding Outlook of Nursing in Oriental Medicine)

  • The purpose of this research is to provide basic data for the demanding outlook and the Nursing education by grasping the present educational conditions of nursing in oriental medicine. The results of survey and study on 112 nationwide schools (47 Department of nursing and 65 Nursing junior colleges) and data of the Korean Oriental Medicine Association and General Assembly in Sep. 1998 are as follows. 1. The number of nurse Oriental Medicine hospitals require in 2002 will have increased at least 700 or more. Public health centers' and other centers' requirements will increase more and more. 2. Education of nursing in oriental medicine is offered at practice in 14 universities (29.8%) among 47 Universities and 35 colleges (53.8%) among 65 colleges. 3. Nursing education in Oriental Medicine is mostly offered in the first term of the second year at universities with 5 to 10 points, and the second term of the third year at colleges with 2 points. According to this result, we would like to suggest as follows; A nurse whose main interesting subject is human beings will be an important figure as a medical information administrator in 21 century, namely, a nurse will play the leading part in the hospital administration more than a doctor. We must take much more interest arid exert ourselves in developing education of nursing in oriental medicine because in the future it will be the one and only field in the world, so we can export our Nursing manpower.
