복시(複視)(Diplopia)를 호소하는 Guillain-Barre Syndrome(GBS) 환자 치험 1례 증례보고 (A Clinical Case Report on Guillain-Barre Syndrome with diplopia)

  • Guillain-Barre Syndrome (GBS) is an acute inflammatory demyelinating polyneuropathy. It is disorder in which the body's immune system, attacks parts of the peripheral nervous system. It is characterized by the rapid onset of weakness, paralysis of the legs, arms, breathing muscles and face. But the analysis of CSF and electrical tests on nerve and muscle function can be performed to confirm the diagnosis. Most cases occur shortly after a viral infection. This is a clinical ease report on Cuillain-Barre Syndrome with diplopia. The patient, a 52-year-old man had a weakness in both legs and diplopia. His weakness and diplopia improved after oriental medical treatment, so this is reported as a potential treatment. Objective: This study was designed to evaluate the effects on oriental medicine therapy on Guillain-Barre Syndrome with diplopia. Methods & Result: The Clinical data was analyzed on a patient with Cuillain-Barre Syndrome whose main symptoms were diplopia. The patient was treated by acupuncture and oriental medicine. As a result, symptoms was improved remarkably. Conclusion: The patient showed weakness and diplopia. After acupuncture and oriental medicine treatment, weakness and diplopia was improved in 4weeks after visit to clinic. The study suggests that oriental medicine treatment is effective on Cuillain-Barre Syndrome.

한.양방 협진체계 개발에 관한 연구 (A Study on Cooperative Medical Treatment System between Oriental and Western Medical Practitioners)

  • A study on cooperative medical treatment system between oriental and western medical practitioners was conducted from March through August 1997 in order to determine a productive model of cooperation of two medical parts for treatment of patients. Questionnaires were distributed to 195 medical doctors(M.D.) and 195 doctors of oriental medicine(O.M.D.) working in clinics and hospitals in six major cities. Statistical analysis tools used for this study were frequency, t-test and multiple regression by using SPSS/P.C package. The results are summarized as follows: 1. The respondents were composed of male docotors(78%) and female doctors(22%) and 68.2% of M.D. and 97% of O.M.D. were interested in the other medical part. The doctors of both disciplines had some limitation on treatment of patients but they were reluctant to refer their patients to other doctors in different discipline. 2. M.D assumed that oriental medicine was more effective for chronic diseases, and O.M.D. thought that western medicine was more effective for acute diseases. 92.3% of O.M.D.s responded that even though they do not utilize western medical technology for diagnosis, they believed the results of diagnoses by western medical technology. 3. 60.5% of O.M. and 93.3% of O.M.D. said that cooperative medical treatment system could be necessary for patients and it would be effective for control of diseases and 69% of western medical doctors and 99% of oriental medical doctors agreed that oriental medical practice would be more effective for cerebellar vascular accidents than other diseases. 77.4% of western medical doctors and 92.3% of oriental medical doctors responded that the students of two different disciplines have to be taught on the other disciplines. 82.6% of western medical doctors and 83.3% oriental medical doctors agreed that it would be necessary to have collaborative research between scientists in two disciplines. 81.5% of M.D.s and 93.3% of O.M.D.s believed that they had prejudice and distrust on other discipline 4.90% of the doctors were not satisfied with the government medical policies on health insurance, legal matters, and health delivery system. 5. 75.4% of M.D.s and 50.2% of O.M.D.s said that two medical disciplines should be integrated, but they were skeptical on that. 75.3% of M.D.s thought that western hospitals should employ O.M.D.s Finally this study recommended that western medical students study oriental medicine and the vice versa, and M.D.s and O.M.D.s should have seminars and workshops to exchange knowledge, and experiences. It is also recommended that medical laws be revised to allow medical doctors refer the patients whom they can not handle to oriental medical doctors and the vice versa.

서울보훈병원 한방진료과 내원환자에 대한 통계적 고찰 (A Statistical Study of Patients Came to Seoul Veterans Hospital)

  • Objectives : Among the patients at Seoul Veterans Hospital, there is a great demand for oriental medical treatments, and the number of patients of oriental medical part is on the increase. The purpose of this study is to show patients inclination statistically. Methods : The study was carried out on the 1045 patients came to Seoul veterans hospital from April 2003 to May 2004. Results & Conclusions : 1. Most of men patients were veterans. They had neurological disorder or cerebrovascular disease that was ongoing for 1~5 years before diagnosis and they were treated less than 10 times. 2. Most of women patients were nursing their family. They had musculoskeletal disease that was ongoing 1 week before diagnosis and they were treated less than 10 times. 3. Under the current system, only acupuncture and other limited treatments are provided to patients due to the limited funding from the government. There were few patients who had an internal disease because they needed to be herbal treated. If the government funding were provided, they would be able to be treated.

  • 1. Objectives In this study, we analysed the up-to-the-present data in the SCIB and the chart of Iksan Wonkwang Oriental Medical Clinic, to develop the system and factors of data for the SCIB(Sasang constitution Information Bank). 2. Methods SCIB data is composed of analyses about the QSCC II(Questionnaire for the Sasang Constitution Classification II), Cold-hot questionaire, Ban-chi constitution questionnaire, Body composition, Body measurement, 24-hour food intake & Activity examination, MBTI & MMTIC, Informed consent & Blood sampling, and data presented by committee of Sasang constitution diagnosis expert. And the chart data of Iksan Wonkwang Oriental Medical Clinic is composed of analyses about Inspection, Ausculfation and olfaction, Inquiry, Pulse feeling and palpitation, and treatment field. 3. Results and Conclusions Almost data in the SCIB are lack of regular forms because they are based on mainly the QSCC II and additionally other examinations. Conclusionly important matters of the SCIB data are as follows: (1) the standard form including 4 whole diagnostic factors (2) the standard form including symptoms, pattern identifications, treatment processes and treatment results (3) objectivity and practicality to collect data.

침 자극이 인체의 자율신경계에 미치는 영향 고찰 (Review on the Effects of Acupuncture Stimulation on Autonomic Nervous System)

  • Objectives: Acupuncture has been shown to relieve pain and modulate biological function by stimulating the organ-specific meridians and enhances parasympathetic activity and suppresses sympathetic activity. The aim of this review is to summarize and understand the effects of acupuncture on autonomic nervous system. Methods: We reviewed a total of 29 studies published from 2000 to 2010 searched by PueMed and various domestic oriental medicine journals to obtain acupuncture studies related with ANS. Each article was classified by ANS measurement index and reviewed for study objectives, outcomes, acupuncture points, experimental and control interventions. Results: In the study of acupuncture about EEG, HRV, SCR, the experiment results are not the same by acupoints. Although same acupoint the results differ by subject's condition. But the study showed some clear tendency. In brief, in normal states acupuncture enhanced either vagal or sympathetic tone depending on the stimulated acupuncture point sites. On the other hand, most of studies demonstrated that acupuncture restored the autonomic dysfunctions in various kinds of tired or stressful states. Conclusions: We reviewed studies that contributed to an understanding of the effects and mechanisms of acupuncture on autonomic nervous system. Although the relationship between acupuncture and ANS response is still uncertain, acupuncture could be a excellent treatment method for modulating autonomic dysfunction.

한의 외래 비급여 진료비의 연도별 추이 (Annual trends of Outpatients' Out-of-pocket Spending in Using of Korean Medicine)

  • Background : The introduction of policies expanding the coverage of uninsured Korean Medicine (KM) services have requires an understanding of the following components of the service : current financial expenses, degree of financial burden on the patient, and financial effect of the coverage expansion. Objectives : This study aims to determine the annual trend of outpatients' characteristics and the category of out-of-pocket spending in KM. Methods : This study uses data from the Korea Health Panel to analyze use of KM in the Korean population. Using the user characteristics and behavior drawn from the Korea Health Panel data, out-of-pocket spending trends of KM were analyzed by year. The diagnosis and prescription of out-of-pocket spending were also analyzed. Results : The proportion of patients receiving uninsured medical treatment and the number of uninsured medical treatment in outpatient clinics have increased. However, the average out-of-pocket spending per person and out-of-pocket spending per visit are consistent or have decreased. Meaningful trends are the increase of R00-R99 (unclassified symptoms) and the decrease of K00-K93 (digestive system disease) and J00-J99 (respiratory system disease). Conclusions : Expansion of KM medical service and insurance is influenced by uninsured medical treatment of KM. Hence, research to increase medical treatment categories for out-of-pocket spending or explore diseases where KM diagnosis has been proven effective should be further developed.

  • Guillain-Barre syndrome, or acute inflammatory polyneuritis, is a disorder in which the body's immune system attacks parts of the peripheral nervous system. The causes and mechanisms of this syndrome are unknown. Typically, Guillain-Barre syndrome can be diagnosed from the patient's symptoms and physical examination such as the rapid onset of weakness, paralysis and loss of reflexes. The analysis of CSF and electrical tests on nerve and muscle function can be performed to confirm the diagnosis. Most cases occur shortly after a viral infection. This is a clinical report about one patient suspected as having Guillain-Barre syndrome. The patient, a 62-year-old man had weakness in both legs after gastroduodenal disease. His weakness and general condition improved after Korean medical treatments, so this is reported as a potential treatment.

한의사들이 원하는 한의학 지식체계에 대한 고찰 (A study of Korean medical knowledge system which Korean medical doctors want)

  • Objectives : This study was carried out to know the status of clinical utilization of Korean medical knowledge and the Korean medical knowledge system which Korean medical doctors want. Methods : Questionnaires were distributed to Korean medical doctors registered for the Association of Korean medicine via the email and 249 answered questionnaires were analyzed. Results : 50.6 % of Korean medical doctors utilized the textbooks for Korean medicine when they treated their patients. The reason why they did not utilize the textbooks for Korean medicine was mostly that 'it could not be applied directly into clinical application with only contents of textbook because clinical approach (diagnosis and treatment methods) was not clearly described in the textbook'. 48.6 % of Korean medical doctors utilized the EBM materials when they treated their patients. EBM materials that they referred included domestic articles, other EBM materials excluding papers and international academic papers based on PubMed in order. The reasons why they did not utilize the EBM materials were mostly that 'it was difficult to find the evidence' and 'EBM materials were not actually helpful for the clinical practice.' It was found that Korean medical doctors approached the disease in aspects of Korean and Western Medicine concurrently upon the diagnosis of patients and performed the diagnosis and treatment of Korean medicine on the diseases which seemed to be effective with Korean medicine. It was also found that they thought that they needed to know the Western medical knowledge as much as general doctors who have a doctor's license or family medicine specialists who comprehensively treat the general diseases know. Korean medical doctors wanted to have the systematic summary of modern research performance in fields of acupoint, acupuncture, herbal drugs and formula. They thought that constitutional medicine and Sa-am acupuncture therapy were representative characteristics in Korean medicine differentiated from Traditional Chinese medicine and Kampo medicine. Conclusions : Korean medical doctors wanted to establish the Korean medical knowledge system based on evidence focusing on clinical and practical contents. New Korean medical knowledge system should be established based on these requirements in the future.

다한증 입원 프로그램을 통해 치료한 소아 및 청소년 수족다한증 11례에 대한 증례보고 (A Case Series Report on 11 Patients of Primary Palmar/Plantar Hyperhidrosis in Children and Adolescents Treated with Hospitalization Program of a Hyperhidrosis Clinic)

  • Objectives : The purpose of this study was to report the clinical effect of short- term admission program on primary palmar/plantar hyperhidrosis in children and adolescents. Methods : Between January 2011 and January 2012, a total of 11 patients with clinical diagnosis of primary palmar/plantar hyperhidrosis were treated with hospitalization which consisted of iontophoresis, electro-acupuncture, herbal medicine, topical therapy and physical therapy. The effects were evaluated as quality of life with dermatology life quality index (DLQI), severity of discomfort using visual analogue scale (VAS), and the patients' subjective satisfaction. We analyzed the patients' improvement before and after admission treatment, as well as six month later. Results : After treatment, the 11 patients' quality of life and severity of discomfort improved significantly. They also were satisfied with oriental treatments for hyper hidrosis. Six months later, 6 of 11 patients had visited the hyperhidrosis clinic. Their symptoms had improved and remained stable. Conclusions : The short-term admission program is effective on primary palmar/plantar hyperhidrosis. Further studies with a larger number of cases will be needed in the future.

  • Objective : To review the method, parameters, system of electrogastrography and to study its oriental medical application. Method : We investigate the research results through internet search engines such as Pubmed, China National Knowledge Infrastructure. Conclusion : Electrogastrography is believed to be of great use to the verification of treatment effect of herbal medicine that is used in internal medicine and neuropsychiatry, along with the objectification and quantification of diagnosis of oriental medicine related to gastric peristalsis and balance of autonomic nervous system. I would therefore like to conclude that there should be continued study and active application of electrogastrography in the wide variety of oriental medicine.
