노인의 구강건강상태와 전신건강의 상태 (The relationship of oral state and health condition among elderly people)

  • The purpose of this study was to examine the relationship of oral state and health condition among elderly people. The subjects in this study were 220 senior citizens who were randomly selected from among dwellers in Eunpyeong-gu and Jongno-gu, Seoul. A survey was conducted by one-on-one interview to find out their oral state and health condition. The collected data were analyzed with SPSS 10.0 program, and variance analysis, correlation analysis and regression procedure were employed. The findings of this study were as follows: First, Regarding subjective oral health state, 52.3 percent of the elderly people, more than the half, considered their mouth to be in good health. The number of their mean remaining permanent teeth was 14.85, which was one of the indexes to indicate their objective oral state. Second, Concerning their abnormal oral health experience, 75 percent didn't feel that their masticatory force was good enough, and 55.9 percent felt pain in their teeth, 70.0 percent suffered from mouth dryness. Those fact showed that a lot of the elderly people felt there was something wrong with their oral cavity, and the unhealthy oral state made it difficult for them to eat food, as their masticatory force weakened and they couldn't get a good taste for food. Third, Those who viewed their oral cavity as healthy thought that they were in good shape, and there was a significant correlational relationship between their subjective oral and overall physical health states. Their own perception of oral state made a difference to their subjective health status, and the subjective oral health was significantly related to ADL as well, which implied that their ADL was different according to the way they looked at their own oral health status.

비강수술 후 구강간호 방법에 따른 갈증과 구강상태 효과 비교: 냉수 거즈 활용 구강 내 닦아주기와 냉수 거즈 물려주기 (Comparison of the effects of two oral cares on thirst and oral status following nasal surgery: Oral swab versus non-swab using cold water gauze)

  • Purpose: This study aimed to compare the effects of oral swab and non-swab using cold water gauze on patients' thirst and oral status following nasal surgery under general anesthesia. Methods: A non-equivalent control group was applied. Participants were divided into a control group (n=30) that was treated by cared for with cold water gauze application without oral swab and an experimental group (n=29) that was treated cared for with oral swab using cold water gauze after nasal surgery at the G University Hospital in Korea. Data were collected from May 14, 2020, to April 30, 2021, and analyzed using descriptive statistics, a chi-squared test, independent t-test, Wilcoxon signed-ranks test, and Mann-Whitney test. Results: The results showed no significant differences between the two groups in thirst (U=-0.04, p=.693) and overall oral condition (U=-0.34, p=.813) after the intervention. However, participants' thirst and oral condition were significantly improved in each group after intervention. Conclusions: It was confirmed that both oral care methods reduced thirst and improved oral condition after nasal surgery. These findings indicate the need for intervention for patients' thirst and oral condition after nasal surgery. Furthermore, they show that these oral care protocols can be used as a safe and effective nursing intervention for patients who undergo nasal surgery under general anesthesia.

  • Ridge expansion osteotomy(REO) technique is a simple and more conservation method to widen a narrow alveolar ridge in the maxilla. This method is superior to drilling method in soft and narrow maxillary alveolar ridge and allows the surgeon to widen the ridge in routine office procedure. Therefore, it is the treatment of choice to implant the maxilla with narrow alveolar ridge. This article presents clinical cases and discusses the advantages, rationale and surgical protocol of REO technique.

  • Various dental procedures, such as injection administration, surgical treatment, and endodontic treatment, can cause injury to the nerves. The most commonly injured nerves are the inferior alveolar and lingual nerves. This can manifest as altered sensation to the area of innervation of the injured nerve, such as the lower lip, chin, teeth, tongue, and mucosa. Altered sensations or loss of sensation are relatively infrequent complications in daily dental practice. Here, we report an uncommon case of altered sensation in the midfacial region caused by an endodontic procedure and discuss the need to consider local dental causes in the differential diagnosis of numbness in the facial region.

  • Mucoepidermid tumors are usually indistinquishable from benign tumors clinically, and accounts for approximately 6% to 8% of all salivary gland tumors. Although rare in children, it is the most common "malignant" salivary gland tumor, haying been reported in major and minor (intraoral) salivary gland sites as well as in the maxilla and the mandible. In children, as in adults, it most often occurs in the parotid gland, but a significant percentage is found in the palate. Presently, there is no unanimity of opinion about whether to consider all mucoepidermoid tumors malignant or what the most appropriate treatment regimen is. The importance of submitting, for microscopic diagnosis, all tissue removed during surgical procedure is illustrated in this case report and a review of the literature is presented.

  • AVMs is not a neoplasm, but a congenital developmental anomalies.$^{5)}$ In oral and maxillofacial area, the high recurrence rate and more facial disfigurement give a difficult problem to surgen and patient.$^{8)}$ Conventional treatment modality nowadays is presurgical embolization and surgical resection. In treatment planning, we consider the possibility of complete surgical resection and the esthetics of soft tissue reconstruction. But, two things are very difficult to achieve. We present a patient with AVMs in left upper lip, he had presurgical embolization with Ivalon and Bucrylate, and conservative surgical procedure. So we present this possible treatment modality.

  • Ketamine HC1(Phencyclidine derivative) is a white, crystalline substance with melting point 259℃. It is soluble in water to a 20% clear, colorless solution. a 10% aqueous solution has a pH 3.5. The chemical structure is 2-(O-chlorophenyl)-2-(methylamino) cyclohexanone hydrochloride. It is a rapid acting, nonhypnotic, nonbarbiturate drug with a wide safety margin. The author used the anesthetic ketamine hydrochloride for oral surgery procedure in 16 patients(2 to 33 years). Anesthesia was achieved with a single intramuscular injection by introduction of manufacture. Result obtained as follows : 1. It was easily administered. 2. Onset time was rapid and duration was short. 3. The mild cardiac stimulation resulted in moderate increase in the blood pressure and pulse. 4. There was no significant respiratory depression. 5. The airway can be maintained without artificial support or endotracheal intubation. 6. All protective reflex maintained, which was important in oral surgery. 7. The best results were achieved in pediatric patients than adults.

  • Recently, clinical application of autogenous tooth bone-graft materials has been reported. Autogenous tooth bone graft has been used in implant surgery. Familial tooth bone graft is a more advanced procedure than autogenous teeth bone graft in that extracted teeth can be used for bone graft materials of implant and teeth donation between siblings is possible. We used autogenous tooth and familial tooth bone-graft materials for ridge augmentation and sinus bone graft and obtained satisfactory results. The cases are presented herein.

  • The inferior alveolar nerve block is the most common method of local anesthesia for intraoral surgery at the posterior mandibular region. However, unexpected complications may occur when administering the local anesthesia. One of these uncommon complications is the fracture of the needle. If the injection needle is broken during the surgery, it should be removed immediately. However, this is one of the most difficult procedures. In this report, we present two cases of needle fracture during the procedure, and its successful removal under general/local anesthesia administration.

Oral Health of Stone Mine Workers of Jodhpur City, Rajasthan, India

  • Background: Occupational injuries cause major health problems, which the developed, developing, and underdeveloped nations worldwide are facing today. The present study aimed to assess dental caries, periodontal health of stone mine workers, and the relationship between wasting diseases and the years of working experience. Methods: The study population comprised 510 men, selected based on the stratified cluster sampling procedure. Clinical oral examinations were carried out, and periodontal disease, dental caries, and wasting diseases were recorded. Results: Workers were in the age group of 17-56 years; the prevalence of dental caries in the workers was found to be 74%, with a mean decayed, missing, filled teeth index of 2.89. A periodontal pocket of more than 6 mm was observed in 6% of the workers. Conclusion: The oral health of mine workers is in a poor state; steps should be taken so as to provide basic medical and dental care facilities.
