Long Term Evaluation of the Effect of Botulinum Toxin A Injection on the Masseteric (교근 비대 환자에서 보툴리눔 A형 독소 주사 효과의 장기적 평가)

  • In this experiment, eleven volunteers were followed up for 15 months after the injection of botulinum toxin type A on right and left masseter muscles. The measurement of masseter muscle atrophy for each volunteer was performed by CT(computed tomography) three times: before the injection, three and fifteen months after the injection. The thickness and area of muscle were measured in three positions which are 10 (position 1), 20 (position 2), and 40 mm (position 3) above the inferior border of mandible(the injection site was nearest the position 1). The thickness of masster muscle was decreased in all three positions three months after the injection, but no significant change was observed fifteen months after the injection. On the other hand, the area of masster muscle was decreased in all three positions three months after the injection. Furthermore, the area was decreased significantly in positions 1 and 2, but not in position 3 fifteen months after the injection. As a result, toxin is still in effect even fifteen months after the injection. Finally, the present study shows that the measurement of muscle area provides more precise informations than that of muscle thickness does.

  • In this study, 94 patients with temporomandibular disorders were interviewed to evaluate the effect of the treatment they received at the department of Oral diagnosis and oral Medicine in Seoul national university Hospital. The treatment administered to the patient were patient education, relaxation procedures, physical therapy, occlusal splint therapy and selective grinding of teeth. The following results were disclosed : 1. 97.9% of the patients reported no pain or considerably less pain than they had reported at their first visits to the department. 2. 89.3% of the patients thought that the treatment provided was either complete or considerably successful. 3. TMJ and facial pain was resolved within average 3.0 months 4. Of the 70 patients who previously reported having jaw joint sound, 34 patients (48.6%) no longer reported int. 5. Of the 48 patients who previously reported having headache regularly, 3 patients (68.8%) no longer reported headache regularly. 6. Of the 66 patients who previously reported having masticatory muscle pain, 41 patients (62.1%) no longer reported masticatory muscle pain. 7. Analysis of the data did not disclose a subgroup or factor, such as age, the stage of internal derangement, capsulitis, bruxism, MPD, duration of symptoms, that could be correlated with the reduction of pain or the patient's perception of success of treatment.

  • Angiomyoma is the vascular type of leiomyoma that the tumor cells are originated from vascular smooth muscle cells. It's frequently found in the subcutaneous tissues of the lower extremities. Such case of an angiomyoma within the oral cavity is rarely found. From a series of 7748 smooth muscle tumors of all types, only 0.06% were found in the oral cavity. This is a rare case of a young woman appeared with oral angiomyoma located in the left mandibular posterior region with plain radiograph, CT and histologic review.

  • The alar base on the cleft side in unilateral complete cleft lip, alveolus and palate is markedly displaced laterally, caudally and dorsally, By incising the pyriform margin from the cleft margin of the alveolar process, including mucosa of the anterior part of the inferior turbinate, to the upper end of the postnasal vestibular fold, the alar base is released from the maxilla, A physiological correction of nasal deformity can be accomplished by careful reconstruction of nasolabial muscle integrity, functional repair of the orbicular muscle, raising and rotating the displaced alar cartilage, and finally by lining the lateral nasal vestibule, The inferior maxillary head of the nasal muscle complex is identified as the deeper muscle just below the web of the nostril, The muscle is repositioned inframedially, so that it is sutured to the periosteum that overlies the facial aspect of the premaxilla in the region of the developing lateral incisor tooth, And then, the deep superior part of the orbicular muscle is sutured to the periosteum and the fibrous tissue at the base of the septum, just in front of the anterior nasal spine, The nasal floor is surgically created by insertions of the nasal muscle complex in deep plane and of the orbicular muscle in superficial one, The upper part of the lateral nasal vestibular defect is sutured by shifting the alar flap cephalically, The middle and lower parts of this defect are closed by use of cleft margin flaps of the philtral and lateral segments, respectively, Authors stress the importance of nasal floor reconstruction at primary surgery and report the technique and postoperative results.

Long-Lasting Muscle Relaxant Effect of Patch Containing Centrally Acting Muscle Relaxant (중추성 근이완패취제의 지속성 근이완효과)

  • Due to its profound intestinal first-pass metabolism, oral eperisone hydrochloride produces a very low bioavailability and a very short muscle relaxant activity. To improve the efficacy and compliance of eperisone, we designed a new dosage form, the transdermal patch and evaluated the muscle relaxant effects of this patch in rats. The muscle relaxant activity was assessed by measurement of forelimb grip strength and hanging test in rats. The transdermal patch of eperisone hydrochloride showed significant muscle relaxant activity at 0.5, 1.5 and 3 cm$^2$/200 g rat (1.39, 4.17 and 8.33 mg of eperisone hydrochloride/kg, respectively) in a dose-dependent manner and the effects were lasted over 24 hours. The oral eperisone hydrochloride showed significant activity at 12.5, 25 and 50 mg/kg in a dose-dependent manner but the activity was within 1 or 2 hours after administration. These results suggest that the eperisone hydrochloride is absorbed efficiently from transdermal patch and the patch can exert potent and long-lasting muscle relaxant activity. This transdermal patch will increase the efficacy and compliance in the clinical use of eperisone hydrochloride.

  • A 16-year-old female was admitted to Wonkwang dental hospital with a chief complaint of painful ulceration on right buccal mucosa around mandibular 3rd molar area. Computed tomography and magnetic resonance imaging showed relative large soft tissue mass on the infratemporal fossa and masseter muscle region. By the feature of T1-weighted and T2-weighted of MR imaging, we suspected this mass as a kind of myogenic sarcoma. Histopathological and immunohistochemical studies established a definitive diagnosis of embryonal rhabdomyosarcoma. A review of the literature was also presented.

Temporal Abscess Mimicking Temporomandibular Disorders

  • Facial abscess is a suppurative condition that is caused by infection and that its infected materials built up within the loose connective tissues or a fascial space of the head and neck. Facial abscess should be treated with a caution since it can make threat to patient's life. When pus collects near masticatory muscles, it may lead to masticatory muscle disorder reducing the range of mouth opening and the mobility of jaw. The authors review an uncommon case of facial abscess which occurred in temporal muscle and induced mouth opening limitation.

Neuromedin B modulates phosphate-induced vascular calcification

  • Vascular calcification is the heterotopic accumulation of calcium phosphate salts in the vascular tissue and is highly correlated with increased cardiovascular morbidity and mortality. In this study, we found that the expression of neuromedin B (NMB) and NMB receptor is upregulated in phosphate-induced calcification of vascular smooth muscle cells (VSMCs). Silencing of NMB or treatment with NMB receptor antagonist, PD168368, inhibited the phosphate-induced osteogenic differentiation of VSMCs by inhibiting Wnt/β-catenin signaling and VSMC apoptosis. PD168368 also attenuated the arterial calcification in cultured aortic rings and in a rat model of chronic kidney disease. The results of this study suggest that NMB-NMB receptor axis may have potential therapeutic value in the diagnosis and treatment of vascular calcification.

The Effects of Oral Care with Difflam Spray 0.3% and Sodium Bicarbonate-Normal Saline Solution on Postoperative Oral Comfort, Sore Throat, and Halitosis (디프람 스프레이(Difflam spray 0.3%)와 중조 생리식염수 분무요법이 전신마취 하 수술 환자의 구강 안위감, 인후통 및 구취에 미치는 효과)

  • Purpose: This study aimed to compare the effects of the Difflam spray 0.3% with the sodium bicarbonate-normal saline spray on oral care for postoperative patients. Methods: The participants were randomly allocated to either the Difflam (n=22) group or the saline solution (n=23) group. The data were collected at the 1-, 4-, 7-, and 10-hour marks after returning to the ward post operation using the Beck's subjective oral discomfort scale, Visual Analogue Scale for sore throat, and portable halitosis detector. Results: The sore throat (F=7.25, p=.001) score significantly decreased after oral care using the Difflam spray 0.3%. However, the difference in the scores of oral comfort (F=0.34, p=.797) and halitosis (F=0.91, p=.443) between the two groups was not statistically significant. Conclusion: These findings suggest that the Difflam spray 0.3% is effective in improving postoperative sore throat. A further study that explores the effect of various oral solutions for postoperative patients is needed to present systemic and effective evidence-based oral care guidelines.

INFILTRATING LIPOMA OF THE CERVICAL AND PAROTID AREA : REPORT OF A CASE (경부 및 이하부에 발생한 침습성 지방종의 치험례)

  • The lipoma is the neoplasm of mesenchymal origin. Although most lipomas occur on the trunk and the proximal portion of the extremities, lipomas of the oral and maxillofacial regions are relatively rare, approximately 13% of all cases. Lipomas have been found in all age, but usually found between 40 and 60 years of age. Clinically the lipoma is a painless, slowly growing, nearly always benign soft mass. The treatment of choice is a surgical excision, and the recurrence is rare. But the infiltrating lipoma originated from muscle has high recurrence rate. It has ill-defined border and little or no evidence of encapsulation. Histologically there is a consistent infiltration with dissociation of the surrounding muscle fiber. The infiltrating lipoma should be excised with surrounding normal muscle and tissue to prevent the recurrence. This case was a 57-year-old female with a painless swelling of the right cervical and parotid areas which was diagnosed as large infiltrating lipoma by clinical examination and radiographic findings. The patient was treated by surgical excision and showed good functional and esthetic results. Histologically the tumor was diagnosed as infiltrating lipoma with no evidence of malignancy.
