A Study on the Correlation of Dental Anxiety Based on Dental Fear Scale (DFS) (치과불안척도(DFS)에 근거한 치과공포 연관성 연구)

  • Objective: Dental fear is a feeling of anxiety and fear without intervention during a dental visit, mainly due to past negative dental treatment experiences, and in adults, despite the recognition of the need for dental treatment, it can lead to avoidance of dental treatment, which may cause deterioration of oral health. Therefore, this study was conducted to measure dental fear and anxiety level in some adults, and to identify related factors and their causes. Methods: This study used Kleinknecht's Dental Fear Survey scale and the SPSS 21 program (IBM SPSS Statistics 21) was used for analysis. Result: When the dental fear was the higher, the "access to the dental clinic" was found to be the highest, followed by "sitting in dental unit chair" and "smell of dental clinic" in that order. Conclusions: Dentists and dental hygienists should be able to understand the fear, anxiety, and concerns of the dentist office and to find various ways to provide systematic medical services.

The Burden and Social Support of Mothers of Nephrotic Syndrome Patients (신증후군 환아 어머니의 부담감과 사회적지지)

  • This study was conducted to identity the degree of burden and social support perceived by mothers of nephrotic syndrome patients. Also, relations between burden and social support were investigated to provide basis data for their family health and nursing intervention. The study subjects were mothers of nephrotic syndrome patients whose children were hospitalized in 2 Pediatric wards of University Hospital in Seoul and 1 in Pusan from Jun. 1, 1998 to Jun. 30, 1998. Burden measurement Instrument for this study was designed by the researcher and its basis in one developed by Montgomery et al.(1985) and the reliability was .78. Also. P.R.Q. Part I, II by Brandt and Weinert(1981) was used as social support measurement instrument and the reliability .71. The data analysis was done by SPSS, t-test, ANOVA Pearson correlation. The result were as follows. 1. Burden felt by mothers shows an average value of 60.82 (Maximum 86, Standard deviation 1.244). 2. Of the mother characteristics, the score of burden was high in case of no religion and low income. Of the patient characteristics, the score burden ranked as high MCNS, doing oral therapy, injection therapy at the same time and negative perceived patients condition. 3. The mean score of support was 77.54(Maximum 96, Standard deviation 1.096). 4. The main supporters were husband (the highest), brother, sister, health speciality and the subject expressed the highest satisfaction toward supporters in chronic disease. 5. Of the patient characteristics, the higher age group and the elder group showed high support. Also, positive perceived patient's condition, high support. 6. The relationship between burden and social support is not significant. In conclusion to the above study, the researcher suggests. 1. The Qualitative research to investigate influential factors on burden of family of nephrotic syndrome patients is needed.

  • This study applied an original survey of an objective National Health Nutriment Research. It picked the youth up from 14 years old to 19 years old and examined an influence relationship of four heads such as DT, DI, DMFT, DMFI which investigated in heads of dental inspections based on an intake frequency questionnaire by food group. It was proved that Korean dietary culture which eats rice and kimchi as staple food had an great effect on DT, DI, DMFT, DMFI And rice and cereals, fruits and vegetables like a cabbage turned out to be factors that can lower dental caries. The dental caries can be reduced by the more intake frequency of rice. And also, intake of fish like anchovy and meats like pork and beef can reduce development of dental caries too. Especially, the larger intake frequency of carbonated drinks and fast food such as hamburger and pizza can make a growth of dental caries.

Anti-Obesity and Lipid Metabolism Effects of Ulmus davidiana var. japonica in Mice Fed a High-Fat Diet

  • The root bark of Ulmus davidiana var. japonica (Japanese elm) is used in Korea and other East Asian countries as a traditional herbal remedy to treat a variety of inflammatory diseases and ailments such as edema, gastric cancer and mastitis. For this study, we investigated the lipid metabolism and anti-obesity efficacy of ethyl alcohol extract of Ulmus davidiana var. japonica root bark (UDE). First, HPLC was performed to quantify the level of (+)-catechin, the active ingredient of UDE. In the following experiments, cultured 3T3-L1 pre-adipocytes and high-fat diet (HFD)-fed murine model were studied for anti-obesity efficacy by testing the lipid metabolism effects of UDE and (+)-catechin. In the test using 3T3-L1 pre-adipocytes, treatment with UDE inhibited adipocyte differentiation and significantly reduced the production of adipogenic genes and transcription factors PPARγ, C/EBPα and SREBP-1c. HFD-fed, obese mice were administered with UDE (200 mg/kg per day) and (+)-catechin (30 mg/kg per day) by oral gavage for 4 weeks. Weight gain, epididymal and abdominal adipose tissue mass were significantly reduced, and a change in adipocyte size was observed in the UDE and (+)-catechin treatment groups compared to the untreated control group (***p < 0.001). Significantly lower total cholesterol and triglyceride levels were detected in UDE-treated HFD mice compared to the control, revealing the efficacy of UDE. In addition, it was found that lipid accumulation in hepatocytes was also significantly reduced after administration of UDE. These results suggest that UDE has significant anti-obesity and lipid metabolism effects through inhibition of adipocyte differentiation and adipogenesis.

The Relationship between Tooth Brushing and Hand Washing in Hypertensive Patients amidst COVID-19 Pandemic (COVID-19 상황에서 고혈압 기저질환자의 칫솔질과 손 씻기 실천율과의 관련성)

  • It has been found that the incidence and mortality of COVID-19 are high in patients with underlying chronic diseases. Amid COVID-19 pandemic, tooth brushing is as important as hand washing for health care. The aim of this study was to identify factors related to the rate of tooth brushing and hand washing in hypertensive patients. We analyzed 62,762 hypertensive patients over the age of 40 from the Community Health Survey 2020. The practice rate of hand washing was higher among women, younger age, higher education level, mask wearers when going out, and tooth brushing practitioners after lunch. The hand washing practice rate was higher in hypertensive patients who practiced tooth brushing. In conclusion, there was the relationship between oral hygiene and personal quarantine. It is necessary to prepare sustainable measures to increase the practice rate of infectious disease prevention behaviors for patients with underlying chronic diseases who are susceptible to COVID-19.

The Effects of Endogenous and Exogenous Estrogen on Risk Factors of Coronary Heart Disease -Part I : On Serum Lipid Levels- (내인성 및 외인성 Estrogen이 관상심장질환 위험인자에 미치는 영향 -제1보 : 내인성 및 외인성 Estrogen이 혈액 지질 농도에 미치는 영향-)

  • To determine the effects of endogenous and exogenous strogen on serum lipid levels, twenty nonsmoking healthy Korean women were participated in this experiment for 12 weeks. They were assigned to three groups : (1) eight women aged 22 to 30(yr) for the premenopausal(Pre) group, (2) eight, aged 49 to 60(yr) for the postmenoparusal(Pst) group, (3) four, aged 23 to 30(yr) for the oral contraceptive(OC) group which used triphasic OC formulation. Fasting blood samples representing every phase of the hormonal levels were obtained from the subjects of the Pre and the OC group. From the subjects of the Pst group, fasting blood samples were obtained once per three weeks for 12 weeks. All the serum data were adjusted for dietary effects, exercise, personality type and body mass index(BMI) by using analysis of covariation(ANCOVA). Serum lipid levels of the three groups were significantly different. While serum levels of triglycerides(TG)(p<0.0001), low density lipoprotein-chloesterol(LDL-C)/high density liporotein-cholesterol(HDL-C) ratio (LDC-C/HDL-C)(P<0.01) and total cholesterol (TC)/HDL-C ratio (TC/HDL-C)(P<0.001) were significatnly high in the Pst group, serum HDL-C(P0.001) level was significantly high in the Pre group. The OC group showed significantly low serum TC(P<0.0001) and LDL-C(P<0.0001) levels. There was no signidicant difference in the fluictuation of serum lipid levels during the menstrual cycle of the Pre group. However, in the OC group, serum TG level was significantly increased at phase 2(P<0.05) where exogenous estrogen administration was highest. Even though other serum lipid levels of the OC group were not significantly fluctuated according to the exogenous estrogen administration, there was a trend of increased levels of serum TC, LDL-C, LDL-C/HDL-C and TC/HDL-C and decreased level of HDL-C during the menstruation period. Also, serum TC level was high(P<0.005) and serum TG level was low (P<0.005) at the baseline of the OC group compared with the periods of OC administration. When screening and counseling the female population at risk for coronary heart disease(CHD), the result of this study suggest that in may be desirable to divide the population into several groups according to their personal physiological characteristics, such as age, OC administration, menstrual cycle and menopause, as well as general risk factors for CHD.

Effect of Dental Treatment Fear and Distrust of Dentists with Dental Caries Experience among Middle and High School Students in Daegu City, Korea (일부 지역 중·고등학생들이 치과치료공포감 및 치과의사불신감이 치아우식증에 미치는 영향)

  • The purpose of this study is to analyze factors impacting on the middle & high school students' dental treatment fear and distrust of dentists with dental caries experience to provide empirical data for the development of students oral health. The subjects of this study consists of 720 middle & high school students in Daegu area. The data were collected from July 1, 2009 to September 30, 2009 by self-administrative questionnaires. Among sub-scales of Dental Fear Survey(DFS), stimulate-response factor was the highest ($3.42{\pm}0.98$), and patient distrust toward dintist factor was the highest score($3.07{\pm}0.912$) among sub-scales of Dentist Distrust Survey(DDS). Mean score of DFS were DDS were higher in female than male student. The students who have higher DFS and DDS score, the also have high DMFT index. The higher DDS, the higher DFS, and it was statistically significant(p<0.001). The results suggest that oral health status of adolescent can improve diminishing dental fear and dentist distrust if patient-specific medical service may be provided.

TREATMENT OF ECTOPICALLY ERUPTING FIRST PERMANENT MOLAR IN CHRONIC MYELOGENOUS LEUKEMIA (전신 질환을 동반한 환아에서 나타난 제1대구치 이소맹출의 처치)

  • The ectopic eruption is defined as abnormal eruption which gives to displacement of the teeth and abnormal root resorption of adjacent teeth. The prevalence of ectopic eruption is reported to vary 2~6%, most of them are in the maxilla. Etiologic factors include narrow maxilla, large maxillary teeth, inclined eruption path of the first molar, retruded position of the maxilla and hereditary factor. Irreversible ectopic eruption where the second primary molar is lost often causes mesial tipping and rotation of the permanent molar, unfavorable occlusion and space deficiency for the second premolar. Ectopically erupted teeth should be treated early to maintain normal development of the dentition, harmony of facial growth and occlusal support. The method of the treatment are classified as follows : appliances that is positioned at the contact point for unlocking and the distal movement, fixed appliance that is connected to more than one tooth, and occlusion guiding method after disking or extraction of the second deciduous molar. A case report of a patient with bilaterally ectopic eruption of maxilla and mandible first permanent molar was present. Also, the patient who had experienced the chronic myelogenous leukemia, show various dental developmental complications. The ectopic eruption was treated with a Halterman appliance that was a effective way of correcting of ectopic eruption of the permanent first molar.

The association of mask selection and wearing time with dry mouth and bad breath

  • The purpose of this study is to provide basic data on the negative factors of oral health in masks and the importance of oral health management according to the use of masks to prevent novel coronavirus infection (COVID-19). From May 3 to 31, 2021, 232 adults aged 20 to 59 across the country were surveyed and statistically analyzed. As for the mask selection, 63.9% of men and 61.3% of women chose the KF_94 mask for both men and women, and it was found that the older they were, the more they chose the KF_94 mask. Self-recognition of dry mouth and bad breath due to wearing a mask showed that the group wearing a cotton mask felt high dry mouth, and there was a statistically significant difference. There was a significant positive correlation between mask type, dry mouth(r=.142, p<.05), and age(r=.234, p<.01). There was a significant positive correlation between mask wearing time and age(r=.158, p<.05), and it was found to be negatively correlated according to occupation, and was statistically significant(r=-.472, p< .01). Dry mouth had a statistically significant positive correlation with bad breath(r=3.04, p<.01) and age(r=.224, p<.01).

The Factors Influencing to the Implant Patients Discomfort (임플란트 식립 후 불편감에 영향을 주는 요인)

  • To get the evidences for decreasing the discomfort, dissatisfaction, and for improving management of the implant prothesis through assessment implant patients' awareness and discomforts about care of the prostheses. The subjects were 300 implant patients who treated in the 20 dental clinics in Pusan, Ulsan and Daegu. The general characteristics, recognition, maintenance, discomfort and dissatisfaction, and oral hygiene status were collected. Level of the discomfort and dissatisfaction was measured by a 5 Likert scale, and analyzed by the equivalent of 100. The life-span of the implants was overestimated, however, periodic recall check and scaling were underwent. The level of discomfort and dissatisfaction was generally good, but many of them (65.3%) complained the food impaction. As a result of the multiple regression analysis, level of the discomfort and dissatisfaction increased in case of more number of the prosthesis ($\beta$=0.864), however decreased in case of more frequent teeth brushing ($\beta$=0.737), more frequent periodic check ($\beta$=-0.737), longer duration from the implantation ($\beta$=-0.090). It is very important for the implant patients to be recognized that the lack of care may shorten the life-span of the implant prosthesis. Therefore, efforts of the continuous oral hygiene care such as, periodic recall check, scaling, teeth brushing, the correct using of additional oral hygiene product, and education would be more emphasized.
