정유를 이용한 구강자가간호가 재가노인의 구강건강에 미치는 효과 (The Effects of an Essential Oil Mouthrinse on Oral Health in the Community Indwelling Elderly)

  • Purpose: The purpose of this study was to investigate the effects of an essential oil mouthrinse with gingival massage on oral health in community indwelling elderly. Method: The subjects were composed of 61 healthy elderly at a Senior Welfare Center in J city. Thirty subjects in the experimental group were given toothbrushing education ongingival massage toothbrushing with an essential oil mouthrinse for 2 weeks(4 times per day, 3 minutes per session). The 31 subjects in the control group were given toothbrushing education ongingival massage toothbrushing. The effects of the treatment were measured by salivary pH, salivary IgA level, halitosis, oral subjective symptoms, and oral self care behavior scores before, right after and 2 weeks after the experiment. Results: Salivary pH was significantly increased(p=0.018) in the experimental group. Salivary IgA levels were not significantly different between the experimental and control groups; however, IgA levels of the experimental group were significantly increased(p=0.006) after time had passed. Halitosis was significantly decreased(p=0.002) in the experimental group. Oral subjective symptom scores were significantly decreased(p=0.000) and oral self care behavior scores were significantly increased(p=0.000) in the experimental group. Conclusion: Regular gingival massage toothbrushing with an essential oil mouthrinse could be an effective oral health nursing intervention for the elderly.

중년층의 구강건강지식, 실천 및 보철치료 만족실태 조사 (A study for middle-aged on oral health knowledge, oral health care and satisfaction with prosthetic treatment)

  • Objectives : The purpose of this study was to examine the oral health knowledge and actual oral health care of the selected subjects, their decision making about prosthetic treatment, the state and characteristics of their prosthetic treatment and their satisfaction with prosthetic treatment in an attempt to provide some information on the improvement of the quality of life related to oral health and the promotion of oral health. Methods : The subjects in this study were 250 people who received education in two different lifelong education institutions in the city of Busan. After a survey was conducted from May 23 to June 15, 2011, the answer sheets from 217 respondents were analyzed. Results : 1. As for the general characteristics of the respondents, the men(52.1%) outnumbered the women. Those who were in their 60 and up(47.5%) made up the largest age group, and the married people(65.4%) outnumbered the unmarried ones. By occupation, the company employees(20.3%) made up the biggest group. By education, the greatest group was high-school graduates(36.1%). By monthly mean income, the biggest group gained an income of 2.01 million won or more(36.9%). As to medical security, community-based insurance was most prevailing (36.9%). In terms of health promotion, the largest group worked out to stay fit(52.4%). 2. Regarding oral health knowledge, they had a good knowledge about the cause of dental caries(56.7%), but they weren't well aware of the right time for regular dental checkup(47.9%). In relation to oral health care, regular scaling wasn't prevailing(16.9%). 3. Regarding connections between the general characteristics and satisfaction with prosthesis, the less-educated respondents expressed significantly more dissatisfaction(p=0.015). By monthly mean income, those who had a smaller income were dissatisfied in general(p=0.028). Conclusions : The findings of the study illustrated that it's required to spread awareness of the importance of oral health among people in general, and that differentiated incremental oral health care should be provided for different age groups. In order to raise the satisfaction of patients with prosthesis, how to relieve their pain and anxiety and how to adjust prosthetic treatment cost properly should carefully be considered. In addition, the government should take measures to offer assistance for the low-income classes in preparation for an increase in the elderly population.

일부 보건과 비보건계열 대학생의 구강보건 행태 및 관리수준에 관한 비교조사연구 (A Study on the Oral Health and Oral-Health Care of Some Health-Related and Health-Unrelated Majors)

  • The purpose of this study was to examine the oral health awareness and oral health care of health-related and health-unrelated majors in an attempt to stress the importance of oral health education and boost the efficiency of oral health care. The subjects in this study were 363 college students. Out of the selected students, 174 were health-related majors, and 189 weren't. After a survey was conducted, the collected data were analyzed for the purpose of comparison, and the following findings were given: 1. Concerning daily mean toothbrushing frequency, the largest number of the male students who accounted for 55.2 percent brushed their teeth once a day on average, and the greatest number of the female students who represented 52.9 percent did that twice a day. As for the influence of their major, the largest group of the health-related majors who numbered 89(51.1%) brushed their teeth twice a day, and the greatest group of the health-unrelated majors who numbered 93(49.2%) did that once a day. The gaps between them were statistically significant(p<0.05). 2. As to the length of toothbrushing time by gender, it took more than two or three minutes for the largest groups of the male and female students that respectively numbered 96(76.8%) and 184(77.3%) to do toothbrushing(p<0.01). The greatest groups of the male and female students that respectively numbered 72(57.6%) and 183(76.9%) brushed their teeth after meals. 3. In regard to oral hygiene supplies, the largest groups of the health-related and health-unrelated majors that respectively numbered 78(44.8%) and 115(60.8%) had chewing gum and candy with them. As to the use of oral hygiene supplies, 99 health-related majors(56.9%) and 133 health-unrelated majors(70.4%) didn't put oral hygiene supplies to use. Thus, the use of oral hygiene supplies was statistically less common among the health-unrelated majors, and the gap between the two was significant(p<0.01). 4. As for health-related concern by gender, the largest group of the male students that numbered 56(44.8%) showed the most interest in preventing dental caries, and the greatest group of the female students that numbered 103(43.3%) were most concerned about tooth whitening. The gap between the male and female students was statistically significant(p<0.05), but the track of their major made no statistically significant difference to that. 5. Regarding the experience and awareness of scaling, the largest number of the students never got their teeth scaled regardless of gender and major, and the greatest group didn't care about scaling irrespective of gender and major, either, though they considered it advisable to do.

구강건강지표를 활용한 건강수명 연구경향 분석: 최근 10년간의 논문분석(2010-2020) (Research trend of health life expectancy using oral health indicators (2010-2020))

  • Purpose: The purpose of this article is to clearly describe research trends on health life expectancy using oral health indicators that have been published from 2010 to 2020 then suggest the direction of future research. Methods: Online academic databases in English (PubMed, Web of Science and Embase) were used to find those articles by applying a variety of keywords, including terms (adjusted life year, adjusted life expectancy, dental and oral). We identified relevant articles based on the following classification method of Mathers: (1) health gaps, (2) health expectancies. Results: Among 1,728 articles from the online databases, the final 13 studies satisfied the inclusion criteria and were selected for analysis. Health life expectancy studies indicate that research growth was recently achieved overseas. Among the literature collected in this study, 10 studies using health gap indicators yielded seven Disability-Adjusted Life Year (DALY), and three calculated Quality-Adjusted Life Year (QALY), which differed in the nature of the survey data used in the study measuring DALY and QALY. There are only three health expectancies and the number of papers were smaller than the health gap study. Conclusion: Establishing a foundation to calculate health life expectancy indicators through the development and improvement of oral health level are needed. More studies in the area of health life expectancy estimation research is based on actual prevalence and oral health-related quality of life are also needed.

국내 장애인의 구강 관련에 대한 문헌고찰(2000년-2015년) (Literature review on oral health among people with disabilities in Korea (2000-2015))

  • Objectives: This study consisted of a literature review on oral health among people with disabilities, and to present the factors that should be considered in developing an oral health program for people with disabilities. Methods: The key words 'people with disabilities' and 'oral health' were searched in 4 Korean academic journals and 5 online search engines and a total of 635 papers were identified. Duplicate papers were removed, and the literature selection criteria were applied to the remaining papers. Finally, a total of 45 papers were used in the review. Results: First, people with disabilities were the most common research subjects, followed by dental students, dental hygiene students, dentists, dental hygienist and guardians and special education teachers. Second, the most frequently studied research topic was dental examination, followed by oral health behavior and behavior of using dental clinics. Third, research purposes included the status of oral health, the quality of oral health, dental treatment, the use of oral health service, and oral hygiene behavior and perception. Fourth, the most frequently discussed policy task was expansion of research subjects, followed by oral hygiene and policy development. Conclusions: To enhance the oral health of people with disabilities who have difficulty in keeping their teeth clean and healthy on their own, development of an oral hygiene training program is required not only for people with disabilities, but also for guardians and teachers. It is equally important to examine oral health behavior that could potentially affect the status of oral health and create a more accurate and systematic oral hygiene method. In addition, the government, together with various other research institutes, should conduct an oral health survey of a representative sample of people with disabilities to determine oral health status and facilitate improvements to oral hygiene programs.

초등학생 대상 구강건강교육 프로그램 적용 및 효과 (Application of Oral Health Educational Program and it's Effects among Elementary Students)

  • The purpose of this study was to develop an educational program to promote oral health habits and to analyze it's effects among elementary students. The study was a "one group, pre-test, post-test design", which included 158 3rd grade students from three elementary schools in Seoul. The effects were measured after each group had received a 40 minute program each week for three weeks. The program consisted of a lecture, 1:1 tooth brushing demonstration and practice, and viewing the oral health DVD. The students were given a questionnaire with 26 questions at the beginning and at the end of the study (6 questions about their toothbrushing habits, 5 oral health related habit questions, and 15 oral health related knowledge questions). All students received an oral-health practice pamphlet at the beginning of the program. The students who participated in the program had higher levels of oral health knowledge (t=27.23, p<.001), better toothbrushing habits (t=13.72, p<.001), and oral related habits ($X^2=34.22$, p<.001) after receiving the structured oral health educational program.

일부 특수학교 교사의 구강보건지식, 실천 및 인식도 조사 (Knowledge, practice, and awareness toward oral health in the special school teachers)

  • Objectives : The purpose of this study is to investigate the knowledge, practice, and awareness toward oral health in the special school teachers. Methods : A self-reported questionnaire was filled out by 133 teachers in special education schools in Seoul, Gyeonggi-do, Chungbuk, and Jeonbuk. Data were analyzed using the statistical package SPSS WIN 12.0 for frequency, t-test, one-way ANOVA, Scheffe test(post hoc test), and Pearson's correlation coefficient. Results : In oral health knowledge, the average score of female teachers was 8.31 in comparison to 7.75 in male teachers(p<.05). In oral health practice, the average score of female teachers was 3.71 and that of male teachers was 3.69. In oral health awareness, the average score of female teachers was 4.12 and male teachers had 3.93. The teachers in special schools had a close positive correlation between oral health awareness and practice(r=.448, p<.05). Conclusions : The teachers tended to have better knowledge of oral health but they did not practice the oral health behavior. The teachers have the important role in oral health management for the disabled children by providing the right toothbrushing method.

노인의 구강건강관련 삶의 질과 요인분석 (Factors analysis of the oral health-related quality of life in the elderly)

  • Objectives: The purpose of this study was to compare the differences in oral health-related quality of life among elderly people aged over 65 years, in terms of physical, mental and oral health status and to analyze factors affecting their oral health-related quality of life. Methods: From May 9 to June 23, 2017, we randomly visited aged-care community centers in the metropolitan area, and recruited 222 elderly, aged 65 or older. First, each participant completed a questionnaire consisting of 4 general items: 1 systematic disease, and 3 subjective oral conditions. Afterwards, the researchers interviewed the participants to assess their mental status, using MMSE-DS and recorded the responses. Finally, an oral examination was performed to determine the number of remaining teeth. The average oral health-related quality of life according to each characteristic was analyzed by t-test and ANOVA. Hierarchical multiple regression analysis and Pearson's correlation coefficient analysis were used to analyze the correlations between factors and the factors affecting oral health-related quality of life. Results: The mean oral health-related quality of life was 4.15. Participants with 20 or more remaining teeth demonstrated better oral health-related quality of life than those with 19 or less teeth. Higher oral health-related quality of life was also found among elderly without gingival bleeding, self-reported halitosis and dry mouth. In addition, positive correlation with the number of remaining teeth and negative correlation with gingival bleeding, self-reported halitosis and dry mouth, were noted. Finally, the results of the hierarchical multiple regression analysis indicated that remaining teeth, gingival bleeding, self-reported halitosis and education were influential factors in determining the oral health-related quality of life among the elderly. Conclusions: The results of this study confirmed the necessity of better policy support, and the importance of implementing delivered, elderly-centered oral health education program by professionals to prevent tooth loss and manage periodontal diseases.

지역사회거주 노인의 주관적 구강건강상태와 구강건강문해력이 구강건강관련 삶의 질에 미치는 영향에 대한 융합 연구 (A Convergence Study about Influences of Subjective Oral Health Status and Oral Health Literacy on Oral Health-related Quality of Life among Elderly in Community)

  • 이 연구는 노인의 주관적 구강건강상태와 구강건강문해력이 구강건강관련 삶의 질에 미치는 영향을 파악하기 위하여 시행되었다. 충북 제천시에 거주하는 65세 이상 노인 248명을 대상으로 단면조사를 실시하였다. 주관적 구강건강상태, 구강건강문해력 및 구강건강관련 삶의 질을 자가보고 설문지를 사용하여 2016년 7월 3일부터 21일까지 자료를 수집하였다. 다중회귀분석결과, 노인의 구강건강관련 삶의 질에 영향을 주는 유의한 변수로 주관적 구강건강상태, 구강건강문해력, 동거인 유무, 치아 수, 교육수준 및 종교 등으로 나타났고 이들 변수들은 노인의 구강건강관련 삶의 질을 44.3% 설명했다. 따라서 건강관리 제공자는 노인의 구강건강관련 삶의 질을 증진시키기 위하여 치아 상실을 예방하고 구강건강상태와 구강건강문해력을 향상시키는 중재 프로그램을 고려할 필요가 있다.

취약계층 노인의 구강건강상태에 영향을 미치는 요인 (Factors Affecting the Oral Health Status in Vulnerable Elders)

  • Propose: This study was to identify the factors affecting the oral health status in vulnerable elders receiving home visiting health care service. Methods: A total of 444 elderly people over the age of 65 at 33 public health centers in Daegu and Gyungbuk areas were included in this survey. The data were collected by personal interview with nurses using oral health knowledge, behavior, and status questionnaires from the $10^{th}$ to $22^{nd}$ of July, 2012. The data were analyzed with t-test, one-way ANOVA, $Scheff{\acute{e}}$ test, and stepwise multiple regression, using SPSS program. Results: Factors affecting the oral health status were chewing satisfaction with denture 14.5% (${\beta}=.391$, p<.001), denture care (wash with water) 3.2% (${\beta}=-.187$, p<.01), and types of health insurance (medical aid type1, 2) 1% (${\beta}=-.111$, p<.05). The factors explained 18.7% of variance in the oral health status of vulnerable elderly subjects. Conclusion: The results of this study indicate that in order to improve the oral health status of vulnerable elders, it is necessary to provide oral health education that includes the proper usage of denture to elders who receive home-visiting care.
