55~84세 한국 성인의 현존치아수와 구강기능의 관계 (The Association between Number of Present Teeth and Oral Function in Korean Adults Aged 55~84 Years)

  • 본 연구는 2007~2009년 제4기 국민건강영양조사 자료를 분석하여 만 55~84세 한국인의 현존치아수와 인구사회경제적 요인, 주관적 건강수준, 그리고 구강기능 요인간의 관련성을 파악하고자 하는 목적으로 수행되었으며, 그 결과 성별과 교육수준, 부정구강진료여부, 주관적 건강 및 구강 건강상태, 씹기와 말하기 행동이 현존치아수와 관련된 변수임을 확인하였다. 인구사회경제적 요인 중, 성별에 따른 결과에서는 남성이 여성에 비해 현존치아수가 1.12개 적었으며(p<0.001), 교육수준이 높을수록 현존치아수가 많았다. 부정치과진료시술 유무와 현존치아수의 분석 결과에서는 부정치과진료 경험자가 비경험자보다 2.43개 적은 치아를 가지고 있었다(p<0.001). 주관적 건강 및 구강건강상태는 현존치아수에서 유의한 차이를 보였다. 구강기능 요인과 현존치아수의 관계에서 씹기 행동과 구강기능간의 유의한 차이를 확인하였고, 특히 말하기 행동은 불편함의 정도가 높다고 응답한 그룹일수록 현존치아수가 통계적으로 확연하게 감소하였다(p<0.001). 이러한 결과를 통해 구강기능 중에서 씹기보다는 말하기가 현존치아수와 더 밀접한 관련성을 가진다는 사실을 확인할 수 있었다. 따라서 본 연구의 결과는 만 55~84세 한국인의 구강건강 및 구강기능 향상을 위한 기초자료로 활용될 수 있을 것이다.

구술기록의 온라인 서비스 수립방안 연구 (A Study on the Online Service of Oral History Archives)

  • 최근의 디지털 시대를 맞이하여 구술기록의 온라인 서비스는 기록으로 남지 않은 소중한 역사적 경험과 기억을 범사회적으로 공유 가능한 기반을 제공해주는 유용한 방안이다. 그동안 구술기록은 기록의 결락부분을 보완한다는 차원에서 활발히 생성되어 왔지만, 주로 개인적인 연구 차원에서 이용될 뿐 적극적인 활용방안 모색은 미진하게 이루어져왔다. 구술기록 온라인 서비스는 디지털 기술을 활용한 온라인 접근방식을 통해 현재 및 잠재적 이용자가 필요로 하는 구술기록을 검색하고 활용할 수 있도록 도와주는 서비스라고 할 수 있다. 최근 들어 정보검색 환경의 변화와 구술기록의 다양하고 적극적인 활용방안 모색, 그리고 디지털 구술기록의 생성은 온라인 서비스에 유리하다는 점 등의 여러 관점에서 구술기록의 온라인 서비스가 활발히 검토되고 있지만 아직 초보적인 단계라 할 수 있다. 구술기록 온라인 서비스에서 우선적으로 고려되어야 할 점은 구술기록의 정체성이다. 구술기록이 어떠한 과정과 연구 작업을 통해 생산되었는지, 그리고 이를 통해 무엇을 전달하고자 하는지, 구술기록이 어떤 목적과 의도를 바탕으로 생산되었는지, 그것의 의미는 무엇인지에 대한 기본적인 물음에서 구술기록의 정체성을 발견할 수 있다. 이러한 구술기록의 정체성은 구술기록 맥락관계에 기반을 둔다. 이러한 점에서 구술기록 온라인 서비스는 구술기록의 정체성 형성을 위한 구술기록의 맥락을 고려해야 하며, 나아가 기관의 목적 및 구술기록의 성격에 따라 대중성과 전문성을 적절히 반영하는 것이 필요하다. 이러한 배경에서 본고에서는 구술기록의 정체성을 확보한 온라인 서비스 수립 방안에 대해 고찰하려고 한다.

당뇨상태에 따른 구강상태 및 행태: 제5기 국민건강영양조사 (Oral health and behavior by diabetic status: the fifth Korea national health and nutrition examination survey)

  • Objectives: The aim of this study was to evaluate the oral health status and behavior in Korean diabetic adults. Methods: The study subjects were 11,840 adults who participated in the fifth Korea National Health and Nutrition Examination Survey(2010-2012). Diabetic status was defined by doctors and fasting blood sugar(FBS) level. Oral health status was assessed by decayed-missing-filled teeth(DMFT), community periodontal index(CPI), periodontal disease, denture needs, limitation of oral function, and chewing difficulty. The oral health behavior was evaluated by oral examination within a year, brushing times a day, and use of auxiliary oral product. The data were analyzed by descriptive analysis, chi-square tests and multiple logistic regression analyses. Results: The prevalence rate of diabetes mellitus diagnosed by doctor and FBS was 8.3% and 10.2%, respectively. The prevalence rate of periodontitis was 25.4%. The proportions of $DMFT{\geq}20$, $CPI{\geq}3$, periodontal disease, denture needs, oral function limitation and chewing difficulty in the confirmed diabetic group by doctor were significantly higher than those of the non-diabetic group(p<0.05). In multiple logistic regression analysis, the adjusted odds ratio(aOR) for periodontal disease(aOR=1.73, 95% CI=1.41-2.12), presence of denture needs(aOR=1.40, 95% CI=1.06-1.84), limited oral function(aOR=1.43, 95% CI=1.15-1.78) and chewing difficulty(aOR=1.41 95% CI=1.13-1.77) in diabetic subjects were significantly higher than those of the non-diabetic subjects. There were similar associations between diabetes defined with FBS and oral health. In oral health behavior, diabetic subjects had significantly lower odds ratios for oral examination(aOR=0.76, 95% CI=0.60-0.98), brushing time ${\geq}2$ times(aOR=0.73, 95% CI=0.57-0.93), and auxiliary oral products(aOR=0.74, 95% CI=0.59-0.94). Conclusions: There was a significant relationship between oral health status and behavior in Korean diabetic adults. Further study is needed to evaluate the underlying mechanisms between diabetes mellitus and oral health status.

구강 및 구인두종양수술시 구순-하악골이단 접근법의 이용에 관한 임상적 연구 (A CLINICAL STUDY ON THE APPLICATION OF LABIO-MANDIBULOTOMY APPROACH FOR ORAL AND OROPHARYNGEAL TUMORS)

  • The labiomandibulotomy approch to the oral cavity and oropharygeal region was first described by Roux in 1836 and become popular for oral and oropharyngeal tumors in cases where there are no clinical and radiological signs of mandible invasion. Anterior labiomandibulotomy and swing procedure provides excellent access and facilitate a mandibular resection and subsequent repair. In last two years, 8 cases of oral and oropharyngeal tumors were treated by this approach at the Dept. of Oral & Maxillofacial Surgery, Holy Family Hospital, Catholic University of Korea. And we analyzed postoperative complications as well as functional evaluations, and the results were as follows; In 4 cases, marginal mandibulectomy were combined with labiomandibulotomy and in case of malignancies, neck dissection was performed simultaneously. In almost case, plate and miniscrew fixation was used for osteotomy sites. Histologic evaluation of the resection margins of the specimens revealed tumor free in all cases. The postoperative complications were occured in 3 cases, one case of nonunion, one case of orocutaneous fistula, and one case of wound dehisence. Occlusal stability, jaw movement and swallowing function were acceptable postoperatively in 3 months. From above results, we concluded that, this approach not only provides wide exposure, permitting radical removal of benign and malignant lesions but also preserves function with minimal complications.

구강기능향상운동과 노인의 구강기능 및 의치만족도와의 관련성 (Effects of Oral Exercise on Oral Function and Denture Satisfaction of the Elderly)

  • 이 연구는 구강기능향상운동을 통한 의치사용 노인들의 구강기능의 향상과 의치만족도 및 구강건강관련 삶의 질과의 관련성을 파악하고자 2013년 9월부터 2014년 4월까지 경상북도 청송군 관내 경로당 13개소에서 실시되었다. 구강기능향상운동 프로그램 실시군 79명과 미실시군 71명을 대상으로 구강기능, 의치만족도, 구강건강관련 삶의 질(OHIP-14)을 조사하여 다음과 같은 결과를 얻었다. 구강기능은 실험군에서 타액분비량이 사전보다 사후에 $1.53m{\ell}$ 유의하게 증가하였고(p<0.001), 개구량은 사전보다 사후에 0.33 cm 유의하게 증가하였다(p<0.01). 또한 발음은 사전보다 사후에 6.91회 유의하게 증가하였고(p<0.001), 미각 기능도 0.52점 유의하게 증가하였으며(p<0.001), 실험군과 대조군의 타액분비량, 개구량, 발음의 개선 정도 및 미각의 두 군 간 변화량은 유의한 차이가 있었다(p<0.01). 의치만족도는 실험군에서 고정기능 만족도가 사전에 비해 사후에 0.32점 유의하게 증가하였고(p<0.01), 일반적 치료 만족도는 사전에 비해 사후에 0.26점 유의하게 증가하였으며(p<0.01). 의치만족도 총점은 사전에 비해 사후에 0.22점 유의하게 증가하였다(p<0.01). 실험군과 대조군의 저작기능 만족도, 고정기능 만족도, 일반적 치료만족도, 의치만족도 총점은 두 군 간의 변화량이 유의한 차이가 있었다(p<0.05). 구강건강관련 삶의 질은 사회적 불리와 구강건강관련 삶의 질의 총점에 있어서 두 군 간의 변화량에 유의한 차이가 있었다(p<0.05) 이상의 연구결과로 구강기능향상운동이 의치사용 노인의 구강기능 개선과 의치만족도를 높이는데 긍정적인 영향을 미친다는 것을 확인하였으며, 향후 노인들의 구강건강관련 삶의 질을 향상시킬 수 있는 방안들이 모색될 필요가 있다.

40세 이상 성인의 구강건강상태와 폐 환기장애의 관련성 (Association between oral health status and pulmonary ventilatory defects in adults 40 years or older)

  • Objectives: The purpose of this study is to identify the association between oral health status and pulmonary ventilatory defects. Methods: The 6th (2013-2015) National Health and Nutrition Examination Survey data was used. The study subjects were those aged 40-79 who had pulmonary function examination. Complex samples general linear model analysis, Complex samples cross-tabulation analysis, and Complex samples logistic regression analysis were conducted. Results: In terms of restrictive ventilatory defects, the study subjects (8.3%) who recognized that their oral health status was bad outnumbered those who recognized that their oral health status was good (6.1%) (p<0.05). Many of the study subjects who experienced difficulty in biting, chewing and/or speech, and who had an unhealthy periodontal had restrictive ventilatory defects and obstructive ventilatory defects (p<0.05). The association between oral health status and pulmonary ventilatory defects was analyzed. The findings showed that those who had unhealthy periodontal had a 1.33 times higher probability of pulmonary ventilatory defects than those who had a healthy periodontal (p<0.05). After taking into account general characteristics (age, sex, incomes, education, and smoking) of the subjects, the association between oral health status and pulmonary ventilatory defects was analyzed. The result found that only in cases where one experienced dental caries was an association with pulmonary ventilatory defects found. In other words, those who had dental caries showed a 0.73 times higher probability of pulmonary ventilatory defects than those who had no dental caries (p<0.05). Conclusions: Based on the findings of this research, oral health status was found to be associated with pulmonary ventilatory defects. To improve oral health, it is necessary to provide life-cycle stages based oral health education. Therefore, it is required to develop an oral health education program and develop a national oral health policy.

우리나라 40-50대 중년층의 구강 보건행태 및 상태와 건강 관련 삶의 질(EQ-5D)의 관련성 연구 (A study on the relationship between the oral health behaviors and conditions of middle-aged people in their 40s and 50s in Korea and their health-related quality of life)

  • Objectives: To provide basic data for oral health policy by identifying the differences in oral health behaviors and conditions among middle-aged people in their 40s and 50s and relating these to their health-related quality of life, and to identify factors affecting health-related quality of life. Methods: In this study, secondary data from the National Health and Nutrition Examination Survey were used. Data were gathered from 8,127 participants, of which 2,353 were middle-aged (40-59 years old). Covariance analysis was performed using a complex sample general linear model to investigate the changes in the subjects' health-related quality of life (EQ-5D) according to their general characteristics, oral health behaviors, and oral health status. Results: Oral health behavior was not significantly related to health-related quality of life. In contrast, a higher oral health status (chewing function) was associated with better health-related quality of life. Conclusions: In conclusion, this study suggested a close relationship between oral health status (chewing function) and health-related quality of life. Therefore, it is necessary to recognize the importance of oral health in improving the health-related quality of life of middle-aged people in their 40s and 50s, as well as in informing oral health policies and preventive measures for individuals and communities.

Risk factors for orthodontic fixed retention failure: A retrospective controlled study

  • Objective: To investigate the potential correlation between fixed orthodontic retention failure and several patient- and treatment-related factors. Methods: Patients finishing treatment with fixed appliances between 2016 and 2017 were retrospectively included in this study. Those not showing fixed retention failure were considered as control group. Patients with fixed retention failure were considered as the experimental group. Additionally, patients with failure of fixed retainers in the period of June 2019 to March 2021 were prospectively identified and included in the experimental group. The location of the first retention failure, sex, pretreatment dental occlusion, facial characteristics, posttreatment dental occlusion, treatment approach and presence of oral habits were compared between groups before and after treatment separately by using a Fisher exact test and a Mann-Whitney U test. Results: 206 patients with fixed retention failure were included, 169 in the mandibular and 74 in the maxillary jaws. Significant correlations were observed between retention failure in the mandibular jaws and mandibular arch length discrepancy (P = 0.010), post-treatment growth pattern (P = 0.041), nail biting (P < 0.001) and abnormal tongue function (P = 0.002). Retention failure in the maxillary jaws was more frequent in patients with IPR in the mandibular jaws (P = 0.005) and abnormal tongue function (P = 0.021). Conclusions: This study suggests a correlation between fixed retention failure and parafunctional habits, such as nail biting and abnormal tongue function. Prospective studies with larger study populations could further confirm these results.

Channel Function of TRPML1 Prompts Lipolysis in Mature Adipocytes

  • Increased intracellular levels of $Ca^{2+}$ are generally thought to negatively regulate lipolysis in mature adipocytes, whereas store-operated $Ca^{2+}$ entry was recently reported to facilitate lipolysis and attenuate lipotoxicity by inducing lipophagy. Transient receptor potential mucolipin1 (TRPML1), a $Ca^{2+}$-permeable non-selective cation channel, is mainly expressed on the lysosomal membrane and plays key roles in lysosomal homeostasis and membrane trafficking. However, the roles of TRPML1 in lipolysis remains unclear. In this study, we examined whether the channel function of TRPML1 induces lipolysis in mature adipocytes. We found that treatment of mature adipocytes with ML-SA1, a specific agonist of TRPML1, solely upregulated extracellular glycerol release, but not to the same extent as isoproterenol. In addition, knockdown of TRPML1 in mature adipocytes significantly reduced autophagic flux, regardless of ML-SA1 treatment. Our findings demonstrate that the channel function of TRPML1 partially contributes to lipid metabolism and autophagic membrane trafficking, suggesting that TRPML1, particularly the channel function of TRPML1, is as therapeutic target molecule for treating obesity.
