  • The submandibular gland is the second largest major salivary gland, which secretes 40% of the total daily saliva. Owing to its anatomic characteristics as well as the high viscosity and basicity of the saliva, sialolithiasis is found most commonly in the submandibular gland. Sialolithiasis that cannot be treated by conservative treatment is conventionally removed by an excision of the submandibular gland. Generally, an excision of the submandibular gland is performed via an extra-oral approach but the disadvantages of this treatment include a risk of injuring the facial nerve and scar formation. Case reports have revealed an even less invasive intraoral surgical technique for the removal of sialolith that does not affect the submandibular gland function. The functional recovery of the gland, complications and recurrence rates after surgery with this conservative intraoral procedure were all successful. We report 5 patients from the department of Oral and Maxillofacial Surgery at Dental Hospital, Yonsei University, who had undergone a resection of the sialolith though the intraoral approach with successful results.

  • A new surgical approach to the area of the infratemporal fossa and parapharyngeal space is described. This approach results in a wide-field exposure of the infratemporal fossa, pterygomaxillary space and parapharyngeal space. We used two osteotomies on the patient's mandible and temporary resection of zygomatic arch for superior margin of tumor. Lower lip splitting was not needed because the incision was started in the frontal scalp, curved in front of and below the external auditary canal, and extended anteriorly to the greater horn of hyoid bone on the neck along a skin crease. We had good results without sacrifice of the facial nerve, mandibular function and sensory supply of the face and oral cavity.

  • Objectives : This study was to examine the satisfaction level of participants in a oral exercise program geared toward the improvement of oral function, their reuse intention of the program and their willingness of recommendation in an effort to facilitate the advancement of the program. Methods : The subjects in this study were 85 senior citizens who used senior welfare centers and nursing homes in Seoul. A oral exercise program was implemented 24 times from April to June 2009, twice a week, and the selected elderly people participated in the program 20 times or more. An interview survey was conducted by two trained dental hygienists, and the collected data were analyzed. Results : 1. As for satisfaction level with each part of the program, the elderly people investigated expressed the best satisfaction with the service supplier(a mean of 3.81), followed by the usefulness of the procedure of the program(3.77) and oral exercise(3.64). 2. The satisfaction level with oral exercise had a significant impact on their overall satisfaction with the program, reuse intention and willingness of recommendation, and the entire satisfaction level with the program was correlated to reuse intention and willingness of recommendation(p<0.001). 3. Out of the demographic characteristics, marital status and whether to live with their families or not made a significant difference to their satisfaction level with oral exercise(p<0.05). Conclusions : It took 20 minutes to get oral exercise, which consisted of warming-up, exercise for each part of oral and cooking-down, and they found it appropriate to spend that time and had no difficulties in repeating after the instructor.

  • Objectives: The purpose of the study is to investigate the change of the salivary flow rate, xerostomia, and oral health-related quality of life in the elderly people after the application of oral massage. Methods: The subjects were 101 elderly people at two senior welfare centers in Mokpo from November, 2012 to January, 2013 by the application of oral massage. The final subjects were 56 elderly people who participated in the massage more than 15 times of 20. They were measured for saliva flow rate, xerostomia, and OHIP. On the first and the last day, a self-reported questionnaire was completed by the elderly people. The questionnaire consisted of general characteristics of the subjects, oral health related knowledge, symptoms and behavior of xerostomia, and OHIP. OHIP included functional limitation, physical pain, psychological discomfort, physical disability, psychological disability, social disability, and handicap, and was measured by Likert 5 point scale. Results: The salivary flow rate of the elderly people increased after the implementation of the program. There was a significant improvement in xerostomia, functional limitation, physical pain, psychological discomfort, and social efficacy (p<0.001). Conclusions : The oral massage program enhanced the oral function of the elderly people, and had an influence on the improvement of oral health-related quality of life.

  • The present report describes the case of a patient who underwent maxillary sinusitis right after dental implant installation with sinus lifting. Computed tomography scan revealed a dental implant (#16) was protruded inside the right maxillary sinus and confirmed the obstruction of ostium. A symptom remission was gained with the dual approaches combined by functional endoscopic sinus surgery and an intra-oral approach. Fully recovered function and healing of sinus were identified after 10 months follow-up. We report the case of sinusitis caused by protrusion of implants with sinus floor lift procedures and propose that practitioners should be aware of the possible its complications and management.

  • The sialolithiasis is one of the most common disease that is found in the submandibular gland and they can be usually observed in the extra glandular area. As the lumen of Wharton's duct is larger and more expandable than that of Stensen's duct, the symptom of them is initially painless. Obstruction of salivary secretion by a sialolith can result in sudden swelling and pain, as well as infection of the gland. It can be observed with clinical exam and the scintigraphy. After removal of sialoliths, the majority of them can get the recovery of function. Our department performed the transoral sialolithotomy. One of three cases was recurred sialolithiasis with sialadenitis and was performed by the re-sialolithotomy and extraoral sialoadenectomy.

  • There are many kinds of techniques for imaging temporomandibular joints. None of them for imaging "best fit" for every patient of temporomandibular disorder is recommended ideally. It is more important to be able to select the modality that is most adequate and appropriate for a given clinical issue.

  • Temporomandibular disorders have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory muscles, and associated structures. Since Dr. James Costen has mentioned about the relationship between TMD and posterior bite collapse, there have been many controversies about the etiology and comorbidities of TMD. The purpose of this study was to investigate the relationship between TMD and hearing acuity in 158 volunteers with or without TMD symptoms. The subjects were examined clinically about TMJ sound, missing teeth, tinnitus, TMJ pain and masticatory muscle pain and the hearing acuity were measured by Audiometer(ITO AE-1000, Japan). The result of this study indicated that there was no significant differences between TMD symptoms and hearing acuity relatively.

  • Temporomandibular disorders (TMDs) are diseases that affect the temporomandibular joint and supporting structures. The goal of treatment for TMDs is elimination or reduction of pain and return to normal temporomandibular joint function. Initial treatment for TMDs is non-invasive and conservative, not surgical. Oral and maxillofacial surgeons should fully understand and actively care about non-invasive treatments for TMDs. The purpose of this study is to review the validity and outcomes of non-invasive and surgical treatment modalities for TMDs.
