Electromyographic Activities of the Sternocleidomastoid Muscle during Masticatory Function (저작기능이 흉쇄유돌근의 근활성도에 미치는 영향)

  • The author has synchronously recorded the average electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muacles and the masseter muscles. The marious levels of occlusal force were checked during clenching the resin plates which are made to fit each of the maxillary and the mandibular teeth. These activities were recorded in order to study the EMG activity pattern of the sternocleidomastoid muscle during the masticatory function of the jaw in 11 healthy subjects. The obtined results were as follows : 1. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles were increased as the occlusal force increased, and the sternocleidomastoid muscles have a lineal correlationship with the occlusal force. 2. The sternocleidomastoid muscles and the masseter muscles showed higher EMG activity during clenching at the ventroflexed head position rather than at the extended head position. (p<0.05) However the EMG activities of the anterior temporal muscles showed no difference between the ventroflexed position and the extended position of the head. 3. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles during clenching are similar at the habitual position and at the retruded condylar position. 4. The electromyographic activities of the sternocleidomastoid muscles, the anterior temporal muscles and the masseter muscles on the working side during mastication of gum and almonds are significantly higher than on the balancing side except the masseter muscles during mastication of almonds. (p<0.05, P<0.01) 5. The asymmetry of gum are lower than that during mastication of almonds. (p<0.05) The asymmetry indices of the sternocleidomastoid muscles are higher than those of the anterior temporal muscles and masseter muscles.

  • The management of mandibular condyle fractures continues to be a subject of much debate. It is suggested that, if not properly managed, these fractures may give rise to serious problems, such as malocclusion, mouth opening limitation, temporomandibular joint disorders. Treatment planing of mandibular condyle fractures is very important. The aim of this present study was to evaluate the long-term results according to treatment methods in condylar fractures. Also, it was to evaluate results of treatment according to condylar fracture level. We conducted a retrospective analysis of 43 mandibular condyle fractures. 43 patients followed for average period of 9.00 years(mini. 7yr, max. 12yr). All patients underwent a clinical and radiologic evaluation focusing on mouth opening, mandibular movements, TMJ function, change of ramal height, condylar remodelling. If the level of fracture was positioned in high, especially in level II, mandibular movement disability and ramus length loss was more prominent. This results were similar to the cases of treatment of fragment removal. In high level fracture and fragment removal cases, It is thought that more intensive and long term management are needed than other treatment cases using different operation methods. Also, direct fixation by each approach showed good results in mandibular movement, ramal height change and condylar shape. Through this results, accurate reduction of the mandibular condyle fractures was a very important factor in postoperative prognosis.

Effects of Dysphagia Treatment Applied to Infants with Pierre Robin Syndrome - Single Subject Research Design

  • Pierre Robin syndrome is characterized by micrognathia, glossoptosis, and cleft palate. Infants with Pierre Robin syndrome causes feeding difficulty, upper airway obstruction, and other symptoms. This study aims to examine the effects of applying dysphagia treatment to infants with Pierre Robin syndrome. The study participant was an infant who was born four weeks premature and referred for dysphagia treatment approximately 100 days after birth. At the initial assessment, the infant showed oral sensory sensitivity, a high level of facial and masticatory muscle tension, and a low stability of the chin and cheeks with almost no normal "sucking-swallowing-breathing" pattern. We set the baseline period and intervention period using the AB design. During the baseline period, non-nutritive sucking training using a rubber nipple was conducted without implementing an oral stimulation intervention. During the intervention period, non-nutritive sucking training and an oral stimulation intervention were performed. After the intervention period, the infant's daily oral intake and oral intake per time significantly increased compared to that during the baseline period. We observed that the oral intake time of the infant decreased during the intervention period compared to that in the baseline period, which indicated an improvement in control over the chin, tongue, and lip movements, a change in muscular tension, and stabilization of the "sucking-swallowing-breathing" pattern. We provided dysphagia treatment before breastfeeding, it was positive effects such as normal development of the infant, transition from tube feeding to bottle feeding, and enhancement of overall oral motor function.

EFFECTS OF CHITOSAN ON HUMAN OSTEOBLASTS (키토산이 조골세포에 미치는 영향)

  • As the result of the study concerning "bone inducibility of chitosan", 1. "BMP-2" was observed mainly through the test when the "osteoblast" is exposed to the "chitosan". The expression of BMP-2 was 542.63 times compared to control after 2 hours exposure and it was maintained 16.60 times till 24 hours. 2. The expression of BMP-4 was decreased compared to control during exposure. 3. The expression of BMP-7 revealed two peaks during exposure. 4. The expression of osteocalcin was increased in early phase, and then decreased. Although it is not clear whether the "chitosan" is clinically effective material as a "bone induction material", we could say that it has a function for bone induction. Further detailed study will be required.

  • Aucubin, the natural product, which is isolated from Aucuba japonica, has a variety of pharmacological effects such as liver-protective function, inhibition of liver RNA and protein biosynthesis, hypotensive activity and antimicrobial effect, etc. This study was performed to investigate the effects of iridoid compounds on wound healing. The author prepared 0.1% aucubin solution and 0.1% aucubin ointment as an active form, aucubigenin to which aucubin was converted by ${\beta}$-glucosidase. Artificial surgical wound was made on either 1cm lateral side of the dorsal midline along the axis of spine of Sprague-Dawley rats under sterile technique. Application of 0.1% aucubin solution or 0.1% aucubin ointment to surgical wound was done daily. Light microscopic examination was performed on the postsurgical 3 days, 5 days, and 9 days. The 0.1% aucubin solution group epithelialized earlier than the control group and the fibrosis of granulation tissue of both aucubin groups were more prominent than the control group. Collectively, this study suggests the possibility of aucubin as a topical agent. Further research should be performed on the mechanism of aucubin on wound healing and proper formulation for effective topical agents.

  • Objectives : The purpose of this study was to investigate the oral health center operation and to activate the program development of public oral health center. Methods : Delphi analyses were utilized by the recruited expert panel from August to September in 2012. Results : The difficulty in operation of the oral health centers is the human resources management. In order to solve the human resources operation, utilization of the local human resources is the best choice of the solution. The next choice was the designation of the oral health expert in Ministry of Health and Welfare. So the expert can activate the public oral health care system for the disabled and the underprivileged classes in the long term. Conclusions : In order to activate the oral health centers, Ministry of Health and Welfare should promote the public health center function connected with the cooperation of the Association of dental hygienists.

Removal of submandibular calculi by surgical method and hydraulic power with curved needle: a case report

  • Sialolithiasis, the most common salivary gland pathology, is caused by calculi in the gland itself and its duct. While patients with small sialoliths can undergo conservative treatment, those with standard-size or larger sialoliths require sialolithotomy. In the present case study, we removed two sialoliths located beneath the mucosa in the posterior and anterior regions of Wharton's duct, respectively. For the posterior calculus, we performed sialolithotomy via an intra-oral approach; thereafter, the small anterior calculus near the duct orifice was removed by hydraulic power. This method has not previously been reported. There were no complications either during the operation or postoperatively, and the salivary function of the gland remained normal.

  • The literature on alloplastic total temporomandibular joint (TMJ) replacement is encouraging, with acceptable improvement of treatment outcomes in terms of both pain level and jaw function. This is a case report on patients who suffered from degenerative joint disease and ankylosis after mandibular condyle fracture or prior TMJ surgery and were treated by TMJ replacement with condyle prosthesis. We obtained good results from the procedures, including total TMJ replacement.

  • Benign neural sheath neoplasms are not common in the maxillofacial region. These lesions can occur as solitary tumors, or they can affect many sites in the form of multiple neurofibromatosis. A solitary neurofibroma is seldom undergo sarcomatous transformation, since solitary neurofibroma is relatively radioresistant and its recurrence rate seems to be low, the treatment of choice is surgical excision. This case showed a solitary neurofibroma in the left side of the floor of mouth which occurred in a 33 - year -old female. The tumor was excised. And there is no evidence of disease. She is satisfied in function and esthetic aspect.

Relationship between Chemical Structure and Antimicrobial Activities of Isothiocyanates from Cruciferous Vegetables against Oral Pathogens

  • We evaluated the potentials of 10 isothiocyanates (ITCs) from cruciferous vegetables and radish root hydrolysate for inhibiting the growth of oral pathogens, with an emphasis on assessing any structure-function relationship. Structural differences in ITCs impacted their antimicrobial activities against oral pathogens differently. The indolyl ITC (indol-3-carbinol) was the most potent inhibitor of the growth of oral pathogens, followed by aromatic ITCs (benzyl ITC (BITC) and phenylethyl ITC (PEITC)) and aliphatic ITCs (erucin, iberin, and sulforaphene). Sulforaphene, which is similar in structure, but has one double bond, showed higher antimicrobial activity than sulforaphane. Erucin, which has a thiol group, showed higher antimicrobial activity than sulforaphane, which has a sulfinyl group. BITC and iberin with a short chain exhibited higher antimicrobial potential than PEITC and sulforaphane with a longer chain, respectively. ITCs have strong antimicrobial activities and may be useful in the prevention and management of dental caries.
