Cases Report on Conservative Treatment for the Chronic Closed Lock of Temporomandibular Joint

  • Some treatment methods have been proposed for patients with chronic closed lock of temporomandibular joint. We report a conservative treatment for patients who had chronic closed lock of temporomandibular joint and who did not want surgical treatment. Two patients who had been treated in the Template clinic, Soonchunhyang University Bucheon Hospital, are the subjects of this report. The subjects had chronic closed lock symptom for over 3 months after an onset of locking; conventional therapies had no effect. The subjects were treated by making them wear a Template appliance while sleeping and exercise for 10 hours a day. After periodic follow-up, significant improvement was observed for Template treatment in terms of the maximal mouth opening range. When conventional therapy is expected to be ineffective, The Template appliance can be used as conservative treatment for temporomandibular disorders patients with chronic closed lock of temporomandibular joint.

The Effects of Bilateral Chewing Exercise on Occlusion Force and Masseter Muscle Thickness in Community-Dwelling Elderly (양측저작 운동이 지역사회 거주 노인의 교합력과 깨물근에 미치는 효과)

  • Objective : The purpose of this study is to investigate the effects of bilateral chewing exercise applied for 6 weeks on occlusion force and masseter muscle thickness in the elderly living in the community. Methods : This study recruited 25 community residents. All participants performed bilateral chewing exercise using equipment developed for the purpose of oral chewing exercise. The chewing exercise was divided into isometric and isotonic type and applied for about 20 minutes a day, five times a week for six weeks. For the evaluation, the masseter muscle thickness and the maximum occlusion force were measured three times at three week intervals using a portable ultrasound instrument and an occlusion force gauge. Results : As a result of the change in masseter muscle thickness, baseline, 3 weeks later, and 6 weeks later referred to 7.51±0.43, 7.63±0.44, and 7.83±0.46, respectively (F=3.819, p<.05). The post hoc test resulted in a significance between baseline and 6 weeks later (p=0.023). Similarly, as a result of the change in occlusion force, baseline, 3 weeks later, and 6 weeks later referred to 265±9.22, 268±9.57, and 271.59±10.16, respectively (F=3.031, p<.05). The post hoc test resulted in a significance between baseline and 6 weeks later (p=0.048). Conclusion : This study confirmed that bilateral chewing exercise was effective for increasing masseter muscle thickness and occlusion force in the elderly. Therefore, bilateral chewing exercise can be applied as a therapeutic exercise method for improving oral function.

Performance of elderly oral health management and related factors among care workers in long-term-care hospitals (요양병원 요양보호사의 노인 구강건강관리 수행도 관련요인)

  • Objectives: Oral health management is important to improve the quality of life among the elderly. This study investigated the performance of elderly oral health management among some care workers in long-term-care hospitals. Methods: The study subjects were 174 care workers in 10 long-term-care hospitals. Data on general characteristics of care workers, attitude, recognition and knowledge of elderly health, performance of elderly oral health management were collected by a self-administered questionnaire. Data were analyzed through descriptive analysis, t-test, ANOVA, correlation and multiple regression analysis by using a SPSS version 23.0 statistical program. Results: The performance score of oral health management was $4.34{\pm}0.64$ on the 5-point Likert scale. The subjects who exercised more than 2 times a month were significantly higher in their performance of elderly oral health management compared to subjects who did not exercise (${\beta}=0.232$, p=0.035). And, the subjects who cared 10-19 persons were significantly higher in performance of elderly oral health management compared to subjects who cared more than 20 elderly (${\beta}=0.246$, p=0.020). The oral health behavior of care worker (${\beta}=0.271$, p<0.001) and the knowledge of oral health care (${\beta}=0.055$, p=0.008) were positively related to the performance of elderly oral health management. Conclusions: The educational program designed to improve knowledge of care workers in accordance with the standard textbook for training care workers should be developed, and the long term education program should be reinforced to improve the performance for elderly oral health care. If care workers can care a proper number of old persons, they will give oral health care to them.

Effects of Salvianolic Acid B Against Oxidative Stress in Skeletal Muscle and Brain Tissue following Exhaustive Exercise in Rats (Salvianolic acid B가 고강도 운동부하에 의한 흰쥐 골격근과 뇌조직의 Oxidative Stress에 미치는 영향)

  • Objectives : Salvianolic acid B (SAB) is an active ingredient in Salvia miltiorrhiza frequently used for cardiovascular and cerebrovascular diseases. The present study investigated the antioxidant effects of SAB on the skeletal muscle and the brain tissue of rats following exhaustive exercise.Methods : The rats were treated with oral administration of SAB (30 mg/kg) daily for 5 days prior to the exhaustive exercise. The exhaustive exercise was performed as swimming for 150 min with 5% body weight attached to the tail on the 5th day. The antioxidant effects of SAB was evaluated by measuring the superoxide generation in the gastrocnemius and the 4-HNE expression in the hippocampal tissue. In addition, c-Fos-expressing cells in the brain tissue was observed using immunohistochemistry.Results : Histological features and muscle fiber type composition were not different between the SAB group and the exhaustive exercise group. SAB significantly reduced the upregulation of superoxide generation in the muscle tissue. SAB significantly reduced the increase of c-Fos-expressing cells in the cerebral cortex, paraventricular thalamic nucleus, dorsomedial hypothalamic nucleus, the CA1, CA3, and DG regions of hippocampus. SAB significantly reduced the upregulation of 4-HNE expression in the CA1 and DG regions of hippocampus caused by the exhaustive exercise.Conclusions : The results suggest that SAB exerts antioxidative effect against oxidative stress in the skeletal muscle and the brain tissue following exhaustive exercise, while SAB may has an anti-stress effect on stress responses in the brain.

Effects of long-term endurance exercise and Salvia miltiorrhiza vinegar on body composition and insulin resistance in high fat diet-induced obese rats (장기간의 지구성 운동과 단삼식초 섭취가 고지방식 유도 비만 흰 쥐의 신체구성과 인슐린 저항성에 미치는 영향)

  • The purpose of this study was to examine the effects of long-term endurance exercise and salvia miltiorrhiza vinegar on body composition and insulin resistance of high-fat diet (30% carbohydrate, 50% fat and 20% protein) induced obese rats. After 8 weeks of high fat diet (50% of total calories), rats were divided into 4 groups (sedentary group, n=10; exercise group, n=10; Salvia miltiorrhiza vinegar group, n=10; exercise+Salvia miltiorrhiza vinegar group, n=10) for 8 weeks. Body weight, body composition, diet intake volume, oral glucose tolerance test, plasma total cholesterol were measured. The results showed that Salvia miltiorrhiza vinegar plus endurance exercise training for 8 weeks significantly improved body weight control, visceral fat weight, and insulin resistance. However, only Salvia miltiorrhiza vinegar treatment did not significantly improve body composition and insulin resistance. In addition, there was no additive by the combination of Salvia miltiorrhiza vinegar and endurance exercise in insulin, body fat, and total cholesterol. The reduction of body fat, glucose, insulin and cholesterol by combination was resulted from the exercise. These results suggest that Salvia miltiorrhiza vinegar has slight effect on anti-hyperglycemia and anti-obesity.

TREATMENT OF TRISMUS BY CORONOIDECTOMY (오훼돌기 절제술에 의한 개구장애의 치료)

  • There are many causes of trismus. Aetiology can be roughly divided into muscle spasm, mechanical interference, extracapsular ankylosis, intracapsular ankylosis. Trismus caused by mechanical interference between postero-lateral wall of zygoma-maxillary complex and coronoid process following reduction of fractured facial bone is rare. Especially on maxillary bone fracture, when we faced the trismus following removal of intermaxillary fixation, we got used to solve that problem by physical exercise. We obtained good results by coronoidectomy on patients with limited mouth opening who were referred from department of plastic surgery, St. Marys' hospital, the Catholic university of Korea. We report our experience with literature review.

Analysis of Dental Care Clinic Visits and Causes of Tooth Injuries by Age and Sex (외상에 의한 치아손상의 연령 및 성별에 따른 의료이용분석)

  • Objectives: Using data from the 1styear of the 7th National Health and Nutrition Survey, this study administered a questionnaire to patients with oral damage and examined the type of medical care used based on the nature of the injury, further characterized by age group and sex. Methods: Of the 8,150 respondents, this study selected those who participated in the health survey and oral exam survey, excluding surveys with missing values, and set 7,681 people as the research participants. This study used SPSS Windows version 23.0(SPSS Inc. IL, USA) as the statistics program and applied the chi-square test (p) based on a complex sample and logistic regression analysis. Results: In the analysis of the type of tooth damage, there was a significant difference (p<.001) in the number of male patients who were injured due to exercise, violence, and safety accidents compared to females. However, there was no significant difference in their type of medical security and whether or not they visited a dental clinic. The patients with private insurance showed relatively high use for tooth extraction and oral surgery (p<.001). Conclusions: The best way to prevent tooth damage is to prevent the accident. For sports or exercises which may cause tooth damage, protective gear, such as a mouthpiece, should be used to prevent tooth damage.

  • Management of migraine, one of common primary headache disorders, involves the diverse strategies non-pharmacological treatment, such as headache diary, lifestyle modification, regular exercise and relaxation, cognitive behavioral therapy and neurostimulation, and pharmacological treatment. Among the treatments, this review described a pharmacologic treatment of migraine, classified into acute and preventive treatment based on the severity and the frequency of headache. It introduced the way to optimize pharmacological treatment and updated the latest treatment for migraine.

Extensive Bilateral Subcutaneous Emphysema after Dental Treatment: Two Case Reports

  • We report two rare cases of extensive bilateral subcutaneous emphysema that occurred during ordinary dental procedures. An air-driven high-speed handpiece, routinely used in dental procedures may cause subcutaneous emphysema when high pressure air is introduced into the loose connective tissues below the dermal layer. The first case occurred with surgical extraction of the lower third molar. The air introduced into the fascial spaces near the surgical field spread to the contralateral spaces, as well as the neck and chest areas. The second case also showed extensive bilateral subcutaneous emphysema caused by the introduction of compressed air from the handpiece during crown preparation without any invasive procedure. Cases where the emphysema extends beyond the treatment site to involve the contralateral cervicofacial areas have been rarely reported. Predicting the occurrence of subcutaneous emphysema is difficult, so it is important to exercise caution during routine dental treatment. If significant bilateral cervicofacial swelling is suspected to be due to subcutaneous emphysema, prompt diagnosis with securing the patient's airway will be necessary.

Factors Influencing Osteoporosis (폐경기 전.후 여성의 골다공증 영향요인)

  • Purpose: This study was carried out to identify factors influencing osteoporosis in women at pre- and post-menopausal state. Methods: The subjects of this study were 52 pre-menopausal and 125 post-menopausal women who were assessed of bone density in one general hospital. The data were collected through review of clinical records and telephone interviews using a questionnaire. Results: In the pre-menopausal women, the factors influencing osteoporosis were regular exercise (protective factor) and age (risk factor). Regression analysis showed that the factors attributable to osteoporosis included educational level, weight, age and number of pregnancy, accounted for 41.89% of the total variance. In the post-menopausal women. the factors influencing osteoporosis were age (risk factor), low educational level (risk factor), low economical state (risk factor), high parity (risk factor), and intake of coffee (protective factor). Regression analysis also showed that factors attributable to their osteoporosis included age, educational level, number of delivery, intake of the coffee, regular exercise, number of pregnancy and duration of oral pill intake, accounted for 37.41 % of the total variance. Conclusion: In pre-menopausal women, regular exercise was one of the most powerful determinant of their bone mass. Therefore, it is necessary to participate in a regular exercise program to maintain peak bone mass density prior to the onset of menopause. In post-menopausal women, increased age was the most influencing factor of their bone mass. Therefore, it is essential to establish early diagnosis and management of osteoporosis after menopause.
