Therapeutic Outcomes of Pectoralis Major Muscle Turnover Flap in Mediastinitis

  • Background: This study aimed to evaluate the therapeutic results and safety of pectoralis major muscle turnover flaps in the treatment of mediastinitis after coronary artery bypass grafting (CABG) procedures. Methods: Data regarding 33 patients with post-CABG deep sternal wound infections (DSWIs) who underwent pectoralis major muscle turnover flap procedures in the Emam Reza and Ghaem Hospitals of Mashhad, Iran were reviewed in this study. For each patient, age, sex, hospital stay duration, remission, recurrence, and associated morbidity and mortality were evaluated. Results: Of the 2,447 CABG procedures that were carried out during the time period encompassed by our study, DSWIs occurred in 61 patients (2.5%). Of these 61 patients, 33 patients (nine females [27.3%] and 24 males [72.7%]) with an average age of $63{\pm}4.54$ years underwent pectoralis major muscle turnover flap placement. Symptoms of infection mainly occurred within the first 10 days after surgery (mean, $10.24{\pm}13.62days$). The most common risk factor for DSWIs was obesity (n=16, 48.4%) followed by diabetes mellitus (n=13, 39.4%). Bilateral and unilateral pectoralis major muscle turnover flaps were performed in 20 patients (60.6%) and 13 patients (39.4%), respectively. Complete remission was achieved in 25 patients (75.7%), with no recurrence in the follow-up period. Four patients (12.1%) needed reoperation. The mean hospitalization time was $11.69{\pm}6.516days$. Four patients (12.1%) died during the course of the study: three due to the postoperative complication of respiratory failure and one due to pulmonary thromboembolism. Conclusion: Pectoralis major muscle turnover flaps are an optimal technique in the treatment of post-CABG mediastinitis. In addition to leading to favorable therapeutic results, this flap is associated with minimal morbidity and mortality, as well as a short hospitalization time.

A STUDY OF CLINICAL RESULTS ON STERI-OSS ENDOSSEOUS IMPLANTS (Steri-Oss 임플랜트의 임상 결과에 관한 연구)

  • This investigation evaluated patients who received Steri-Oss implants from the Dental Hospital of Chosun University during the period from March 1989 to August 1997. 346 fixtures of 127 patients were included in this study. The results were as follows ; 1.The follow-up period was defined as the period between the surgical placement of the implants and the last follow-up examination. The mean follow-up period was $2.17{\pm}1.21$ years. 2.The period between fixture installation and second surgery was $0.71{\pm}0.44$ years in the maxilla and $0.46{\pm}0.21$ years in the mandible. 3.The number of fixtures which were installed in the upper jaw(112) was less than that in the lower jaw(234) and in the posterior region(260) was more than in the anterior region(86). 4.The length of fixture which was most frequently used was 12 mm and least was 8mm. Screw implants were installed more than cylindrical implants. 3.8mm implant was the most common implans, followed by 4.5mm and 3.25mm. 5.The number of augmentation cases was more than that of non-augmentation cases and the rate of augmentation cases in the maxilla was more than that in the mandible. 6.Implant restorations for partial edentulos patients(94cases) were more than single- tooth implant restorations(33cases) or implant restorations for complete edentulos patients(10cases). 7.Free-standing prostheses for partially edentulous patients were more commom than any other type of connection between implants and natural teeth. 8.Plaque Index($0.95{\pm}0.74$) and Gingival Index($0.31{\pm}0.52$) were very similar around the natural teeth and reflected an acceptable level of plaque and gingivitis control. Mean value for keratinized mucosa index($1.93{\pm}1.20$) remained fairly constant around level 2(1-2 mm keratinized epithelium). 9.Patients were generally satisfied with implant in terms of comfort, function, speech and esthetics. 10.There was not a statistically significant differences in overall survial rate between implants placed in the maxilla (91.5%) and those placed in the mandible (93.8%). Fourteen implants lost before the prosthetic rehabilitation and eleven implants lost following variable periods in function after the prosthetic phase of the treatment. 11.Cause of implant failures was exfoliation or removal of fixture due to non-osseointegration before the prosthetic rehabilitation or due to fracture of fixture, masticatory pain after the prosthetic rehabilitation. 12.The survival rate of Steri-Oss implants using the Kaplan-Meier statistical analysis was 93.8% at 2 year and 86.6% at 5 year, In all cases, implant losses occured predominantly in the healing period. There was a steep decline in the rate of implant loss after the first year. 13.The survival rate of Steri-Oss implants in the anterior region was 94.8% at 2 year and 94.8% at 5 year and that in the posterior region was 92.8% at 2 year and 75.9% at 5 year. In conclusion, this study revealed a number of parameters and guidelines for achieving an optimal success rate in osseointegration.

Creation and labeling of multiple phonotopic maps using a hierarchical self-organizing classifier (계층적 자기조직화 분류기를 이용한 다수 음성자판의 생성과 레이블링)

  • Recently, neural network-based speech recognition has been studied to utilize the adaptivity and learnability of neural network models. However, conventional neural network models have difficulty in the co-articulation processing and the boundary detection of similar phonmes of the Korean speech. Also, in case of using one phonotopic map, learning speed may dramatically increase and inaccuracies may be caused because homogeneous learning and recognition method should be applied for heterogenous data. Hence, in this paper, a neural net typewriter has been designed using a hierarchical self-organizing classifier(HSOC), and related algorithms are presented. This HSOC, during its learing stage, distributed phoneme data on hierarchically structured multiple phonotopic maps, using Kohonen's self-organizing feature maps(SOFM). Presented and experimented in this paper were the algorithms for deciding the number of maps, map sizes, the selection of phonemes and their placement per map, an approapriate learning and preprocessing method per map. If maps are divided according to a priorlinguistic knowledge, we would have difficulty in acquiring linguistic knowledge and how to alpply it(e.g., processing extended phonemes). Contrarily, our HSOC has an advantage that multiple phonotopic maps suitable for given input data are self-organizable. The resulting three korean phonotopic maps are optimally labelled and have their own optimal preprocessing schemes, and also confirm to the conventional linguistic knowledge.

  • This Paper deals with the experimental study of the axisymmetric air jet impinging vertically on the flat heating surface with and without swirl. The purpose of this study is to investigate the characteristics of flow, augmentation of heat transfer rate, turbulent intensity, and the comparison of heat transfer rate, the optimal swirling condition about the swirl and nonswirl axisymmetric air jet. In order to augment the heat transfer on the flat heating surface without introducing any additional power, the technique used in the present work was placement of twisted tape inserted pipe in front of the nozzle exit in order to make a swirl. The effect of swirl degree is investigated in case of S=0., 0.056, 0.111, 0.222 and the velocity of the jet was 14, 20, 26, 32, 38, 44m/s. The distance between the nozle exit and the stagnation point on the impinging plate was the H/D=$1{\sim}14$. In order to analyze of the flow structure which increase heat transfer, the velocity and the turbulent intensity of the axisymmetric jet was measured by a hot wire anemometer according to the swirl number and H/D.

A Numerical Analysis on the Diaphragm and Cutout Structures for Improvement of Structure Performance in Orthotropic Steel Decks (강바닥판 구조성능 개선을 위한 보강재 설치에 관한 매개변수해석)

  • Transverse rib web of orthotropic steel decks is highly susceptible to stress concentration due to out-of-plane and oil canning deformation caused by longitudinal rib distortion. In particular, stress concentrations are observed in the crossing point of longitudinal rib-transverse rib-deck plate, and cutout parts of transverse rib. The main objective of this study is to improve structure performance and to reduce the stress concentration of aforementioned susceptible parts. It is known that the installation of diaphragm alleviates stress concentrations between crossing point ant cutout. The influence of transverse rib placement and cutout width on stress concentrations was thoroughly investigated through numerical analyses. The numerical result showed that diaphragms produce the structural details for improved structure performance, when the transverse rib was placed in the same location with diaphragms. In any case, it is certain that the installation of diaphragms has more advantageous than the case without diaphragms in terms of structure performance of orthotropic steel decks. In this study, the distance ratio($y_i/y_{total}$) is defined as the ratio of the distance($y_{total}$) between the deck plate and longitudinal rib bottom to the distance($y_i$) between the deck plate and crossing point of longitudinal rib-transverse rib in cutout part. It has been found that the optimal distance ratio was 0.85 from the numerical simulation.

Three Dimensional Measurement of Ideal Trajectory of Pedicle Screws of Subaxial Cervical Spine Using the Algorithm Could Be Applied for Robotic Screw Insertion

  • Objective : To define optimal method that calculate the safe direction of cervical pedicle screw placement using computed tomography (CT) image based three dimensional (3D) cortical shell model of human cervical spine. Methods : Cortical shell model of cervical spine from C3 to C6 was made after segmentation of in vivo CT image data of 44 volunteers. Three dimensional Cartesian coordinate of all points constituting surface of whole vertebra, bilateral pedicle and posterior wall were acquired. The ideal trajectory of pedicle screw insertion was defined as viewing direction at which the inner area of pedicle become largest when we see through the biconcave tubular pedicle. The ideal trajectory of 352 pedicles (eight pedicles for each of 44 subjects) were calculated using custom made program and were changed from global coordinate to local coordinate according to the three dimensional position of posterior wall of each vertebral body. The transverse and sagittal angle of trajectory were defined as the angle between ideal trajectory line and perpendicular line of posterior wall in the horizontal and sagittal plane. The averages and standard deviations of all measurements were calculated. Results : The average transverse angles were $50.60^{\circ}{\pm}6.22^{\circ}$ at C3, $51.42^{\circ}{\pm}7.44^{\circ}$ at C4, $47.79^{\circ}{\pm}7.61^{\circ}$ at C5, and $41.24^{\circ}{\pm}7.76^{\circ}$ at C6. The transverse angle becomes more steep from C3 to C6. The mean sagittal angles were $9.72^{\circ}{\pm}6.73^{\circ}$ downward at C3, $5.09^{\circ}{\pm}6.39^{\circ}$ downward at C4, $0.08^{\circ}{\pm}6.06^{\circ}$ downward at C5, and $1.67^{\circ}{\pm}6.06^{\circ}$ upward at C6. The sagittal angle changes from caudad to cephalad from C3 to C6. Conclusion : The absolute values of transverse and sagittal angle in our study were not same but the trend of changes were similar to previous studies. Because we know 3D address of all points constituting cortical shell of cervical vertebrae. we can easily reconstruct 3D model and manage it freely using computer program. More creative measurement of morphological characteristics could be carried out than direct inspection of raw bone. Furthermore this concept of measurement could be used for the computing program of automated robotic screw insertion.

A Study on Reinforcement Method of Concrete Block for Direct Fixation Tracks on Serviced Light Rail Transit (공용중인 경전철 직결 궤도 콘크리트 도상블록의 보강 방안 연구)

  • In this study, numerical analysis was performed based on field investigation to derive an appropriate reinforcement method by analyzing the displacement behavior characteristics of concrete blocks generated in the direct fixation track on the bridges of the serviced light rail transit. The track of this study was a direct fixation track on a sharp curved track, and the problem of movement of the concrete blocks installed on the bridge deck in the longitudinal and lateral directions occurred. In this study, based on the finite element model using 3D solid elements, the behavior of the direct fixation track that could be occurred under operating load conditions was analyzed. In addition, the reinforcement effect of various reinforcement methods was analyzed. As a result of analyzing the lateral displacement before and after reinforcement, it was analyzed that the maximum lateral displacement after reinforcement under the extreme lateral wheel loads significantly decreased to about 3% (about 0.1mm) compared to before reinforcement. In addition, as a result of examining the generated stress of the filling mortar, bridge decks, and reinforcing bar, it was analyzed that all of them secured a sufficient safety factor of 2.6 or higher, and the optimal conditions for the reinforcement method were derived. Therefore, it is judged that the number of anchoring reinforcements and symmetrical anchor placement reviewed in this study will be effective in controlling the occurrence of lateral displacement of concrete blocks and securing the structural integrity of bridges and concrete blocks.

Exploring the role and characterization of Burkholderia cepacia CD2: a promising eco-friendly microbial fertilizer isolated from long-term chemical fertilizer-free soil

  • In the pursuit of sustainable and environmentally-friendly agricultural practices, we conducted an extensive study on the rhizosphere bacteria inhabiting soils that have been devoid of chemical fertilizers for an extended period exceeding 40 years. Through this investigation, we isolated a total of 80 species of plant growth-promoting rhizosphere bacteria and assessed their potential to enhance plant growth. Among these isolates, Burkholderia cepacia CD2 displayed remarkable plant growth-promoting activity, making it an optimal candidate for further analysis. Burkholderia cepacia CD2 exhibited a range of beneficial characteristics conducive to plant growth, including phosphate solubilization, siderophore production, denitrification, nitrate utilization, and urease activity. These attributes are well-known to positively influence the growth and development of plants. To validate the taxonomic classification of the strain, 16S rRNA gene sequencing confirmed its placement within the Burkholderia genus, providing further insights into its phylogenetic relationship. To delve deeper into the potential mechanisms underlying its plant growth-promoting properties, we sought to confirm the presence of specific genes associated with plant growth promotion in CD2. To achieve this, whole genome sequencing (WGS) was performed by Plasmidsaurus Inc. (USA) utilizing Oxford Nanopore technology (Abingdon, UK). The WGS analysis of the genome of CD2 revealed the existence of a subsystem function, which is thought to be a pivotal factor contributing to improved plant growth. Based on these findings, it can be concluded that Burkholderia cepacia CD2 has the potential to serve as a microbial fertilizer, offering a sustainable alternative to chemical fertilizers.

Video-assisted Talc Poudrage for the Treatment of Malignant Pleural Effusion: Analysis of Effects and Benefits (악성 늑막 삼출증에서의 비디오 흉강경하 탈크 분무의 효과 및 장점 분석)

  • Background: Malignant pleural effusion is a common condition in neoplastic patients and palliative therapy is the usual treatment. Talc has been generally accepted to be the most effective sclerosant for chemical pleurodesis, but the optimal route of administration remains controversy. We compared the results of video-assisted thoracoscopic talc poudrage (VTP) with administering a bedside talc slurry through a chest tube (BTS) for the treatment of malignant pleural effusion. Material and Method: From December 2004 to May 2006, 20 patients with malignant pleural effusion underwent chemical pleurodesis via VTP (group A, n=10), and BTS (group B, n=10). Result: The durations of chest tube placement after the procedure were $7.0{\pm}4.0$ days (group A) and $6.7{\pm}3.6$ days (group B). The hospital stays were $24.3{\pm}9.4$ days (group A) and $30.7{\pm}21.5$ days (group B), respectively. The symptoms of dyspnea were much more improved in group A (p-value=0.014) after discharge (mean f/u group $A=8.5{\pm}2.2$ months, group B $8.0{\pm}7.4$ months). The collapsed portions of lung were better expanded in group A than in group B (p-value=0.011). Conclusion: We recommend VTP for the selected patients with malignant pleural effusion because of the advantages of dissecting the fibrous peel to relieve the atelectasis and dyspnea, and excising the pleura for diagnosis with direct viewing of the lesion.

The effects of tissue punch diameter on healing around implants in flapless implant surgery (무피판 임플란트 수술에서 연조직 펀치의 크기가 임플란트 주위 조직의 치유에 미치는 영향)

  • Statement of problem: Flapless implant surgery using a soft tissue punch device requires a circumferential excision of the mucosa at the implant site. To date, Although there have been several reports on clinical outcomes of flapless implant surgeries, there are no published reports that address the appropriate size of the soft tissue punch for peri-implant tissue healing. Purpose: In an attempt to help produce guidelines for the use of soft tissue punches, this animal study was undertaken to examine the effect of soft tissue punch size on the healing of peri-implant tissue in a canine mandible model. Material and methods: Bilateral, edentulated, flat alveolar ridges were created in the mandibles of six mongrel dogs. After a three month healing period, three fixtures (diameter, 4.0 mm) were placed on each side of the mandible using 3 mm, 4 mm, or 5 mm soft tissue punches. During subsequent healing periods, the peri-implant mucosa was evaluated using clinical, radiological, and histometric parameters, which included Gingival Index, bleeding on probing, probing pocket depth, marginal bone loss, and vertical dimension measurements of the peri-implant tissues. Results: The results showed significant differences (P <0.05) between the 3 mm, 4 mm and 5 mm tissue punch groups for the length of the junctional epithelium, probing depth, and marginal bone loss during healing periods after implant placement. When the mucosa was punched with a 3 mm tissue punch, the length of the junctional epithelium was shorter, the probing depth was shallower, and less crestal bone loss occurred than when using a tissue punch with a diameter $\geq$ 4 mm. Conclusion: Within the limit of this study, the size of the soft tissue punch plays an important role in achieving optimal healing. Our findings support the use of tissue punch that 1 mm smaller than implant itself to obtain better peri-implant tissue healing around flapless implants.
