In the design and maintenance of buildings, identifying the degree of damage in the event of a fire is an important factor in fire prevention and fire safety design. In order to predict fire damage, safety measures should be established by predicting the nature of evacuation according to fire, smoke and in-house characteristics, and the effects of the operation of fire safety facilities should also be considered, but in Korea, the risk analysis due to the operation of fire safety facilities is insufficient. Accordingly, this study uses fire statistics and sprinkler inspection data to analyze the degree of fire damage caused by the operation of sprinkler facilities in a probabilistic manner.
가스제조 시설들은 불활성 가스공급을 통하여 제조업 시설의 안전한 운영 및 유지하는 역할을 하며, 폭발성, 가연성 및 독성가스를 공급하여 화학제품 제조에 필요한 기초 원료의 공급을 담당하는 역할을 한다. 또한 가스의 제조과정에서 고온, 고압, 초저온 및 촉매반응 등의 운전조건 하에서 시설이 운전되기 때문에 안전하고 신뢰성 있는 운영이 반드시 필요하다. 이러한 공장들은 공정관리가 복잡하며 제조물질의 누출로 인한 화재, 폭발 및 독성가스 누출로 인한 중대산업사고의 위험성이 있고, 불활성가스로 인한 질식재해, 고온 및 초저온으로 인한 화상 등 잠재적인 위험요소를 많이 가지고 있다. 본 연구는 신뢰성 있는 준 정량적 위험성평가 기법인 GRA(Generic Risk Analysis) 모델을 공기분리시설(ASU)에 적용하여 초기위험도(Initial Risk) 산정, 안전방벽(Safety Barriers) 적용, 잔여위험도(Residual Risk) 산정 및 중요안전요소(EIS, Elements Important for Safety)를 도출 하였으며, 위험성 평가 결과로 도출된 중요안전요소에 대한 효과적인 안전관리 및 시행절차의 구축을 제안하였다.
The development of the aviation industry and the changes in the military operation mission environment are demanding more long - distance operation (long - time flight), and such a flying environment is a risk factor for fatigue - related accidents. For the aviation related organizations such as ICAO and FAA, fatigue risk management system (FRMS) are applied along with flight time restriction regulations to prevent fatigue related accidents. The most important process in FRMS is fatigue risk management. Fatigue risk management systematically manages fatigue through scientific fatigue risk data collection and fatigue risk assessment. The purpose of this study is to applicate the assessment of scientific fatigue risk management to pilots of airplanes engaged in long flight. We reviewed the current state of risk management and FRMS through previous research. We also developed fatigue risk management indicators and examined the validity of internationally recognized fatigue risk data collection methods and fatigue risk assessment tools. There are 134 mission (flight) data used for development. In order to verify the indicators, the fatigue risk score between the items was assigned through pair-wise comparison. In addition, the verify test results were normalized.
Purpose: This study was conducted to evaluate the incidence and risk factors for surgical glove perforation during operation. Methods: During the month of december 2008, a total of 1,400 pairs of surgical gloves used in major operations was collected in a tertiary hospital. All gloves were examined immediately after operation using the standardized water-leak method to detect any perforation. Incidence of the glove perforation was counted according to the type of operation, operation time, the number of involved personnel, perforation sites, and the manufacturing companies. Results: Out of 2,800 gloves examined, 312 perforations were detected comprising 11.1% of samples. In terms of the type of operation, the perforation incidence varied from 5% to 20%, and the perforation rates in CS (20%) and NS (18%) (p<.001) were significantly higher than those in other departments. The 1st assistant or scrub nurse got glove perforation more frequently than the 2nd assistant or operator (p<.001). Longer operation time was associated with higher incidence of perforation evidently (p<.001). In terms of the sites, the thumb and index finger were more frequently perforated than other sites (4.1% and 3.4% respectively) without any differences between left and right side. Conclusion: Risk factors for glove perforation including the department of operation, operation time, participating personnel, and location of perforation should be taken into account to improve surgical safety.
Previous methods to evaluate the risk of dropped objects rely on personnel experience of the engineer or operator without analyzed data. However analyzing historical statistic data is the best approach to find the safest operation route and to achieve more reasonable and reliable calculation results. By counting the failure frequency and fatal accident rate the risk can be quantified, and so controlled or mitigated with best economical risk reducing measures. This analysis gives a crane operator with useful information for selecting the best crane operation route, and a designer with an estimation of risk level for the dropped objects from a safety point of view.
증가하는 도심지 전철 급행화의 요구에 따라, 완행과 급행철도를 동시에 운영할 수 있도록 하는 부본선의 건설이 필요하다. 그러나 기존 대피선 설치 방안은 열차의 운행을 차단한다는 점에서 긴 공사기간이 필요할 뿐만 아니라 막대한 경제적 손실을 발생시키는 문제점이 있다. 이를 개선하고자 기존 지하철 터널 운영 중이라는 특수한 상황에서의 대피선 설치 방안 연구가 필요하며 대피선 건설방안 적용에 따른 시공 리스크 분석 또한 요구된다. 따라서 본 논문에서는 지하철 4호선 과천정부청사역을 가상의 대피선 건설 대상역으로 선정하여 최적의 대피선 건설방안을 도출하였으며, 기존 지하철 터널운영 중 대피선 건설 예비 설계단계 시 발생 가능한 리스크 사건에 대한 리스크 식별 및 대응계획 수립, 리스크 평가, 리스크 통제 및 관리의 일련의 리스크 관리 프로세스를 수행하였다. 총 8가지의 발생 가능한 리스크 사건과 리스크 저감 대책을 선정하였으며 5단계의 리스크 사건 발생확률 및 영향도 기준을 활용한 리스크 평가 매트릭스를 구축하여 리스크 및 잔류 리스크 평가를 수행하였다. 리스크 평가 결과를 바탕으로 각 리스크 사건의 평가 점수와 리스크 저감 대책공법의 저감효과를 확인하였다.
Personal noise exposure was assessed in railroad operators working in 40 operation units including the Gyeongbu line, Honam line, and Janghang line between 2004 and 2006. 8-hour TWA were assessed based on the MOL (Ministry of Labor) guideline on noise evaluation, and 17% of these measurements exceeded 85 dBA while 70% were over 80 dBA. When the ACGIH TLV is adopted, 40% of measurements were over TLV (85 dBA). The noise exposure risk of railroad operators was higher for diesel locomotive operation and longer operational distances; in addition, the risk was higher for passenger car operation compared to freight car driving. Given that in Korea, over the past 30 years diesel locomotives have provided most of the railway's motive power and that hearing protective equipment is not likely to be used by workers during operation, railroad operators are at a high risk of noise-induced hearing loss. The result of audiometric test among 568 railroad operators showed that 32.6% of those tested had a hearing threshold shift of more than 40 dB. In conclusion, this study calls for more fundamental measures including noise control countermeasures within the operation areas, development of equipment that generates less noise and adopting limitation on the operation distance for a work shift.

And present operation of safety of the level crossing is just regular checking of reaction and functions of safety equipments physically, we need to react to the changes of what's going on around of level crossing and incestigation of traffic flow and surrounding conditions more spontaneously. So, we have to prepare the strategies against accidents of level crossing cost-effectively based on regular risk assessments for sustainable safety improvements and overcome operational problems of present level crossing's safety. Here, we're going to investigate the level of safety level and present risk factors of the level crossing using risk assessments for accidents/error of the equipments to show operational strategies of level crossing based on risk assessments.
Purpose: The purpose of this research was to provide patients with safe preoperative preparatory procedures by removing any risk factors from the preparatory procedures by using failure mode and effects analysis, which is a prospective risk-managing tool. Methods: This was a research design in which before and after conditions of a single group were studied, Failure mode and effects analysis were applied for the preparatory procedures done before operations. Results: The preparation omission rate before the operation decreased from 2.70% to 0.04%, and operation cancellation rate decreased from 0.48% to 0.08%. Conclusion: Failure mode and effects analysis which remove any risk factors for patients in advance of the operation is effective in preventing any negligent accidents.
According to the increase of the requirement for the safety and environmental protection at sea, safety is a core design factor of maritime systems. Safety is defined as a perceived quality that determines to what extent the management, engineering and operation of a system is free of danger to life, property and the environment. There are many possible approaches in dealing with the safety of ships. The risk-based approach is one of the most systematic and reasonable way currently in design, operation and regulatory works for ship safety. This paper proposes a concept of total risk management for ship safety, and introduces a prototype system implemented based on this concept as a framework of design and operation of ships.
