Operation Fee and Insurance Charge of Hallux Valgus Surgery (무지 외반증의 수술비 및 보험)

  • Purpose: The purpose of this study was to figure out the appropriate and systemic insurance charge for the hallux valgus operations. Materials and Methods: 5 Hospitals for hallux valgus operations were analyzed how they have been charging the national health insurance corporation for their operation fees and how to use the estimated guide and authoritive interpretation through the guide book of health insurance medical treatment grant expense and the guide book of Health insurance medical treatment. Results: There are nothing for guiding principle of hallux valgus operations in both books but a guide of Mcbride operation which is approved 'JA-93-KA and JA-31' for operation fee. So majority of hospitals have charged operation fee depending on their own interpretations they like. According to the guide books, there was a authoritive interpretation that simultaneous operation of osteotomy and tendon transfer for cerebral palsy and flat foot can be eatimated as 'osteotomy+JA-93-NA'. Conclusion: Distal soft tissue procedure should be approved as 'JA-93-NAx100%+JA-31x50%' according to the the estimated guide and authoritive interpretation if transected adductor hllucis is transfered to first metatarsal head. So distal chevron osteotomy could be 'JA-30-1-RAx100%+JA-31x50%', proximal metatarsal osteotomy could be 'JA- 93-NAx100%+JA-31-50%+JA-30-1-RAx50%', first metatarsocuneiform joint arthrodesis could be 'JA-93-NAx100%+ JA-31x50%+JA-73-RAx50%'.

A Study on Driver's Physiological Response in Train Simulator (열차 시뮬레이터 조작 시 운전자의 생체신호 변화에 대한 연구)

  • he purpose of this study is to measure bio-signal to investigate the driver's physiological response change under real situation using train simulator. The train simulator used in this study is KTX model and according to changes of driving situation, The bio-signal controlled by autonomic nervous system, such as GSR(Galvanic Skin Response), SpO2(Saturation percent O2), HR(Heart Rate), ECG(Electrocardiograph), EEG(Electroencephagram) and movement and response of eye were measured. Statistically significant difference in bio-signal data and eye movement activity pattern were investigated under several different driving speeds using analysis of variance (p<0.05). The GSR and HR value measured in average and mission speed operation is higher than in high-speed operation. β wave of EEG in average speed operation become more activated than in high speed operation. In accordance with a characteristic of rail vehicle, movement and response of eye in high-speed operation requiring relatively simple maneuver become less activated than in either average or mission speed operations. Conclusively, due to more careful driving controls in average and mission speed operation are required than in high-speed operation, level of mental and physical stresses of train driver was increased and observed through changes of bio-signal and eye movement measured in this study.

  • We report five patients of esophageal injuries confirmed by clinical signs and radiological evidences. They include a partial tear and a perforation which were not noticed during the operation, a perforation which was primarily repaired during the operation, and two perforations which occurred during the reoperations for the removal of mal-positioned screws or plate. The partial tear was not repaired. The perforation which occurred during the operation was primarily sutured and didn't receive further treatment. Two perforations which occurred during the reoperations were treated by irrigation, debridement with surgical drainage, and systemic antiobiotics. One who was diagnosed later after the operation showed the poorest outcome and required longest hospital days among our series. Early detection and appropriate treatment of esophageal injury following anterior spinal surgery can only improve the prognosis by preventing secondary complications.

  • The purpose of this study was to develop a evaluation system of customer service department operation in business. Four sources including related literature, managers of 4 outstanding customer service departments, SOCAP, and 7 OCAP administrative members provided useful information in various ways. The finally suggested evaluation system is consisted of three parts; work to do, resources needed, and operation standards. It is the type of checklist, which business can utilize it to examine whether they are doing well or not in terms of customer service department operation.

  • A yard in a container terminal is a temporary storage space before containers are loaded onto the target vessel or delivered to consignees. For improving the utilization of the space in the yard and the efficiency of loading and discharging operations, it is important that operation plans must be carefully constructed in advance. A heuristic method is suggested to solve operation space planning problems considering workloads on handling equipment as well as space availabilities. The operation plans in this paper includes quay crane (QC) schedules and space plans for multiple vessels considering the workload in the container yard of container terminals. This paper evaluates the effectiveness of a space planning method and the performance of a new QC scheduling method using a simulation model.

Parents' toothbrushing guidance on the children by operation of toothbrushing room (양치교실 운영여부에 따른 학부모의 자녀에 대한 칫솔질 지도 상태)

  • Objectives: The purpose of the study is to investigate the parents' toothbrushing guidance on the children by operation of toothbrushing room. Methods: A self-reported questionnaire was completed by 790 parents of first, fourth, and sixth grade students of J elementary school in Changwon from June 1 to 30, 2013. J elementary school did not have toothbrushing room. The questionnaire consisted of general characteristics of the subjects (3 items), toothbrushing room operation (3 items), and toothbrushing (7 items). Data were analyzed using SPSS 20.0 program. Results: Parents of high school diploma demanded the establishment of toothbrushing room (p<0.05) and parents having undergraduate course tended to want the operation of toothbrushing room (p<0.05) and children's dental health care (p<0.05). Parents within forties tended to have higher concern for their children's toothbrushing (p<0.05) and toothbrushing room operation (p<0.05). They tried to have much interest in their dental health checkup and toothbrushing frequency. Conclusions: The establishment and operation of toothbrushing room is very important to elementary school children. The toothbrushing guidance by parents will enhance children's dental health knowledge and maintain lifelong dental healthcare for the children.

Effects of Combined Antiplatelets on Bleeding in Off-Pump Coronary-Artery Bypass Surgery (술 전 항혈소판제 복합투여가 무심폐기하 관상동맥우회술 후 출혈에 미치는 영향)

  • Background: Antiplatelet agent administration is critical in managing coronary-artery disease, but there is a concern regarding operation-related bleeding and an increase in blood transfusion in such, especially when delivering combined antiplatelet agents. This study was conducted to evaluate the effect of the administration of antiplatelet agents on off-pump coronary-artery bypass surgery (OPCAB). Methods: From March 2003 to December 2009, 49 patients who had undergone OPCAB were collected retrospectively. The patients were divided into three groups according to the administration of antiplatelet agents before the OPCAB operation: 21 patients were given an aspirin agent (group 1), 19 patients were given combined agents (aspirin+clopidogrel) (group 2), and nine patients were not given any antiplatelet agent (group 3). The three groups' perioperative hematologic and coagulation profiles, including their platelet counts, hemoglobin levels, hematocrit, prothrombin times, and aPTTs (activated partial thromboplastin times), and their postoperative bleeding, related complications, transfusion requirements, and operation times, were compared. Results: The operation time in group 2 was 4.3 hours, longer than those in the two other groups, and urgent operation was significantly most frequent in group 2 (63%). The amount of blood loss and the number of patients who received blood transfusion were not different in the three groups. The perioperative hemoglobin level, hematocrit, platelet count, prothrombin time, and aPTT were also not significantly different among the three groups. Conclusion: The continuous administration of antiplatelet agents to the patients in this study did not increase their postoperative bleeding or operation-related complications. Therefore, OPCAB may well be considered even if combined antiplatelet agents are being administered.

Development and Verification of Indicators for a Foodservice & Nutrition Management Evaluation at a Hospital Nutrition Department (병원 영양부서의 급식 및 영양관리 평가 지표 개발 및 검증)

  • The purposes of this study were to develop the standard indicators to evaluate the food and nutrition systems in hospitals and to test the validity of those items scientifically. The results were as follows: First, the conceptual validity was examined with recognition degrees of importance from the hospital nutrition department managers. All of the hospital nutrition department's operation evaluation standards and the indicators' conceptual validity tested were in the range of 3.71~4.93 out of 5.0, and the mean score was 4.36. Therefore, the conceptual validity was verified. Second, to verify the factor validity of the items of the standards and indicators for the hospital nutrition department's operation evaluation, the standards and indicators were analyzed as key-factors. Key-factor analysis after vertical rotation showed that four factors appeared and were composed of (a) facilities management, (b) sanitation management, (c) operation & foodservice management, and (d) nutrition management. Third, the reliability of the standards and indicators for the hospital nutrition department's operation evaluation was analyzed and resulted in a score of 0.98, which showed good internal consistency. Fourth, the discriminative power of each item of the standards for the hospital nutrition department's operation evaluation was tested by checking the differences between groups with first quartile and forth quartile of total evaluation scores. The indicators having low distinction power were modified into obligatory items or eliminated for better differentiation.

Secure and Energy-Efficient Join-Leave Operations in ZigBee Network

  • Since security plays an important role in several ZigBee applications, such as Smart Energy and medical sensor applications, ZigBee Specification includes various security mechanisms to protect ZigBee frames and infrastructure. Among them, the Join and Leave operations of ZigBee are investigated in this paper. The current Join-Leave operation is protected by the network key (a kind of group key). We claim it is not adequate to employ the network key for such purpose, and propose a new Join-Leave operation protected by the application link key (a kind of pairwise key), which is based on a more efficient key management scheme than that of ZigBee. Hence, the original Join operation consists of a total of 12 command frames, while the new Join operation consists of only 6 command frames. In particular, the security of the proposed Join-Leave operation is equivalent to or better than that of the original Join-Leave operation. The new Join-Leave operation is extensively analyzed in terms of security and efficiency, and compared with the original Join-Leave operation of ZigBee.

  • This paper in order to efficiently operate zero energy buildings developed a methodology for optimal operation of PV + ESS active systems. This program consists of three steps. First step is PV optimal operation and second step is PV + ESS optimal operation. Third step is the analysis of the results by PV + ESS optimal operation. The optimal operation of PV + ESS was calculated by using Dynamic Programming (DP). Therefore, the optimal capacity and operating plan of PV + ESS in this study are calculated for electric load at building. This paper conducted case study to verify the validity of the developed algorithm. Also, the sensitivity analysis analyzed the effect of each variable on the optimal operation.
