The Operative Management of Navicular Stress Fractures in Adolescence (청소년기에 발생한 주상골 피로 골절의 수술적 처치)

  • Purpose: The aim of this study is to evaluate the effectiveness of surgical treatment in adolescent patients suffering from navicular stress fracture. Materials and Methods: A total of 11 adolescent patients aged 14 to 19, who underwent an operation for navicular stress fracture between 2005 and 2008 were recruited. Clinical outcomes were evaluated by the American Orthopaedic Foot and Ankle Society (AOFAS) midfoot score and visual analogue scale (VAS) score. Results: The mean VAS score before the operation was 7.7. A score of 2.9 was confirmed at final follow up after the operation. The mean final follow-up period was 22.1 months. There was a statistically significant improvement in the VAS score between before and after the operation (p=0.01), and similarly, the AOFAS score also showed an improvement, from 46.5 to 80.7 (p=0.01). The pain that remained after the operation, according to the VAS score, was severe in three patients (27.3%), tolerable in seven patients (63.6%), and free of pain in one patient (9.1%). Conclusion: In navicular stress fracture in adolescents, careful selection of patients who could benefit from surgical treatment is recommended.

A Simulation Evaluation of Aisle Design and Operation Policies for an Automated Storage and Retrieval System with Narrow-/Wide-Width Racks (다품종 조립라인 자동화 물류창고의 이형 랙 배치 및 운영정책 시뮬레이션 평가)

  • In an automotive plant, an automated storage and retrieval system (ASRS) synchronizes material handling flows from a part production line to an auto-assembly line. The part production line transfers parts on small-/large-sized pallets. The products on pallets are temporarily stored on the ASRS, and the ASRS retrieves the products upon request from the auto-assembly line. Each ASRS aisle is equipped with narrow-/wide-width racks for two pallet sizes. An ASRS aisle with narrow-/wide-width racks improves both storage space utilization and crane utilization while requiring delicate ASRS aisle design, i.e., the locations of the narrow-/wide-width racks in an ASRS aisle, and proper operation policies affect the ASRS performance over demand fluctuations. We focus on operation policies involving a common storage zone using wide-width racks for two pallet sizes and a storage-retrieval job-change for a crane based on assembly-line batch size. We model a discrete-event simulation model and conduct extensive experiments to evaluate operation policies. The simulation results address the best ASRS aisle design and suggest the most effective operation policies for the aisle design.

Clinical experience with single-port access laparoscopic cystectomy and myomectomy

  • Objective: This study was performed to assess our clinical experience with single-port access (SPA) laparoscopic cystectomy and myomectomy and the surgical outcomes of those procedures at our institution. Methods: The authors evaluated the surgical outcomes of SPA laparoscopic cystectomy in 293 patients and SPA laparoscopic myomectomy in 246 patients. The surgical outcomes comprised operation time, the amount of blood loss during the operation, the change in hemoglobin (before and after the operation), the change in hematocrit (before and after the operation), switching to the multi-port access method, complications, transfusions, and the duration of the postoperative hospital stay. Results: The Pearson correlation coefficient and the Spearman correlation coefficient between the operation time and the amount of blood loss were 0.312 and 0.321 for SPA laparoscopic cystectomy, respectively, and 0.706 and 0.674 for SPA laparoscopic myomectomy, respectively. The drops in hemoglobin and hematocrit were $1.33{\pm}0.78g/dL$ and $4.14%{\pm}2.45%$, respectively, in SPA laparoscopic cystectomy, while the corresponding figures were $1.34{\pm}1.13g/dL$ and $4.17%{\pm}3.24%$ in SPA laparoscopic myomectomy, respectively. Conclusion: This study reported the surgical outcomes of SPA laparoscopic cystectomy and myomectomy and compared them to previously published findings on traditional laparoscopic cystectomy and myomectomy. No significant differences were found in the surgical outcomes between SPA and traditional laparoscopic cystectomy and myomectomy.

Ultrasound-guided Femorosciatic Nerve Block by Orthopaedist for Ankle Fracture Operation (족관절 골절 수술을 위한 정형외과 의사의 초음파 유도 대퇴좌골 신경 차단)

  • Purpose: The purpose of this study is to investigate the usefulness of ultrasound-guided femorosciatic nerve block by orthopaedist to operate the fracture around ankle. Materials and Methods: Twenty-two patients, who had an operation for fracture around the ankle under a ultrasound-guided femorosciatic nerve block from January to April 2010, were the targets of this study. We measured the time spent for the ultrasound-guided femorosciatic nerve block, the time taken to start the operation after the nerve block, the time taken to deflate the tourniquet because of a tourniquet pain, the time passed until feeling a postoperative pain after the operation, etc. We also studied the complications and satisfaction of the anesthesia. Results: It took 6.2 (3 to 12) minutes for the nerve block, 46.1 (28 to 75) minutes to start the operation, 52.5 (22 to 78) minutes until feeling a tourniquet pain and 11.5 (7.5 to 19) hours until starting to feeing a postoperative pain. There was no complication by anesthesia and 21 people (95.5%) were satisfied with anesthesia by ultrasound-guided femorosciatic nerve block. Conclusion: Ultrasound-guided femorosciatic nerve block by orthopaedist in the fracture around ankle reduces anesthetic and nerve injury complication, and leads to high anesthetic success rate. Also it is considered as an effective method to alleviate postoperative pain.

Clinicopathologic Analysis of Remnant Gastric Cancer after Distal Partial Gastrectomy: Experience of Single Center during 15 Years

  • Purpose: Remnant gastric cancer (RGC) are generally detected at advanced stages or infiltration of adjacent organs. We retrospectively reviewed the surgical outcomes and clinicopathologic results of remnant gastric cancers that have operated during fourteen years in one institution of Korea. Materials and Methods: 34 patients who were diagnosed with RGC at Ajou University Hospital from April 1995 to October 2009 were enrolled. We analyzed the features of previous operation, and according to these results, surgical outcomes and clinicopathologic results for RGC were analyzed. Results: Of 34 patients, 20 patients had previously undergone distal gastrectomy for malignant disease, and 14 patients for benign disease. The period between previous operation and surgery for RGC in the patients underwent operation for malignant disease was shorter than that in benign patients (P<0.001). In surgical field, 31 patients (91.0%) were resected and curative resection was possible in 23 patients (67.6%). When 31 patients who underwent resection for RGC were divided into previous malignant and benign disease, there was no significantly different in terms of surgical outcomes and pathologic findings between two groups. Meanwhile, the patients who recently (after 2005) underwent surgery for RGC showed less advanced stage compared with the patients who underwent surgery before 2004. Conclusions: Resection was possible in the higher proportion (91.0%) of patients diagnosed with RGC compared with previous reports. The cause of previous operation did not effect on the surgical outcomes for surgery of RGC. Recent trend of RGC is to increase the proportion of early stage gastric cancer. Therefore, surgeons should consider curatively surgical resection for RGC the regardless of pattern of previous operation.

  • this paper analyses how much acceleration and deceleration of urban rail vehicle should be applied and how to choose an operation mode to minimize energy consumption when train runs between stations within the fixed operation time. The decided operation pattern satisfying the minimum energy consumption becomes a target trajectory and a basis for the controller design criteria. To make this goal it grasps the characteristics of urban rail vehicle, realize operation energy model of urban rail vehicle and verity the accuracy of embodied model the Matlab simulation with the same operation result of real route. It searches for operation pattern to minimize operation energy by changing the acceleration and deceleration on the imaginative route and proposes operation pattern minimizing energy consumption by applying real operation data between stations of Seoul Metropolitan Subway Line 6.

A Study on the Location and Area Composition of the Operation Department in Regional Public Hospitals - Focused on Net Floor Area (지방의료원 수술부 배치 및 규모에 관한 연구 - 순면적을 중심으로)

  • Purpose: To establish the building guidelines and to analyze the function of Regional Public Hospitals, the survey was conducted primarily on the current status focusing on the location and relationship of the department, area composition ratio, area per bed, and area per operating room. Methods: The research methods of this paper are as follows. 1) A literature survey on the function of the operating and related departments, 2) A drawing analysis for spatial composition and net area calculation. Results: The area of the Operation Department in the Regional Public Hospitals with 200 to 300 beds are as follows. Area ratio: 3.35%, area per Bed: 2.53㎡/bed, the number of beds per operating room: 64.37bed, area per operating room: 146.46㎡/n, area per operating room by plan type: integration corridor 133.84㎡/n, separation corridor 184.82㎡/n. Implications: This paper analyzed data on the current state of Operation Departments for setting up the function and role for the Regional Public Hospital. In the future, it is also required to provide size that takes into account user behavior and the user's psychological aspect in order to suggest appropriate area.

  • Purpose: The purpose of this study is to report four operative cases of recurrent peroneal tendon dislocation being effectively treated by superior peroneal retinaculum reconstructive operation and rerouting of calcaneofibular ligament. Materials and Methods: Four male adults with recurrent peroneal tendon dislocation including one of patient with redislocation after the treatment of plication of inferior peroneal retinaculum were followed up after being treated with superior peroneal retinaculum reconstructive operation using Achilles tendon and rerouting of calcaneofibular ligament. Results: All patients followed up for over one year revealed no pain and no redislocation. One patient who experienced redislocation after the treatment of plication of inferior peroneal retinaculum had no other complication after the treatment of superior peroneal retinaculum reconstructive operation using Achilles tendon. Conclusion: The reconstructive operation presumed to be effective for recurrent peroneal tendon dislocation.

Clinical Evaluation After Periodontal Flap Surgery with/without Non-Surgical Periodontal Therapy (비외과적 치주처치의 선행 유무에 따른 치주 판막술 후 임상적 평가)

  • In the treatment of chronic adult periodontitis, scaling and root planing have been generally performed prior to periodontal flap surgery. The purpose of this study was to evaluate the clinical significance of nonsurgical periodontal therapy prior to periodontal flap surgery in patients with chronic adult periodontitis. Fifty six molars showing bilateral bony defects and 4-6mm periodontal pocket in twelve patients with chronic adult periodontitis were selected. By randomized split-mouth design, in one side, flap operation was performed 4 weeks after scaling and root planing, in the other group, flap operation was only performed without scaling and root planing. Probing pocket depth, gingival recession, clinical attachment level, gingival index, plaque index, and tooth mobility were measured at baseline, before flap operation and post-operation

  • Purpose : The purpose of this study was to determine radiologic variables affecting extraction of impacted mandibular third molar. Materials and Methods : This study was investigated on 44 consecutive extractions of unilateral impacted mandibular third molars (Male;34, Female;10, Mean age;24.98, Age range;19-35). Fourteen radiologic variables, which were actual value except only one, were evaluated to establish their relation to the operation time. The Pearson correlation coefficient and Stepwise procedures for regression analysis were carried out. Results : Eight variables (depth, angulation, ramus width, relationship to the second molar, distal ramus depth, mesial ramus depth, mesial periodontal ligament width and follicle) showed a statistically significant relation to the operation time(Pearson correlation coefficient, p<0.05). Two variables, depth and mesial periodontal ligament width, showed the most powerful and just relation to the operation time($r^2$ = 0.349, p<0.05). Conclusion : This study shows that our results can help general practioner to estimate the operation time of extraction of impacted mandibular third molars by measuring two radiologic variables ; depth and mesial periondontal ligament width.
