  • A cost analysis for hospitalized patients was performed based on the KDRG classification in order to determine an appropriate nursing fee under the PPS(Prospective Payment System). The data was collected from 20 nursing units of three tertiary hospitals and two secondary hospitals from August 26 to September 15, 1996. The study consisted of 148 inpatients diagnosed for lens procedures(KDRG 03900), tonsillectomy &/or adenoidectomy(KDRG 16100), Cesarean section(KDRG 37000), or vaginal delivery(KDRG 37300) without any complications. The direct or indirect nursing hours of each patients were measured. Then, direct or indirect nursing expenditures of four nursing units, operating room and delivery room were computed. Finally, the resources used including average total nursing hours, average length of stay and average nursing cost of each KDRG were estimated as follows; 1) The average total nursing hours were 640 minutes for lens procedures, 403 minutes for tonsillectomy &/or adenoidectomy, 934 minutes for appendectomy with complicated principal diagnosis, 1,094 minutes for Cesarean section and 631 minutes for vaginal delivery. Significant differences were found in average total nursing hours among hospitals. 2) The average length of stay in lens procedures were 5 days, 4 days for tonsillectomy &/or adenoidectomy, 6 days for appendectomy with complicated principal diagnosis, 8 days for Cesarean section and 3 days for vaginal delivery. All results were within normal determined by the Ministry of Health and Welfare although significant differences existed among hospitals, especially with average length of stay for leng procedures between tertiary hospitals and secondary hospitals which was greater than for those of others. 3) The average nursing cost were 87,146 Won for lens procedures, 69,600 Won for tonsillectomy &/or adenoidectomy, 128,337 Won for appendectomy with complicated principal diagnosis, 151,769 Won for Cesarean section and 85,403 Won for vaginal delivery. These costs were 7.6%, 13.0%, 13.0%, 16.0% and 22.0%, respectively, of the official price fixed by the Ministry of Health and Welfare under the prospective payment system. Research for the analysis of nursing costs according to the severity of illness for those KDRGs shoud be carried out within the period of the PPS pilot project. In addition, a proper nursing fee schedule for a new reimbursement system based upon the result of the above research should be prepared in the near future.

A Study on Network Construction Strategies for Long-Haul Low-Cost Carrier Operations

  • Purpose - This study aims to analyze the characteristics of network construction by Norwegian Air and AirAsia X, which are recognized as leading airlines in the long-haul LCC market. Based on this analysis, this study intends to provide implications for networking strategies for Korean LCCs that seek to enter the long-haul market when the aviation market stabilizes again upon the end of the COVID-19 pandemic. Design/methodology - To conduct the network analysis on long-haul low-cost airlines, the Official Airline Guide (OAG) Schedule Analyzer was used to extract long-haul data of Norwegian Air and AirAsia X. To analyze the trend of the long-haul route network, we obtained the data from 3 separate years between 2011 and 2019. The network was analyzed using UCINET 6.0 in order to examine the network structure of long-haul low-cost airlines and the growth trend of each stage. Findings - Analyzing the network of long-haul routes by visualizing the network structure of low-cost carriers showed the following results. In its early years, Norwegian Air's long-haul route network, centering on regional airports in Spain and Sweden, connected European regions, the Middle East, and Africa. As time passed, however, the network expanded and became steadily strong as the airline connected airports in other European countries to North America and Asia. In addition, in 2011, AirAsia X showed links to parts of Europe, such as London and Paris, the Middle East and India, and Australia and Northeast Asia, centering on the Kuala Lumpur Airport. Although the routes in Europe were suspended, the network continued to expand while concentrating on routes of less than approximately 7,000 km. It was found that instead of giving up on ultra-long-haul routes such as Europe, the network was further expanded in Northeast Asia, such as the routes in Korea and Japan centering on China. Originality/value - Until the COVID-19 pandemic broke out, Norwegian Air actively expanded long-haul routes, resulting in the number of long-haul routes quintupling since 2011. The unfortunate circumstance, wherein the world aviation market was rendered stagnant due to the outbreak of COVID-19, hit Norwegian Air harder than any other low-cost carriers. However, in the case of AirAsia X, it was found that it did not suffer as much damage as Norwegian Air because it initially withdrew from unprofitable routes over 7,000 km and grew by gradually increasing profitable destinations over shorter distances. When the COVID-19 pandemic ends and the aviation market stabilizes, low-cost carriers around the world, including Korea, that enter the long-haul route market will need to employ strategies to analyze the marketability of potential routes and to launch the routes that yield the highest profits without being bound by distance. For stable growth, it is necessary to take a conservative stance; first, by reviewing the business feasibility of the operating a small number of highly profitable routes, and second, by gradually expanding these routes.

EMR 인증제 교육을 위한 보건의료정보관리 실습 프로그램 모델 연구 -환자정보관리 중심- (A Study on the Health Information Management Practice Program Model for EMR Certification System Education -Focus on Patient Information Management-)

  • 본 연구에서는 한국보건의료정보원에서 실시하는 EMR 인증기준울 이해할 수 있도록 보건의료정보관리 실습 프로그램에 인증기준을 추가한 모델을 연구하여 제시하였다. 실습 프로그램은 EMR 인증제의 기능성 기준에 해당하는 환자정보관리를 실습하고 이해할 수 있도록 보건의료정보관리 교육시스템에 환자정보관리에 대한 인증기준 기능을 추가하였다. 환자정보관리를 위한 EMR 인증기준 실습프로그램은 다음과 같은 인증기준으로 구성되었다. 등록번호 및 인적사항 관리, 진료예약 일정관리, 인적사항 수정이력관리, 동명이인 구분자 표시, 다중 등록번호 통합관리, 식별정보를 이용한 환자 검색, 진료형태에 따른 환자 검색, 수술시술 동의서 기록·조회, 개인정보활용동의서 기록·조회, 연명의료결정정보 표시, 외부 의료기관문서 등록·조회, 외부 검사결과 등록·조회. 이와 같이 인증기준에 의한 보건의료정보시스템의 기능을 운영하여 실습해봄으로써 인증기준의 기능성 영역에서 환자정보관리의 인증기준과 내역을 이해하고 실습할 수 있다. EMR 인증 기준에 맞춰 환자정보관리 실습을 수행해봄으로써 전자의무기록시스템에서의 표준화된 환자정보관리를 이해할 수 있다. 또한 EMR 인증기준의 기능을 확인할 수 있기 때문에 의료기관에서 보건의료정보관리사의 전자의무기록시스템의 관리 능력을 향상시킬 수 있을 것이다.

상용망 기반의 항만터미널 효율적인 관제시스템 설계 (Design of an Efficient Control System for Harbor Terminal based on the Commercial Network)

  • 해상을 통한 물동량이 전체 97% 차지하고 있어 효율적인 항만 운영 관리 시스템을 통해 작업효율을 높이는 동시에 운영비용을 절감하고, 관리자가 작업 지연 및 장비 지원이 필요한 상황이 발생한 경우 신속하게 이를 확인하여 대처할 필요가 있다. 기존 시스템은 GPS 을 이용한 야드 자동화 장비의 실시간 위치정보 확인을 토대로 작업 완료 혹은 작업 시작에 따라 입력된 정보로 위치 정보를 모니터링하고 있다. 기존 시스템보다 태블릿의 GPS 시스템으로 실시간 위치정보 확인 시스템이 야드 조업 장비의 위치 확인에 있어 더욱 정확한 정보 제공이 가능하다. 야드장 내의 통신망에서도 컨테이너로 인한 음영이 없는 상용 LTE 서비스를 활용한 망 구성이 컨테이너 처리 지연을 줄인다. 마지막으로 안드로이드나 IOS를 사용하는 스마트단말기의 도입과 인공지능을 활용한 컨테이너 처리 스케줄링을 통해 컨테이너 처리 어플리케이션의 스마트 단말 사용과 컨테이너 작업 스케줄의 최적화를 통한 최소 지연시스템을 구축한다. 스마트 단말의 도입과 인공지능을 활용한 컨테이너 처리 지연의 최소화는 컨테이너 정보 요구자인 소비자에게 실시간으로 컨테이너의 처리과정을 확인시킴으로써 항만 서비스의 질적 향상이 예상된다.

다중이용시설 내 식생바이오필터 시스템의 PM10, PM2.5 저감효과 및 측정방법에 대한 연구 (Study on PM10, PM2.5 Reduction Effects and Measurement Method of Vegetation Bio-Filters System in Multi-Use Facility)

  • 2019년 3월 미세먼지 비상저감조치가 일주일 동안 발령되면서, 미세먼지로 인한 국민의 불안감은 점차 가중되고 있다. 본 연구는 공기정화식물이 적용된 바이오필터의 다중이용시설 내 적용성 평가를 위해 입자상 오염원의 실내 연속방출환경을 조성하여 오염원 저감효과에 대한 측정방법을 제안하고, 시스템의 실내공기질 개선 여부를 확인할 수 있는 기초연구를 진행하였다. 강의실을 대상으로 춘절기에 모니터링 1시간 전 모기향을 오염원으로 배경농도를 조성한 후, 스케줄에 따라 2시간 관수, 1시간 송풍하여 미세먼지의 저감능을 확인하였으며, 바이오필터 2m 전방에 PM10, PM2.5 및 온습도 센서를 설치하고, 3개 송풍구 중 중앙에 풍속 프로브를 설치하여 시계열 모니터링을 수행하였다. 바이오필터에 구비된 총 3개소의 송풍구 평균 면풍속은 0.38±0.16 m/s로 댐퍼 면적이 제외된 송풍구별 면적 0.29m×0.65m을 적용한 총 공조풍량이 776.89±320.16㎥/h로 산출되었다. 시스템 가동으로 평균온도 21.5~22.3℃, 평균상대습도 63.79~73.6%를 유지하여, 선행연구의 다양한 조건별 온습도 범위에 부합하는 것으로 판단된다. 시스템 공조부 구동을 통해 급격하게 상대습도를 상승시키는 효과를 효율적으로 운용할 경우, 계절에 따른 실내 미세먼지 저감과 적정한 상대습도 확보도 가능할 것으로 판단된다. 미세먼지 농도는 바이오필터 시스템 가동 전의 모든 주기에서 상승 현상이 동일하게 집계되었으며, 시스템 가동 후 1주기 송풍구간(B-1, β=-3.83, β=-2.45)에서 미세먼지(PM10)는 최대 28.8% 수준인 560.3㎍/㎥, 초미세 먼지(PM2.5)는 최대 28.0% 수준인 350.0㎍/㎥까지 저감되었다. 이후 미세먼지(PM10, PM2.5)의 농도는 2주기 송풍구간 감소(B-2, β=-5.50, β=-3.30)로 각각 최대 32.6% 수준인 647.0㎍/㎥, 32.4% 수준인 401.3㎍/㎥까지 저감되었고, 3주기 송풍구간감소(B-3, β=5.48, β=-3.51)로 최대 30.8% 수준인 732.7㎍/㎥, 31.0% 수준인 459.3㎍/㎥까지 저감된 것으로 확인되었다. 본 연구는 식생 바이오필터의 다중이용시설 내 설치와 유관한 관련 표준 및 규정을 참조하여, 객관적인 성능평가환경의 구축 방안을 제시할 수 있었다. 이를 통해 일반 강의실 환경 내에 보다 객관화된 모니터링 인프라를 조성하여, 상대적으로 신뢰성 있는 데이터 확보가 가능했던 것으로 판단된다.

심리적 계약 위반이 OS이용자의 배신 행동에 미치는 영향: 윈도우 XP 기술적 지원서비스 중단 사례 (The Effects of Psychological Contract Violation on OS User's Betrayal Behaviors: Window XP Technical Support Ending Case)

  • Technical support of Window XP ended in March, 8, 2014, and it makes OS(Operating System) users fall in a state of confusion. Sudden decision making of OS upgrade and replacement is not a simple problem. Firms need to change the long term capacity plan in enterprise IS management, but they are pressed for time and cost to complete it. Individuals can not help selecting the second best plan, because the following OSs of Window XP are below expectations in performances, new PC sales as the opportunities of OS upgrade decrease, and the potential risk of OS technical support ending had not announced to OS users at the point of purchase. Microsoft as the OS vendors had not presented precaution or remedy for this confusion. Rather, Microsoft announced that the technical support of the other following OSs of Wndow XP such as Window 7 would ended in two years. This conflict between OS vendor and OS users could not happen in one time, but could recur in recent future. Although studies on the ways of OS user protection policy would be needed to escape from this conflict, few prior studies had conducted this issue. This study had challenge to cautiously investigate in such OS user's reactions as the confirmation with OS user's expectation in the point of purchase, three types of justice perception on the treatment of OS vendor, psychological contract violation, satisfaction and the other betrayal behavioral intention in the case of Window XP technical support ending. By adopting the justice perception on this research, and by empirically validating the impact on OS user's reactions, I could suggest the direction of establishing OS user protection policy of OS vendor. Based on the expectation-confirmation theory, the theory of justice, literatures about psychological contract violation, and studies about consumer betrayal behaviors in the perspective of Herzberg(1968)'s dual factor theory, I developed the research model and hypothesis. Expectation-confirmation theory explain that consumers had expectation on the performance of product in the point of sale, and they could satisfied with their purchase behaviors, when the expectation could have confirmed in the point of consumption. The theory of justice in social exchange argues that treatee could be willing to accept the treatment by treater when the three types of justice as distributive, procedural, and interactional justice could be established in treatment. Literatures about psychological contract violation in human behaviors explains that contracter in a side could have the implied contract (also called 'psychological contract') which the contracter in the other side would sincerely execute the contract, and that they are willing to do vengeance behaviors when their contract had unfairly been broken. When the psychological contract of consumers had been broken, consumers feel distrust with the vendors and are willing to decrease such beneficial attitude and behavior as satisfaction, loyalty and repurchase intention. At the same time, consumers feel betrayal and are willing to increase such retributive attitude and behavior as negative word-of-mouth, complain to the vendors, complain to the third parties for consumer protection. We conducted a scenario survey in order to validate our research model at March, 2013, when is the point of news released firstly and when is the point of one year before the acture Window XP technical support ending. We collected the valid data from 238 voluntary participants who are the OS users but had not yet exposed the news of Window OSs technical support ending schedule. The subject had been allocated into two groups and one of two groups had been exposed this news. The data had been analyzed by the MANOVA and PLS. MANOVA results indicate that the OSs technical support ending could significantly decrease all three types of justice perception. PLS results indicated that it could significantly increase psychological contract violation and that this increased psychological contract violation could significantly reduce the trust and increase the perceived betrayal. Then, it could significantly reduce satisfaction, loyalty, and repurchase intention, and it also could significantly increase negative word-of-month intention, complain to the vendor intention, and complain to the third party intention. All hypothesis had been significantly approved. Consequently, OS users feel that the OSs technical support ending is not natural value added service ending, but the violation of the core OS purchase contract, that it could be the posteriori prohibition of OS user's OS usage right, and that it could induce the psychological contract violation of OS users. This study would contributions to introduce the psychological contract violation of the OS users from the OSs technical support ending in IS field, to introduce three types of justice as the antecedents of psychological contract violation, and to empirically validate the impact of psychological contract violation both on the beneficial and retributive behavioral intentions of OS users. For practice, the results of this study could contribute to make more comprehensive OS user protection policy and consumer relationship management practices of OS vendor.
