진료재료의 관리개선을 통한 운영재고 절감 방안 (Measures to Reduce Medical Supplies in Stock through Improved Manageme)

  • Research Problem: 45-65% of all medical supplies in hospital are used in operating rooms. Medical supplies in operating rooms are difficult to manage in general because many of them are frequently used and come in a variety of types. Purpose: Our aim was to strive for user-friendliness and reduce the inventory through efficient management of medical supplies stocked in operating rooms. Medical Facility: Korea University, Ansan Hospital Quality Improvement Activity: On the last day of each month, we checked the inventory of medical supplies in all operating rooms by identifying the amount of medical supplies in each room, warehouse, and OCS, identified problems, and presented the ways to improve. Improvement Outcome: We increased the number of post-processing medical supply items by 8%, and reduced the inventory cost by 15% through improved management of medical supplies.

수술실 내 CCTV 설치 의무화 법안의 향후 과제 (Future Tasks of the Law Forcing CCTV Installation in Operating Rooms)

  • 2021년 9월 24일 전신마취 등 의식이 없는 환자를 수술하는 의료기관의 수술실 내 CCTV 설치 의무화 조항(의료법 제38조의2)이 공포되었다. 개정된 「의료법」은 수술실 내에서 발생할 수 있는 불법행위를 효과적으로 예방하고, 의료분쟁 발생 시 적정한 해결 도모를 목적으로 한다. 이로 인해 2023년 9월 25일까지 전신마취 등 의식이 없는 상태의 환자를 수술하는 의료기관은 수술실 내 CCTV를 의무적으로 설치해야 하고, 의료인의 동의 여부에 관계없이 환자와 환자 보호자의 요청만으로도 수술장면을 촬영해야 한다. 해당 법안은 기본권 침해 최소를 위한 입법 장치를 법률에 규정하지 않고 하위법령에 위임하였다(제38조의2 제10항). 정보주체의 기본권 침해를 최소화하기 위한 가장 현실적인 정책방안은 구체적인 규정을 마련하는 것이다. 이에 본 논문에서는 수술실 내 CCTV 설치 의무화 법안의 입법 배경과 개정안의 주요 내용을 살펴보고, 주요 쟁점을 분석함으로써 하위법령 마련 시 검토되어야 할 사안을 제안하였다. 수술실 내 촬영 대상인 정보주체의 기본권 침해 최소화 원칙 준수를 기준으로 촬영 요건, 촬영 거부 정당화 사유, CCTV 설치 위치, 촬영 범위·대상, 영상정보 안전 조치 의무와 처벌의 적절성 등을 검토하였다. 수술실 내 CCTV의 정보주체는 수술에 참여하는 보건의료인과 환자일 것으로 이들의 개인정보자기결정권, 인격권, 인권 등의 침해를 최소화하면서 설치 의무화법의 목적이 달성될 수 있도록 하위법령 마련 시 고려되어야 할 사안을 제안하였다. 본 논문이 하위법령 논의 시 검토되어 정보주체의 기본권 침해 최소화 방안 마련에 기여할 수 있기를 기대한다.

한 대학병원의 수술실 이용 지연요인과 개선방안에 관한 연구 (Delayed use of Operating Rooms in a University Hospital)

  • Advanced surgical technology demands more precise, meticulous, and time-consuming procedures. In addition, the patient's preference of tertiary health providers makes over crowding of the University Hospitals. Therefore, it has been necessary to maximize utilization of the operating room of such hospitals to accommodate these requirements. This study, targeting 1,302 surgical cases performed in 22 operating rooms at a university hospital in Seoul from October 8 to November 1, 2001, analyzed reasons for delay, and factors that caused delayed use of operating rooms. This study also assessed that the rate of operating room use would increase if the sources for possible reform were improved. 1. Among total of 1,302 cases of surgery, the incidence of surgeries in which there were no time delays and no factors for delay were discovered is 71.4% or 930 cases: the incidence in which surgeries were delayed was 28.6% or 372 cases. 2. As results of logistic regression for delay, procedures involving women were delayed 1.4 times more frequently than those of men. Compared to Department A, Department B was 1.8 times more likely to be delayed, and Department H was 0.4 times less likely to be delayed. Regional anesthesia was 2.4 times more likely to be delayed than general anesthesia, and surgeries that PCA was applied were 0.6 times less likely to be delayed than those when it was not. Surgeries performed on the Thursday were 1.7 times more likely to be delayed than those performed on the Monday. Compared to surgeries performed between 07:00-07:59, those performed between 08:00-08:29 were 4.3 times higher. 3. The reasons for delay were related to surgeon, surgical department, patient, anesthesia, administrative system, sick ward, and support services. Among these, 5,755 minutes for 276 delayed cases could be resolved easily, and resolving delays of 3,320 minutes for 131 cases would be more difficult. Among the causes for delay that could be improved, delays due to patient's transfer and surgeon's factor were the most common, 21.6% and 17.4% respectively. 4. If resolvable delays are improved, pre-anesthesia room is administered, and regional anesthesia and PCA are done ahead of time, use of emergency operating rooms will increase, we can increase overall utilization by 4.09%, we will save 744 minutes a day, we can reduce the time the operation room is used after 4 PM by 35%, and we can resolve the operation cancellations due to insufficient operating rooms. For the increase in the use of operating rooms, we need to maximally decrease the delays that could be improved, by allocating block time based on used totals hours of elective cases, giving accurate information on surgery schedule, voluntary cooperation by staff participating in surgeries in reducing delay time, and the hospital management's will to improve delay.

Investigation of influence of linear diffuser in the ventilation of operating rooms

  • Air quality in hospitals has always concerned hospitals' health officials due to its dangerous particles and gases. Because of the importance of air conditioning in the operating room, a system must be embedded in operating rooms to direct the contaminated air outside, and inject fresh filtered air from outside back into the room. In this study, laminar flow air conditioning system is implemented in the operating room by slot linear diffusers and with the help of air curtain. For this, stimulation Computational Fluid Dynamic (CFD) was used due to its efficiency. The aim of the present study was to find a proper solution to overcome the unfavorable factors, namely, contamination, humidity, and also temperature, velocity and pressure inside the room. These factors were implemented with different values and then stimulated through FLUENT software program. Results showed that the aforementioned factors can be overcome using air curtain and slot linear diffusers.

수술실 간호사의 환자안전 확보 경험 (Operating Room Nurses' Experiences of Securing for Patient Safety)

  • Purpose: This study was done to evaluate the experience of securing patient safety in hospital operating rooms. Methods: Experiential data were collected from 15 operating room nurses through in-depth interviews. The main question was "Could you describe your experience with patient safety in the operating room?". Qualitative data from the field and transcribed notes were analyzed using Strauss and Corbin's grounded theory methodology. Results: The core category of experience with patient safety in the operating room was 'trying to maintain principles of patient safety during high-risk surgical procedures'. The participants used two interactional strategies: 'attempt continuous improvement', 'immersion in operation with sharing issues of patient safety'. Conclusion: The results indicate that the important factors for ensuring the safety of patients in the operating room are manpower, education, and a system for patient safety. Successful and safe surgery requires communication, teamwork and recognition of the importance of patient safety by the surgical team.

일개 병원의 수술실 약제관련 업무 적정화 방안연구: 설문조사를 중심으로 (Evaluating Appropriateness of Medication Use in the Operating Rooms of a Tertiary Hospital: Based on Survey)

  • Background: Since the use of opioid analgesics is frequent in operation rooms (OR), the risk of medication error is high; however the use of medication in the OR has been operating independently with the hospital pharmacy. Therefore, the assessment on management of medication use in operation and the pharmacist's role is needed. Methods: We conducted the literature review and survey from anesthesiologists, operating nurses at Chung-Ang Hospital on management of medication for operation use, awareness on need for medication management efficiency, need for satellite pharmacy in the operating room and its effect. Results: 56% of medical staffs responded that management of medication in the operating room is efficient; however, 82.6% responded that they felt the inconvenience in medication delivery to the OR when additional prescription was ordered. 51.5% also responded that extra time was required for management of narcotics and inventory/record keeping. 80% agreed that there could be lost costs due to prescription missed. Medical staffs responded improving the drug management system could increase the OR efficiency (87%), and eventually bring the increase in hospital revenue (80.4%). Those who responded that implementation of OR satellite pharmacy was needed include physicians (84.6%), nurses (63.6%), and also responded that it'd bring more profit to the hospital by increasing the efficiency in OR (60.9%). Conclusion: For efficient management of medications, implementation of OR satellite pharmacy would lead to improved drug management and increased efficiency in OR and reduced cost and improved patient care.

수술실의 간호조직특성, 조직문화 유형 및 팀 효과성과의 관계 (A Study on the Relationships among Nursing Organizational Characteristics, Types of Nursing Organizational Culture and Team Effectiveness at Operating Rooms)

  • Purpose: This study was a literary investigation in the development of an effective team-raised and cultural nursing organization targeting operating room nurses. This was done by grasping organizational features, different types of nursing organizational culture and team effectiveness, and by analyzing the relationships among variables. Method: The participators were 373 nurses. The data was collected between March 10th and April 28th, 2004. SPSS Win 11.0 program was used for data analysis. Results: Results of this study were as follows: A nursing organization at operating rooms was found to have an organizational culture with the characteristic of a centralized power and hierarchy-oriented organizational culture. With respect to a relationship between operating room organizational characteristics and the types of nursing organizational culture, there was a significant correlation in relationship-oriented culture and innovation-oriented culture. In the relation between the types of operating room organizational culture and team-effectiveness, it showed a significant correlation in team-devotion, peer-groupsatisfactional perception of a team's outcome in decreasing order. Conclusion: The organizational characteristic at operating rooms was power centralization and the culture type was hierarchy-oriented. These are factors that have a negative effect on a team-effectiveness-affecting relationship-oriented culture, innovation-oriented culture and work-oriented culture.

수술실 간호사의 피로 예측요인 (Factors Influencing on Fatigue in Operating Room Nurses)

  • Purpose: The purpose of this study was to measure the fatigue of operating room nurses and examine factors contributing to their fatigue. Methods: The subjects of this study were 115 nurses who worked in operating rooms of three hospitals. Data were collected from September 15, 2009 to September 30, 2009. Fatigue and job stress were measured by using the Symptom's Fatigue Scale and the job stress measurement tools in operating room nurses. Results: The mean score of fatigue level was $3.10{\pm}0.61$. We analyzed fatigue according to characteristics of subjects and found that there were significant differences in marital status, number of children and sleeping hours in general characteristics. Career, daily participation hours in the surgery relating to job related characteristics. Significant positive correlations were found between job stress and fatigue(r=.233, p= .012). Three significant variables influencing fatigue of operation rooms nurses were job stress, daily participation hours in the surgery, and sleeping hours (Adj. $R^2$=0.284, F=4.773, p<.001). Conclusion: An integrative care program that takes job stress, daily participation hours in the surgery and sleeping hours into consideration is essential to reduce fatigue in operating room nurses.

종합병원 수술실 종사자의 마취가스 노출에 관한 연구 (Employee Exposures to Waste Anesthetic Gases in Hospital Operating Rooms)

  • This study was performed to investigate employee exposures to waste anesthetic gases, such as enflurane and sevoflurane in operating rooms of general surgical, children's and dental clinics of a large hospital located in Seoul and to analyze factors affecting the concentrations of waste anesthetic gases. The results of the study are summarized below. 1. Based on results of personal and area samples for airborne enflurane, all of the employees investigated in this study were exposed to airborne enflurane concentrations below the ACGIH-threshold limit value (TLV) of 75 ppm. 2. However, based on results of personal samples for sevoflurane, employees of two (2) out of eleven (11) operating rooms were exposed to sevoflurane concentrations in excess of the NIOSH recommended exposure limit (REL) of 2 ppm. A similar trend was found in the area samples. 3. To investigate the source of sevoflurane emissions, airborne sevoflurane concentrations were measured on an anesthesia machine, a drug cabinet and a desk. It was indicated that the geometric means were 0.93 ppm, 0.83 ppm and 0.72 ppm, respectively. 4. Factors affecting waste anesthetic gas concentrations were the age of anesthesia machine, the volume of operating room and the extent of ventilation (p<0.05). 5. It is recommended that the use of anesthesia equipments be properly controlled, the operating room be well ventilated, and the airborne concentrations of anesthetic gases be continuously monitored.

중소병원 수술실 간호사의 간호근무환경이 직무몰입에 미치는 영향: 심리적 임파워먼트의 매개효과 (The Effect of Nursing Work Environment on Job Involvement in Operating Room Nurses in small- and medium-sized hospitals: Mediating Effect of Psychological Empowerment)

  • 본 연구의 목적은 중소병원 수술실 간호사의 간호근무환경과 직무몰입의 관계에서 심리적 임파워먼트의 매개효과를 규명하기 위함이다. 연구 대상은 전국 24개 중소병원의 수술실에서 근무하는 간호사 150명으로, 구조화된 설문지를 이용하여 2018년 7월부터 9월까지 자료를 수집하였다. 수집된 자료는 t-test, ANOVA, Pearson's correlation coefficients, 회귀분석으로 분석하였다. 연구결과, 직무몰입은 간호근무환경 또는 심리적 임파워먼트와 정 상관관계가 있었고. 심리적 임파워먼트는 간호근무환경과 정 상관관계가 있었다. 심리적 임파워먼트는 간호근무환경이 직무몰입에 미치는 영향을 부분 매개하는 것으로 나타났다 중소병원 수술실 간호사들이 간호근무환경을 긍정적으로 인식하여 직무에 몰입하도록 돕기 위해 심리적 임파워먼트 향상 프로그램을 개발하여 적용하는 것이 필요하다.
