수술실에서 중심정맥관 삽입 시 번들적용이 중심정맥관 관련 혈류감염에 미치는 영향 (Impact of Central Line Insertion Bundle on the Adherence of Bundle and Central Line-Associated Bloodstream Infections in the Operating Room)

  • Background: This study purposed to examine how the incidence of the central line-associated bloodstream infection (CLABSI) in a operating room (OR) is affected by bundle application on central line insertion (CLI) practice. Methods: The study design was a pretest-posttest experimental trial. The subjects were 83 patients before the bundle application on CLI and 70 patients after. Results: The compliance(%) of bundle on CLI of among those who observed all of the five items increased from 7.2% before the intervention to 72.9% after. By items, compliance with the maximal barrier precaution was 100% for the use of a mask and cap before and after the intervention, but increased from 73.5% before the intervention to 88.6% after for the hand hygiene, from 73.5% to 88.6% for the use of a sterile gown, and from 9.6% to 75.7% for the use of a sterile large drape covering the whole body. CLABSI did not happen on CLI either before or after the application of the bundle intervention. Conclusion: Bundle application increased compliance with the use of a sterile gown and the use of a sterile large drape. However, its effect in the prevention of CLABSI was not clear probably due to the short period of intervention in a single hospital.

Factors Affecting the Postoperative Mortality in the Ruptured Abdominal Aortic Aneurysm

  • Background: Although patients with a ruptured abdominal aortic aneurysm (RAAA) often reach the hospital alive, the perioperative mortality is still very high. We retrospectively reviewed thirty patients who underwent repair of RAAA to identify the factors affecting postoperative mortality in a single hospital. Materials and Methods: Between September 2007 and May 2011, thirty patients with RAAA underwent emergent surgery (n=27) or endovascular aneurysm repair (n=3). Their medical records were retrospectively reviewed regarding three categories: 1) preoperative patient status: age, gender, vital signs, serum creatinine, blood urea nitrogen, hematocrit, and hemoglobin level: 2) aneurysmal status: size, type, and rupture status; and 3) operative factors: interval time to operating room, operative duration, and amount of perioperative transfusion. Results: The 30-day postoperative mortality rate was 13.3% (4/30); later mortality was 3.3% (1/30). On multivariate analysis, the initial diastolic blood pressure (BP), interval time to operating room and amount of preoperative packed cell transfusion were statistically significantly linked with postoperative mortality (p<0.05). Conclusion: In this study, preoperative diastolic BP, preoperative packed cell transfusion amount and interval time between arrival and entry to operating room were significantly associated with postoperative mortality. It is important to prevent hemorrhage as quickly as possible.

수술실의료진의 환자안전문화 인식수준 및 관련요인 (Perceived Level and Associated Factors of Patient Safety Culture among Health Care Providers in an Operating Room)

  • Purpose: The purposes of this study were to compare the level of perception and to identify factors associated with perception on patient safety culture among health care providers working in an operating room(OR). Methods: A cross-sectional survey design was used. Data were collected conveniently from 154 RNs and 116 physicians working in a tertiary hospital in Seoul. Patient safety culture was measured using "The Hospital Survey on Patient Safety Culture" developed by the Agency for Healthcare Research and Quality (AHRQ). Descriptive statistics, t-test, ANOVA, and Spearman rank correlation coefficients were used for statistical analysis with the SPSS version 17.0. Results: The perception level of nurses and physicians on patient safety culture was "moderate" (3.14). Compared to physicians, nurses showed a significantly lower perception on the items of "teamwork within units" (t=-6.904, p<.001) and "overall perception of patient safety" (t=-4.327, p<.001), but had a higher perception about "frequency of events reported" (t=2.769, p=.006). The physicians' professional positions, years of working experience, age, and working hour per week were identified as factors associated with patient safety culture. Conclusion: Level of perception on patient safety culture may vary among health care providers working in the OR. The study finding suggests that a tailored education and training strategies should be considered to develop an effective safety culture for healthcare professionals working in OR.

Treatment of a penetrating inferior vena cava injury using doctor-helicopter emergency medical service and direct-to-operating room resuscitation in Korea: a case report

  • Inferior vena cava (IVC) injuries can have fatal outcomes and are associated with high mortality rates. Patients with IVC injuries require multiple procedures, including prehospital care, surgical techniques, and postoperative care. We present the case of a 67-year-old woman who stabbed herself in the abdomen with a knife, resulting in an infrarenal IVC injury. We shortened the transfer time by transporting the patient using a helicopter and decided to perform direct-to-operating room resuscitation by a trauma physician in the helicopter. The patient underwent laparotomy with IVC ligation for damage control during the first operation. The second- and third-look operations, including previous suture removal, IVC reconstruction, and IVC thrombectomy, were performed by a trauma surgeon specializing in cardiovascular diseases. The patient was discharged without major complications on the 19th postoperative day with rivaroxaban as an anticoagulant medication. Computed tomography angiography at the outpatient clinic showed that thrombi in the IVC and both iliac veins had been completely removed. Patients with IVC injuries can be effectively treated using a trauma system that includes fast transportation by helicopter, damage control for rapid hemostasis, and expert treatment of IVC injuries.

환기정도에 따른 수술실용 신발 종류가 수술실 오염에 미치는 영향 (Efficiency of Footwear and Ventilation Systems of Operating Rooms : How to Choose Suitable Shoes?)

  • Background : Various types of protective footwear have been used to minimize bacterial contamination in operating rooms. In recent years, debate has arisen concerning the need for use of such protective footwear. This study was designed to provide useful data about choosing shoes most suitable for the surgical environment. Methods : Between November, 1999 and January, 2000, we performed this experimental study by comparing effect of three types of shoes (i.e., disposable shoescover, operating room-restricted shoes, and ordinary shoes) on bacterial contamination of operating rooms equipped with two different ventilation systems (i.e., high air-change, low air-change) respectively. Data were collected during two- hour sham operations in which subjects and their activities were strictly standardized. Bacterial flora were sampled from the study area floor and air colony counts were measured. Results : In experiments involving high air-change ventilation system, there was a significant difference of floor contamination between three types of shoes, but no difference of air contamination. Under low air-change system, there was a significant difference of both floor and air contamination between three types of shoes. Conclusion : The results show that protective footwear would be unnecessary in the operating room with high air-change ventilation system, but it is important to choose suitable shoes carefully under low air-change system. Therefore, the use of outdoor shoes can be considered under high air-change system, but it would seem sensible to apply their first use in less bloody operations at the day surgery center or out-patient department to prevent transfer of body fluid into the outside environment.

종합병원 수술실 종사자의 마취가스 노출에 관한 연구 (Employee Exposures to Waste Anesthetic Gases in Hospital Operating Rooms)

  • This study was performed to investigate employee exposures to waste anesthetic gases, such as enflurane and sevoflurane in operating rooms of general surgical, children's and dental clinics of a large hospital located in Seoul and to analyze factors affecting the concentrations of waste anesthetic gases. The results of the study are summarized below. 1. Based on results of personal and area samples for airborne enflurane, all of the employees investigated in this study were exposed to airborne enflurane concentrations below the ACGIH-threshold limit value (TLV) of 75 ppm. 2. However, based on results of personal samples for sevoflurane, employees of two (2) out of eleven (11) operating rooms were exposed to sevoflurane concentrations in excess of the NIOSH recommended exposure limit (REL) of 2 ppm. A similar trend was found in the area samples. 3. To investigate the source of sevoflurane emissions, airborne sevoflurane concentrations were measured on an anesthesia machine, a drug cabinet and a desk. It was indicated that the geometric means were 0.93 ppm, 0.83 ppm and 0.72 ppm, respectively. 4. Factors affecting waste anesthetic gas concentrations were the age of anesthesia machine, the volume of operating room and the extent of ventilation (p<0.05). 5. It is recommended that the use of anesthesia equipments be properly controlled, the operating room be well ventilated, and the airborne concentrations of anesthetic gases be continuously monitored.

중소병원 수술실 간호사의 간호근무환경이 직무몰입에 미치는 영향: 심리적 임파워먼트의 매개효과 (The Effect of Nursing Work Environment on Job Involvement in Operating Room Nurses in small- and medium-sized hospitals: Mediating Effect of Psychological Empowerment)

  • 본 연구의 목적은 중소병원 수술실 간호사의 간호근무환경과 직무몰입의 관계에서 심리적 임파워먼트의 매개효과를 규명하기 위함이다. 연구 대상은 전국 24개 중소병원의 수술실에서 근무하는 간호사 150명으로, 구조화된 설문지를 이용하여 2018년 7월부터 9월까지 자료를 수집하였다. 수집된 자료는 t-test, ANOVA, Pearson's correlation coefficients, 회귀분석으로 분석하였다. 연구결과, 직무몰입은 간호근무환경 또는 심리적 임파워먼트와 정 상관관계가 있었고. 심리적 임파워먼트는 간호근무환경과 정 상관관계가 있었다. 심리적 임파워먼트는 간호근무환경이 직무몰입에 미치는 영향을 부분 매개하는 것으로 나타났다 중소병원 수술실 간호사들이 간호근무환경을 긍정적으로 인식하여 직무에 몰입하도록 돕기 위해 심리적 임파워먼트 향상 프로그램을 개발하여 적용하는 것이 필요하다.

학교도서관의 디지털자료실 설치 효과에 관한 연구 (A Study on the Effect of Establishing School Library Digital Resource Room)

  • 교육인적자원부에서는 지식기반사회에서 요구하는 자율적이고 창의적인 인재를 육성하고, 제7차 교육과정을 구현하는데 있어서 학교도서관이 필수적이라는 인식 하에 "학교도서관 디지털자료실" 시범 구축사업을 추진하고 있다. 학교도서관 디지털자료실을 효과적으로 설치하고 교육적 효과를 극대화하기 위해서는 시범 운영중인 디지털자료실의 운영 실태와 교육적 효과를 분석함으로써 향후 개선 및 발전 방안을 모색할 필요가 있다 이에 본 연구는 2001년도에 설치된 96개의 학교도서관 디지털자료실을 대상으로 운영 실태와 교육적 효과를 분석함으로써 초.중등학교의 도서관정보화 사업을 효율적으로 추진할 수 있는 토대를 마련하고자 한다. 마련하고자 한다.

바닥 난방공간의 다인자 제어에 관한 실험적 연구 (An experimental study on the multiple parameter switching control for floor heating system)

  • An experimental facility consisting of two $3{\times}4.4{\times}3.8m$ rooms identical in construction is built. Each room has a control system and storage tank supplying hot water to the radiant floor heating system. The facility enables simultaneous comparision of two different control stratigies each implemented in a separate room. The operating performance of three kinds of flow control scheme is tested and compared in this study : (i) conventional on-off control based on feedback from room air temperature (ii) TPSC(two parameter switching control) (iii) TPOC(two parameter on-off control). Results show that TPSC and TPOC using room air and surface temperature sequentially as feedback signal to control hot water supply is the better temperature regulation scheme than conventional control based on feedback from only room air temperature. They are good candidates for the room with radiant floor heating system under continuous and intermittent heating mode.

퍼지제어를 통한 오수-합병정화조의 오니 측정 및 제어시스템에 관한 연구 (The Development using Fuzzy Control of sludge amount inspection and discharge materials of outlet water and affiliated water-purification tank)

  • The individual rotten water purification tank recently discharges wastewater and sewage through the outlet without purification ability. The outlet water and affiliated water purification tank with microorganism cultivator tank cultivates microorganism and then drops the value BOD, COD of sewage and discharges the quality of water into the outlet. The blower and water pump operating continuously prompts the waste of energy and deterioration of equipment. Each room of deposition tank, foaming tank, microorganism cultivator tank is equipment with the sludge detection senses so it can detect the density of each room. The power-drive plant of the blower and water pump which are the system cultivating the microorganism must be made as fuzzy controlization (If the sludge amount of each room become higher, the rate of operation of blower and water pump must heighten, on the contrary, in case of row sludge amount, the total handling amount and microorganism handling amount of each room of control. Tank reducing the rate of operation must be DB. At present, the blower amount in proportion to the sludge and oxyzen demanding amount has to control. Each room mus be checked outlet level of the outlet, also each room must flow backward discharge materials, and must operate feed-back control until we want to be come as a below value of BOD/COD(10PPM ; KS).
