Purpose: The purpose of this study is to examine the effect of leg crossing on reducing orthostatic hypotension and orthostatic hypotension symptoms in hemodialysis patients. Methods: A one-group pretest-posttest design was used. A total of 40 post-hemodialysis adult patients were enrolled, excluding the case of intradialysis hypotension, unbalance of standing with leg crossing, adding antihypertensive medications. Blood pressure (BP) and heart rate (HR) were measured in supine and standing positions. After a week, BP and HR were measured in supine and standing with leg crossing position. Orthostatic hypotension symptoms were also measured by self-reported structured questionnaire in standing without leg crossing and with leg crossing position. Results: We found out that systolic blood pressure, diastolic blood pressure, and mean arterial pressure increased significantly in standing with crossing leg position applied (p=.006, p=.001, p=.006). However, presences of orthostatic hypotension symptoms were not significantly decreased in standing with leg crossing position (p=.500, p=.318, p=.306, p=.241, p=.356, p=.500, p=.241, p=.308). Conclusion: This study shows that leg crossing is effective for reducing orthostatic hypotension without additional cost or instruments. Leg crossing as one of the preventive interventions to reduce orthostatic hypotension is easier and simpler to be implemented in hemodialysis patients.
In the patients with hemiplegia caused by stroke and TBI. postural sway is increased and open displaced laterally over the non-affected leg, reflecting asymmetry in weight bearing on lower extremities during standing balance. Recovery of symmetric weight bearing and postural stability is an important aim in physical therapy. Plastic AFO has been used for hemiplegic patients in order to help their abnormal walking patterns. Past studies have mainly focused on the AFO influences on hemiplegic walking patterns without balance function approaches. The purpose of this study was to identify the immediate effects of plastic AFO and shoes on the static balance in hemiplegic patients. The scale for static balance were weight bearing on affected leg(%), sway area(mm2), sway path(mm), maximal sway velocity(mm/s), anteroposterior sway angle($^{\circ}$ ), and lateral angle($^{\circ}$ ). Seventeen hemiplegic patients participated in this study: 13 men and 4 women, with an average age of 50.18 years. Static balance was measured using BPM(balance performance monitor; dataprint software version 5.3) under four standing condition namely bare-foot standing. standing in shoes, standing with AFO, and standing in shoes with AFO. In order to assure the statistical significance of the results, an one-way ANOVA, the independent t-test. and a pearson's correlation were applied at the .05 level of significant. The results of this study were as follows: 1) There were statistically significant differences in weight bearing(%) on the static balance between affected leg and non-affected leg(p<.01). 2) There were statistically significant differences in sway reverse frequence(Hz) in standing with AFO between affected leg and non-affected leg(p<.05). 3) Sway area(mm2) on standing in shoes with AFO was lower than bare-foot standing(p<.05), Lateral sway angle($^{\circ}$ ) on standing in shoes with AFO was lower than bare-foot standing and standing in shoes(p<.05). 4) Weight bearing in affected leg was not significantly correlated with postural sway.
Purpose : The purpose of this study was to evaluate the influence of sensory integration training on equilibrium ability in the college students of the twenties. Methods : The sensory integration training group(male: n=10, female: n=9) was measured an equilibrium ability by Kinesthetic Ability Trainer(KAT) at pre-intervention and post-intervention in 4weeks. Results : Mean value of eye close both leg standing and eye close one leg standing tests of the sensory integration training group were significantly reduced among the intervention period(p<.05). Conclusion : We were found that sensory integration training could reduce mean value of eye close both leg standing and eye close one leg standing tests and improve a balance for the college students of the twenties.
Purpose: The purpose of this study was to investigate the kinematic characteristics and muscle activities during the following two conditions: transition from half-kneel to standing on the affected leg and non-affected leg. Methods: Twenty-one hemiplegic patients participated in the study. A motion analysis system was used to record the range of motion and angle velocity of the hip, knee and ankle from the half-kneel to the standing position. Electromyography was used to record the activity of 4 muscles. Results: The statistical analysis showed that the minimum ROM of the hip joint was less on the affected leg during transition from half-kneel to standing. However, the minimum ROM of the knee and ankle joints was less on the non-affected leg during transition from half-kneel to standing. The angle velocity of the knee and ankle joints was less during transition from half kneeling to standing on the non-affected leg. Muscle activity of the rectus femoris and tibialis anterior was less while moving from half-kneel to the standing position on the affected leg. Conclusion: These results show that greater active ROM of the knee and ankle was required on the affected leg for transition from half-kneel to the standing position than for normal gait. Muscle activity of the rectus femoris and tibialis anterior is normally required for movement from the half-kneel to the standing position during normal gait. Further studies are needed to investigate the antigravity movement in healthy subjects and hemiplegic patients in order to completely understand the normal and abnormal movement from the half-kneel to the standing position.
Purpose: The purpose of this study was to determine the effects of a fall prevention program on gait ability, one leg standing-right, one leg standing-left, fear of falling, fall efficacy, and depression in the elderly. Methods: A fall prevention program was consisted of fall preventing exercise with laughter therapy. Quasi-experimental design with one group pre- and post-test was used. Twenty-two elderly were participated in this study from a community health center located in a rural area, South Korea. SPSS/WIN 18.0 was used for descriptive analysis and paired t-test. Results: After 12 weeks with a fall prevention program, gait ability (t=3.44, p=.002), one leg standing-right (t=-3.73, p=.001), one leg standing-left (t=-4.17, p<.001) and fear of falling (t=2.12, p=.046) in the elderly were significantly improved. Conclusion: A fall prevention program was effective on physical and psychological functions. This study suggested that control groups and a larger sample should be included in order to validate the effects of a fall prevention program for the elderly. Objective: The aim of this study was to determine the effect of abdominal-compression belt in one leg standing on balance in normal adult. Background: With the effects of increased intra-abdominal pressure, the abdominal-compression belt is contributing to a static balance control. However, specific study is still insufficient. Method: Forty subjects were randomly allocated to two groups: control(n=20) and experimental group(n=20), respectively. The experimental group used an abdominal-compression belt, whereas the control group did not that. All subjects were educated using pressure biofeedback unit and ultrasound imaging for exact application by abdominal-compression belt. Main outcome measurement was used a general stability index, fourier harmony index, weight distribution index, and fall index in tetrax balance system. Results: Experimental group improved significantly on general stability, only 2 factors(eyes closed with head turned forward and eyes closed with head turned backward) among fourier harmony index, and fall index, However, weight distribution index did not revealed significant difference. Conclusion: The findings suggest that application of abdominal-compression belt could be effective on improving balance ability in one leg standing of normal adults. Application: The results of the abdominal-compression belt might help to control balance in workers.
Purpose: The purpose of this study was to investigate the effects of a four week unilateral isokinetic exercise program applied to ankle on the one-leg stance balance performance of ipsilateral and contralateral lower-limbs. Methods: Subjects were randomly assigned to either a right ankle training program (n=12) or a control group (n=12). The training group received unilateral ankle isokinetic exercise of the dominant side for 4 weeks, whereas control group did not. Ipsilateral and contralateral one-leg balance were measured before and after intervention using the Biodex Balance System. Results: Improvements of stability scores, such as APSI, MLSI, and OSI, from pre-test to post-test were significantly different greater for the training group when the control. Conclusion: The results of this study suggest unilateral ankle strengthening exercise transfers benefit to the untrained limb by a cross-education effect, and that this type of exercise should be considered to improve one-leg standing balance of trained and untrained lower-limbs.
PURPOSE: The purpose of this study was to compare the activity of the abductor hallucis (AbdH) muscle during short foot exercise (SFE) using foot orthosis (SFE-FO) and the windlass effect (SFO-WE) while sitting and in a one-leg standing position. METHODS: We recruited fourteen subjects with normal feet for this study. Surface electromyography (EMG) was used to measure the muscle activity of the AbdH muscle during SFE, SFE-FO, and SFE-WE while sitting and in a one-leg standing position. Three trials consisting of a 5 s hold for each of the three exercises were performed to measure the EMG activity of the AbdH muscle. Exercise type and position were randomly assigned. Two-way repeated-measures ANOVA was used to analyze the effects of exercise type (SFE vs. SFE-FO vs. SFE-WE) and position (sitting vs. one-leg standing position) on AbdH muscle activity. A statistical significance was set at ${\alpha}=.05$. RESULTS: The EMG activity of AbdH muscle in the SFE-WE exercise was significantly greater than that during SFE and SFE-FO in both exercise positions (p<.001). In addition, the EMG activity of the AbdH muscle in the one-leg standing position was significantly higher than that while sitting (p<.001) during all three exercises. CONCLUSION: These results suggest that SFE-WE is a more effective strengthening exercise than SFE or SFE-FO for activating the AbdH muscle.
PURPOSE: Muscle fatigue is a cause to change proprioception. The purpose of this study was to investigate the effects of calf muscle fatigue and visual control on postural balance during single-legged standing in healthy adults. METHODS: Nineteen healthy adults (male) were participated in this study (mean age: 24.36 years; mean height: 171.32 cm; mean weight: 64.58 kg). The postural balance (sway length, sway area, sway velocity of COG displacement) was measured by Balance Trainer System (BT4) in before and after calf muscle fatigue feeling in single legged stance. In this study, repetitive single-legged heel rise test was used to induce fatigue of the calf muscle. Paired t- test was used to compare the postural balance between before and after calf muscle fatigue. Data of subjects were analyzed using SPSS 22.0 (SPSS Inc., Chicago, IL, USA). Level of significance was set to .05. RESULTS: The sway length, sway area, sway velocity of COG (center of gravity) displacement after calf muscle fatigue feeling was significantly increased compared to before calf muscle fatigue feeling during single leg standing both eye open and close conditions (p<.05). CONCLUSION: This study suggested that calf muscle fatigue feeling has affected on postural balance when standing one leg both eye open and close conditions and postural control was disturbed by muscle fatigue and visual feedback in single leg standing.
Purpose : The purpose of this study was to investigate the effects of the combined patterns of PNF(proprioceptive neuromuscular facilitation) on the static balance ability. Methods : The measurements of the static balance ability were completed by 10 subjects for 6 weeks, from October to November 2007. The combined patterns of PNF were carried out by means of self-exercising suggested by Dietz, which were designed as four cases: two positions (standing and quadruped) for both patterns(sprinter and skater), respectively. The exercises were practiced once a day, 3 times a week in same condition. By using the GOOD BALANCE system, assessment of the static balance ability was taken at before and after exercise from 6 positions: normal standing, one leg left and one leg right standing when eye open and close, respectively. For each case, the experimental data was obtained in 3 items: mean X speed, mean Y speed and velocity moment. Results : The results of this study were as follows : 1. There were statistically significant differences of Mean X speed, Mean Y speed and Velocity moment between the before and the after exercise in the case of normal standing when eye open and close(NSEO and NSEC), respectively. 2. There was statistically significant difference of Mean X speed between the before and the after exercise in the case of one leg left standing when eye open(OLLEO). In this case, however, the statistically significant differences were not found in both terms of Mean Y speed and Velocity moment. 3. There were statistically significant differences of Mean X speed and Mean Y speed between the before and the after exercise in the case of one leg left standing when eye close(OLLEC). In this case, however, the statistically significant difference was not found in term of Velocity moment. 4. There were statistically significant differences of Mean X speed, Mean Y speed and Velocity moment between the before and the after exercise in the case of one leg right standing when eye close(OLREC). 5. There was statistically significant difference of Mean X speed between the before and the after exercise in the case of one leg right standing when eye open(OLREO). In this case, however, the statistically significant differences were not found in both terms of Mean Y speed and Velocity moment. 6. There were statistically significant differences of total Mean X speed, total Mean Y speed and total Velocity moment between the before and the after exercise. Conclusions : The above results from this study indicated that the combined patterns of PNF have improved the static balance ability. However the used self-exercise can be applied to normal people, i.e., the exercise is difficult to apply into clinical patients. The further study should be focused at development of various modified forms of the combined patterns of PNF in keeping up the improvement effect of this exercise.
