An Anatomic Study and Clinical Application of Medial Plantar Septo-cutaneous Flap (내족저변 격막 피판의 해부학적 고찰 및 임상적 적용)

  • Several investigators have reported clinical experience of medial plantar septo-cutaneous flap for reconstruction for soft tissue defect of the hand and digits. Jayme and Hamilton first described the anatomy of superficial branch of medial division of the medial plantar artery used in this flap through cadavaric study in 1997. But, they had a few cases for this flap and there was no anatomic study in Korean. We experienced the reliability of medial plantar septo-cutaneous flap for reconstruction for soft tissue defect of hand and digits through an anatomic study (20 fresh specimens dissected) and clinical application (17 patients). An anatomic study revealed that there were differences in diameter and length of the vessels between Korean and Caucasian. The diameter of vessels in Korean is larger than Caucasian one in each area. Based on this anatomic knowledge, we could harvest this flap safely, and have performed reconstruction on 17 patients with soft tissue defects of hand and digits using a thin, flexible medial plantar septo-cutaneous flap similar to the volar aspect of the hand and digits in anatomical characteristics of the skin and subcutaneous tissue covering. The vessels used for this flap were superficial branches of medial division of the medial plantar artery and vena comitants, or the subcutaneous veins. The mean size of the flap was $2.82cm{\times}4.15cm$. All the flaps survived without significant complications. A medial plantar septo-cutaneous flap possesses several advantages : (1) It is very thin in comparison with other standard free flap; (2) it has two draining venous pathways; (3) it provides a good color and texture match for hand and finger; (4) a good recovery of protective sensation is achievable.

Correlation Analysis of Ocular Dominance and Levator Palpebrae Superioris Muscle Function (주시안과 눈꺼풀 올림근 기능의 상관관계 분석)

  • Purpose: Most of the bilateral structures in our body are not perfectly balanced, such that one side is preferred than the other or it has physiological superiority. Eyes also have an imbalance; the eye with sensory and motional superiority compared to the other is called dominant eye. Authors of this study focused on analyzing the correlation between the dominant eye and levator palpebrae superioris muscle. Methods: The subject of this study was 42 patients with no ptosis and with no past history of blepharoplasty. Hand dominance was identified through questionnaire and dominant eye was identified by hole-in-the-card dominance test (Dolman's test) in all patients. The function of levator palpebrae superioris muscle was measured by MLD (marginal limbal distance). During the measuring procedure, frontalis muscle was not inhibited to avoid the eyelid skin hooding. Results: Out of 42 patients, 27 patients (64.3%) were right ocular dominant, 15 patients (35.7%) were left ocular dominant, 36 patients (85.7%) were right hand dominant and 4 patients (9.5%) were left hand dominant. Out of 27 right ocular dominant patients, right MLD was larger than the left in 26 patients (96.3%). It was larger in average of 0.47 mm (p<0.001) in 27 right ocular dominant patients. Also, left MLD was larger than the right in 11 patients (73.3%) out of 15 left ocular dominant patients. It was larger in average of 0.57 mm (p=0.003) in 27 left ocular dominant patients. MLD on the side of the dominant eye was larger in average of 0.50 mm (p<0.001) than the MLD of non-dominant eye side. Right MLD was larger than the left in average of 0.28mm (p=0.010) in right hand dominant patients, and left MLD was larger than the right in average of 1.15 mm (p=0.025) in left hand dominant patients. Conclusion: The function of levator palpebrae muscle differs in right and left, and the difference correlates with the dominant eye. Also, the function of levator palpebrae muscle is stronger in the dominant eye. We were able to present statistical evidence regarding the difference of the function in right and left levator palpebrae muscle. This may be a factor worth consideration in terms of balancing the eyes during the blepharoplasty.

Fishing efficiency of LED fishing lamp for squid Todarodes pacificus by training ship (실습선에 의한 LED 집어등의 오징어 어획성능)

  • This study surveyed the fishing efficiency for Japanese common squid based on improvements made to LED fishing lamps by utilizing the training ship of Gangwon Provincial College. The training ship, Haesong-ho (24 tons), was equipped with seventy-two 150W electric power LED fishing lamps (10.8kW), and their fishing efficiency and fuel consumption level were surveyed for a total of ten times during the period between June 15, 2009 and July 27, 2009. In addition, the training ship was equipped with seventy-one 300W electric power LED fishing lamps (21.3kW) and their fishing efficiency and fuel consumption level were surveyed for a total of five times during the period between January 17, 2010 and August 4, 2010. In order to compare the fishing efficiency of LED fishing lamps, the catch of another fishing vessel equipped with Metal Halide (MH) fishing lamps of 120kW for the same period and at the same fishing grounds. The fuel consumption levels during the fishing operation of Haesong-ho was about 1,047.7 liters which was approximately 19.9% of the total fuel consumption level of 5,262.6 liters, and the fuel consumption level per operation hour was on average 9.2 liters. The number of Japanese common squid caught by the LED fishing lamp-equipped fishing vessel ranged from 12 to 1,640 squid for each fishing trial and the average quantity was 652. The number of Japanese common squid caught by the MH fishing lamp-equipped 10 fishing vessels ranged from 40 to 4,800 squid and the average quantity was 2,055. The fishing of Japanese common squid was done by the use of hand lines and an automatic jigging machine. The number of Japanese common squid caught per hand line and a single roller of the automatic jigging machine was in the proportion of 50.8% to 49.2% with respect to the LED fishing lamp-equipped fishing vessel. However, the number of Japanese common squid caught per hand line and a single roller of the automatic jigging machine was in the proportion of 86.4% to 13.6% with respect to the MH fishing lamp-equipped fishing vessel where most of the catch was made by hand lines. On the other hand, in comparing the number of Japanese common squid caught per automatic jigging machine, the number of squid caught by the LED fishing lamp-equipped fishing vessel was about the same or greater than the number of squid caught by the MH fishing lamp-equipped fishing vessel.

A Survey on the Band Injury Patients of Physical Therapy (수부손상환자의 물리치료 실태조사)

  • The purpose of this study is to provide the fundamental information fer the rehabilitation of hand injury and to emphasize the point of early treatment of hand injury after operation. The subjects are composed with the physical therapists who care hand injury and are engaged in 76 university hospitals,59 general hospitals and 88 semi and local clinics respectively. Investigator take the research form chart with 41 questionnaire and use the cross tabulation frequencies and one-way ANOVA of SPSS WIN(ver 10.0) for the statistic analysis. The results of this research are as follows :1. The number of physical therapists who care hand injury are 110 men and 113 women. 2. The beginning period of active assistive exorcise on the various type of injuries are as follows: At the case of flexor tendon injury, the most part of physical therapists make response to the period that is above 4 weeks in the rate of 29.1%. At the case of fracture, the most part of physical therapists make response to the period those are from 3 weeks to less than 4 weeks and from 4 weeks to less than 5 weeks in the rate of 28.7% respectively. At the case of crushing injury, the most part of physical therapists response to the period that is above 4 weeks in the rate of 32.2%. At the case of amputation injury, the most part of physical therapists response to the period that is above 4 weeks in the rate of 40.4%. 3. In the physical therapy request, treatment period are mentioned in 22.0%. 4. The most difficult factors in the hand treatment is to contracture soft tissue and joint to physical therapist in the rate of 59.6%. 5. Among the reasons of the intervention between physical therapy request and actual treatment, the case of wrong physical therapy request is examined in 69.4%.

The Effects of Hand Function on the Angle of Holding a Smartphone (스마트폰 사용 시 팔의 지지 각도가 손의 기능에 미치는 영향)

  • The purpose of the study is to prevent pain and malfunction of the hand by correcting the angle of holding a smartphone. Subjects comprised of 21 healthy students. This research was conducted from April 21 2014 to April 27 2014. The three groups were as follows: first group included 7 students who held the phone at an angle of 90 degrees without a sling, and second group included 7 students who held the smartphone at an angle of 90 degrees with a sling; and the third group included 7 students who held the smartphone at an angle of 120 degrees with a sling. VAS was measured through the Finkelstein Test. Also, pain rating and muscle strength were assessed four times 30 minutes before and after the experiment for one and a half hour with pinch grip, dynamometer, and visual analogue scale. Smartphone was used. In all of the measurements, period showed a significant difference (p<.05) between both sides and a significant difference was not found between the group. In the time ${\times}$ group, pain score and grip strength were significantly different in the right side only (p<.05). According to the results of this experiment, providing support to the hand helps to increase the hand function by reducing the stress.

Microbiological Hazard Analysis of Ginseng Farms at the Cultivation Stage to Develop a Good Agricultural Practices (GAP) Model (인삼의 GAP 실천모델 개발을 위한 재배단계의 미생물학적 위해도 평가)

  • This study validated microbiological hazards of ginseng farms at the cultivation stage and suggested recommendations to develop a good agricultural practices (GAP) model. A total of 96 samples were collected from cultivation environments (soil, irrigation water, and atmosphere), plants (ginseng and its leaf), personnel hygiene (glove, cloth, and hand) of 3 ginseng farms (A, B, and C) and were tested to analyze sanitary indicator bacteria (aerobic plate count, coliforms and Escherichia coli), major foodborne pathogens (E. coli O157:H7, Listeria monocytogenes, Salmonella spp., Staphylococcus aureus, and Bacillus cereus), and fungi. Total bacteria, coliform, and fungi in the 3 ginseng farms were detected at the level of 1.3~6.0, 0.1~5.0, and 0.4~4.9 v/g (or mL, hand, and $100cm^2$), respectively. Only irrigation water collected from one ginseng farm was confirmed to be E. coli positive. In case of pathogenic bacteria, B. cereus was detected at levels of 0.1~5.0 log CFU/g (or mL, hand, and $100cm^2$) in all samples, but other pathogen bacterias were not detected in any samples from all farms. Although E. coli were detected in irrigation water, the level of microbial for the three farms was lower than the regulation limit. According to the results, the ginsengs produced from the 3 farms were comparatively safe with respect to microbiological hazard. However, cross-contamination of bacteria from environments and workers to ginseng has been considered as potential risks. Therefore, to minimize microbial contamination in ginseng, GAP model should be applied for ensuring the safety of ginsengs.

Change of working length in curved canals by various instrumentation techniques (만곡근관에서 근관형성법에 따른 근관작업장의 변화)

  • To evaluate the change of working length with various instrumentation techniques in curved canals, working length and canal curvature were determined before and after canal instrumentation in buccal or mesial canals of extracted human molars. Stainless steel K-files ($MANI^{(R)}$, Matsutani Seisakusho Co. Takanezawa, Japan), nickel-titanium K-files (Naviflex $NT^{TM}$, Brassier, Savannah, USA) , $ProFile^{(R)}$, and ProTaper (Dentsply-Maillefer, Ballaigues, Switzerland) were used to prepare the canals with crown-down technique. In two hand instrumentation groups coronal flaring was made with Gates Glidden burs. Apical canals were instrumented until apical diameter had attained a size of 30. Positional relation between the tooth apex and the $\#10$ K-file tip was examined by using AutoCAD 2000 (Autodesk Corp., San Rafael. CA, USA) under a stereomicroscope before and after coronal flaring, and after apical instrumentation. Degree of canal curvature was also measured with Schneider's method in radiographs. Data of working length and canal curvature changes were statistically analyzed with one-way ANOVA and Tukey's studentized range test. Working length and canal curvature were decreased significantly in each step in all instrumentation groups. Coronal flaring using Cates Glidden burs in hand instrument groups and whole canal instrumentation using stainless steel hand K-files caused significantly more working length change than in ProFile instrumentation group (p<0.05). The result of this study demonstrates that all of the above kinds of instrumentation in curved canals cause reduction of working length and canal curvature at each instrumentation steps, and hand instrumentation causes more working length change than ProFile.

Development of Surface Myoelectric Sensor for Myoelectric Hand Prosthesis (근전의수용 소형 표면 근전위 센서의 개발)

  • This paper proposes a compact-sized surface myoelectric sensor for the myoelectric hand prosthesis. To fit the surface myoelectric sensor in the socket for the myoelectric hand prosthesis, the sensor should be a compact size. The surface myoelectric sensor is. composed of a skin interface and a single processing circuit that are mounted on a single package. The skin interface has one reference and two input electrodes, and the reference electrode is located in the center of two input electrodes. In this paper we propose two types of sensors with the circle- and bar-shaped reference electrode, but all input electrodes are the bar-shaped. The metal material of the electrodes is the stainless steel (SUS440) that endures sweat and wet conditions. Considering the conduction velocity and the median frequency of the myoelectric signal, we select the inter-electrode distance (IED) between two input electrodes as 18mm, 20mm, and 22 mm. The signal processing circuit consists of a differential amplifier with a band pass filter, a band rejection filter for rejecting 60Hz power-line noise, amplifiers, and a mean absolute value circuit. We evaluate the proposed sensor from the output characteristics according to the IED and the shape of the reference electrode. From the experimental results we show the surface myoelectric sensor with the 18mm IED and the bar-shaped reference electrode is suitable for the myoelectric hand prosthesis.

Infection control of dental implant hand drivers using ethanol solution (에탄올을 이용한 치과 임플란트 핸드 드라이버의 감염 관리에 대한 연구)

  • Purpose: The purpose of this study was to study the effects of the utilization of ethanol solution in infection control of dental implant hand drivers, a common practice in dental prosthodontic clinics. Materials and Methods: Infection control methods were divided into two groups. One swabbed with 83% ethanol gauze and the other immersed in 83% ethanol solution for 30, 60, 90, 120, 150, 180 and 300 second intervals after inoculation of the dental implant hand drivers with Staphylococcus aureus. After measuring the number of colony forming units and analyzing the optical density, the effects of infection control in the experimental group were compared with the positive control group without infection control after inoculation with bacteria and the negative control group without inoculation with bacteria after sterilization. Results: The number of colony forming units and optical density analysis showed a statistically significant difference compared to the positive control. On the other hand, there was no statistically significant difference between the negative control and the group immersed in the 83% ethanol solution for more than 150 seconds. Conclusion: It is recommended to use the ethanol solution as a pre-cleaning process before sterilization, since the intermediate-level disinfection method using ethanol solution alone for the infection control of the dental implant hand driver cannot clinically secure the sterility.

The Effects of the Cognition Improvement Activities of the Elderly in the Welfare Center on Hand Dexterity, Cognition, Instrumental Activity of Daily Living and Depression (복지관 노인의 인지증진활동이 손의 기민성 정도와 인지능력, 도구적 일상 생활 및 우울에 미치는 영향)

  • Objective : The purpose of this study is to investigate the effects of hand dexterity, cognition skill, instrumental activities of daily living, and depression after cognition improvement activities in elderly welfare centers. Method : The cognition improvement activities were performed for 10 ristimes on 20 elderly people. This study assessed hand dexterity, cognition skills, IADL, and depression using grooved pegboard, MOCA-K, K-IADL, and K-GDS before and after cognition improvement activities. Result : After cognition improvement activities, hand dexterity improves positively and cognition skills and depression improves significantly. However, instrument activities of daily living was not affected by cognition improvement activities. Conclusion : The cognition improvement activities affect positively on hand dexterity, cognition skills and depression. The elderly people of welfare center were highly satisfied with cognition improvement activities. However, this study were held in one welfare center in K city, so further study required to recruit more sample size and develop regional programs to generalize the result.
