암 환자 삶의 질에 대한 근거이론적 접근 (Quality of Life in Cancer Patients;Grounded Theory)

  • The purpose of the study was to understand and explore the quality of life in cancer patients in the context of a Korean culture and society. Grounded theory method guided the data collection and analysis. A total of 10 cancer patients was selected by a theoretical sampling. The data were collected by an open question. All interviews were audio taped and transcribed verbatim. Constant comparison analysis was employed to analyze the data. As the results, eighty-five concepts and twelve categories were emerged, and "keeping well-being status" identified as a core category. Quality of life related categories derived from this study will academically contribute to the understanding and expansion of theoretical bases for quality of life of Korean cancer patients and to the contexts for the development of measurement of quality of life. Furthermore, they will function as the foundation of a intervention development for quality of life.

Experiences of Nursing Students in Caring for Pediatric Cancer Patients

  • Background: This study was performed to determine the experiences of nursing students in caring for paediatric cancer patients and their families. Materials and Methods: This qualitative survey was carried out with 14 students studying in the nursing department of the Faculty of Health Sciences of a university in Edirne, Turkey. Data were obtained through focus group interviews and evaluated based on a qualitative content analysis. Results: It was determined that students, for the most part, experienced problems related to communication, sadness, helplessness, fear, anxiety, resentment and anger. In addition, the students were affected most often by effects of the disease and invasive procedures on paediatric cancer patients and their families during the process of caring for them in the oncology clinic. Conclusions: It would be useful to inform nursing students, prior to clinical practice, about the special needs of paediatric cancer patients and families who stay in oncology clinics, and to follow up with appropriate guidance during the clinical practices.

발마사지가 유방절제술 환자의 수술후 불안과 통증에 미치는 효과 (Effect of Foot Massage on Anxiety and Pain in Mastectomy Patients)

  • Purpose: The purpose of this study was to examine the effects of foot massage as a nursing intervention on the anxiety and pain of the mastectomy patients. Method: This research was quasi-experimental study of a total of 28 patients with 14 in the experimental group and 14 in the control group. For the experimental group, foot massage for 30 minutes was given by a foot-massage specialist. Anxiety and pain were measure once before the foot massage and five times after the massage. The collected data were analyzed by repeated ANOVA and t-test using the SPSS 12.0 program. Result: There was a statistically significant decrease in anxiety and pain in the experimental group compared to the control group over 5 different times. Conclusion: The foot massage was effective on anxiety and pain in mastectomy patients in this study. Therefore, foot massage would be usefully utilized as a nursing intervention for mastectomy patients.

죽음준비교육 참여군과 비 참여군의 삶의 의미 및 죽음에 대한 태도 비교 (Comparison of Meaning in Life and Death Attitude between Participants and Non-participants in Well-dying Education)

  • Purpose: The purposes of this study was to compare the meaning in life and death attitude between the participants and nonparticipants of the well-dying education program. Methods: This study adopted the descriptive comparative design. Data were collected by interviewing 85 participants and 94 non-participants of well-dying education. The instruments used for this study were a self-report questionnaire. Results: There were significant differences in age, gender, marital status, health status, and volunteer experience. The program participants showed higher scores in the death attitude than non-participants. There were significant correlations between meaning in life and death attitude in participant group. Death attitude was significantly associated with meaning in life in participant group with 6.0% variance. Conclusion: Based on the results, well-dying education program was effective to prepare good death with more comprehensive vision. Therefore, this program should be served for patient with life-threatening illness by nurse and this is the expended role of oncology and hospice palliative nurses.

용서간호중재 프로그램이 여성 암환자의 희망과 삶의 질에 미치는 영향 (Effects of a Forgiveness Nursing Intervention Program on Hope and Quality of life in Woman Cancer Patients)

  • Purpose: The purpose of this study was to analysis the effects of a forgiveness nursing intervention program on hope and quality of life in woman with cancer. Methods: The used design was a nonequivalent control group design with pretest and post-test. Thirty patients diagnosed with uterine cervical cancer, breast cancer, ovarian cancer were recruited from a University Hospital in Busan, Korea. An experimental group (n=15) was given the forgiveness nursing intervention program, and a control group (n=15) was done a common nursing intervention. The period of data collection was from June 10 to September 10, 2004. The obtained data were analyzed using ${\chi}^2-test$, Fisher's exact test, t-test, and ANCOVA. Results: The first hypothesis, "The experimental group would have higher hope state than control group" was supported(F =16.967, p = .000). The second hypothesis, "The experimental group would have higher quality of life state than control group" was supported (F =4.850, p =.036). Conclusions: The findings showed that the forgiveness nursing intervention program was effective to increase hope and quality of life in women with cancer. Therefore, this program may be used for a nursing practice for cancer patients who are suffering from emotional distress.

국내 암환자를 대상으로 한 간호중재 연구현황 및 분석 (The analysis of Trends and Contents of Nursing Intervention Research for Cancer Patients in Korea)

  • Purpose: In this study, the articles which developed and applied nursing interventions for patients with cancer were reviewed and analyzed. Methods: The analysis was performed in light of 171 pieces of literature on nursing interventions for patients with cancer published between 1991 and 2010. Results: Approximately half of the studies have been published between 2006 and 2010. The quasi-experimental designs using nonequivalent control group and no-synchronized design were the most common form of research design among the 71 papers constituting 41.52% of the total. The most commonly used nursing intervention was education (25.2%), followed by massage (20.4%) and exercise (11.5%). The most common outcome variable was anxiety (11.8%), followed by pain (8.0%) and nausea and vomiting (7.3%). Conclusion: The results of this study can provide useful information for the development of nursing interventions in specialized areas. Systematic literature review or meta-analysis is needed in this area.

Does Clinical Experience Help Oncology Nursing Staff to Deal with Patient Pain Better than Nurses from other Displines? Knowledge and Attitudes Survey Amongst Nurses in a Tertiary Care in Malaysia

  • Background: Successful implementation of pain management procedures and guidelines in an institution depends very much on the acceptance of many levels of healthcare providers. Aim: The main purpose of this study was to determine the level of knowledge and attitudes regarding pain among nurses working in tertiary care in a local setting and the factors that may be associated with this. Materials and Methods: This cross-sectional research study used a modified version of the Nurses' Knowledge and Attitudes Survey (NKAS) regarding pain. Basic demographic data were obtained for further correlation with the level of pain knowledge. Results: A total of 566 nurses, 34 male and 532 female, volunteered to participate in this study. The response rate (RR) was 76%, with an overall mean percentage score of $42.7{\pm}10.9$ (range: 5-92.5). The majority of participants were younger nurses below 40 years of age and more than 70% had worked for less than 10 years ($6.6{\pm}4.45$). Up to 92% had never had any formal education in pain management in general. The total mean score of correct answers was $58.6{\pm}9.58$, with oncology nursing staff scoring a higher percentage when compared with nurses from other general and critical care wards ($63.52{\pm}9.27$, p<0.045). Only 2.5% out of all participants obtained a score of 80% or greater. The majority of the oncology nurses achieved the expected competency level (p<0.03). Conclusions: The present findings give further support for the universal concern about poor knowledge and attitudes among nurses' related to the optimal management of pain. The results reflected that neither number of years working nor age influenced the level of knowledge or attitudes of the practising nurses. Oncology nursing staff consistently scored better than the rest of the cohort. This reflects that clinical experience helped to improve attitudes and knowledge concerning better pain management.

암 병동 간호사의 공감피로 경험 (Compassion Fatigue Experience of Oncology Nurses)

  • 본 연구는 암 병동에 근무하는 간호사의 공감피로 경험의 의미와 본질을 이해하기 위해 수행되었다. 연구 참여자는 암 병동 근무기간이 최소 6개월 이상인 11명을 대상으로 하였다. 심층면담을 실시하여 자료를 수집하였으며, Giorgi의 현상학적 방법으로 분석하였다. 연구결과 신체적으로 힘듦, 환자의 상황에 갇힌 마음, 암이라는 병 앞에서 무너짐, 되새겨지는 환자관련 기억, 변화된 자신의 모습을 발견함, 회복의 길을 찾아감의 6개의 범주로 도출되었다.

  • Purpose: This study was purposed to identify the effect of pain monitoring program for improving the knowledge, attitude and assessment ability of cancer pain in oncology nurses. Methods: In this study pain monitoring program was developed based on the literature, and constructed it with various teaching method including several group lectures, workshop, focus group and implementation at the clinical practices. In order to test the effect of this program, contrast group, pretest-post test quasi-experimental design was designed. 46 oncology nurses randomly assigned to either control or experimental group. The tool used are 32 item scale for the evaluation of the nurses' knowledge, attitude toward pain modified by Kim(1997), which was originally developed by McCaffery and Ferrel(1995). Results: The pain monitoring program for the oncology nurses was statistically significant for improving the level of the knowledge about and attitude toward pain management, and pain assessment behavior. Conclusion: More sensitive multiple measurement tool for the attitude toward pain management needs to be developed. Nurses need more knowledge and effective attitude toward cancer pain management. Pain monitoring program in this study is effective to improve the quality of pain management.

Effects of Distant Reiki On Pain, Anxiety and Fatigue in Oncology Patients in Turkey: A Pilot Study

  • Background: Fatigue, stress and pain are common symptoms among cancer patients, affecting the quality of life. The purpose of the present study was to determine the effect of distant Reiki on pain, anxiety and fatigue in oncology patients. Materials and Methods: Participants in the control group received usual medical and nursing care during their stay. The intervention group received usual care plus five distant Reiki sessions, one each night for 30 min. A face to face interview was performed and patient personal and illness related characteristics were evaluated using the Patient Characteristics form. Pain, stress and fatigue were evaluated according to a numeric rating scale. Results: The experimental group was predominantly composed of women (71.4%), married individuals (40%), and primary school graduates (40%). The control group was predominantly male (72.7%), married (60%), and primary school graduates (60%). The control group demonstrated greater levels of pain (p=0.002), stress (p=0.001) and fatigue (p=0.001). The Reiki group pain score (p <0.0001), stress score (p <0.001) and fatigue score were also significantly lower. Conclusions: The results of this study indicate that Reiki may d ecreasepain, anxiety and fatigue in oncology patients.
