  • The purpose of this survey was to give data and information about type and needs of Home Physical Therapy, Visiting Physical Therapy and School Physical Therapy for physical and nurse. The subjects were 154(99 physical therapists and 55 nurses) who were working at geriatric rehabilitation hospitals and children hospitals. The period of questionary collection was from the 15 of August to the 15th of September 2011. And data was analysis from 99 articles such as journals related to physical therapy, and searched with keyword 'home and visiting physical therapy' by web site and Korea National Assembly Library from 1991 to 2011. The data was analysis with percentage, mean, standard deviation and ANOVA by SPSS PC 12.0. The results were as follows; 1. The definition of 'Home Physical Therapy' has been community based on physical therapy service for the patient who had diagnosis by medical doctor, has been based on medical law. The definition of 'Visiting Physical Therapy' has been community based on physical therapy service at home for the patient who had diagnosis by medical doctor, for the national basic living security, and senior citizen over 65 years who lives alone, has been based on law for community health and law of long term health insurance. The definition of 'School Physical Therapy' has been school based on physical therapy service at school after class for the disabled children who are studying at school, has been based on special education law article 28. 2. As for the knowledge of the Home and Visiting and School Physical Therapy, both groups PT and nurse were 'I do not know'125(81.3%) of the difference the concept of 3 definitions, so it means to need education and information about the different concept of three physical therapy. As for the needs of home and visiting physical therapy, both groups of PT and Nurse were 'needs' 151(98.1%). Physical therapist showed of 'Needs' on visiting physical therapy 35(35.4%), home physical therapy 32(32.3%), and schole physical therapy 32(32.3%). Nurse showed of 'Needs' on home physical therapy 23(41.8%). visiting physical therapy 19(34.5%), school physical therapy 13(23.6%). • The effects of artificial interference on the vegetation landscape in Mt. Nam of Seoul, Korea were clarified by analysing the distribution of vegetation landscape element and the number and size of patch depicted as a vegetation map in terms of landscape ecological principles. The effects of artificial interference on vegetation were also confirmed from the environmental gradient analysis on plant community extended from the lowland to the peak of that mountain. Vegetation landscape elements were divided into plantation and secondary forest in actual vegtation map. The ratio of plantation to secondary forest was higher in the lowland below mid-slope and the southern slope. Most afforested land were occupied by Robinia pseudoacacia and Populus tomentoglandulosa, Pinus rigida, P. koraiensis, Metasequoia glyptostroboides, Alnus hirsuta and so on are localy planted. In addition, projects to replace those afforested trees by P. densiflora as a kind of campaign for "Restoration of the one original feature of Mt. Nam" or to replace those tree species by planting young Abies holophylla or P. koraiensis under the mature afforested trees are also carried out in recent years. In cases of secondary forest, the southern slope was dominated by P. densiflora and the northern one by Q. mongolica. But the lowland of the northern slope is dominated by P. densiflora as the same as that in the southern slope. Vegetation landscape elements in Mt. Nam were much simplified comparing with that of suburban area around Seoul. The number of patches, which reflects the degree of diverse artificial interference was more in the lower area than in the upper area and more in the southern slope than in the northern one. On the other hand, the size of patch showed the antagonistic tendency to that of the number of patch. As a result of environmental gradient analysis, vegetation distribution in Mt. Nam was different from that in suburban area around Seoul. For example, Alnus japonica community, Zelkova serrata community, and Carpinus laxiflora community, which is established in mountain comparatively rare in artificial interference disappeared in Mt. Nam. As a result of analysis on vegetational succession in P. densiflora community and Q. mongolica community, both communities showed a tendency of retrogressive succession differently from that in control site located in suburban area around Seoul. In addition, species composition of P. densiflora and Q. mongolica communities in Mt. Nam were also different from those in Mt. Surak located around Seoul. It was interpreted that those results were originated from the environmental pollution and excessive arti ficial interferences.rferences.

  • This study reviewed the type of the project and developmental plan related to ocean energy development in the coastal land boundaries and the ocean, and suggested the efficient scoping method through the diagnosis of the key items of environmental impact assessment (EIA) in the coastal area. The major projects are the construction of tidal power plant, tidal current power plant, and offshore wind power plant in the public water, and also those are the construction of solar power plants in the coastal land boundaries. While the project plans on a large scale such as the construction of tidal power plant, it is important to consider both property of usefulness of the designated areas and harmony analysis with marine space availability based on the adequacy of the site selection and relevance of plan with the master plan for reclamation and strategic environmental assessment (SEA). And also it needs to be considered the careful checkup on the EIA checklist referring to the type of project, effective post-monitoring, and suggestion of mitigating methods to minimize the environmental impacts during the stage of actual environmental impact assessment. Introduction of a system of integrated marine environmental impact assessment should be considered for reasonable and effective manage to developmental projects on the marine spatial area.

  • Purpose: Anastomotic leakage (AL) is associated with high morbidity and mortality, high reoperation rates, and increased hospital length of stay. Here we investigated the risk factors for AL after anterior resection for rectal cancer with a double stapling technique. Patients and Methods: Data for 460 patients who underwent primary anterior resection with a double stapling technique for rectal carcinoma at a single institution from 2003 to 2007 were prospectively collected. All patients experienced a total mesorectal excision (TME) operation. Clinical AL was defined as the presence of leakage signs and confirmed by diagnostic work-up according to ICD-9 codes 997.4, 567.22 (abdominopelvic abscess), and 569.81 (fistula of the intestine). Univariate and logistic regression analyses of 20 variables were undertaken to determine risk factors for AL. Survival was analysed using the Cox regression method. Results: AL was noted in 35 (7.6%) of 460 patients with rectal cancer. :Median age of the patients was 65 (50-74) and 161 (35%) were male. The diagnosis of AL was made between the 6th and 12th postoperative day (POD; mean 8th POD). After univariate and multivariate analysis, age (p=0.004), gender (p=0.007), tumor site (p<0.001), preoperative body mass index (EMI) (p<0.001), the reduction of TSGF on 5th POD less than 10U/ml (p=0.044) and the pH value of pelvic dranage less than or equal to 6.978 on 3rd POD (p<0.001) were selected as 6 independent risk factors for AL. It was shown that significant differences in survival of the patients were AL-related (p<0.001), high ASA score related (p=0.036), high-level EMI related (p=0.007) and advanced TNM stage related

  • Background and Objectives : Branchial anomaly is a common neck pathology seen by otolaryngologist. Although there is still controversy regarding the origin, almost all surgeon agree that branchial anomalies result from incomplete involution of the branchial apparatus. Depending on its anatomic location, branchial anomalies can be classified into first, second, third and fourth anomaly. The purpose of this study is to evaluate the incidence of different categories of branchial anomalies and to determine proper method of diagnosis and treatment. Subjects and Method : A retrospective chart review was conducted for 72 patients with branchial anomalies operated on at the Korea University Anam Hospital from 1996 to 2007. The patient's sex, age, presenting symptoms, classification, site of lesion, diagnostic studies, surgical therapy and post-operative surgical complication were reviewed. Results : Patients were commonly seen in the 3rd decades. Eleven patients(15.3%) were first branchial anomalies, 52(72.2%) were second, 1(1.4%) was third, and 8(11.1%) were fourth. Of the types of anomalies, cyst were most commonly seen. In cases of 3rd and 4th branchial anomalies, barium contrast study can delineate the course of sinus or fistula. All cases was operated on for branchial anomalies, there were no major post-operative complication. Conclusion : Cervical mass and recurrent cervical infection or abscess are suspect for branchial anomalies. Especially, clinical history of dysphagia, and recurrent painful swelling in the thyroid region in young patients should raise the suspicion of 3rd and 4th branchial anomalies. Careful history, clinical examination and radiographic study were essential parts in diagnosis of branchial anomalies. Although surgical management of branchial anomalies depend on its type, nature and extent, complete excision is the major surgical procedure.

  • The result of the study on the origin and preservation status of the 'Gongju Junghakdong old missionary house', the Registered Cultural Property No. 233, reveals that the building was approved on October 23, 1921, and missionary Alice H. Sharp was living in the building until her retirement from her missionary life of 39 years in Korea in 1939. In order to review internal and external preservation status of the building, condition of wood material, the composition of the window, the damage of the wall, and the leaning of the building were examined. In particular, in the case of window facilities, it is necessary to restore it to the original upper and lower sliding window. As a result of investigation of the preservation status of the external facilities, it is necessary to restore the original shape of the staircase and a deck of the building on south western side and the well. In addition, the results of the non-destructive diagnosis of the ground revealed that the building was built on uneven surface layer of 2-5 m thickness and the boundary between the surface layer and the upper part of the weathered rock is inclined following geomorphology. This phenomenon shows that when the water content of the ground increases in the rainy season, the bearing capacity of the ground is lowered, and there is a possibility of uneven subsidence. Especially, landslides may occur in case of heavy rain. Therefore, it is desirable to install a masonry facility at the southwest boundary of the site, and it is recommended to install a drainage facility to ensure rapid drainage.

  • Purpose: This study aimed to investigate the computed tomography and magnetic resonance imaging features of giant cell tumors in the temporomandibular joint region to facilitate accurate diagnoses. Materials and Methods: From October 2007 to June 2020, 6 patients (2 men and 4 women) at Yonsei University Dental Hospital had histopathologically proven giant cell tumors in the temporomandibular joint. Their computed tomography and magnetic resonance imaging findings were reviewed retrospectively, and the cases were classified into 3 types based on the tumor center and growth pattern observed on the radiologic findings. Results: The age of the 6 patients ranged from 25 to 53 years. Trismus was found in 5 of the 6 cases. One case recurred. The mean size of the tumors, defined based on their greatest diameter, was 32 mm (range, 15-41 mm). The characteristic features of all cases were a heterogeneously-enhancing tumorous mass with a lobulated margin on computed tomographic images and internal multiplicity of signal intensity on T2-weighted magnetic resonance images. According to the site of origin, 3 tumors were bone-centered, 2 were soft tissue-centered, and 1 was peri-articular. Conclusion: Computed tomography and magnetic resonance imaging yielded a tripartite classification of giant cell tumors of the temporomandibular joint according to their location on imaging. This study could help clinicians in the differential diagnosis of giant cell tumors and assist in proper treatment planning for tumorous diseases of the temporomandibular joint.

  • Purpose: Percutaneous Kirschner wire fixation is common method for hand fracture. It is simple but has risk of ascending infection through the pin and bony injury by multiple drilling. Ascending infection through pin tract is mostly superficial and can be treated with antibiotics and aseptic dressing. This is a case review of subacute osteomyelitis on phalangeal bones after Kirschner wire fixation with literature review. Methods: A 40-years-old man with distal phalangeal fracture on right second finger is presented. He went to a local clinic and had percutaneous Kirschner wire fixation under local anesthesia. He was transferred to our hospital for ulcerative wound on DIP joint at 4 weeks after operation. Radiography showed osteolytic change around medulla of middle and distal phalanges, leading to diagnosis of a subacute osteomyelitis. We treated it with amputation at the level of shaft of middle phalanx. Results: The postoperative course was uneventful. We thought several possible reasons for osteomyelitis in our case. First, it could resulted from ascending infection through the wire. Second, it could be resulted from a bony burn by repeated drilling. And bony necrosis could be a consequence of arterial insufficiency caused by 2 pin insertion. Conclusion: We suggest that a precise pinning based on accurate anatomical understanding is required for a percutaneous Kirschner wire fixation. The frequency of drilling should be minimized. Careful observation and patient education for pin site care are essential.

  • Purpose: Fewer than 100 cases of calcifying aponeurotic fibroma have been reported in the literature since this entity was initially described by Keasbey in 1953 who called it calcifying juvenile aponeurotic fibroma. The tumor is a slowly growing, painless mass. In most cases the mass is poorly circumscribed and causes neither discomfort nor limitation of movement. Most lesions occur in children, with a peak incidence ages of 8-14 years. There is no evidence of any increased familial prevalence. Predilection sites are palm, finger, toe, but it also occurs in the wrist, forearm, elbow, upper arm, neck, abdominal wall, lumbar paravertebral area, leg and ankle. We herein describe a rare case of calcifying aponeurotic fibroma occurring on the chin with review of the literature. Methods: A 14-year-old male had painless, slowly growing mass(${\phi}2.5cm$) on a chin for a year. The tumor was excised elliptically under local anesthesia and the excisional site was repaired directly. Due pathological examination was processed. Results: Histological examination revealed an illdefined fibrous growth that extends with multiple processes into the surrounding tissue with centrally located foci of calcification. The tumor is composed of short spindled plump fibroblasts with round or ovoid nuclei separated by collagenous stroma, showing vaguely palisading pattern. Diagnosis of calcifying aponeurotic fibroma was conferred. Postoperatively, the patient did well, and the lesion had not recurred. Conclusion: Fewer than 100 cases of calcifying aponeurotic fibroma have been reported in the literature. The most common occurring sites are palm, finger & toe, but it has been reported in the wrist, forearm, elbow, upper arm, neck, abdominal wall, lumbar paravertebral area, leg and ankle. Two cases of calcifying aponeurotic fibroma occurring on the neck have been reported in the literature. To the authors knowledge, our case of calcifying aponeurotic fibroma occurring on the chin is the first to be reported.

  • Objectives This study was designed to analyze the clinical studies on Mental retardation(MR) in traditional Chinese medicine(TCM). Methods For this study, we searched the clinical studies on MR, which had been published from 2003 to 2007, through web site CNKI(中國知識基礎設施工 http://www.cnki.net). There were 17 clinical studies and we focused on those studies. Results 1. In those Chinese studies, they used following words to describe Mental retardation; 小人弱智($xi\check{a}o\acute{e}rru\grave{o}zh\grave{i}$), 智能發育不全($zh\grave{i}n\acute{e}ngf\bar{a}y\grave{u}buqu\acute{a}n$), 智力低下($zh\grave{i}l\grave{i}d\bar{i}xi\grave{a}$), 精神發育遲滯($j\bar{i}ngsh\acute{e}f\bar{a}y\grave{u}ch\acute{i}zh\grave{i}$), 智能落后($zh\grave{i}n\acute{e}nglu\grave{o}h\grave{o}u$), 智能落后($zh\grave{i}n\acute{e}ngch\acute{i}hu\check{a}n$), 失天愚型患人($xi\bar{a}nti\bar{a}ny\acute{u}x\acute{i}nghu\grave{a}n'\acute{e}r$). 2. There were many kinds of TCM treatment methods for MR, such as herbal medicine, acupuncture, electroacupunture, acupoint injection, Chuna therapy, and special education. And those TCM treatments methods showed higher efficacies in the treatment of MR compared with Western medicine. 3. Mental retardation was related with the deficiency of heart, kidney, liver, spleen(心虛, 腎虛, 肝虛, 脾虛) and the pathological mechanism of Phlegm(絹) and Blood stasis(慫沂)in the studies about the Bian Zheng-the types of differential diagnosis- of MR. 4. Most of studies used Intelligence Quotient(IQ) to assess the efficacy of TCM treatment of MR. And the duration of treatment, the degree of illness, the age of patient, and the cause of illness affected the prognosis of MR. Conclusions These results suggest that traditional medicine could be one of the useful treatments on MR. And these results could be used in the clinical practices and studies on MR in Korea.
