Comparison of vegetation recovery according to the forest restoration technique using the satellite imagery: focus on the Goseong (1996) and East Coast (2000) forest fire

  • This study was conducted to compare the level of vegetation recovery based on the forest restoration techniques (natural restoration and artificial restoration) determined using the satellite imagery that targeted forest fire damaged areas in Goseong-gun, Gangwon-do. The study site included the area affected by the Goseong forest fire (1996) and the East Coast forest fire (2000). We conducted a time-series analysis of satellite imagery on the natural restoration sites (19 sites) and artificial restoration sites (12 sites) that were created after the forest fire in 1996. In the analysis of satellite imagery, the difference normalized burn ratio (dNBR) and normalized difference vegetation index (NDVI) were calculated to compare the level of vegetation recovery between the two groups. We discovered that vegetation was restored at all of the study sites (31 locations). The satellite image-based analysis showed that the artificial restoration sites were relatively better than the natural restoration sites, but there was no statistically significant difference between the two groups (p > 0.05). Therefore, it is necessary to select a restoration technique that can achieve the goal of forest restoration, taking the topography and environment of the target site into account. We also believe that in the future, accurate diagnosis and analysis of the vegetation will be necessary through a field survey of the forest fire-damaged sites.

인터넷을 이용한 관절염 환자의 건강상담 내용분석 -대한류마티스 건강전문학회 홈페이지를 대상으로- (Analysis of Health Counseling by the Internet - on the Home page of Korean Rheumatology Health Professions Society)

  • Recently with the development in computer technology and its communication system, many changes in medical informatics enable us to use various medical information regardless of time or place. There are many home pages on the web, which provide medical counseling and hospital information. On May 11th 1999, Korean Rheumatology Health Professions Society began its new service as a home page on the web with various rheumatologic health information, questioning/ answering, and so on. This study was undertaken to examine the content and the purpose of health counseling on the web. The data was collected from 173 questioners who visited questioning/ answering site in the KRHPS home page for May 11th, 1999 through November 10th, 1999. Most of the questioners consulted the health problems of their or their families. Over two thirds of them were already diagnosed medically. Rheumatoid arthritis was the most frequent one. Other diseases, such as, osteoarthritis, ankylosing spondylitis, Still's disease were also on the list. Most of the questioners wanted to know treatment strategies, to consult about their symptoms, and to make diagnosis. And many questioners wanted detailed explanations about their diseases or the informations regarding the hospital. These findings suggest that the health counseling on the web may be used to supplement the lack of direct medical interviews with doctors. It also is expected to guide the patients to the right direction.

  • Over the past 40 years, Korea's defense industry has been deepening into a low-efficiency industrial structure as the government directly controls prices, quantities, and costs. By implementing the Defense Industry Building Act in 2021, the government is creating a healthy ecosystem for the defense industry and strengthening its global competitiveness. In this study, based on KPC's Productivity Management System (PMS), a diagnostic model of defense companies implemented since 2013, the on-site diagnosis was performed from 4 to 28 days depending on the size of the company data was collected based on the results. The causal relationship was analyzed through structural equation model path analysis for the effect of innovation capability on productivity performance. As a result, it suggests that defense materials suppliers should focus on which core processes to innovate and strengthen and improve their innovation capabilities.

해외 노후 수력발전시스템 수용가 맞춤형 현대화 기술개발 및 실증 (Development of modernizing Old Small-Hydropower system with custom-built at Overseas)

  • 연구목적: 본 연구는 운용중인 해외 노후 수력발전 시스템 수용가 맞춤형 현대화 기술개발 및 실증이 그 목적이다. 연구방법: 현지에서 운용중인 발전소의 일간, 주간, 윌간, 년간 정보를 바탕으로 현대화 설계를 수행 하였다. 연구결과: 본 논문에서는 국내 시설물 안전관리에 관한 특별법에 의거 필리핀에 위치한 노후 수력발전소의 현대화를 위해 최초 기존 시설물에 대한 안전진단을 실시하여 토목 및 건축물의 보강을 위한 방법을 선정하였다. 결론: 현지 환경에 적합한 수차선정과 노후 시설물의 유지보수를 국내 토목 보수 보강 기술로 설계하였다.

원자력발전소 전력케이블 부분방전 진단 사례 (Partial Discharge Measurement of Power Cables for Nuclear Power Plant)

  • Electric cables are one of the most important components in a nuclear power plant since they provide the power needed to operate electrical equipment. Despite their importance, cables typically receive little attention since they are considered passive, long-lived components that have been very reliable over the years when subjected to the environmental conditions for which they were designed. The operating experience reveals that a defect of the insulator or poor construction causes the initial failure of cable. However, the number of cable failures increase with plant aging, and these cable failures are occurring within the plants' 40-year licensed period. These cable failures have resulted in plant transients, shutdown, loss of safety functions or redundancy, entries into limiting conditions for operation, and challenges for plant operators. Therefore, diagnosis of MV cable installed in NPPs has become one of the most urgent issues in recent years. In accordance with PSR, condition maintenance for cables is also continuously required. Recently, HFPD tests have been widely performed to diagnose cable in the transmission and distribution cable system. However, on-line HFPD wasn't used in the NPPs because of the danger of plant shutdown, measurement sensitivity and application problems, etc. In this paper, HFPD measurement with portable device was performed to evaluate the integrity of the 4.16kV & 13.8kV cable lines. The test results show that HFPD is highly attractive to the diagnosis of MV cables in NPP by high detection sensitivity on-site.

Clinical Utility of Two Interferon-gamma Release Assays on Pleural Fluid for the Diagnosis of Tuberculous Pleurisy

  • Background: The release of interferon-gamma (IFN-${\gamma}$) by T lymphocytes increases after rechallenge with Mycobacterium tuberculosis antigen, especially, at a localized site of tuberculosis (TB) infection. We aimed to compare the clincial efficacy of two commercial IFN-${\gamma}$ release assays from pleural fluid for the diagnosis in tuberculous pleurisy. Methods: We performed T-SPOT.TB and QuantiFERON-TB Gold tests simultaneously on pleural fluid and peripheral blood samples from patients with pleural effusion, in South Korea, an area with intermediate TB burden. Results: Thirty-six patients were enrolled prospectively, and tuberculous pleurisy was found in 21 patients. Both the numbers of IFN-${\gamma}$ secreting T cells and the concentration of IFN-${\gamma}$ were greater in the pleural tuberculous group, comparing with the non-tuberculous group. Moreover, in the tuberculous group, there was a significant difference in IFN-${\gamma}$ producing spot-forming cells using the T-SPOT.TB method between pleural fluid and peripheral blood. The receiver operating characteristic (ROC) curve, was the greatest for pleural fluid T-SPOT.TB test, followed by peripheral blood T-SPOT.TB test, peripheral blood QuantiFERON-TB Gold test, and pleural fluid QuantiFERON-TB Gold test (area under the ROC curve of 0.956, 0.890, 0.743, and 0.721, respectively). The T-SPOT.TB assay produced less indeterminate results than did QuantiFERON-TB Gold assay in both pleural fluid and peripheral blood. Conclusion: These findings suggest that the pleural fluid T-SPOT.TB test could be the most useful test among the IFN-${\gamma}$ release assays for diagnosing tuberculous pleurisy in an area with an intermediate prevalence of TB infection.

손가락에 발생한 평활근종: 증례보고 (A Case of Leiomyoma of the Thumb)

  • Purpose: Leiomyoma is a rare benign tumor of nonstriated muscles. Leiomyoma is most commonly found in the uterus and rarely occur in the hand. 150 cases of leiomyoma of the hand have been reported in the English literature; however, to the best of our knowledge only four of these have been in children and none were reported in Korean literature. We present a case of leiomyoma in the hand of a 8 - year - old boy, which is a rare site for localization and unusual for age. Methods: A 8 - year - old boy presented with a painless mass on the ulnar side of his thumb. Physical examination revealed a $1.2{\times}1.2cm$ round, rubbery mass that was nontender to palpation. The vascular, sensory, and motor exams were otherwise unremarkable. Further evaluation with CT demonstrated an enhancing mass at dorsoulnar aspect of 1st proximal phalangeal region suggestive of a hemangioma versus other enhancing solid mass. The diagnosis of a leiomyoma was confirmed following surgical excision with histologic evaluation. Results: At 3 months follow - up, the incision was healed, motor and sensory function were intact, and there were full range of motion. Neither recurrence nor postoperative complication were observed. Conclusion: Leiomyoma is a rare tumor of the hand, especially in children. Diagnosing hand tumors in children is more difficult than in adults, hand surgeons should be aware of the diagnostic possibilities based on examination and imaging of a hand tumor ; however, surgical excision with histologic examination is required for definitive diagnosis.

  • 파킨슨병은 노년층에 가장 흔한 퇴행성 뇌질환 중의 하나로 진행성핵상마비, 다중계 위축, 루이체 치매 등과 같은 비전형 파킨슨병과 임상적으로 감별이 어려울 수 있다. 파킨슨병과 비전형 파킨슨병의 감별은 치료방침 결정과 예후평가뿐만 아니라 파킨슨병의 원인과 병태생리를 연구하고 새로운 치료법 개발에 있어서도 매우 중요하다. 파킨슨병과 비전형 파킨슨병과 같이 파킨슨 증후군을 유발하는 질환은 선조체내 도파민 신경의 퇴행성 변화를 흔히 동반하지만 병태생리학적으로 서로 다른 뇌피질 및 피질하 구조물에서의 신경세포 소실을 동반하고 있다. 따라서 국소 시냅스 활성도와 신경 및 시냅스의 손상, 그리고 원발병변과 기능적으로 연결된 원격부위의 기능이상 등을 대변하는 뇌포도당 대사를 F-18FDG PET으로 평가하는 것은 파킨슨 병의 감별진단과 병태생리를 평가하는데 유용하다.

경피적 내시경 요추 추간판 절제술 후 발생한 추간판과 경막 사이의 누공 형성 - 증례 보고 - (Fistula Formation Between the Disc and Dura after Percutaneous Endoscopic Lumbar Discectomy - A Case Report -)

  • 연구 계획: 증례 보고 목적: 경피적 내시경하 요추 추간판 절제술 후 인식하지 못한 경막 손상에 의해 생긴 추간판과 경막 사이의 누공에 대해 보고하고자 한다. 선행 연구문헌의 요약: 경피적 내시경하 요추 추간판 절제술시 경막 손상의 위험은 낮은 것으로 알려져 있으며, 인지하지 못한 경막 손상이 보고 되고 있으나, 이를 정확히 진단할 수 있는 도구는 아직 알려지지 않고 있다. 대상 및 방법: 50세 여자 환자는 제 4-5요추 추간판 탈출증에 대해 두 차례 경피적 내시경하 요추 추간판 절제술을 시행 받았다. 하지만, 환자는 술 후 6개월에도 지속적으로 좌측 하지에 참을 수 없는 동통과 운동 신경 마비를 호소하였다. 이에 신경 조직 손상을 찾기 위해 수술을 계획하였다. 결과: 이전 수술 부위에서 경막 손상이 발견되었으며, 또한 경막 손상 부위에서 추간판과 경막 사이에 누공이 형성된 것을 발견 하였다. 경막 손상 부위는 경막 전측부에 위치 하였으며, 크기는 5 mm 정도 였다. 5~0 나일론과 접착 봉합제로 경막 손상 부위를 봉합하였다. 술 후 환자의 술 전 증상은 호전되었다. 결론: 경피적 내시경하 요추 추간판 절제술 후 인지하지 못한 경막 손상은 추간판과 경막 사이의 누공 형성의 원인이 될 수 있다. 추간판 조영술은 인지하지 못한 경막 손상에 의해 생성된 누공에 대한 신뢰할 수 있는 진단 도구이다.

뇌하수체상부 배아세포종의 방사선치료 성적 (Radiation Therapy of Suprasellar Germ Cell Tumors)

  • 1979년부터 1985년까지 서울대학교병원 치료방사선과에서 외부 방사선조사를 시행한 15명의 뇌하수체상부 배아세포종(전이송과선종) 환자에 대한 후향적 분석을 시행하였다. 생존자의 추적기간은 $30\~91$개월이었다. 10명의 환자는 방사선치료 전 조직학적으로 진단되었으며 나머지 5명은 조직학적 진단이 없이 방사선치료를 시행하였단 조직학적으로 진단된 9명중 배아세포종 환자 6명은 전뇌와 척추에 3명은 전뇌 조사를 시행하였다. 혼합 배아세포종 및 종양 marker양성인 5명의 환자 중 2명은 전뇌, 그리고 1명은 원발병소 부위에만 방사선치료를 시행하였다. 총 방사선량은 원발병소에 $5,000\~5,500 cGy$, 전뇌에 $3,000\~4,400 cGy$ 그리고 척추에 >$1,300\~3,000 cGy$였다. 상기 그룹 14명의 환자에서 원발병소는 완전 관해 되었으며 척추실패는 관찰되지 많았다. 조직학적 진만이 없고 marker의 상승이 없었던 한 환자에서 전뇌 방사선조사를 시행하였으나 원발병소의 완전관해 없이 척추 재발이 발생하였다. 방사선치료는 뇌하수체상부 배아세포종에 유효한 치료방법이며 신경 내분비학적 양상과 함께 조직학적 진단이 불가능한 경우에 있어서 소량의 방사선치료 후 관해정도 관찰은 이후의 치료방향설정에 유용한 수단으로 이용될 수 있다.
