Juxtacortical Osteogenic Sarcoma of Chondroblastic Type on the Maxilla (상악골에 발생된 연골형성현 측피질골육종)

  • 39-year-old female had been treated for the exophytic mass on buccal aspect of the left, maxillary posterior area 2 years and 8 months ago. Tentative diagnosis was obtained as fibrous dysplasia on clinical and radiographic examinations and histopathologic findings revealed as osteochondroma after bone trimming at that time. She revisited for the treatment of recurred lesions. We reviewed this case with clinical. radiologic and histopathologic standpoints retrospectively, and came to a conclusion that the tumor primarily occurred was juxtacortical osteogenic sarcoma and recurred due to inadequate treatment and then expanded over intramedullary. This case shows that the diagnosis of osteosarcoma should take account of the patient history, clinical. radiographic and histopathologic findings and it requires attentive follow up check. Retrospectively reviewed results were as follows ; At first visit, oral examination revealed a bony hard swelling on the buccal aspect of the left maxillary posterior area. Radiographically, a dense radiopaque mass was noted on the site. The lesion showed hot uptake of /sup 99m/Tc-MDP. Histopathologic diagnosis was done as osteochondroma, but it was considered as osteogenic sarcoma when compared with the recurrent lesion. When she revisited for the treatment of multiple bony swelling on the left maxilla, radiograms showed typical features of malignancy such as widening of periodontal ligament space and sunray appearace, and coincided with benign characters as follows; relatively well circumscribed lesion and expansion and displacement of the adjacent structures. Finally, histopathologic findings of the lesion was well differentiated chondroblastic osteogenic sarcoma.

A Study on the Evaluation of Energy Consumption of the Air Compressor (공기압축기 소비에너지 평가에 관한 연구)

  • Various efforts have been initiated to reduce the energy consumption of the compressor as it is one of the approaches to saving a large portion of the fixed cost of the production site. Various results of reducing the energy consumption of the compressor have been reported, but to reduce the energy consumption of the compressors fundamentally, regular management of the compressor should ensure optimum operation. This requires periodic on-site visits by experts, but is often overlooked as a cost issue, resulting in the use of the compressor in low-efficiency conditions. Thus, it is necessary to develop a low-cost evaluation technology for compressor condition monitoring and efficiency analysis to ensure that the compressor is always driven at the optimum efficiency without imposing undue burden on the compressor user. In this study, a sensor was installed at the inlet, outlet, and power supply of the compressor, and a method for evaluating the energy consumption of the compressor using the minimum sensor was derived. The experimental results are presented to show the validity of the proposed method. It was confirmed that the energy consumption of the compressor can be easily as well as efficiently evaluated by using the method developed in this study.

A Case of Pilomatrixoma after Split Thickness Skin Graft (식피술 후 발생한 모기질종 1례)

  • Purpose: Pilomatrixoma is a benign, usually asymptomatic tumor. It presents clinically as a solitary superficial subcutaneous nodule measuring between 0.5 cm and 5 cm in diameter on the head or upper extremeties and has not been reported after skin graft. The objective of this article is to report our experience in treating pilomatrixoma which occurred after split thickness skin graft on the lower extremity. Methods: A 56-year-old female was treated in August 2005 with a $0.5{\times}0.5cm$ firm subcutaneous nodule at recipient site of split thickness skin graft on the left medial thigh. The tumor was successfully removed by complete excision and histologic examination was followed. Results: The diagnosis was pilomatrixoma which was characterized by a dual population of proliferating basophilic cells and diagnostic shadow cells. Conclusion: The tumor was successfully treated by complete resection. The authors report this very rare case of pilomatrixoma which occurred at recipient site of split thickness skin graft.

  • Emergency transport is directly related to the life of the patient, and rapid transport to the hospital is crucial. However, external environmental factors such as traffic or weather, interfere with hospital transport. In this study, we investigated the external environment affecting hospital transport time. We examined the transfer time and patient treatment time of emergency patients in an area of northern Gyeonggi-do from 2018 to 2020. Diagnosis after arrival at the hospital was used, and on-site treatment time was measured from paramedic arrival time at the scene to departure. Furthermore, we examined whether there was a correlation between the time paramedics left the scene and hospital arrival time through the reason for the delay as recorded in the emergency log. Traffic jams had the greatest impact on patient transport, while transport delays occurred due to heavy rain, but not snow. Among injured patients, electrical accidents were the most problematic in terms of on-site treatment time. This was because a lot of first aid is needed in electrical accidents. It must be necessary to mobilize two ambulances in an emergency through the expansion of infrastructure, prepare a plan for rapid transport in heavy rain, and implement strong laws against transport obstruction.

Hands-on Tools to Prevent Human Errors in Highway Construction (고속도로 건설현장의 인적오류 예방을 위한 실무자용 도구 개발)

  • Objective: The aim of this study is to reclassify human errors and to develop hands-on tools to apply the new classification for preventing human error accidents in highway construction site. Background: The main cause of accidents in highway construction was reported as the carelessness of workers. However, such diagnosis could not help us operationally prevent accidents in real workplace. Method: The accidents in highway construction were reanalyzed and the causes of human error were reclassified in order to educate and improve the awareness of human error in highway construction. Field survey and interview with safety managers and workers were conducted to find the causal relationship between the actual accidents and the human errors. Results: The most frequently observed human errors in highway construction were classified into six categories such as mis-perception, distraction, memory fail, slip, cognition error and mis-judgment. In order to provide hands-on tools to increase the awareness of human error in construction field, the human error checklist and card sorting diary were developed. Especially, the card sorting diary was designed to increase the ability in human error inspection of safety manager at construction site. Moreover, posters were developed based on actual accident cases. Conclusion: We suggested that the improved awareness and analytical report on checklist, card sorting diary and posters for construction field could collectively prevent the accident. Application: The classification of human error, hands-on tools and posters can be directly applicable on highway construction site. This analytical and collective approach preventing human error-related accident could be extended to other construction workplaces.

  • Lately teaching-learning system on the web site offers broad educational environment without any restriction of time and place to the teachers and learners. Using a web site in music education turned out to be effective. It provokes learner's interest and guides self-directed learning. Therefore, on this research, the web site for self-directed learning in music subject was designed and embodied as an assistant material of learning which is suitable for learner's level and demand As a result of using the web site in music class, there are some responses of students. First, there were 27 students positive responses$(79.4\%)$ of 34 students in the learning effect of the web site, and 25 students$(73.5\%)$ of 34 showed their will to use the web site continually if it is modified and supplemented. Second, some students answered that using this program in Diagnosis Evaluation and Formative Test could have an instant result, which was used for the material of feedback. Third, there was a positive evaluation of helping self-directed learning.

Clinicoradiologic evaluation of styloid process calcification

  • Purpose: This study was performed to investigate the prevalence, morphology, and calcification pattern of the elongated styloid process in the Mathura population and its relation to gender, age, and mandibular movements. Materials and Methods: The study analyzed digital panoramic radiographs of 2,706 adults. The elongated styloid process was classified with the radiographic appearance based on the morphology and calcification pattern. The limits of mandibular protrusion were evaluated for each subject. The data were analyzed by using a Student's t-test and chi-squared test with significance set at p=0.05. Results: Bilateral elongation having an "elongated" type styloid process with a "partially mineralized" pattern was the most frequent type of styloid process. No correlation was found between styloid process type and calcification pattern on the one hand and gender on the other, although elongated styloid was more prevalent in older and male populations (p<0.05). Further styloid process elongation showed no effect on mandibular protrusive movement (p>0.05). Conclusion: Dentists should recognize the existence of morphological variation in elongated styloid process or Eagle syndrome apparent on panoramic radiographs. We found higher prevalence of elongated styloid process in the population of the Mathura region when compared with other Indian populations. The calcification of the styloid process was more common in the older age group with no correlation to gender, mandibular movement and site. "Type I" with a "partially calcified" styloid process was observed more frequently in the population studied.

A Study on the Safety Inspection System Improvement of Agricultural Reservoir Considering Fill-Dam Characteristics (필 댐의 특성을 고려한 농업용 저수지 정밀안전진단체계 개선 연구)

  • In 2008, 17, 596 dams and reservoirs are scattered across South Korea, and 17, 505 of them (99.5 %) are used for agriculture and 99.3 % are fill dam types. This study aimed to review literature related to the precise safety diagnosis system for agricultural reservoirs established by Korea Rural Community Corporation (KRCC) and analyze problems of its evaluation method. And then, it proposed ways to improve the system including a modified diagnosis system, which was applied to pilot districts in order to verify the utility. For assessment model development of agricultural reservoir, we reviewed status of precision safety inspections systems of agricultural reservoir. There are many problems such as assess agricultural reservoir not by sheet which used in fill dam but by block which used in concrete dam construction and diversion tunnel which main element in reservoir levee is treated as water intake facility. For considering diversion tunnel in reservoir levee, previous precision safety inspection systems which summed in separated phenomenon, separated element, separated site, separated facility was change to new systems which summed in site, phenomenon, element, and facility. Compared results of previous inspection system calculated total assessment index (Ec) with new system calculated total assessment index (Ec) are not show statistical difference.

Cytologic Analysis of Malignant Tumor Cells in Cerebrospinal Fluid (뇌척수액에서 진단된 악성 종양세포의 세포학적 분석)

  • Cytologic evaluation of cerebrospinal fluid(CSF) is an effective tool in diagnosing many disorders involving the central nervous system(CNS). CSF examination has been found to be of particular value in the diagnosis of metastatic carcinoma, lymphomatous or leukemic involvement of CNS and certain primary CNS tumors. As a survey of metastatic tumors to CSF and an evaluation of the preparation techniques increasing cellular yield in our laboratory, 713 CSF specimens examined between July 1995 and April 1997(1 year 10 months), were reviewed. There were 75 positive and 5 suspicious cases, the latter have had no evidence of tumors clinically. Primary tumors of 75 positive cases were classified as follows; 4(5.3%) as primary brain tumors, 40(53.3%) as secondary carcinomas, 13(17.3%) as leukemias, and 18 (24.0%) as lymphomas. The most common primary site of metastatic carcinomas was the lung in 17 cases(42.5%) followed by the stomach in 13(32.5%), breast in 8 (20.0%), and unknown primary in 2(5.0%). Four primary brain tumors were 3 cerebellar medulloblastomas and a supratentorial primitive neuroectodermal tumor (PNET). All 40 metastatic carcinomas were adenocarcinoma presented as single cells or cell clusters. Although signet ring cells were frequent in the cases of gastric primary cancers, no significant cytologic differences according to the primary site were observed. The cytologic features of leukemia and lymphoma were characterized by hypercellular smears presenting as individual atypical cells with increased N/C ratio, presence of nucleoli, and nuclear protrusions. In medulloblastomas and PNET, the principal cytologic findings were small undifferentiated cells arranged singly or in loose clusters with occasional rosettoid features. This study suggests that the CSF cytology is useful in the diagnosis of malignancy, especially metastatic extracranial tumors and the diagnostic accuracy can be improved by increasing cellular yield using cytocentrifuge.

Bullae-Forming Pulmonary Metastasis from Choriocarcinoma Presenting as Pneumothorax

  • Gestational trophoblastic disease (GTD) is a condition of uncertain etiology, choriocarcioma, or placental-site hydatidiform moles, invasive moles, choriocarcinoma, and placental-site trophoblastic tumors. It arises from the abnormal proliferation of trophoblastic tissue and spreads beyond the uterus hematogenously. The early diagnosis of GTD is important to ensure timely and successful management and the preservation of fertility. We report the unusual case of a metastatic choriocarcinoma that formed bullae on the lung surface and presented as recurrent pneumothorax in a 38-year-old woman with elevated beta-human chorionic gonadotropin (hCG) levels. She underwent thoracoscopic wedge resection of the involved lung and four subsequent cycles of consolidation chemotherapy. No other evidence of metastatic disease or recurrent pneumothorax was noted during 22 months of follow-up. GTD should be considered in the differential diagnosis of spontaneous pneumothorax in reproductive-age women with an antecedent pregnancy and abnormal beta-hCG levels.
