Comparison of Video Lecture and Instructor-Led Lecture for the Recognition of Cardiac Arrest : Korean Youths (심정지 인지를 위한 동영상 교육과 강의식 교육의 비교 연구 : 청소년을 대상으로)

  • It is the first step to survive cardiac arrest for the general public to recognize cardiac arrest rapidly. The purpose of this study is to investigate the effective method of education by analyzing the degree of cardiac arrest recognition after performing the agonal breathing education in a video lecture or instructor-led lecture. The study subjects were assigned to either a video lecture or instructor-led lecture in a randomized way and were compared after the education according to the degree of recognition of cardiac arrest in the randomized controlled study. The study was conducted from October 30, 2015 to October 31, 2015. And the study subjects were 104 youths aged 15 years or older, of which 52 were selected as the experimental group and the remaining 52 as the control group. The results did not show a significant difference between these two groups when the subjects are given the video lecture where patients showed no reaction or sign of breathing(p=0.741). However, in the video lecture where there was no reaction of patients but still sign of agonal breathing, 43 people(82.7%) in the experimental group and 33 people(63.5%) in control group have successfully performed CPR and there has been a significant difference (p=0.006). Therefore, we could conclude that video lecture was more efficient than instructor-led lecture when teaching CPR.

Socioeconomic Costs of Stroke in Korea: Estimated from the Korea National Health Insurance Claims Database (건강보험 청구자료를 이용한 우리나라 뇌졸증 환자의 사회경제적 비용 추계)

  • Objectives : To estimate the annual socioeconomic costs of stroke in Korea in 2005 from a societal perspective. Methods : We identified those 20 years or older who had at least one national health insurance (NHI) claims record with a primary or a secondary diagnosis of stroke (ICD-10 codes: I60-I69, G45) in 2005. Direct medical costs of the stroke were measured from the NHI claims records. Direct non-medical costs were estimated as transportation costs incurred when visiting the hospitals. Indirect costs were defined as patients and caregivers productivity loss associated with office visits or hospitalization. Also, the costs of productivity loss due to premature death from stroke were calculated. Results : A total of 882,143 stroke patients were identified with prevalence for treatment of stroke at 2.44%. The total cost for the treatment of stroke in the nation was estimated to be 3,737 billion Korean won (KRW) which included direct costs at 1,130 billion KRW and indirect costs at 2,606 billion KRW. The per-capita cost of stroke was 3 million KRW for men and 2 million KRW for women. The total national spending for hemorrhagic and ischemic stroke was 1,323 billion KRW and 1,553 billion KRW, respectively, which together consisted of 77.0% of the total cost for stroke. Costs per patient for hemorrhagic and ischemic stroke were estimated at 6 million KRW and 2 million KRW, respectively. Conclusions : Stroke is a leading public health problem in Korea in terms of the economic burden. The indirect costs were identified as the largest component of the overall cost.

Heterogenous Clinical Manifestations in Adult Patients with Late Diagnosis of Glycogen Storage Disease type Ia (성인기에 진단된 당원병 제 Ia형의 다양한 임상 양상)

  • Glycogen storage disease (GSD) type Ia is rare inborn metabolic disorder, caused by glucose-6-phosphatase deficiency. It characterized by hepatomegaly, hypoglycemia, lactic acidosis, hypertriglyceridemia, and hyperuricemia and it is usually manifested in the infantile period. In addition, it is also associated with growth failure, pubertal delay, anemia, platelet dysfunction, osteopenia, and pulmonary hypertension. Hepatocellular adenoma and renal dysfunction are frequent late complications. Delayed diagnosis and inappropriate therapy lead to many complications such as growth failure, osteoporosis, refractory gout, renal failure, hepatocellular carcinoma (HCC), and pulmonary hypertension. Here, two Korean sisters diagnosed with GSD Ia, aged 33 and 36 respectively, were described and compared to recent articles about four adults with late diagnosis of GSD Ia. One sister had typical manifestations of GSD Ia including short stature (height, 145 cm), multiple hepatic adenoma, chronic kidney disease stage IV, and severe osteoporosis, whereas the older sister had normal stature (162 cm), one tiny hepatic nodule, and normal renal function. Direct sequencing of G6PC in two sisters identified a homozygous splicing mutation, c.645G>T, which is a prevalent mutation in Korea. Interestingly, our cases and four adults from recent reports had asymptomatic mild hypoglycemia and various manifestations including renal failure, HCC, fatty liver, or uncontrolled hyperlipidemia. These adult cases represent not only heterogenous phenotype to genotype within family members with GSD Ia but also long-term complications such as gouty arthritis, renal failure, and osteoporosis in untreated adult GSD Ia patients. In addition, lactic academia and hypertriglyceridemia are good markers of GSD Ia to distinguish from metabolic disease.

Effects of Metabolic Syndrome on Periodontal diseases in Korean Adults (한국 성인의 대사증후군이 치주질환에 미치는 영향)

  • This study aimed to evaluate the correlation between metabolic syndrome and periodontal in the adult Korean population on the basis of the 2009 Korea National Health and Nutrition Examination Survey(NHANES) data. Of 19-year-old or older adult patients suffering from their disease diagnosed as metabolic syndrome, 1,315 subjects whose community periodontal index(CPI) information was fully available were finally analyzed, and the following conclusions were drawn. 1. It was found that of sociodemographic characteristics, age and educational background correlated to the prevalence of periodontal diseases. With increasing age and poorer educational background, the rates of periodontal diseases increased(p<0.001). 2. It was revealed that of general health behaviors, smoking, alcohol drinking and BMI correlated with the prevalence of periodontal diseases. Higher rates of periodontal diseases were shown in the group of patients who were current smokers(p<0.05), had the higher frequency of drinking per week(p<0.001), and had higher BMI(p<0.005). 3. It was ascertained that of oral health behaviors, the history of oral health examination in the previous year, the use of proxabrush or dental floss, and perceived(i.e., subjective) oral health status correlated to the prevalence of periodontal diseases. The higher rates of periodontal diseases were seen in those who had no history of oral health examination in the previous year(p<0.001), had used neither proxabrush nor dental floss(p<0.05), and perceived that their own oral health was poor(p<0.001).

Distribution of Sexually Transmitted Viral Diseases in Busan (부산지역 바이러스성 성병 감염양상 연구)

  • This study was performed to evaluate the prevalence of high risk Human papilloma virus (HPV), Herpes simplex virus type 1, 2 (HSV-1,2), Hepatitis B virus (HBV) and Human Immuno deficiency virus (HIV) infection with sexual transmitted viral diseases in Busan during 2004 to 2005. six hundred seventy four samples of cervical swabs were tested for sexually transmitted viral diseases. Among the isolated viruses, 23 (3.4%) samples were HPV and 3 (0.4%) and 9 (1.3%) samples were HSV 1 and 2, respectively. Among the 586 serum samples tested for viruses, HSV IgM 121 (3.6%), HSV-1 IgG 487 (83.1%), HSV-2 IgG 135 (23.0%), HBsAg 26 (4.4%), HBeAg 7 (1.2%), and HIV (0%) types were found. HPV genotypes were detected in 16 patients, of which 13 cases were high risk type HPV, 3 cases were low risk type HPV, and multi infection were detected in 7 cases. In the age distribution of the patients, 7.2% of infection tested from cervical swabs occurred in under the age of 20, while 100% of infection was found to occur in those who were 40 years old or older in the serum samples. The outbreak pattern in their occupations was found to be the highest at the health organization (amusement quarter) for the cervical swabs, and at infirmary (commercial sex worker) for the serum samples, respectively.

Comparative Study on the Clinical Characteristics and Effects of Korean Medical Treatment between the Upper and Lower Lumbar Single Level Disc Herniation (상부요추와 하부요추 단분절 추간판 탈출증 환자의 임상적 특성과 한방치료 효과 비교연구)

  • Objectives Upper lumbar disc herniation (LDH) (L1/2, L2/3) has specific anatomical characteristics and different outcome after conventional treatment compared to lower LDH (L3/4, L4/5, L5/S1). The purpose of this study was to compare the clinical features and effects of korean medical treatment of upper LDH between lower LDH. Methods We retrospectively reviewed the clinical data collected from 121 patients who was had admitted at the Haeundae Jaseng Hospital of Korean Medicine from June 1st, 2014 to August 31th, 2018. The patients who had treated at L1/2, L2/3 level LDH were grouped and compared with those treated at the L3/4, L4/5, L5/S1 level. We reviewed the patient characteristics such as age, the positive rate of Straight Leg Raise Test (SLR test), the presence or absence of previous lumbar surgery. Numeric Rating Scale (NRS) and Oswestry Disability Index (ODI) were evaluated at adimission and discharge. Results Mean age was significantly higher at upper LDH group and positive rate of SLR test was higher at lower LDH group. There was no significant difference of gender and previous lumbar surgery between two groups. Each group had a significant improvement of NRS, ODI scores. But between two groups, there was no significant difference of NRS, ODI scores. Conclusions Upper LDH group was older than lower LDH group. SLR test was useful tool to exclude upper LDH. Korean medical treatment was significantly effective to both upper and lower LDH groups. Further well designed prospective comparative studies are needed.

The Efficacy of Saccharomyces boulardii CNCM I-745 in Addition to Standard Helicobacter pylori Eradication Treatment in Children

  • Purpose: This study aims to investigate Saccharomyces boulardii CNCM I-745 during Helicobacter pylori eradication in children. Methods: One hundred ninety-four H. pylori positive children were randomized in two groups. Therapy (omeprazole+ clarithromycin+amoxicillin or omeprazole+clarithromycin+metronidazole in case of penicillin allergy) was given to both groups during two weeks. In the treatment group (n: 102) S. boulardii was added to the triple therapy, while the control group (n: 92) only received triple therapy. The incidence, onset, duration and severity of diarrhea and compliance to the eradication treatment were compared. A $^{13}C$ urea breath test was done 4 weeks after the end of eradication therapy in two groups of 21 patients aged 12 years and older to test the H. pylori eradication rate. Results: In the treatment group, diarrhea occurred in 12 cases (11.76%), starting after $6.25{\pm}1.24days$, lasting $3.17{\pm}1.08days$, and compliance to eradication treatment was 100%. In the control group, diarrhea occurred in 26 cases (28.26%), starting after $4.05{\pm}1.11days$, lasting $4.02{\pm}0.87days$, and in six cases eradication treatment was stopped prematurely (p<0.05). The $^{13}C$ urea breath test showed successful H. pylori eradication in 71.4% of the patients in the treatment and in 61.9 % in the control group (not significant). Conclusion: S. boulardii has a beneficial effect on the prevention and treatment of diarrhea during H. pylori eradication in children. Although S. boulardii did only slightly increase H. pylori eradication rate, compliance to eradication treatment was improved.

Factors related to the willingness of chronic patients to quit smoking among adult smokers in Korea: Focusing on the data of the 7th National Health and Nutrition Survey (우리나라 성인 흡연자 중 만성질환자의 금연의지 관련 요인: 국민건강영양조사 제7기 자료를 중심으로)

  • This study aims to confirm the association with the willingness to quit smoking in adult smokers with chronic diseases, and the research method was analyzed using data from the 7th National Health and Nutrition Survey (2016-2018). The subjects were frequency analysis, chi-square test, and logistic regression analysis of 960 chronically ill patients aged 19 years or older, which were analyzed using the SPSS 26.0 Program. As a result of the study, depending on the level of education, high school graduation (OR=1.328, 95%CI=1.004-1.757), college graduation or higher (OR=1.556, 95%CI=1.167-2.075), the more stressed you are (OR=1.602, 95%CI) =1.217-2.109), there was an attempt to quit smoking (OR=5.263, 95% CI=4.287-6.462), which was associated with the willingness to quit smoking. It is necessary to prevent chronic diseases by reducing the smoking rate through active participation in smoking cessation programs targeting groups with high willingness to quit smoking, and by preparing measures to relieve appropriate stress.

  • The purpose of this study was to examine job stress, job satisfaction, and turnover intention of the nurses who work at the comprehensive nursing service units in two academic medical institutions that provide healthcare service at the tertiary care level in Korea. A descriptive study was conducted with the nurses by using self-reported questionnaires. We used descriptive statistics to summarize the data and Mann-Whitney U test for group comparison. The mean scores of job stress, job satisfaction, and turnover intention were $89.17{\pm}12.56$, $107.25{\pm}6.85$, $37.19{\pm}4.86$, respectively. Nurses with religion showed more job stress than those without. Nurses with their age below 30 years old had significantly less job satisfaction than those older than 30 years old. Monthly income was significantly associated with turnover intention in our data. The top three sub-category of the job stress were conflict with physicians, patients and their caregivers, and conflicts with others; and those of the job satisfaction were pay and promotion and task requirement. Job stress, job satisfaction, and turnover intention were significantly correlated. In order to reduce turnover intention of nursing professionals in the comprehensive nursing service, job stress related to issues on relationships with patients, their family, health care providers need to be taken into consideration. Policy and administrative support are needed to improve their work environment.

Effects of Music Therapy on Depression and Rehabilitation Motivation of Inpatients with Stroke (뇌졸중으로 입원한 환자에게 제공한 음악요법이 우울과 재활 동기에 미치는 영향)

  • This study aimed to evaluate the effect of music therapy on depression and rehabilitation-motivation provided to stroke patients, thus achieving evidence-based music therapy as a nursing intervention method for patients. Thirty-three people who were hospitalized for stroke diagnosis at K-Hospital were required to listen to 10 pre-selected songs for three days before bedtime. According to the general characteristics, the proportion of male participants (25, 75.8%) was higher than female (8, 24.2%), and by age group, 6(18.2%) were in 40s, 19(57.6%) were in 50s and 8(24.2%) were aged 60 or older. The study data were analyzed using Wilcoxon-signed rank test. As a result, music therapy does not affect depression and rehabilitation-motivation. However, it was shown to be effective in reducing depression in MMSE group 1 (≤ 29) with low cognitive levels. It is expected that this study can be used as an evidence for music therapy which can be applied as a way to increase the effectiveness of nursing intervention.
