Two Cases of Non-Calcifying Pilomatrixoma in Elderly Individuals (성인에서 발생한 비석회화 모기질종 2예)

  • Pilomatrixoma is an unusual benign cutaneous tumor, originating from precursor cells that normally differentiate into hair matrix cells. The majority of pilomatrixoma is seen in the first two decades of life, and they are extremely rare in older patients. Thus, pilomatrixoma is often not considered in the clinical differential diagnosis of solitary, firm nodules in older adults. Most present as firm, solid nodules in the head and neck region. We present two cases of a non-calcifying pilomatrixoma in older patients. One lesion arose from the upper neck in 54 year-old male and the other from the cavum concha of left ear in 35 year-old male.

The Effects of Discharge Planning for the Elderly with Pulmonary Disease in the Emergency Room (퇴원계획중재가 응급실 내원 호흡기 질환 노인의 퇴원준비도, 간호서비스 만족도, 재입원율에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of discharge planning on patient satisfaction, the readmission rate and preparedness for discharge in the elderly admitted to the emergency room (ER) for pulmonary disease. Methods: A quasi-experimental intervention study design was used. Older adults with pulmonary health problems in the ER in one general hospital were randomly allocated to either an experimental (n=21, 74.2 years) or control group (n=19, 70.7 years). The experimental group participated in a discharge planning program by a geriatric nurse practitioner. Data were collected from medical records, physical measurements and structured questionnaires including information on demographics, patient satisfaction, readmission, and preparedness for discharge. Results: Participants in the experimental group had significantly better outcomes with regard to patient satisfaction with nursing services (p=.003) and preparedness for discharge (p=.034). However, there was no significant effect on the readmission rate (p=.392) Conclusion: The results suggested that a discharge planning program could bolster nursing service satisfaction and preparedness for discharge in older patients admitted to the ER for pulmonary health problems. To clarify the effects of discharge planning on older patients admitted to the ER, a larger sample population, better instruments for various measures, a new manual on discharge planning and frequent follow-up will be necessary.

Trends Analysis of Characteristics in Patient with Dementia (치매환자 특성의 추이 분석)

  • South Korea is becoming an aging society, increasing the population with dementia. This study analyzed the characteristics of the trend of dementia patients by utilizing 12 years' worth of data from five V hospitals within metropolitan cities. The results show that there has been an 11.3 times increase in the total number of new dementia patients in these five hospitals from 410 in 2003 to 5,048 in 2014; and among them, the rate of increase was highest in P hospital with a 30.3 times increase. Conversely, there has been a 6-times increase in D hospital, illustrating a noticeable gap between these hospitals. Overall, these hospitals have shown an enormous increasing ratio. In an analysis according to age, it's been shown that those aged between 75 and 84 showed the highest prevalence, and most patients, approximately 93.5%, were 65 years or older, while 20.2% were 85 years or older, showing a high ratio of elderly among dementia patients. In an analysis by type of dementia, it's been shown that Alzheimer's was the most prevalent with 44.4%, followed by unspecified dementia with 33.3%, and vascular dementia with 20.4%. As evident by the outcome of the study, there's been a drastic increase in the number of dementia patients in the past 12 years; since the prevalence of dementia was shown to be higher with older age, it is expected that the number of dementia patients will rise further in these five hospitals, where there's a high ratio of elderly patients. More elaborate and continuous plans are necessary for the management of dementia patients following the growing number of dementia patients.

Age Distribution of Breast Cancer from a Thailand Population-Based Cancer Registry

  • Background: Breast cancer is a common cancer worldwide. With the establishment of Thailand's population-based cancer registry and availability of complete data from 2002-2011, it is of interest to investigate the epidemiologic and clinic-pathological profiles of breast cancer based on the population-based registry data. Methods: The data of all breast cancer patients in the registry for the period of 2002-2011 were included. All medical records of the patients diagnosed from documents of National Cancer Registry of Thailand were retrieved and the following information abstracted: age, clinical characteristics, and histological variables. Thailand census data for the period of 2002-2011 were used to provide the general population's statistics on age, gender, and other related demographic factors. Results: Over the 10 year-period, 7,711 breast cancer cases were included. The disease incidence under age 40 years was relatively low (4.13/$10^5$) while the incidence in the age groups 40 and older was very high (39.2/$10^5$). The vast majority of breast cancer cases (88.8%) were diagnosed by histology as primary lesions in the breast. The most common of patients with breast cancer (36.4%) had regional lymph node involvement and the most common of histopathology diagnosed in patients (84.2%) was an infiltrating duct carcinoma. Conclusions: This study showed a high incidence of breast cancer in older subjects, and high rate of breast cancer in Thailand. Future studies should explore clinical and molecular disease patterns.

Factors Related to Blood Glucose Control in Patients with Diabetes (당뇨병 유병자의 혈당 조절 관련 요인)

  • Objectives: This study was conducted to determine factors related to glycemic control among Korean patients with diabetes. Methods: Data was collected from the Fourth (2007-2009) Korea National Health and Nutrition Examination Survey (KNHANES) which was a nationwide, cross-sectional survey. The study participants who are available for hemoglobin A1c (HbA1c) were 1,441 patients with diabetes aged 30 years and older. Poor glycemic control was defined as $HbA1c{\geq}7.0%$. Data were analysed using logistic regression. Results: The proportion of patients with the poor glycemic control ($HbA1c{\geq}7.0%$) was 49.5%. After adjusting for confounding factors, younger aged persons (30-49 years) were less likely to control the blood glucose than older aged persons (60-69 years) (Adjusted OR=0.533, 95% CI [0.295-0.960]) and longer duration of diabetes was positively related to poor glycemic control (Adjusted OR=2.018, 95% CI [1.418-2.873]). Conclusions: Age and duration of diabetes were significantly related to blood glucose control. Therefore, considering these factors is helpful for developing a strategy to improve blood glucose control in diabetes.

An Investigation on Solution of Insecurity Sense Using a Music Performing Magnetic Resonance Imaging: Case report (자기공명영상 검사 시 음악을 이용하여 환자의 불안감 해소에 관한 고찰: 사례 보고)

  • The purpose of this study is to know about solution of insecurity sense for solving the tediousness and inducing comfortableness in long time MRI examination. Studies were examined with total of 117 patients that the intention expression of self is possible for a month without dividing man and woman in november 2006. Examination methods were filled in questionnaire after testing using Siemens Impact 1.0T. The musical art were selected through intranet search and record sale inquiry using CD player and speaker. That kind of a music were selected with Korean classical music's 10 chapter, pop song 10's chapter, classic's 10 chapter and Korean popular song's 10 chapter. With analytical method, patients of under 40 year-old, or older and patients using ear-pad and head-phone were classified into groups. As analysis result with question item of questionnaire, most of the patients were showed a favor(36.72%) in playing a music, specially, examination of about 15.38% which uses the headphone prefer a music as older generation of over 40 year-old. In conclusion, with installing of the professional musical program in MRI test rooms, if listening to the music with selection by oneself before testing by season, age, time through development of appropriate musical program will be able to expect the more effect.

Clinical Characteristics of Pediatric Constipation in South Jordan

  • Purpose: Constipation is a common pediatric problem worldwide. This study aims to describe the clinical characteristics of pediatric constipation in south Jordan according to gender and age group. Methods: All patients with constipation managed at our pediatric gastroenterology service between September 2009 and December 2012 were included. Hospital charts were reviewed. Demographic data, clinical characteristics, and final diagnosis were recorded. Data were analyzed according to gender and the following age groups: infants, pre-school, school age, and adolescents. Results: During the study period, 126 patients were enrolled. The number (percentage) of patients according to age were the following infants: 43 (34.1%), pre-school: 55 (43.7%), school age: 25 (19.8%), and adolescents: 3 (2.4%). Males made up 54.8% of the study population. There were no statistical gender differences in any age group. The most common symptom in all age groups was dry, hard stool. Infrequent defecation was found in almost one-half of the patients. Fecal incontinence was more common in school-aged children compared to pre-school-aged children and adolescents. Abdominal pain was seen in almost 40% of the constipated children. Abdominal pain was more prevalent in girls and older children. Fecal mass in the rectum was the most common physical finding, with constipated boys exhibiting higher rates. Functional constipation was the most common etiology. Conclusion: Clinical characteristics of constipation in children vary according to age group and gender. Older children had less frequent bowel motions, a longer duration of symptoms, and a higher prevalence of long-standing constipation compilations (fecal incontinence and abdominal pain).

Analgesic Effects of Intramuscular Stimulation Therapy on Pain in the Elderly (근육내 전기자극에 의한 노인성 통증조절 효과)

  • The purpose of this study was to determine the effect of intramuscular stimulation (IMS) therapy in older persons with musculoskeletal pain. The subjects were 181 older persons (54 males, 127 females) with musculoskeletal pain. Intramuscular stimulation unit with needles (size $.3{\times}30$ mm) was applied for the treatment. The analgesic effects were measured by visual analog scale (VAS). Results showed that the post-treatment VAS score was significantly decreased after IMS therapy for fifteen minutes compared to pre-treatment score. In addition, the post-treatment VAS score was significantly decreased in patients with chronic pain (pain duration of one year after onset) compared to the post-treatment VAS score in patients with subacute pain (pain duration less than three months after onset). There was no significant difference in analgesic effects according to gender and age groups. It is determined from this study that IMS therapy can be beneficial for patients with chronic musculoskeletal pain in clinical setting. Further study is needed to identify whether the IMS therapy can change the pain threshold in patients with neurologic pain.

Durability of the Ionescu-Shiley Valve in Mitral Position (이오네스큐 승모판막의 내구성)

  • A total and consecutive 291 patients of isolated single mitral valve replacement using the Ionescu-Shiley bovine pericardial xenograft valve operated on between October 1978 and June 1983 were retrospectively studied for the durability of the substitute valves based on the structural degeneration of primary tissue failure which had been proved on their re-replacement surgery. The mean age at the initial surgery was 32.4*12.5 years, and the operative mortality rate was 5.2 %. The early survivors of 276 patients were followed up for a total 1148.3 patient-years[mean\ulcornerD, 4.16*2.57 years]at the follow-up end of June 1988. They experienced 4 major late complications: 1.045 % thromboembolism/patient-year [pt-yr]; 0.871 % bleeding/pt-yr; 0.610% endocarditis/pt-yr; and 3.309% overall valve failure/pt-yr or 1.655% primary tissue failure/pt-yr. The actuarial survival rates were 89.4*2.2% and 87.7*2.5% at 5 and 10 years after initial surgery respectively. The probabilities of freedom from thromboembolism were 95.1*1.6% and 93.2*2.0% at 5 and 10 years after surgery. Nineteen patients underwent re-replacement of the Ionescu-Shiley valve because of primary tissue failure, and there was no operative mortality. The incidence of primary tissue failure was highest for the patients less than 15 years of age occurring in 9 out of 27 patients [33.3 %] or 8.68 %/pt-yr, while it was 4.0 % or 0.96 %/pt-yr for the rest of patients older than this age. The probabilities of freedom from primary tissue failure were 96.7*1.4 % and 84.2*3.8% at 5 and 9 years after surgery. The freedom from tissue failure increased as the age limits of cumulative younger patients were increased while it decreased as the age limits of cumulative older patients were decreased. Although it is clear that the Ionescu-Shiley valve would degenerate prematurely in young patients, the definite age limit could not be identified when the risk of early failure was significantly high.

Problematic Behaviors recognized by Caregiver in Demented Older Adults (돌봄제공자가 인지한 치매노인의 문제행동 연구)

  • Behavioral symptoms are frequent and problematic components of dementia. The aim of this study was to detect behavioral problem of the 25 item of dementia problem behaviour(DPB)assesment scale for demented older adults. Seventy-three patients with dementia staying in the day care center, group home, dementia hospital, nursing home were recruited data on problematic behavior obtained through interviews with their caregiver during 2 months from March to May 2004. Results shows that problematic behaviors common occurring in 95% of subjects. The most frequently exhibited problematic behaviors were repetitive movement(1.05), repetitive questions(1.03), restlessness(1.0). indifference(0.97). shouting/screaming(0.92). Problematic Behaviors of the demented older adults were closely associated to the distress for caregiver. Most distressful ones for caregivers were repetitive questions(1.46), repetitive movement(1.42), shouting/screaming (1.42), indifference(1.41). restlessness(1.41). Problematic Behaviors of the demented older adults were divided into six subdivisions, among those restless behavior was positively correlated with aggressive behavior, nervous symptom and psychotic symptom. and then nervous symptom was interrelated to psychotic symptom(P<0.01). This study has limitation that field study data were derived from various primary caregivers.
