In these days, the size of urban is growing and the function of city becomes complicated. And also, in city, people lives a lot. The life of urban is getting closer and linked with neighboring people in many parts. Especially, when peoples are exposing during using public transportation, even though does not be known, in they were living. Seoul is the most crowded place in Korea. In Seoul,The village buses have been serviced to the narrow streets. And people who use this bus, wants to seek the comfort of the ride, air quality and noise during in vehicle. In this paper, we determine the degree of annoyance with the noise inside the town bus in dB scale. And, such a situation was confirmed annoyance see their effect. The Interior noise did not see a big difference in the new car and the old car. Annoyance but also according to the skill of the bus driver remains the difference was confirmed.

Anaplastic lymphoma kinase (ALK) rearrangement, is a kind of driver mutation, accounts for 3%-5% of non-small cell lung cancer (NSCLC). NSCLC patients harboring ALK fusion genes have distinct clinical features and good response to ALK inhibitors. Metastasis from lung cancer to the ovary has rarely been known. We report a case of a 54-year-old woman with bilateral ovarian metastases from ALK rearranged NSCLC. She underwent bilateral salpingo-oophorectomy for ovary masses, which were progressed after cytotoxic chemotherapy although primary lung mass was decreased. Histopathological examination of the ovary tumor showed characteristic adenocarcinoma patterns of the lung and ALK rearrangement.

In 2008, according to Statistics Korea's population over the age of 20 and 26.3% of the smokers, 73.7% were non-smokers, smokers and non-smokers by smoking of 22.0% was reported. 336 people of 255 people (75.9%) is currently also showed that smoking. Hypertension, 48 people(14.30%), the boundary type of high blood pressure 193 people (57.40%) of them, especially for high blood pressure appear as a systematic management is required. 336 people targeting taxi drivers smoking and blood pressure, and the relevance of the reserch results did not show a statistically significant difference. Current smokers of 336 people surveyed 255 people (75.90%), respectively. Men over 15 years old in Korea smoking population (1990) was reported as 73.2%, Lee 2001, according etc. Korea 61.8% of the adult male smoking rate is quite high as compared to what is reported. Taxi drivers are working in very bad working conditions. The health of a taxi driver affect the safety of customers were considered.

Due to the aging effect, older people have relatively weaker muscular performance, less range of motion in the joint articulation, and the lower sense of equilibrium than younger people. These factors attribute to their slow and clumsy ingress/egress motion. In order to analyze ingress/egress motion strategy of the elderly, healthy thirty 65 or more years old volunteers were recruited. The health condition of the each volunteer was verified by the medical checkup and also their physical capabilities were quantified by six fitness tests. Through the video analysis, older driver's ingress/egress motion strategies were classified and statistically investigated. For a comparison purpose, another thirty young volunteers also participated in the same test protocol and their ingress/egress motion strategies were also included in the statistical analysis.

A case of mediastinal seminoma is presented. A male driver, 27 years old Korean, has been suffered from substernal pain, cough and moderate swallowing difficulty since 5 months prior to this admission. At the time of onset, he visited at a local clinic to find some mass in his anterior mediastinum on chest P-A and lateral X-ray check. Recently, intermittent hiccups with much aggravated dysphagia forced him to visit our hospital, and admitted for radical resection under the impression of anterior superior mediastinal tumor of thymus origin. Median sternotomy was done and total resection of the tumor of 8.0X11.0X3.5cm was done without any specific complication and biopsy of the tumor revealed as primary mediastinal seminoma of the mediastinum. Supplementary prophylactic irradiation therapy was done with a tumor dose of 4,000 rad in 4 weeks after operation. Postoperative hospital course was uneventful and patient was joyful with his occupation for 3 months after discharge.

Direct carotid-cavernous fistula [CCF] is a common post-traumatic disease. However, pseudoaneurysm formation after balloon occlusion is a rare complication. The author present such a case with review of the literature. A 26-year-old man involved in a motor vehicle accident as a driver. Only mild conjunctival injection and minimal exophthalmos on the right eye were noted after trauma. However, angiography revealed a direct CCF and dissection of the proximal intracranial internal carotid artery [ICA]. After first balloon occlusion of the CCF, the patient redeveloped fistula due to early deflation of the balloon. After the second balloon occlusion, pseudoaneurysm and diplopia were developed with the change of balloon position and shape. However, visual symptom spontaneously resolved and pseudoaneurysm was also decreased within 6 months after balloon occlusion.

The traffic accidents of the aged are increasing because old people generally experience a decline of physical functions and judgment but their dependence on automobiles increases. In order to investigate the older drivers' response to traffic signs and surrounding environment according to changing driving brightness, this study measured the visual cognitive behavior, driving behavior, and subjective evaluation while the driving brightness was changed using a driving simulator. Furthermore, the drivers who recognized traffic signs and those who did not were compared. As a result, it was found that some older drivers had declined ability of cognition, some failed to adapt to rapid brightness change even if sufficient attention was given to traffic sign, and some could not safely control the vehicle when approaching intersections. Therefore, the development of support systems for aged drivers in traffic environment is necessary in the future.

A 52-year-old man was found dead in the driver's seat of his tank lorry. The tank lorry was used to transport industrial wastewater to a disposal plant; the material was transferred into a storage tank with the help of compressed air. The wastewater contained methylene chloride and 2-chloropyridine. No respiratory protective equipment was used while working under these conditions. The autopsy report showed extensive edema and congestion of the brain, lung, and intraperitoneal organs. The concentrations of methylene chloride in lung and brain were reported at 398 and 67 mg/kg, respectively.

Cardiac tamponade is an acute life-threatening condition that predominantly involves the intra-pericardial space; however, an expanding mediastinal hematoma can also sometimes cause cardiac tamponade. Here we describe the case of a 45-year-old male driver in whom a traffic accident resulted in rupture of the left internal thoracic artery (ITA), extra-pericardial hematoma, and sternal fracture. After resuscitation, he was scheduled to undergo angio-embolization to repair the ruptured left ITA, but he suddenly developed cardiac tamponade that required a decompressive sternotomy. Nevertheless, the patient had an uncomplicated recovery, and this case suggests that extra-pericardial cardiac tamponade should be considered as a possible consequence of retro-sternal hematoma due to traumatic ITA rupture.
