  • Localized mesothelioma is a primary pleural tumor that induces hypoglycemia rarely. We experienced a case of the localized mesothelioma that induced hypoglycemia. The patient was 70 year-old man who was admitted to Severance Hospital because of general weakness and mental confusion in morning before breakfast. He was found to have low level of fasting blood sugar and C-peptide, but had normal serum insulin level. After excision of the tumor, the patient was free from symptoms of hypoglycemia, and fasting blood sugar level was returned to normal range. The most likely speculation of causing hypoglycemia by the mesothelioma was thought to be that the tumor did not secrete insulin itself but the undetectable insulin-like substance and/or antigluconeogenic substances. During the follow up, 5 months after surgical removal of tumor, the patient was in a good condition without symptoms of hypoglycemia.

  • Leiomyoma is the most common benign tumor of the esophagus, and surgical enucleation is the treatment of choice. Recently we successfully performed thoracoscopic enucleation of large esophageal leiomyoma without complication in one patient. The 46 years old male patient complained epigastric discomfort and showed a submucosal mass in lower esophagus under the endoscopic ultrasonography . During operation minimal perforation occurred, it was closed with clipping without conversion to an open procedure. The tumor size was 8cm x 3cm x 1.5cm respectively. There were less post-operative pain, minimal wound size, and early recovery time.Patient was satisfactory these outcome. These result suggest that esophageal enucleation was performed more large size benign tumor and esophageal perforation during operation was treated thoracoscopically.

  • Referred pain is not localized to the site of its cause, but referred to an area that may be an adjacent distant from such a site. With respect to this, there is the possibility of misdiagnosis in the treatment of referred pain patient. We experienced a case of a 31-years-old male patient complaining of right shoulder pain, which subsided after a bursa injection. The patient revisited our clinic after 3 weeks complaining not only of right shoulder pain, but also of right upper quadrant pain and fever. He was diagnosed as having a liver abscess by an imaging study.

  • This case study was performed to investigate the effects of Chongsimyonja-tang and Dong's acupuncture therapy on senile macular degeneration patient who has been treated in the Dong-Eui oriental hospital. A 57-year-old female with senile macular degeneration complained of headaches, heaviness of the ache, night sweat, dizziness. For a duration of 2 months, We prescribed Chongsimyonja-tang, and added 3-4 herbs according to the symtoms and also treated the patient with Dong s acupuncture therapy. After 2 months, the patient's eyesight improved ($0.2{\rightarrow}0.7$) and the headaches disappeared, as did heaviness of the ache, night sweat, and dizziness. In this case Chongsimyonja-tang and Dong's acupuncture therapy present valuable treatment of senile macular degeneration.

Methods A 9-year-old female patient, who was suffering from Hyperhidrosis on both hands and feet, was treated by Korean herbal medicine (Dangguisayuk-tang gami), acupuncture (Joksamri, Taegyue, Taechoong, Souboo hyul symmetricaly) and moxibustion on the abdomen (Joongwan). Also, VAS scales were used to check the improvement. Results About treating for two months, the patient's hyperhidrosis on both hands and feet were improved by Korean herbal medicine, acupuncture and moxibustion on the abdomen. Conclusions This study showed that Korean medical treatments can be an effective for the hands and feet Hyperhidrosis.

  • The authors report the result achieved in the treatment of trigeminal neuralgia patient, especially V2 involved patient, using radiofrequency (RF) thermocoagulation of Gasserian ganglion. A 62-year old female patient had severe burning pain on right cheek usually initiating from upper molar teeth area for 10 years. She was treated with microvascular decompression operation 10 years ago. However, there was no pain relief by operation. She wanted non-surgical treatment. Therefore, we recommended RF thermocoagulation therapy. After 2 times of RF thermocoagulation, there was excellent pain relief without complications. And, for 6months follow-up, there were no pain, and no evidences of complication and recurrence.

  • As laparoscopic surgery becomes more popular, various complications following the laparoscope are also increasing. Nerve injury following the laparoscope is an infrequent but serious complication for both the doctor and patient. A 30-year old female patient suffered severe burning pain of the left buttock, inguinal area, external genitalia and inner side of vagina following laparoscopic surgery for ovarian mass. We successfully treated this patient with ilioinguinal, iliohypogastric nerve block in combination with epidural blocks.

  • A 19-year-old male patient presented with facial hemi-atrophy with unilateral spasms of the masseter and temporalis muscles. Ultrasound therapy and Transcutaneous Electric Nerve Stimulation therapy, known as combination therapy, were given on alternate days for 2 weeks. At the end of 2 weeks of combination therapy the patient reported a drastic reduction in the number of episodes of muscle spasm. The visual analog scale score for tenderness of the masseter and temporalis was also markedly reduced. No one has previously used combination therapy for the treatment of facial hemi-atrophy with hemi-facial spasms. The encouraging results of the combination therapy has prompted us to document this study.

  • A 37-year-old male was admitted for left chest pain during a 6 months period and was given an epidural block for pain control. Vancomycin 500 mg/4 ml instead of 0.25% bupivacaine was accidentaly injected into the epidural space. The patient suddenly complained of an unexpected chest tightness and dyspnea after epidural injection. The patient was treated with steroids and local anesthetics via epidural space and recovered without any neurologic sequelae. The patient completely recovered on the 46th postadmission day and went home without any complaints. We report this case and review the accidental injection of substances into the epidural space.

  • The syndrome of the sinking skin flap was introduced to explain the phenomenon of neurological deterioration after decompressive craniectomy. A 37-year-old man was admitted with acute subdural hematoma and traumatic intraparenchymal hematoma. After decompressive craniectomy, the patient suffered from hydrocephalus for which a ventriculoperitoneal (V-P) shunt was inserted. Following this procedure, the depression of the skin flap became remarkable and his mentation was deteriorated. The patient recovered uneventfully after temporary elevating of valve pressure and cranioplasty. We present a patient who was successfully managed with elevation of valve pressure and cranioplasty for the syndrome of the sinking scalp flap with review of a pertinent literature.
