Supratentorial Cortical Ependymoma in a 21-Month-Old Boy

  • Two-thirds of ependymomas arise in the infratentorial or intraventricles, whereas one-third are located supratentorially. But supratentorial "cortical" ependymomas are very rare. We report a case of a cortical ependymoma in a 21-month-old boy. The patient presented with simple partial seizures. This tumor was located in the postcentral gyrus and he had gross total excision. Microscopy and immunohistochemistry showed grade II differentiation ependymoma.

  • Hydatid cystic disease of the lung is rare in Korea. Recently, the authors encountered 2 patients with this disease who were treated surgically with a successful outcome in both. The first patient, a 27 year old housewife never traveled outside Korea and the second, 25 year old man had a history of spending a year in Kuwait. These 2 cases are the first documented cases of pulmonary hydatoidosis among Koreans reported in the literature.

2 Cases of Nasopharyngeal Tuberculosis (비인강결핵 2례)

  • Nasopharyngeal tuberculosis is a rare pathological condition. It is most often associated with lymph node and pulmonary lesions, but it may be an isolated lesion. The clinical manifestation may resemble a malignant tumor of the nasopharynx and the nasopharyngeal tuberculosis is occurred occasionally primary infection but more frequently secondary infection to pulmonary tuberculosis. The nasal endoscopic evaluation of nasopharynx is necessary in patient with possible pulmonary or extrapulmonary tuberculosis. The author reports two cases of nasopharyngeal tuberculosis in a 45-years old and 34-years old woman with a review of the literature.

  • In 1987, we experienced 2 cases of T.A.P.V.D. corrected successfully under cardiopulmonary bypass. The first case was 28 years old male with supracardiac type drained through left innominate vein. He was oldest patient among T.A.P.V.D. which was reported in Korea. The other case was 14 years old male with cardiac type drained to right atrium directly without communication with coronary sinus. Two patients were well in postoperative 9 and 10 months and have NYHA functional class I.

  • The purpose of the thesis is to develop a module which can measure precise Continuous Cardiac Output(CCO) better than existing equipments for the critical cardiopulmonary patient diagnosis purpose. This module enables more precise and effective diagnosis and treatment for the patient by compensating old method which injected cold liquid to inside of heart to measure the CCO.

  • Lamivudine and adefovir are medications used to treat hepatitis B. We observed the occurrence of tinnitus after administering lamivudine and adefovir to a 49-year-old hepatitis B patient for two months. The patient had no comorbidities and no history of ear diseases, including tinnitus, and was not taking any other medications. In general, neither lamivudine nor adefovir are known to induce tinnitus as an adverse reaction. A literature search revealed that this is the first case in which tinnitus occurred after lamivudine and adefovir were administered to a hepatitis B patient. Therefore, we believe that this case is clinically valuable and decided to report it.

  • Objectives The purpose of this study is to investigate the effect of Jihwangbaekho-tang on chest discomfort of unknown cause in a 52-year-old male patient diagnosed with Soyangin. Methods Patient had chest discomfort. But, the cause of this symptom was not identified by various cardiac examinations. The patient was treated with Soyangin Jihwangbaekho-tang for 2 weeks on the basis of Sasang Constitutional Medicine and the symptoms were evaluated by GAS(Global Assessment Scale). Results After using Jihwangbaekho-tang, GAS score has been decreased from 7 to 3. Conclusions This case study suggests that chest discomfort, whose cause is unknown, can be treated when using Sasang Constitutional Medicine.

  • We present a case of an 8-year-old boy with visceral autonomic neuropathy complicated by Guillain-Barre syndrome. In this pediatric patient, gastroparesis was the major symptom among the autonomic symptoms. Due to the gastroparesis, there was no progress with the oral diet, and nutrition was therefore supplied through a nasojejunal tube and gastrojejunal tube via Percutaneous endoscopic gastrostomy (PEG). After tube feeding for 9 months, the patient's gastrointestinal symptoms improved and his oral ingestion increased. The pediatric patient was maintained well without gastrointestinal symptoms for 3 months after removal of the PEG, had repeated vomiting episodes which lead to the suspicion of cyclic vomiting syndrome. Then he started treatment with low-dose amitriptyline, which resulted in improvement. Currently, the patient has been maintained well for 6 months without recurrence, and his present growth status is normal.

Surgical Treatment of Wolff-Parkinson-White Syndrome Combined with AV Nodal Reentrant achycardia in a Patient with Ebstein`s Anomaly - A report of one case - (Ebstein씨 심기형에 동반된 Wolff-Parkinson-White 증후군 및 방실결절 회귀성 빈맥에 대한 수술치험 1례 보고)

  • A 17 year old female patient with Ebstein`s anomaly received surgical treatment for WPW syndrome and AV nodal reentrant supraventricular tachycardia[SVT] Electrophysiologic study revealed that an anomalous pathway was located in the right posterolateral portion and antegrade dual AV nodal pathway responsible for AV nodal reentrant tachycardia. The patient was underwent surgery on February 18, 1987. Intraoperative mapping was used to define the location of accessory pathway. The accessory pathway was cryoablated through the epicardium. Simultaneously discrete cryoablation around the perinodal area was performed to prevent AV nodal reentrant SVT. The atrialized right ventricle of Ebstein`s anomaly was plicated with 11 pledget mattress sutures under the cardiopulmonary bypass. Two and half years after surgery, the patient has no evidence of WPW syndrome or supraventricular tachycardia.

Reoperation of Failed Tricuspid Mechanical Prosthetic Valve Due to Pannus Formation (Pannus 형성으로 인한 삼첨판막 재치환술)

  • We performed a reoperation of failed tricuspid mechanical valve in a 63-year-old female patient because the overgrown endothelial pannus had entrapped the prosthetic leaflets. Four years ago, the patient underwent mitral and tricuspid valve replacements with 31 and 33 mm Carbomedics, respectively. The patient showed symptoms of neck vein distention, abdominal distention and peripheral edema. The chest film, echocardiography and cineangiography confirmed the diagnosis of tricuspid valve. During the operation, we found the entrapped leaflets of the tricuspid valve in a partially closed state and the endothelial pannus had overgrown into the leaflets. Carpentier-Edward bovine pericardial valve was inserted and the patient was discharged with no significant events.
