  • A 12 years old patient having rampant dental caries on permanent dentition has been treated under operative procedures. In addition to this operative treatment the nutritional survey and the caries activity was checked, and performed the oral hygiene instruction to the patient. As the consequences of above procedures the following results were obtained: 1. The familial history and eating habits checkings were helpful for better dental health guide and caries control. 2. The restriction of mono & disaccharide intake and topical application of 8% stannous fluoride at least two times revealed excellent control effect for recurrent dental caries.

  • Acquired communicated diverticula between the esophagus and respiratory system are infrequent, and they are caused by carcinoma, trauma, infection, and traction. This report reviews the feature of benign esophagobronchial fistula due to midesophageal diverticulum. Patient is twenty year old man with excellent result by surgical intervention . The surgical procedures consist of divertuculectomy and superior segmentectomy of lower lobe of right lung. Clinically and radiologically, the patient is free from substernal distress, regurgitation, esophagorespiratory fistula, and esophageal stricture after surgical treatment.

  • Carcinosarcoma is the group of neoplasms possessing both carcinomatous parenchyma and sarcomatous stroma. These tumors are relatively uncommon and may arise in variable organs. In this case, a 62-year-old male patient was admitted to our department due to left chest pain and general weakness of three months duration. The chest x-ray revealed egg-sized, relatively well defined lobulated soft tissue mass in left paracardiac area. Left lower lobectomy was performed under the preoperative impression of lung cancer. Pathologic examination of the resected lung revealed carcinosarcoma without regional lymph node metastasis. Patient is doing well at present without any chemotherapy.

  • This is a report of a migrated polyethylene catheter from the arm vein cutdown site into the heart. The patient was a 48 years old male who was operated on for mitral stenosis complicated with pulmonary hypertension. On the 8th postoperative day after open mitral commissurotomy, polyethylene catheter embolization occurred. The catheter was removed successfully with an incision on the proximal basilic vein. The patient recovered uneventfully and discharged without complications.

  • Clinical observation is made in 29 cases of patients with esophageal cancer at Department of Chest Surgery in Chunnam University Hospital. 1] The prevalent age of male patient is fifth decade and of female is fourth and fifth decade. The average age of patient is 54 years old. 2] Duration of the disease before admission is relatively long, the duration within 7-9 months is 35% of all patients. 3] On the majority of patients, typical symptoms of esophageal cancer were manifested. 4] Operation was performed on 58% of all patients, in 23% curative and in 35% palliative resection was done.

  • Extramucosal esophagomyotomy by thoracotomy, first described by Heller in 1913 was the principal therapy for achalasia. Recently however, video-assisted thoracoscopic surgery has been implied in esophageal surgery, and we have experienced 23 year old male patient with dysphagia and was diagnosed as achalasia who underwent thoracoscopic esophagomyotomy. The esophagogram showed typical bird beak appearance and the lower esophageal sphincter pressure was increased to 35 mmHg. Thoracoscopic surgery was done twice due to incomplete myotomy in first operation and the patient was released of dysphagia in the second operation.

Leiomyosarcoma of the Pulmonary Artery (폐동맥에 발생한 평활근육종)

  • Primary malignant tumors originating in the pulmonary artery are extremely rare. Recently, We experienced a case of primary leiomyosarcoma occurred on the main pulmonary artery and extended into the Rt and Lt pulmonary artery. The patient was 44 year-old woman and the chief complaint was severe exertional dyspnea. Emergency pulmonary arteriotomy to relieve the pulmonary artery obstruction was performed on cardiopulmonary bypass. The tumor was 6 x6 x4 cm in size and infiltrated into the main pulomary artery. The tumor in the pulmonary artery was removed without any difficulties. The patient was recovered without any specific problems. The tumor was confirmed to be leiomyosamoma histopathologically.

  • Isolated stenotic lesion located at the ostium of the left main coronary artery associated with Takayasu`s aortitis is quite rare. This report herein described a case of 25 year old woman with isolated ostial stenosis of the left main coronary artery underwent pericardial patch [fixed with 0.6 % glutaraldehyde] angioplasty. An anterior approach was used and postoperative coronary angiogram of the patient showed normal coronary ostial contour with normal runoff.

  • We experienced a case of intralobar pulmonary sequestration preoperatively confirmed. The 10 years old male patient was admitted beacuse of recurrent episode of coughing and production of purulent sputum. the chest X-ray showed a dense mass containing a large cyst with air-fiuid level in right lower lung field. An aortogram was performed and revealed that the sequestrated portion of the RLL was supplied by an aberrant large artery arising directly from the lower thoracic aorta. After division and ligation of the aberrant artery in pulmonary ligament, the right lower lobectomy was performed and the patient`s postoperative course was uneventful.

Complete Vascular Ring Caused by Kommerell's Diverticulum and Right Aortic Arch with Mirror Image Branching

  • Vascular ring, caused by Kommerell's diverticulum and ligamentum arteriosum, in a patient with right aortic arch and mirror image branching is extremely rare. A 10-month-old boy with coughing and stridor was diagnosed as having tracheo-esophageal stenosis, which is caused by a vascular ring with Kommerell's diverticulum, ligamentum arteriosum, right aortic arch, and mirror image branching. Kommerell's diverticulum was successfully resected via a left thoracotomy. The patient has been free from tracheo-esophageal stenosis for a year after the surgery.
